Patienters erfarenheter av att ha genomgått en kranskärlsoperation

Bakgrund: En kranskärlsoperation är ett behandlingsalternativ till dem som har en kranskärlssjukdom. Den utförs främst vid komplicerade fall och innebär att med nya kärl skapa en förbindelse förbi det förkalkade kranskärlet runt hjärtat. Operationen är stor och påfrestande, och innebär flera förändringar i livet.   Syfte: Syftet med denna litteraturstudie är att belysa patienters erfarenheter av att ha genomgått en kranskärlsoperation.   Metod: En litteraturstudie utfördes genom att granska 10 vetenskapliga kvalitativa studier som stödjer författarnas syfte. Studierna analyserades och sammanställdes till ett resultat.   Resultat: Operationen påverkade deltagarna och deras återhämtning på olika sätt. Besvärliga kroppsliga symtom kunde visa sig under återhämtningen. Många tankar och funderingar väcktes som skapade oro och rädsla. Stöd och information hade en betydande roll och operationen krävde livsstilsförändringar som kunde vara svåra att anpassa sig till.   Konklusion: Brist på information och avsaknad av länk till sjukhuset efter utskrivning, ledde till rädsla och oro vilket skapade en otrygghet. Ytterligare forskning om stöd och information behövs för att kunna hjälpa och tillfredsställa patienters behov.

Venoarterial modified ultrafiltration versus conventional arteriovenous modified ultrafiltration during cardiopulmonary bypass surgery

Submitted in fulfilment of the Degree of Doctor of Technology: Clinical Technology, Durban University of Technology, 2009. / INTRODUCTION: The role of modified ultrafiltration (MUF) in removing inflammatory mediators, reducing the need for homologous donor blood and decreasing pulmonary vascular resistance after cardiopulmonary bypass (CPB) has already been established. Different types of MUF systems evaluated illustrated that none of the MUF techniques adhered to the normal venous to arterial blood flow dynamics. OBJECTIVES: This experimental study compared a conventional arteriovenous modified ultrafiltration (AVMUF) system to a custom designed venoarterial modified ultrafiltration (VAMUF) system. This technique of VAMUF was designed to mimic the pro-grade flow pattern of the body and cardiopulmonary bypass circuit as compared to the conventional retrograde AVMUF systems. METHODS: Sixty patients that underwent MUF were divided into two groups, the AVMUF (n = 30) and the VAMUF (n=30) groups. Modified ultrafiltration was performed for a mean time of 12 minutes in both groups. In AVMUF blood was removed from the aorta, haemoconcentrated and infused into the right atrium (RA). In VAMUF blood flow was from the RA through a haemoconcentrator and re-infused into the aorta. RESULTS: There was no significant difference in any of the demographic variables, CPB or crossclamping time. Results showed significant difference in the ventilation times, with the VAMUF requiring a shorter ventilation time than the AVMUF group. Intensive care unit (ICU) stay, Hospital stay and discharge days were all significantly lower in the VAMUF group as well. The VAMUF also showed a lower percentage fluid balance than the AVMUF. The systolic and mean blood pressure was significantly higher after VAMUF with a decrease in heart rate, and central venous pressure (CVP). The VAMUF group showed a significantly greater decrease of Creatinine, serum lactacte and uric acid over time with no significant differences in oximetry. CONCLUSION: Results prove that VAMUF is more effective compared to the conventional AVMUF regarding the haemodynamics and clinical parameters of the patient and is more physiological with regards to blood flow dynamics. The VAMUF is, therefore, a more physiological technique than AVMUF.

Introduktion: Coronar arteriell bypass-operation (CABG) är en stor operation som ofta innebär större livsstilsförändringar för patienten. Sjuksköterskan behöver vara medveten om de delar i livskvaliteten som påverkas efter CABG för att kunna förebygga eventuella försämringar i livskvaliteten. Syfte: Syftet med litteraturstudien var att belysa livskvaliteten hos patienter som genomgått coronar arteriell bypass-operation. Metod: Litteraturstudien genomfördes enligt Polit & Becks niostegsmodell. Litteratursökningar genomfördes i databaserna CINAHL och PubMed. Artiklarna genomgick en urvalsprocess och kvalitetsgranskning. Vid slutförd granskning återstod 10 artiklar där två huvudteman växte fram. Resultat: Resultatet redovisades i två huvudteman med tillhörande underteman. Det första huvudtemat var Patientens funktionalitet med undertemana Förekomst eller frånvaro av kroppsliga symtom och Känslor, ångest och depression. Det andra huvudtemat var Patientens sociala tillvaro med undertemana Stöd och förtroende och Socioekonomisk status. Slutsats: Många patienter upplevde en förbättring i livskvaliteten. Patienternas livskvalitet innan operationen hade betydelse för hur livskvaliteten blev efter. Sjuksköterskan behöver fokusera på att patienten får tillräckligt med information och stöd innan och efter operationen för att patienten inte ska ha orealistiska förväntningar av hur livskvaliteten kommer att bli efter operationen.

Genetic regulation of the host response to cardiac surgery and cardiopulmonary bypass

There is significant variation between individual patients in the magnitude and pattern of their systemic response to cardiac surgery. Poor outcomes in these patients have been associated with a dysfunctional host response. This thesis seeks to define such variability at the level of gene expression by sequential analysis of transcription before and after surgery for a low risk group of patients undergoing elective cardiac surgery and cardiopulmonary bypass (CPB) patients using expression microarray profiling. To that aim, we analysed sequential global gene expression patterns in circulating peripheral blood leukocytes. We also investigated the role of DNA sequence variation in modulating the observed changes in gene expression. This approach allowed us to identify important genetic modulators and novel biological pathways and gain new insights into the mechanisms that regulate the host response to surgery.

EFFECTS OF YEAST-DERIVED MICROBIAL PROTEIN ON TRANSITION DAIRY COW HEALTH AND PERFORMANCE

The transition period for dairy cows is defined as the three weeks pre and postpartum. During the transition period, dairy cows experience a myriad of metabolic, managerial, and nutritional requirement changes. These changes lead to stress and increased susceptibility to diseases which can negatively affect lactational performance in the short and long term. However, dietary amino acid availability can have a dramatic impact on the health and performance of dairy cows around parturition. Thus, the objective of the thesis was to evaluate the effects of supplementing yeast-derived microbial protein, as an alternative protein source for dairy cows during the transition period. This was accomplished by using visual observations and precision dairy monitoring technologies to record disease, feeding behavior, and performance of dairy cows from 21 days prepartum to 150 days postpartum. Yeast-derived microbial protein was found to decrease dry matter intake but not negatively affect milk production or health of the animals. Yeast-derived microbial protein may be used as an alternative protein source for transition dairy cows as it did not negatively affect milk production or health of the animals.
