Panorama assistencial e epidemiológico do cancêr de mama em mulheres do norte de Tocantins - Brasil / Overview of female breast cancer care and epidemiological in North Tocantins-Brazil

Dada a complexidade epidemiológica do câncer de mama e a escassez de estudos sistemáticos sobre este assunto na região norte do Brasil, este trabalho consiste no primeiro levantamento epidemiológico e assistencial em saúde tomando como base a assistência ao câncer de mama no norte do Tocantins, entre 2010 e 2015, tendo como referência o Hospital Regional de Araguaína. Os objetivos foram: verificar a qualidade das informações cadastradas no SIS-RHC quanto à completude do preenchimento dos dados referentes às mulheres com câncer de mama; avaliar a variação temporal dos percentuais de casos em estádios precoce e tardio da doença no momento do diagnóstico; analisar o tempo decorrido para se conseguir a primeira consulta, o diagnóstico e o tratamento e, analisar a associação entre as variáveis sociodemográficas com o estadiamento do tumor no momento do diagnóstico (precoce e tardio). Foram excluídas as variáveis com grau de incompletude acima de 20%. Verificou-se que: a variável ocupação teve 35,1% de preenchimento incompleto e foi excluída da análise; escolaridade apresentou 10,6% de incompletude, alcoolismo 13,5% e tabagismo 12,8%; as demais variáveis apresentaram menos de 10% de incompletude; a maioria das pacientes se apresentou com doença avançada (51,1%); não houve diferença dos percentuais de pacientes com estádios precoces e tardios ao longo dos anos avaliados (p=0,757); a mediana do tempo para consulta foi de 158 dias e para tratamento foi de 182 dias; as mulheres com estadiamento precoce levaram mais tempo para iniciarem o tratamento comparativamente àquelas com estadiamento tardio (p=0,011); as mulheres de raça/cor preta (p=0,012), analfabeta (p=0,003) e de procedência do Pará (p=0,009) apresentaram maior porcentagem de estadiamento tardio no momento do diagnóstico. Assim, concluiu-se que as variáveis da base de dados são fontes de informação consideráveis; a maioria das mulheres foi diagnosticada com doença avançada e a evolução temporal da proporção de casos avançado e precoce não demonstrou mudanças variacionais ao longo dos anos; a qualidade do serviço ambulatorial do HRA para consultas em mastologia mostrou-se precária com tendência a priorizar o atendimento aos pacientes com doença avançada. Por fim, foi identificado associação de câncer de mama em estádio avançado nas mulheres de raça/cor preta, analfabetas e provenientes do estado do Pará. / Considering the epidemiological complexity of breast cancer and the few systematic studies on this subject in the North region of Brazil, this study is the first epidemiological and health care survey based on the assistance to breast cancer in the north of Tocantins, having as reference the Regional Hospital of Araguaína (RHA). The aims were: to verify the quality of the information of the variables registered in SIS-RHC regarding the completeness of the data on women with breast cancer; to evaluate the temporal variation of the percentages of cases in early and advanced stages of the disease at diagnosis; to evaluate the elapsed time to obtain the first consultation, the diagnosis and the treatment, and to analyze the association between sociodemographic variables with the tumor stage at diagnosis (early and advanced). This is an analytical study, with secondary data of breast cancer in women between 2000 and 2015 assisted at the RHA. Variables with a degree of incompleteness above 20% were excluded. It was verified that: the variable occupation was incomplete in 35.1% of the forms and was excluded from the analysis; schooling presented 10.6% of incompleteness, alcoholism 13.5% and smoking 12.8%; the other variables showed less than 10% of incompleteness; most of patients presented advanced disease (51.1%); there was no difference of the percentages of patients with early and late stages over the years assessed (p = 0.757); the median of time for consultation was 158 days and for treatment 182 days; women with early stage took more time to initiate treatment compared to those with advanced stage (p = 0.011); women of black race (p = 0.012) , illiterate (p = 0.003) and from the state of Pará (p = 0.009) had a higher percentage of advanced stage at the time of diagnosis. Thus, it was concluded that the database variables are considerable sources of information; most women were diagnosed with advanced disease and the temporal evolution of the proportion of advanced and early cases did not show variational changes over the years; the quality of the ambulatory service of the RHA for consultations on Mastology was shown to be precarious with tendency to prioritize care for patients with advanced disease. Finally, we identified an association of advanced stage breast cancer in women of black race, illiterate and that come from the state of Pará.

Image Segmentation on Lymph Node Images using Machine Learning to improve Colorectal Cancer Diagnosis

In cancer diagnosis there is a goal of having the treatment being tailored to each patient. This in order to increase efficiency and reduce side effects. Using more data on each patient can help in achieving this. One such data source is histological images on tissues, such as lymph nodes. This report sets out to find a method in which such images on lymph nodes can be automatically segmented. This so that they can later be analysed and maybe tell in what stage a cancer is in. Such work is today done by hand, and this makes it a subjective process, that might differ between doctors and institutions. If there was a method done by a computer, the process would be replicable and objective. Also, a lot of time would be saved. The results show that such a method is reachable in this early stage of development. It is also quite efficient when segmenting the lymph node itself. The segmentation of smaller areas of the lymph nodes is not as efficient, but with further work in the area it might improve enough to be useful. Some issues are still had since the method relies in part on a person to decide a parameter in order to get a clean segmentation. The final conclusion is that one model is to prefer compared to the others and that further work on this might make it a useful tool in analysing histological images.

The advantages of using endoscopic ultrasound in adult patients with early stage rectal cancer : a systematic review

Contexte: Le cancer colo-rectal est la deuxième cause de décès, par ordre de fréquence. L’utilisation de l’imagerie dans la stadification du cancer colo-rectal est un élément important de la prise en charge de la maladie. L’échographie endoscopique est une modalité qui permet de préciser la profondeur de l’atteinte néoplasique. Les données probantes concernant la performance diagnostique dans l’identification de cancers peu avancés sont variables. Objectif : Effectuer une revue systématique sur la performance diagnostique de l’échographie endoscopique dans l’identification de cancer de stade T1 et T2. Devis : Revue systématique. Sources bibliographiques : PubMed, EMBASE, Ovid and Cochrane library Méthodes: Dans un premier temps, une recherche de revue systématique publiée dans les 15 dernières années fût effectuée sur la précision diagnostique de l’échographie endoscopique dans les banques PubMed, Cochrne et trip database. Deux revues systématiques, publiées en 2008 et 2009 fûrent identifiées. Une deuxième recherche portant sur des études primaires a été effectuée pour la période 2009 à 2016, dans les mêmes banques bibliographiques. La qualité des études primaires a été évaluée à l’aide de la grille QUADAS2. Les mots clés utilisés étaient échographie endoscopique, EUS, cancer rectal, histo-pathologie, staging. Sélection d’études : Les critères d’inclusion : population adulte avec diagnostic de cancer du rectum pas avancé, articles complets publiés dans des revues avec comité de pairs, articles en anglais. Critères d’exclusion : population pédiatrique, cancers avancés avec atteinte métastatique, patients évalués avec d’autres modalités (CT ou IRM) sans échographie endoscopique, absence de confirmation histologique. Résultats : Dix articles, publiés depuis 2009, répondaient aux critères d’inclusion. Ces articles furent ajoutés aux articles retenus dans les revues systématiques déjà publiées. Au total,49 articles sont inclus dans cette revue systématique. La performance diagnostique de l’échographie endoscopique a été évaluée en calculant la sensitivité et la spécificité des études regroupées. Pour le stade T1, les valeurs de sensitivité et spécificité étaient 0.84 (CI 0.75-0.91) et 0.93 (CI 0.86–0.97), respectivement. Pour le stade T2 les valeurs de sensitivité et spécificité étaient 0.83 (CI 0.74–0.90) et 0.93 (CI 0.86–0.97), respectivement. Conclusion: L’échographie endoscopique présente une performance diagnostique pour l’identification de cancers de stade T1 et T2. Ceci permet d’orienter des patients vers des chirurgies moins invasives avec une survie égale et un taux de complications inférieures comparativement à des chirurgies plus invasives. / Background: Colorectal cancer (CRC) is the second leading cause of death. The use of preoperative imaging in the staging of (CRC) plays a major role in the management. Endorectal ultrasound (ERUS) is a precise imaging modality to determine the depth of penetration. The data on the precision of (ERUS) to predict early stage of rectal cancer has been variable Objectives: To conduct a systematic review, on the diagnostic performance of (ERUS) in the staging of T1 and T2 CRC. Design: Systematic review. Data sources: A literature search via PubMed, EMBASE, Ovid and Cochrane library. METHODS: An initial search for systematic review articles published in the last 15 years on the diagnostic accuracy of EUS in the staging of CRC using PubMed, Cochrane library, and trip database was conducted. After finding two systematic reviews that were published in 2008 and 2009, a second search of original studies published since the systematic reviews were conducted using the same databases from 2009 to 2016. The primary studies included in the systematic reviews and the primary studies published afterwards were included in the review. Methodological quality was applied using a modified version of the quality assessment of diagnostic accuracy studies (QUADAS2) tool. Terms used for search were endoscopic ultrasound, EUS, rectal cancer, histo-pathological finding, and staging. Study selection: Inclusion criteria includes adult people diagnosed with early stage CRC, all articles in english language and must be a full manuscripts published in peer-reviews journals. Exclusion criteria includes any recurrent or metastasis cancer and children with rectal cancer. Patients who were staged preoperatively by other imaging modality (MRI or CT) and no comparison with post operative pathology. Results: The search identified 420 articles, 97 articles were duplicate and excluded, and 232 refined articles were screened for title and abstract, reviewed. Thirty-two full text studies were assessed for eligibility, and ten published as full text and met the inclusion criteria; they were added to the articles identified in the earlier systematic reviews a total of 49 articles. Results of the evaluation of the accuracy of ERUS analyzed according to the diagnostic measures of sensitivities and specificities calculated for each study. The pooled sensitivity and specificity of EUS for stage T1 CRC was 0.84 (CI 0.75-0.91) and 0.93 (CI 0.86–0.97), and for T2 was 0.83 (CI 0.74–0.90) and 0.93(CI 0.86–0.97) respectively. Conclusion: The range of sensitivity and specificity values suggest that EUS performs well in accurately staging T1 and T2 cancers. Further advancement in this technology will lead to an improved diagnosis, clinical decision-making, and reduce the over staging drawback.
