The first objective is to summarize the theoretical knowledge concerning problems of muscular dystrophy, its classification, hereditary predispositions, diagnostic procedures, subsequent care and to present international grant projects, and the muscular dystrophy world organization and registry. Faculty of Health Sciences / Aim: The aim of this study was to quantify the level of ability and in particular walking
ability of a group of stroke survivors resident in Soweto, between 3-6 months post
incident.
Design: Descriptive study in which comfortable walking speed over ten metres is
correlated with the distance covered in six minutes and two minutes.
Subjects: Forty one subjects after stroke divided into two groups. Twenty six with a
Barthel Index initial score of ≤60 and fifteen with a score >60.
Main Outcome Measures: Walking speed and two and six minute distance were
compared between groups. In addition actual distance walked in six minutes was
compared with the distance predicted by the ten-metre walking speed test and the
distance predicted by normative reference equations. Heart rate was measured during
the six minute walking test. Functional ability was compared using the Barthel Index
within ten days post stroke and three to six months.
Results: Of those who survived 90% of subjects were able to walk independently after
stroke. The mean walking speed of 0.55 m/s demonstrated a strong correlation with the
distance walked in six minutes (R2=0.816). However it underestimated the distance
walked in six minutes by 7.4%. There were no significant differences between groups
for the walking tests. The two minute walking test distance accurately predicted the
results of the six minute walking test distance (R2=0.97). The average distance walked
in six minutes by subjects after stroke was 40% of the distance predicted for healthy
adults. For the functional walk test 95.5% of subjects had a heart rate within normal
recommended limits. The functional walk test together with a measure of exertion (heart
rate) may indicate an individual's ability to sustain submaximal activity. Subjects had
minimal rehabilitation training. Lower limb pain did not significantly lower the walking
speed (p=0.18) or distance walked in six minutes (p=0.17). Mean Barthel Index score at
three to six months was 85.78 indicative of independence with minimal assistance.
Although the mortality rate for the Barthel Index group with a score less than 60 is
30.7%, the prognosis of survivors was not uniformly poor.
Conclusion: This study demonstrates that though a high percentage of subjects
recovered independent walking after stroke in the Soweto community with minimal
rehabilitation, their walking speed and distance walked are indicative of limited walking
capacity. Walking speed and the two minute walking test could be used to predict
functional walk test performance. Pain in the paretic lower limb though commonly
reported did not appear to affect walking speed or distance walked. The use of the
Barthel Index to predict activities and/or survival merits further investigation.

Woman on top: interpreting Barthel Beham’s Judith Seated on the Body of HolofernesGrimmett, Kendra Jo 11 September 2014 (has links)
At no point in the apocryphal text does Judith, a wise and beautiful Jewish widow, sit on Holofernes, the Assyrian general laying siege to her city. Yet, in 1525, Barthel Beham, a young artist from Nuremberg, created Judith Seated on the Body of Holofernes, an engraving in which a voluptuous nude Judith sits atop Holofernes’s nude torso. Neither the textual nor the visual traditions explain Beham’s choice to perch the chaste woman on top of her slain enemy, so what sources inspired the printmaker? What is the meaning of Judith’s provocative position?
The tiny printed image depicts the relationship between a male figure and a female figure. Thus, in order to appreciate the complexity of that relationship, I begin this thesis by reviewing what it meant to be a man and what it meant to be a woman in early sixteenth-century Germany. Because gender roles and the dynamics between the sexes were so complex, I encourage scholars to reevaluate Weibermacht (Power of Women) imagery.
The nudity of Beham’s Judith and her intimate proximity to Holofernes suggest that Judith Seated on the Body of Holofernes is a Weibermacht print. In fact, Judith’s pose specifically echoes that of Phyllis riding Aristotle, a popular Weibermacht narrative. The combined eroticism of Judith’s exposed body and her compromising position would have appealed broadly to male viewers, but Beham likely targeted an erudite audience of well-educated, affluent men when he designed the multivalent print.
Through close visual analysis and careful consideration of which prints circulated in early sixteenth-century Nuremberg, I argue that Beham’s Judith resembles witches riding backwards on goats, crouching Venuses, and a woman in the “reverse-cowgirl” sex position. Admittedly, it is impossible to know which sources Beham studied in preparation for Judith Seated on the Body of Holofernes, but I am inclined to believe that he wanted each of those jocular references to enrich the meaning of his work, providing a witty commentary on the power of women. But regardless of the artist’s intentionality, I think visually literate viewers would have recognized and enjoyed decoding the layers of meaning in Beham’s odd engraving. / text

The practical part of the study consisted of a questionnaire, which was using the questionnaire survey gathered data needed to evaluace the clinical utility, in the period from May 2013 to July 2013. After obtaining the necessary data from both theoretical and practical part of this data was evaluated and analyzed according to the standard metodology for the evaluation of functional measures for spinal patiens (Chapman, J.R. et al, 2007) and the results of the individual measures were mutually compared. O nosso objetivo foi de avaliar evolutivamente a perda da força muscular e das habilidades motoras, bem como a progressão da distribuição da fraqueza muscular nos diferentes segmentos dos membros superiores em pacientes com distrofia muscular de Duchenne em corticoterapia. Selecionamos seguintes testes de fácil aplicação: Teste de força Manual Muscular Escala Medical Research Council; Teste ABC provas de coordenação visual-motora e fatigabilidade provas n° 1, 3, 7 e 8; Grau funcional de Brooke; Índice de Barthel. Os testes foram aplicados em 40 pacientes com idades entre 5 e 15 anos, deambulantes e não deambulantes, os quais foram avaliados três vezes, com intervalos de seis meses entre cada avaliação. Os resultados mostraram que a progressão da distribuição da força muscular nos membros superiores ocorreu dos segmentos proximais para os distais em todos os pacientes e foi maior nos pacientes não deambulantes e com maior idade. O ato de escrever (pegada no lápis e/ou caneta) não foi influenciado pela progressão; no entanto, o aumento da fatigabilidade foi um fator limitante contribuindo para a redução do ritmo e da qualidade da escrita. O grau funcional de Brooke confirmou a variação das medidas de força muscular e nas atividades de vida diária o nível de dependência foi maior nos pacientes com maior idade, acontecendo nestes compensações funcionais importantes que lhes permitiram realizar a atividade de forma adaptada. / Our aim was to analyze the progression of the involvement of muscle strength and functional motor hability as well as the progression of the weakness pattern in the upper limbs of children with Duchenne muscular dystrophy who were receiving steroid therapy. We evaluated 40 patients with DMD, aged 5-15 years, by using the following simple tests: MRC score based on the upper limbs muscles; ABC tests number 1, 3, 7 and 8 for assessing visual motor coordination and fatigability; Brooke functional ability scale and modified Barthel index. All boys were evaluated every 6 months along a period of 18 months. The loss of muscular strength showed a proximal to distal progression and was greater in non- ambulant and in older patients. The act of writing (to hold pencil or pen) was not influenced by the progression; however, the fatigability increased and was a limiting factor for the speed and the quality of the hand writing. The Brooke functional ability scale confirmed the changes in the muscular strength. The performance in daily activities showed a greater dependence in the older patients who adopted functional compensations for performing the activities in an adapted way.
