A novel approach to aesthetically treat arrested caries lesions

Indiana University-Purdue University Indianapolis (IUPUI) / In this thesis, we proposed and investigated the efficacy and safety of dental bleaching as a non-invasive aesthetic treatment option for stained arrested caries lesions (s-ACLs). Chapter 1 reports the suitability of this approach in extracted human teeth, as well as in a selected clinical case. Visual improvement in the color lighteness of the s-ACLs was observed and reported in photographs. In order to systematically study the impact of dental bleaching on the s-ACLs, we developed in vitro models simulating the development of metallic and non-metallic s-ACLs (Chapter 2). Human dental specimens were submitted to incipient caries-like lesion formation, followed by a 5-day cycling protocol based on remineralization and staining episodes. The created lesions were then bleached (simulating in-office/40% hydrogen peroxide). Color change was measured spectrophotometrically at baseline, after lesion creation, staining/remineralization cycling and bleaching; while mineral loss and lesion depth were quantified by transversal microradiography after staining/remineralization cycling. Metallic s-ACLs were darker, more remineralized and more difficult to bleach, compared to the non-metallic ones (p<0.05). In Chapter 3, we tested the efficacy and safety of different dental bleaching systems (simulating at-home/15% carbamide peroxide and in-office/40% hydrogen peroxide) using the in vitro models previously developed. Similar methods and outcomes were used, with the addition of demineralization after bleaching to simulate and test changes in caries susceptibility. At-home bleaching showed greater efficacy in color improvement compared to in-office (p<0.05), but also increased susceptibility to further demineralization (p<0.05), regardless of the type of stain. Overall, bleached non-metallic s-ACLs were more susceptible to demineralization compared to metallic ones (p<0.05). Within the limited laboratory testing conditions, we concluded that dental bleaching can improve the aesthetics of s-ACLs, with efficacy being dependent on the nature of the stain. At-home bleaching presented greater efficacy, but also raised some potential safety concerns, which should be further investigated in clinical conditions. In-office bleaching protocol showed to be an effective and safe procedure for the aesthetic treatment of stained arrested caries lesions.

The effects of variable ultraviolet light on bone weathering in a New England setting

Understanding the natural processes that have taphonomic effects on bone is an important part of accurately determining the postmortem interval (PMI). Ultraviolet radiation is one of these various natural processes that weather bone. The present study quantifies the degree to which differential exposure to sunlight affects bone bleaching and weathering. In this study, 140 Sus scrofa long bones were placed in two different microenvironments (grassland and woodland) within a New England setting. Upon the completion of a one-year observation period, 100% of the bones had reached bleaching level 4, and 9.85% (n = 13) of the bones had reached weathering stage 1, with the majority (n = 11) being from the woodland sample. The results indicated that the microhabitat that each sub-sample was deposited within played a statistically significant role in the degree and rate of bleaching and weathering on the bone, with chi-square tests all indicating a p value of < 0.001. In addition, results indicated that environmental variables that led to the more rapid decomposition of soft tissue, such as temperature, humidity, and the type of plant coverage, may play a greater role in the level of bleaching and weathering achieved by the bones, rather than simply the degree of exposure to UV radiation and light intensity.

This thesis is presented in two parts. In part one, the excess molar volumes and the excess molar enthalpies have been determined for several binary systems at 298.15 K using an LKB flow microcalorimeter and/or a 2277 Thermal Activity Monitor and an Anton Paar Digital Densitometer. The excess molar volumes and excess molar enthalpies, V; and H; , have been determined for systems involving an alkanol (I-propanol, 2-propanol) mixed with a hydrocarbon (1hexene, I-heptene, l-octene, I-hexyne, I-heptyne, l-octyne). The results show trends relating to the degree of unsaturation of the hydrocarbon to the position of the hydroxyl group on the alkanol. The excess molar volumes and excess molar enthalpies, V; and H; of di-n-butylamine + diethyl ether or dipropyl ether or di-l-methylethyl ether or dibutyl ether or 1,1 dimethylethyl methyl ether or 1,1 dimethylpropyl methyl ether or tetrahydrofuran or tetrahydropyran or 1,4 dioxane have been measured over the whole composition range at the temperature 298.15 K in order to investigate di-n-butylamine - ether interactions. The V; values for each of the systems studied are negative with the exception of the mixtures of (di-n-butylamine + dibutyl ether or tetrahydrofuran or tetrahydropyran or 1,4 dioxane. The H; results over the whole mole fraction range are formed endothermically. Measurements were also made on mixtures involving (a cycloalkane + a pseudocycloalkane). The congruency theory was tested for the (cycloalkane + pseudo-cycloalkane) mixture. The cycloalkane mixtures studied here did not satisfy the null test of the congruency principle. In the second part of this thesis the main aim of the investigation was to apply ambient temperature hydrogen peroxide bleaching techniques to a novel non-woven fabric and to optimize the treatment conditions for this technique. Five cold-pad batch bleaching formulas were applied to the non-woven and the sample fabrics were analyzed for the following properties a) fluidity (measure of degree of degradation of the cotton fibre as a result of the bleaching process) b) wettability (absorbency) c) whiteness (using instrumental techniques) d) inherent fibre surface properties (SEM) A method was elucidated for the cold batch bleaching of the non-woven which produced a fabric with minimum fibre damage, an acceptable degree of whiteness and excellent absorbency properties. The treatment parameters of time (Xl), temperature(x2) and hydrogen peroxide concentration (x3) for this method were optimised using a multiple regression analysis for three variables and response surface plots. / Thesis (M.Sc.)-University of Natal, Durban, 1993.

Submicron Calcium Phosphate Spheres for Biomedical Applications : Synthesis and Use

Calcium phosphate spheres as biomaterials have been attracting attention in recent years. Calcium phosphate occurs naturally in bone, and a hollow structure could be advantageous for drug loading and release. The combination of a calcium phosphate chemistry and a spherical-hollow structure could be an optimal strategy for specific biomaterial applications, e.g., certain dental and drug-delivery applications. The focus of this thesis is on the synthesis, formation mechanism and applications of hollow, spherical calcium phosphate particles. First, the thesis describes two methods for the synthesis of calcium phosphate (CaP) spherical particles. The first method involves synthesis of hollow calcium phosphate spherical particles via a supersaturated buffer solution based on a previous study. It was utilised to prepare spheres for applications in drug delivery and dentistry. The second method was developed to explain the mechanism of formation of hollow calcium phosphate spheres. It aimed at revealing the particular function of magnesium in the formation of spherical particles. With the use of this modified method, it could be concluded that the only ions active in the formation of CaP spherical particles are calcium ions, phosphate ions and magnesium ions. Compared with the thermodynamics of micellisation, a new model, called three ions virtual micelle effect, was developed to explain the mechanism of the Mg function. Following this mechanism, a series of spherical particles of other compositions were explored. These spherical particles included strontium phosphate, barium phosphate, calcium fluoride, strontium fluoride and barium fluoride. In this thesis, CaP spheres were studied for the controlled delivery of active ingredients and as active agent for tooth remineralisation. The first investigated application was to control the release of vancomycin from Poly(methyl methacrylate) (PMMA) cement via strontium-doped CaP spheres (SCPS). The results showed that incorporation of CaP spheres into PMMA could enhance antibiotic release while maintaining the mechanical strength. The second application was to control hydrogen peroxide (HP) release from two bleaching gel, in which CP-loaded CaP spheres were the active ingredient. One gel with low HP concentration was developed as an at-home bleaching gel, and one with high HP concentration was developed as an in-office bleaching gel. The results showed that CaP spheres would give a controlled release of peroxide and thus have a potential to increase the efficacy of the bleaching. The third application was to investigate the potential for an anti-sensitivity effect of the spheres, as active agents in toothpaste. We studied the tooth tubules occlusion and the remineralisation effect of CaP spheres. After 7 days of application, the open dentin tubules and surface were fully covered by a newly formed apatite layer, demonstrating the remineralisation potential of the spheres.

Purification, application and immunolocalization of thermostable xylanases

Microorganisms producing thermostable -xylanase and their associated hemicellulases have significant application in the paper and pulp, food, animal feed, and textile industries. The potential of partially purified xylanase from Thermomyces lanuginosus MC 134, Luminase PB 100, Luminase PB 200 (a commercial xylanase) and T. lanuginosus DSM 5826 (Sigma Aldrich) was evaluated in bleaching of bagasse pulp. The temperature and pH optima for all the enzymes were 60°C and pH 6, respectively. The temperature (50- 80°C) and pH (5-8) stability of the enzymes were also assessed. All the enzymes were relatively stable at 60°C and pH 6 for 180 min. T. lanuginosus MC 134 retained 80% of its activity at 60°C and pH 6 for 180 min and PB 200 retained 75% of its activity at 80°C for 180 min. T. lanuginosus MC 134 also exhibited good alkaline stability at pH 8. The commercial xylanases Luminase PB 100, Luminase PB 200, T. lanuginosus DSM 5826 (Sigma Aldrich) were purified to homogeneity using a gel filtration column packed with sephadex G-100 and characterized for Km and Vmax. However extracellular crude xylanases from T. lanuginosus MC 134 was purified to homogeneity using (N )2S04 precipitation and gel filtration column, packed with sephadex G-100. The purified xylanases exhibited a molecular mass of- 26 to 24 kDa, given range as determined by SDS page. The Km and Vmax values of Luminase PB 100, Luminase PB 200, T. lanuginosus MC 134, and T. lanuginosus DSM 5826, xylanases were determined by the Michaelis-Menten equation using birchwood xylan as the substrate. The Km value for Luminase PB 100, Luminase PB 200, T. lanuginosus DSM 5826 and T. lanuginosus MC 134 were, 8.1 mg/mL, 11.7 mg/mL and 14.3 mg/mL respectively. The Vmax for Luminase PB 100, Luminase PB 200, T lanuginosus DSM 5826 and T lanuginosus MC 134 were 232.6, 454.6 and 74.6 !Jl11ol/min/mg. Biobleaching conditions of the xylanases were also optimised and the release of reducing sugars and lignin derived compounds showed that an enzyme dosage of 50U/g of pulp was ideal for biobleaching at pH 6 and 60°C for 180 min. This brightness for T lanuginosus MC 134, Luminase PB 200, Luminase PB 100 was 45.5 ± 0.11%, 44.1 ± 0.007% and 42.7 ± 0.03% respectively at pH 6, compared to untreated samples. Reducing sugars and UV-absorbing lignin-derived compound values were considerably higher in xylanase-treated samples. All the enzymes analysed exhibited similar trends in the release of lignin derived compounds and reducing sugars which indicated their potential in the pulp and paper industry. / PDF Full-text unavailable. Monitoramento do processo de desmineralização e remineralização do esmalte dental humano durante e após o clareamento dental / Monitoring de-remineralizaton process on human enamel during and after dental bleaching

Este estudo in situ tem como proposta geral monitorar os processos de desmineralização e remineralização do esmalte dental humano durante e após o clareamento dental e como objetivos específicos: avaliar se o processo de desmineralização e remineralização do esmalte dental é influenciado pela utilização de agentes clareadores com diferentes composições e estimar o período necessário para que o esmalte dental clareado atinja os níveis de mineralização evidenciados anteriormente ao início do tratamento clareador. Dez voluntários participaram deste estudo utilizando dispositivos intra-orais contendo quatro fragmentos de esmalte dental humano cada que foram submetidos aos seguintes tratamentos: G1 - peróxido de hidrogênio 35% (Pola Office, SDI); G2 - peróxido de hidrogênio 7,5% (Pola Day, SDI); G3 - peróxido de hidrogênio 7,5% (Day White ACP, Discus Dental); G4 - ácido fosfórico 35% (Condicionador de ácido fosfórico 3M ESPE Scotchbond 3M ESPE). O período do estudo compreendeu 21 dias entre tratamento e monitoramento que foi realizado pelo método de fluorescência do tecido dental com o auxílio do equipamento QLF System. A análise estatística realizada pelo teste estatístico de ANOVA 2 fatores para mensurações repetidas e o teste de Tukey revelou haver diferenças estatísticas entre os tratamentos realizados e entre os tempos de mensuração. Após a realização do monitoramento do conteúdo mineral do esmalte dental durante e posteriormente ao término do tratamento clareador pode-se constatar que os agentes clareadores que apresentam composições químicas distintas ocasionaram diferentes níveis de desmineralização no esmalte dental humano, sendo que o composto ACP presente em um dos géis clareadores utilizados foi capaz de reduzir o processo de desmineralização durante o período de tratamento clareador. O período necessário para que o esmalte dental clareado atingisse os níveis de mineralização iniciais variaram em função do clareador utilizado, porém apenas o agente clareador que contém o composto ACP foi capaz de remineralizar completamente após 21 dias. / The overall proposal of this in situ study is to monitor the demineralization and remineralization process of human enamel during and after dental bleaching and its specific objectives are: to assess whether the process of demineralization and remineralization of enamel is influenced by the use of bleaching agents with different composition and estimate the time required to bleached enamel reaches the levels of mineralization observed prior to bleaching treatment. Ten volunteers participated of this study wearing intraoral devices containing four fragments of human enamel submitted to the following treatments: G1 - 35% hydrogen peroxide (Pola Office, SDI), G2 - 7.5% hydrogen peroxide (Pola Day, SDI); G3 - 7.5% hydrogen peroxide (Day White ACP, Discus Dental); G4 - 35% phosphoric acid (Scotchbond 3M ESPE, 3M ESPE). The study period comprised 21 days of treatment and monitoring. Monitoring was conducted by the quantitative light fluorescence method, with QLF System. Statistical analysis performed by the statistical two-way ANOVA for repeated measurements and Tukey tests demonstrated differences between treatments and between measurement times. Monitoring the enamel mineral content during and after the end of the bleaching treatment revealed that the bleaching agents with different chemical compositions resulted in different levels of human enamel demineralization and that, the ACP compound present in one of the bleaching agents used was able to reduce the demineralization process during the bleaching treatment. Time required for the bleaching human enamel reaches the initial levels of mineralization varied depending on the bleaching agent used, but only the bleaching agent containing ACP was able to completely remineralize enamel after 21 days.

Desproteinização do esmalte associada à técnica de remineralização no clareamento em consultório / Enamel deproteinization associated to remineralization technique on in-office bleaching

Objetivo: avaliar cor, brilho, rugosidade e alterações ultraestruturais do esmalte dental clareado com peróxido de hidrogênio a 35 %, submetido ao tratamento prévio com agente desproteinizante, ou ao tratamento posterior com o agente remineralizador fosforopeptídeo de caseína/fosfato de cálcio amorfo (ACP-CPP). Material e Métodos: Os grupos experimentais foram: GC (controle/consultório): H2O2 a 35 % - 4 sessões de 8 min; GE1 (primer+consultório): NaOCl 5,25 % por 1 min, aplicação do H2O2 a 35 % como no GC; e GE2 (consultório+ACP): GC + ACP-CPP diariamente por 7 dias. Fragmentos contendo esmalte e dentina (n=8), obtidos de dentes bovinos, foram utilizados para avaliar cor, brilho e a rugosidade. Alteração de cor (E), parâmetros L* e b* foram determinados com colorímetro e o brilho superficial com glossímetro antes, imediatamente após (1h), 4 e 7 dias após o tratamento. Parâmetros de rugosidades, Ra, Rt e RSm, foram obtidos com perfilômetro de contato antes, imediatamente após o tratamento e 7 dias após os tratamentos. Os resultados de E, brilho superficial e rugosidade foram avaliados separadamente usando ANOVA 2 fatores e teste de Tukey (p=0,05). Para avaliar a alteração ultraestrutural, dentes pré-molares humanos, seccionados nos sentidos vestibulo-lingual e mesio distal foram observados em microscópio eletrônico de varredura, por emissão de campo, e realizada a quantificação de elementos químicos por EDS. Análise tridimensional da estrutura do esmalte foi realizada por microtomografia computadorizada (micro-CT) com resolução 11,24 m (n=8). Foram realizadas análises dos parâmetros estruturais: espessura estrutural (St.Th.), separação estrutural (St.Sp.) e índice de fragmentação (Fr.I.) antes e após os tratamentos em duas regiões: ROI 1= 56,2 m e ROI 2= 110,2 m, ambas a partir da superfície vestibular. Foi utilizado o teste t pareado para análise estatística de cada parâmetro estrutural. Resultados: Não houve diferença estatística entre os diferentes tratamentos de superfície para E, Ra e RSm. Imediatamente após o clareamento (1h) ocorreu maior aumento do L* e queda do brilho superficial que se manteve por 7 dias. O uso de agente desproteinizante em dentes bovinos não acentuou a redução do brilho do esmalte, mas a aplicação de ACP-CPP acarretou em maior perda brilho e aumento nos valores de rugosidade após 7 dias para Rt. A aplicação do agente desproteinizante previamente ao clareamento em dente humano revelou uma superfície mais lisa, sem alterar os parâmetros estruturais. Há uma maior quantidade de cálcio, formação de um manto de recobrimento após aplicação de ACP-CPP em torno dos prismas de esmalte, aumento de St.Th de 4,1m, menor espaçamento entre os cristais de hidroxiapatita e redução de St.Sp em 0,8 m e de Fr.I em 0,01 no ROI-1 após 7 dias. Conclusão: O uso de agente desproteinizante não altera a cor, brilho e a ultraestrutura inorgânica. A aplicação de ACP-CPP após a técnica de clareamento de consultório não contribui para alteração de cor, mas reduz o brilho e altera a ultraestrutura da porção mais externa do esmalte após 7 dias / Purpose: To evaluate color, gloss, roughness and ultrastructural changes of enamel bleached with 35% hydrogen peroxide, subjected to previous treatment with deproteinized agent, or later treatment with remineralizing agent casein phosphopeptide-amorphous calcium phosphate (CPP-ACP). Materials and Methods: The experimental groups were: GC (control + in-office): 35% H2O2 - 4 sessions of 8 min; GE1(primer+in-office): 5.25% NaOCl during 1 min before the application of 35% H2O2 as done in GC, and GE2 (in-office+ACP-CPP): GC + ACP-CPP, daily applied during 7 days. Enamel and dentin blocks (n=8), obtained from bovine tooth, were used to evaluate color, gloss and roughness. Color changes (E), L* and b* parameters were done with a colorimeter and surface gloss with a glossimeter, before, immediately after (1h), 4 and 7 days after treatment. Roughness parameters, Ra, RT and Rsm, were done with a contact perfilometer before, immediately after and 7 days after treatments. ANOVA two-way and Tukeys test were performed to evaluate E, gloss and roughness separately (p=0.05). To access human pre-molar ultrastructural changes, teeth were cross-sectioned buccal-lingual and disto-mesio observed by scanning electron microscope, field emission gun, EDS to quantify chemical elements. Enamel three-dimensional images were analysed with microcomputed tomography (micro-CT) with resolution 11,24m (n=8). Structural parameters were analyzed: structural thickness (St.Th.), structural separation (St.Sp.) and fragmentation index (Fr.I.) before and after treatments in two regions of interest:ROI 1= 56,2m and ROI 2= 110,2 m, both from buccal surface. Paired t-test was done for analyses of each structural parameter. Results: There was no statistical difference among surface treatments to E, Ra and Rsm. Immediately after bleaching (1h) occured highest L* increase and decrease of surface gloss which remained until 7 days. Deproteinized agent applied on bovine tooth not emphasized enamel gloss reduction, but the CPP-ACP has resulted in a higher gloss reduction and roughness increase (Rt parameter) after 7 days. Deproteinized agent application previous to in- office bleaching observed a smooth surface, without structural parameters changes. There is a greater calcium quantity, forming a cover mantle after CPP-ACP application around enamel prisms, St.Th increase of 4,1m, less spacing between hydroxyapatite crystals and reductions of St.Sp of 0,8 m and Fr.I of 0,01 on ROI-1 after 7 days. Conclusion: Application of deproteinized agent does not change bovine enamel color, gloss and human enamel inorganic ultrastructure. CPP-ACP application after in-office bleaching does not contribute to color change, but decrease gloss of bovine enamel and change human enamel outermost ultrastructure portion after 7 days.

Avaliação in vitro do papel da catalase em diversas concentrações na adesividade do material restaurador à dentina clareada / In vitro evaluation of the role of catalase in differents concentrations on resin bond strength to bleached dentin

As alterações cromáticas dos dentes têm etiologia diversa e podem surgir da atuação de fatores intrínsecos e extrínsecos. As alterações decorrentes de fatores intrínsecos, por sua vez, podem ser classificadas como de natureza congênita ou adquiridas. Estas últimas podem manifestar-se antes ou após a erupção do dente e incluem as alterações cromáticas dos dentes portadores de polpa morta e dos dentes endodonticamente tratados. Procedimentos inadequados durante o tratamento endodôntico, como deixar permanecer restos necróticos no interior da câmara coronária após a cirurgia de acesso ou o emprego de substâncias medicamentosas na forma de medicação intracanal ou de cimento obturador, podem promover a pigmentação da coroa. Em 1850, Dwinelle propôs o emprego de uma bolinha de algodão impregnada de peróxido de hidrogênio a 30% que deveria permanecer no interior da câmara coronária entre duas sessões clínicas. Os primeiros efeitos adversos do tratamento clareador foram descritos por Harrington & Natkin em 1979 ao relatarem 4 casos clínicos de reabsorções radiculares cervicais externas. Entre outros efeitos adversos têm-se relatado a diminuição da adesividade do material restaurador à dentina clareada por falhas de ordem tanto adesiva como coesiva. Segundo os pesquisadores tal efeito estaria relacionado às alterações microestruturais e à presença de oxigênio residual na dentina que interfeririam com a polimerização do material restaurador. Em 1993 Rotstein utilizou in vitro a enzima Catalase para impedir sua difusão de oxigênio para a superfície radicular externa. No atual experimento avaliou-se o papel da Catalase na neutralização de peróxido de hidrogênio residual com vistas a devolver ao dente suas características normais de adesividade. Os resultados mostraram haver diferenças estatisticamente significantes entre as soluções de 10mg e 250mg e entre as soluções de 50mg e 250mg, quanto à adesividade do material restaurador, de tal forma que a solução de 250mg promoveu maiores valores de resistência adesiva tanto em relação a de 10mg como em relação a de 50mg. Não verificou-se diferenças estatisticamente significantes entre os grupos experimentais e seus respectivos grupos controle. Dadas as condições experimentais concluiu-se que a Catalase na concentração de 250mg restaurou a adesividade da estrutura dentinária clareada. / The teeth color changes have diverse etiology and they can come out as consequence of intrinsic as extrinsic factors. Intrinsic factors can be congenital or acquired and these last can disclose so before as after tooth eruption. Crown color changes from acquired factors are related to nonvital as to endodontic treated teeth. This way pulp degradation byprocutcs can infiltrate the dentin tubules and change the crown color in nonvital teeht. Root canal therapy iatrogenic factors regarding crown staining comprise the presence of pulp remmanents in pulp chamber, intracanal medicaments and endodontic cements or sealsers. Even in 1850 Dwinelle have suggested that teeth could be bleached with Hydrogen Peroxide and in 1979 Natking & Harrington have reported cervical external root resorption as an adverse effect in bleaching endodontic treated teeth. Among the undesirerable effects of teeth bleaching researchers have been reporting such a reduction in the adhesive bond strength of the bleached dentin that would be probably related to tissue microstructural changes and the presence of residual Oxigen inside the structures. In 1993 Rotstein et al. have employed the enzyme Catalase for inactvating this residual Oxygen to not allow it to diffuse to the external root surface. The current in vitro experiment have evaluated the role of Catalase in the neutralization of Hydrogen Peroxide looking for reestablishent of the normal dentin bond adhesion characteristics. Seccionated forty teeth provide eighty specimens whose were bleached with a paste made of 30% Hydrogen Peroxide and Sodium Perborate. Subsequentely all specimens were immersed in four differents Catalase solutions during 3min. Kruskal- Wallis tests revealed statistically signifcant reduction in bond strengths for the specimens treated with Catalase 250mg.
