Attention Biases Associated with Vulnerability to Bipolar Disorder

Bipolar disorder is associated with significant social and occupational impairments, as well as increased risk for substance abuse and suicide. More research is needed to identify potential mechanisms associated with vulnerability to the disorder. Previous research has identified altered processing of emotional information in bipolar and bipolar-prone individuals, including attentional biases which appear to differ based on the current affective state of the individual. The current study applied a sensitive measure of attention (i.e., eye-tracking) to assess whether vulnerability to bipolar disorder, as indexed by hypomanic personality traits, would be correlated with biases in attention to emotional facial stimuli, independent of mood state. Hypomanic personality traits were hypothesized to be associated with greater attention to happy and angry faces, as indexed by faster initial orientation, more frequent gazes, and longer gaze duration for these stimuli. Participants completed self-report measures assessing current mood symptoms, positive and negative affect, and hypomanic personality traits. They then completed two tasks assessing attention for emotional faces. The first was an eye-tracking task, which measured latency to first fixation, total gaze duration and total number of gazes for each emotional face category. The second was a spatial cueing task which assessed both attentional engagement with emotional faces, and ability to disengage attention from this material. Hypomanic personality traits were significantly negatively correlated with latency to orient attention to happy faces. A trend toward decreased latency to orient to angry faces with higher hypomanic personality traits was also demonstrated. Hypomanic traits were not correlated with attention to sad faces. Furthermore, hypomanic traits were associated only with differences in initial orientation of attention, not with continued engagement or disengagement. The results of this study suggest that individuals with higher levels of hypomanic personality traits, who are hypothesized to be at greater risk of developing bipolar disorder, are characterized by differences in their initial orientation of attention to positive emotional stimuli, independent of their current mood state. This finding is indicative of biased information processing in individuals with vulnerability to bipolar disorder. Such a bias may have important clinical implications for individuals with a vulnerability to bipolar disorder, as it may represent a mechanism by which vulnerability leads to increased, and at times problematic, engagement with rewarding stimuli.

Craving for Attention: Examining Mood and Attention Bias in the Moment as Predictors of Alcohol Craving

Alcohol consumption is prevalent on college campuses. There are a number of theories that seek to explain the link between alcohol use and mood. The affective-processing model posits that negative affect may interact with subconscious cognitive factors, such as attention bias (AB), to promote drug-seeking behavior. In contrast, the incentive-sensitization model suggests positive mood may drive drug seeking behavior, and this drive may be moderated by cognitive factors (e.g., AB). The current study hypothesized that both positive and negative mood would be associated with drug craving in the moment. It was further hypothesized that AB would moderate mood-craving associations. Participants (n = 69) from a Midwestern University carried a mobile device for 15 days and provided ratings of momentary mood (positive mood, anxiety, anger, and sadness), craving, and attention bias. Across assessments, all four moods were positively associated with momentary craving (p < .05). There were significant interactions of Anxiety x AB, Anger x AB, and Positive Mood x AB; all of which varied by gender. For men, Anxiety (B = .15, p = .004) and Positive Mood (B = .22, p < .001) were more robustly associated with momentary craving when their AB was +1SD above their own mean. For women, Anger (B = .14, p = .001) was more robustly associated with momentary craving when their AB was +1SD above their own mean. These results indicate differential effects of AB on mood by gender. Theoretical models of mood x cognition interactions may have more nuanced effects based on one's gender. This suggests that different forms of mood may trigger drug seeking behavior for men and women, offering important gender differences in relapse risk.

Attention Bias in Middle Childhood: The Impact of Effortful Control and Temperament

Identifying whether a stimulus is threatening or not is critical for staying safe. The faster one can detect a threat, the greater chance there is to avoid any potential danger. Factors contributing to the visual attention of threat are therefore informative. Previous research has examined how aspects of temperament and effortful control interact and affect the attention allocated to threats, especially in clinically anxious populations. However, there is a sparsity of this literature existing for nonclinical populations. My study addressed previous gaps by examining whether negative affect and fear impact an attention bias to threat in children aged 6 through 8 while assessing how attentional control and inhibitory control moderate these relations. A modified visual search task with snakes as the threat was given to the participants after the children’s parents completed questionnaires and the children completed an attentional control task. Results showed that an attentional bias to snakes was seen in the sample. Negative affect as a main effect nor as an interaction effect with attentional control predicted for the attention bias to snakes. Fear predicted for the attention bias to snakes as a main effect. Interestingly, inhibitory control moderated the relation between fear and the attention bias to snakes. Only children with high inhibitory control and high fear predicted for the attention bias to snakes. Fear predicted for the attention bias to snakes as a main effect. Interestingly, inhibitory control moderated the relation between fear and the attention bias to snakes, which meant that only children with high inhibitory control and high fear predicted for the attention bias to snakes. Findings may indicate children with this temperament are greater susceptible the development of anxiety.

Evaluating Changes in Attentional Biases following Cognitive Behavioral Therapy for Social Phobia

The purpose of the current study was to evaluate changes in attentional biases following CBT for Social Phobia. It was found that 1.) consistent with previous investigations, the overall sample displayed vigilance toward threatening facial stimuli prior to receiving treatment, and 2.) participants’ pattern of responding to threatening facial stimuli changed following treat-ment, but only when the sample was divided into those who were vigilant and those who were avoidant prior to treatment. Findings provide support for the presence of two distinct sub-groups with differing attentional styles, one with a tendency for vigilance toward social threats, and a second with a tendency to avoid threat cues. These findings have important implications for how individuals may differentially respond to treatment and may help explain some of the mixed findings in the extant literature on Social Phobia and attention bias.

Cost and Probability Biases in Social Phobia: Evaluating Their Relation to Attention Bias and Treatment Outcome

Social phobia is maintained in part by judgmental biases concerning the probability and cost of negative social events. One hypothesized mechanism of action of cognitive behavioral therapy (CBT) for anxiety disorders is its reduction in the exaggerated probabilities and costs associated with feared outcomes, termed the “cognitive mediation hypothesis” (Foa & Kozak, 1986). A number of studies have examined the cognitive mediation hypothesis; some investigations find cost bias to be more important to treatment outcome, whereas others find probability bias to be more important. However, methodological limitations of several of these studies leave open the possibility that changes in judgmental biases are simply correlates or consequences of social anxiety reduction. Attentional processes, which mark the first discrimination of incoming information, may serve as precursors to cognitive processes like probability and cost estimates. Though intuitively linked, whether social phobics’ pattern of attending to external threat cues is correlated with their appraisals of the cost and probability of negative events has yet to be examined empirically. The current project examines cost and probability biases and their relation to attention bias and treatment outcome in a randomized controlled trial of CBT for social phobia. It was found that, contrary to hypotheses, greater attentional bias for threat in either direction (vigilance or avoidance) did not predict higher cost and probability estimates. However, a significant relation was observed between attentional vigilance and outcome probability estimates, such that greater vigilance for threat predicted greater estimates of the likelihood that negative social events will occur. As hypothesized, early changes in cost and probability biases predicted later changes in social anxiety symptoms (and not vice versa). Changes in probability estimates were a stronger predictor of treatment outcome than changes in cost estimates. Broadly, findings provide support for the cognitive mediation hypothesis of social phobia and point to both outcome cost and outcome probability as potential treatment mechanisms. Findings are discussed in the context of extant theories of social phobia, and directions for future research are proposed.

Effects of a Gratitude Intervention and Attention Bias Modification on Emotion Regulation

Much research testing positive psychological interventions (PPIs) has focused on the outcome of emotion regulation (e.g., increased positive or decreased negative emotions and affect). On the other hand, most research testing the effects of attention bias modification (ABM) has focused on the process of emotion regulation (e.g., reducing biased attention towards threatening faces in those with social anxiety disorder). Evidence is sparse and inconsistent on the process of emotion regulation in PPIs and the outcome of emotion regulation in ABM programs. Furthermore, few studies have examined the combined effects of a positive ABM (PABM) with PPIs, which is the focus of the current investigation. The aim of the study is to examine two relationships: 1) the effects of the gratitude letter PPI on the process of emotion regulation and 2) the combined effects of the PABM program and the gratitude letter PPI on the process and outcome of emotion regulation. The researchers used a dot-probe task to bias attention. The dot-probe task presented positive-neutral stimuli pairs (e.g., babies; geometric pattern). In the train-positive group, the probe appeared behind the positive images 90% of the time, compared to 50% in the control group. The researchers used a gratitude letter PPI, where participants wrote a letter for 15 minutes to someone they have never thanked, compared to the control condition who wrote a letter about their morning routine. The results suggest that the gratitude letter PPI does not affect the process of emotion regulation via attentional biases but does increase positive affect in the short term. Further, the PABM program may not affect the outcome of emotion regulation but did demonstrate a biasing of reaction time to positive pictures. This biasing of reaction times in the dot-probe was not consistent with eye-gaze patterns to positive images, suggesting that the dot-probe task does not measure or manipulate attentional biases. Finally, combining the dot-probe task and gratitude letter PPI did not produce a stronger effect on the process and outcome of emotion regulation than the gratitude letter alone. The findings of the current study suggest that the gratitude letter may be an effective and quick intervention to increase positive affect, but that the intervention is not suitable for long-term changes after a single administration. Further, individuals should not expect the dot-probe task to bias attention and should not except the task to influence the outcome of emotion regulation. Instead, researchers could use the task to measure and manipulate one’s decision-making processing speed.

Attentional Bias Across the Dimension of Social Anxiety

The objective of this study was to examine attentional bias for threat in relation to social anxiety. It is well known that socially anxious individuals are likely to exhibit this bias, and this phenomenon may be clearly separated into two subsections: facilitation and disengagement. Facilitation, or vigilance, is the initial drawing of attention toward threat, and disengagement is the process of drawing attention away from threat. Past research focuses only on individuals ranked in the lowest and highest percentiles of any particular social anxiety measure, however, and fails to consider those having median scores. The current study included participants representing the full range of socially anxious individuals. In order to do this, the Fear of Negative Evaluations Scale (FNE) was distributed to undergraduate students (N = 230). A portion of these students was then drawn from each of six scoring ranges on the FNE. Participants in this new sample (N = 19) were asked to complete a computerized dot-probe detection task measuring attentional bias. The face stimuli for the current version of the computerized task included disgusted, happy, and neutral faces. Results of the correlational analyses showed a statistically significant positive correlation between FNE score and disengagement bias for disgust faces. There also was a statistically significant negative correlation between FNE score and vigilance bias for disgust faces. There was no suggestion of a statistically significant correlation between FNE score and vigilance or disengagement biases with happy faces.

Attentional bias retraining in cigarette smokers attempting smoking cessation (ARTS): study protocol for a double blind randomised controlled trial

Yes / Smokers attend preferentially to cigarettes and other smoking-related cues in the environment, in what is known as an attentional bias. There is evidence that attentional bias may contribute to craving and failure to stop smoking. Attentional retraining procedures have been used in laboratory studies to train smokers to reduce attentional bias, although these procedures have not been applied in smoking cessation programmes. This trial will examine the efficacy of multiple sessions of attentional retraining on attentional bias, craving, and abstinence in smokers attempting cessation. This is a double-blind randomised controlled trial. Adult smokers attending a 7-session weekly stop smoking clinic will be randomised to either a modified visual probe task with attentional retraining or placebo training. Training will start 1 week prior to quit day and be given weekly for 5 sessions. Both groups will receive 21 mg transdermal nicotine patches for 8–12 weeks and withdrawal-orientated behavioural support for 7 sessions. Primary outcome measures are the change in attentional bias reaction time and urge to smoke on the Mood and Physical Symptoms Scale at 4 weeks post-quit. Secondary outcome measures include differences in withdrawal, time to first lapse and prolonged abstinence at 4 weeks post-quit, which will be biochemically validated at each clinic visit. Follow-up will take place at 8 weeks, 3 months and 6 months post-quit. This is the first randomised controlled trial of attentional retraining in smokers attempting cessation. Associations between Fear of Negative Evaluation and Covert and Overt Attention Bias Through Eye-Tracking and Visual Dot Probe

Social Anxiety Disorder is characterized by irrational and persistent fears of potential evaluation and scrutiny by others. For socially anxious youth, the core, maladaptive cognition is fear of negative evaluation (FNE). Whereas Cognitive Behavioral Therapy (CBT) targets remediation of intense and unfounded FNE, Attention Bias Modification Treatment (ABMT) targets attention bias. The degree to which FNE and biased attention are related processes is unknown. This study sought to assess the relationship between FNE and two indices of attention bias (dot probe and eye-tracking). In addition, this study examines differences in attention bias between a clinically confirmed group of youth SAD and healthy controls. A significant group difference in average latency to fixate on angry faces was found [F(1,65) = 31.94, p < .001, ηp2 = .33]. However, the pattern was not consistent across the other attention bias metrics (i.e., dot probe bias scores and first fixation direction percentage towards angry faces). In addition, associations between FNE and the attention bias metrics were not statistically significant in either group. Identifying and Intervening on Neural Markers of Attention to Threat in Children with Anxiety Disorders

Objective: Attention Bias Modification Training (ABMT) for anxiety aims to train attention away from threatening stimuli and toward neutral stimuli. Although ABMT shows promising anxiety reduction effects in children and adolescents, no study has examined its influence on neural indicators of attention measured using event-related potentials (ERPs) in children or adolescents (i.e., youths). The present study examined the influence of ABMT on the P1, N170, P2 and P3 ERP components during completion of the emotional faces dot probe task in youths with anxiety disorders who failed to respond to cognitive behavioral therapy. Method: Thirty youths (M age = 11.97, SD = 2.89) with primary DSM-IV-TR anxiety disorders completed the dot probe task while undergoing electroencephalogram (EEG) to obtain ERPs before, immediately after, and eight weeks after eight sessions of either ABMT (n = 14) or a control task regimen (CT), (n = 16). Results: At post-treatment, statistically significant effects were found for P1 and P3 mean amplitudes: P1 was significantly higher during trials showing neutral-neutral (NN) face pairs in the ABMT arm than in the CT arm; P3 was significantly higher during trials showing NN face pairs than during trials showing neutral-threat (NT) face pairs in the ABMT arm, but not the CT arm. At eight-week follow-up, participants in both arms showed significantly higher (more negative) N170 responses for NN trials than for NT trials. Conclusions: Attention Bias Modification Treatment led to increases in neural processing of neutral stimuli in early and late stage attentional processing, as measured by the P1 and P3 components, respectively. These components during the dot probe task are promising neural markers of ABMT’s effects on attentional processing in youth with anxiety disorders.
