Dvořáková Zdeňka: Alkaloids of Narcissus 'Dutch Master' (Amaryllidaceae) and their biological activity II. Diploma thesis 2016, Charles University in Prague, Faculty of Pharmacy in Hradec Králové, Department of Pharmaceutical Botany and Ecology. The content of this work was isolation of compounds from the selected fraction ND-6 obtained by column chromatography of Narcissus 'Dutch Master' alkaloid extract. Preparation of extract and its column chromatography was performed by Mrg. Daniela Hulcová as part of her doctoral studies. By the means preparative TLC was from fraction ND- 6 homolycorine type alkaloid (+)-O-methyllycorenine gained. Its structure was determined on the basis NMR, GC-MS analysis and optical rotation. The obtained data were compared with facts in known literature. By the isolated alkaloid was determined its cholinesterase inhibitory activity against acetylcholinesterase and butyrylcholinesterase. Its inhibitory activity was expressed as IC50 (M) and compared with known standards galanthamine, physostigmine, and Huperzine A. This alkaloid is inactive against cholinesterase (IC50 AChE > 1000 M, IC50 BChE > 1000 M). On the basis of gained results, we can evaluate this alkaloid from the point of view of cholinesterase inhibition as potentially unusable in the treatment of AD. Key...

From a selected fraction ND-6, which was obtained by column chromatography of an alkaloid extract of Narcissus 'Dutch Master' (preparation of the alkaloid extract and column chromatography was performed by Mgr. Daniela Hulcová within her doctoral thesis), lycorine alakloid O-acetylpluviin was isolated using preparative TLC. Its structure was determined on the basis of MS, NMR analysis, and optical rotation, the obtained data were compared with the literature. The isolated alkaloid was tested on its possibility to inhibit human acetylcholinesterase and butyrylcholinesterase. The activity was expressed as IC 50 values (IC50 AChE = 648.03 ± 53.95 μM, IC50 BChE = 602.50 ± 48.50 μM) and compared with IC50 values of galanthamine, huperzine A and physostigmine. O-acetylpluviine showed a very low inhibitory cholinesterase activity, and so, the alkaloid does not seem to be a suitable cholinesterase inhibitor for potential use in the treatment of Alzheimer's disease. Opponent: Title of thesis: Memory disorders assessment in patients with Alzheimer's disease Abstract: Objective: The main objective of the thesis was to investigate the impact of memory disorders on self-care in patients with Alzheimer's disease (AD). Another objective of the thesis was to determine whether age of patients with AD affects occupational performance in daily living activities. Methods: The sample was consist of 50 probands (11 men, 39 women), who were diagnosed AD by physician. For data collection was used a questionnaire Bristol Activities of Daily Living scale (BADLS) evaluating self-care and Contextual Memory Test (CMT) for evaluation of memory disorders. Testing the hypothesis was performed by correlation the corrected Spearman correlation coefficient and regression analysis. Results: The thesis results confirmed a significant correlation between BADLS scores and CMT scores - immediate recall (rSp = .51; p<0.05), delay recall (rSp = .42; p<0.05), total (rSp = .48; p<0.05). Furthermore, research results suggest that memory impairment occur more with PADLS. This diploma thesis is focused on providing speech therapy intervention for seniors in residential facilities. The aim of this special pedagogical work is to show the problems in the communication of seniors and a possible support in the home for the elderly. The thesis has two parts. The theoretical part is divided into three chapters. In this part, secondary communication disorders observed among adults, selected diseases related to old age, the definition of old age and residential services for seniors are described. In the empirical part, the research, case studies and the results are obtained. The research is qualitative. The main goal was to analyze providing speech therapy intervention in the home for the elderly. Partial goals were to find out whether there would be a progress in the senior communication based on speech therapy intervention and to find out whether the support of home workers in cooperation with an expert can help in this regard as well. Title of diploma thesis: Treatment of lower extremity peripheral artery disease The lower extremities ischemia is a disease caused most often by atherosclerosis during which the lumen in lower limb arteries becomes narrow. Its prevalence is increasing, especially in the developed countries. The disease can be asymptomatic and symptomatic. The symptoms are very unpleasant and decrease patient's quality of life. Advanced stages of the disease may be life threatening. Therefore, it is vital to timely and correctly diagnose the illness. During the therapy, it is possible to use pharmacological as well as non-pharmacological procedures, and, preferably, a combination of the two types of treatment. The pharmacotherapy can be divided into several parts. The prevention of atherosclerotic complications makes use of preventive measures and antiplatelet therapy to reduce the cardiovascular risk. Diploma thesis, Charles University, Faculty of Pharmacy in Hradec Králové, Department of Pharmaceutical Botany, Hradec Králové, 2019 The aim of this thesis was the isolation of alkaloids from selected fraction 3 (joined fractions (15 - 36), which was a sub-fraction of the fraction VM 323 - 327. It was obtained from the previous processing of an alkaloidal extract from the Vinca minor L at the Department of Pharmaceutical Botany as a part of elaboration of diploma thesis of Aneta Vítavcová.[78] The fraction VM 323-327 was separated by column chromatography on silica gel and a totally, of 7 subfractions were obtained. Subsequent repeated processing of the selected sub-fraction 3 (15 - 36) by preparative TLC on silica gel resulted in the isolation of (-)-vinoxine and its racemate (±)-vinoxine. Identification of their structure was determined based on MS, NMR and optical rotation. The inhibitory activity against acetylcholinesterase, butyrylcholiesterase and prolyl oligopeptidase were determined for the isolated substances. Key words: Argemone grandiflora Sweet, Papaveraceae, alkaloids, isolation, acetylcholinesterase, butyrylcholinesterase, prolyloligopeptidase, Alzheimerʼs disease, in vitro assay. Diethylether alkaloid extract obtained from stem and roots of Argemone grandiflora Sweet was chromatografically analyzed. Using common chromatografic methods, three alkaloids were isolated in clean form. These substances were identified as allocryptopine, (-)-munitagine and (-)-norargemonine by structural analysis (MS, NMR). These obtained alkaloids were tested for their inhibitory activity against human erythrocyte acetylcholinesterase (AChE) and human plasma butyrylcholinestrase (BuChE) by Ellman's method. This diploma work focuses on demanding and important task of nurses care of patients suffering from Alzheimer's disease. The problems of Alzheimer's disease represent socially serious and very important subject at present due to the fact that our generation is growing old, which will obviously result in increasing frequency of Alzheimer?s disease occurrence. This diploma work is divided in two parts: theoretical part and practical part. In the theoretical part there are causes, signs and symptoms, diagnosis and therapy of Alzheimer?s disease. Furthermore there is a conceptual framework of nursing care applied to the patients suffering from Alzheimer?s disease according to nursing model by Virginia Henderson. In addition to it burn out syndrome is mentioned here because due to the syndrome the nursing personnel is exposed to extreme psychic and physical load. The Alzheimer Programme Plan in the Czech Republic is presented in order to get better idea about activities in our country. In the practical part of this diploma work we laid down the following three objectives. The first objective was to ascertain exacting character of nursing work of the nurses taking care of the patients suffering from Alzheimer?s disease. The second objective was to ascertain what is considered by nurses to be the most problematic area within the range of nursing care of the aforementioned patients suffering from Alzheimer?s disease and the third objective of our research was to ascertain the contribution of specialised Alzheimer centres. The above-specified objectives have been fulfilled. In addition to it another research questions have been defined as follows: what is the level of exacting character of nursing work of the nurses taking care of the patients suffering from Alzheimer?s disease, what do the nurses see as the most problematic area within the range of nursing care of the aforementioned patients suffering from Alzheimer?s disease within the range of nursing care, what is the contribution of specialised Alzheimer centres and what nursing care should be provided in the specialised Alzheimer centres. The research questions have been answered. For the purpose of the development of this diploma work a qualitative examination has been used. Data acquisition and collection have been performed by applying the method of non-standard structured interview. The research work took place in Psychiatric Ward of the Hospital of České Budějovice a.s. (Joint stock company), in Alzheimer Centre named Loucký Mlýn near the town of Vodňany and in Alzheimer Centre in the town of Písek, and in Home of Old People in the village of Dobrá Voda. The answers of the interviewed nurses have been worked out in the form of sixteen case reports and subsequently category matrices (tables) containing the most important data were created from the aforementioned reports. The examination took place in the time period from March 2011 to June 2011. The results of our work could serve as a feedback for hospitals and sanatoria first of all for those where the examinations took place. Furthermore the results may be used for keeping public informed about the possibilities of helping patients suffering from Alzheimer?s disease e.g. in the form of information booklet.

The thesis deals with the lifestyle of patients with chronic intestinal disease. The theoretical part contains topics of theoretical knowledge essential to the orientation in the issue. It describes the principles of a healthy lifestyle, the issue of chronic intestinal disease, especially Crohn's disease and ulcerative colitis as a disease of clinical manifestations, from diagnosis to treatment. An important chapter of the theoretical part of the work are the impacts of IBD on lifestyle at chronically ill patients. The empirical part deals with the qualitative research. For datas collecting was chosen and used the method of questioning with narrative interview technique. Methodology of narrative interview was expanded to include specific methods of collection datas, the life curve method. The content of each interpretation is the final report that summarizes the observed facts associated with answers to research questions. Significant elements of the interviews are recorded in tables. The aim of the thesis was to find out in what areas and how the selected people with chronic intestinal diseases have changed their lifestyle. To provide information to the public about the limitations of patients with chronic intestinal diseases that directly affect their everyday lives. Finally to assist society tolerance to constraints of patients with chronic intestinal disease.

Problematika dětí s Crohnovou chorobou / Problems of Children Crohn's disease

Crohn's disease is a chronic inflammatory disease that affects the entire gastrointestinal tract in all its layers.The theoretical part deals with the characteristics of the disease, its causes, symptoms, complications, treatment and psychological aspects. It also touches on the needs of children and communication with a chronically ill child.At first, this work set out to achieve two goals: mapping the provision of nursing care to children with Crohn's disease in hospitals and determining the impact of Crohn's disease on the everyday life of a child. A third goal, gathering respondents' views on the possibilities of community nurses in collective institutions, was encountered during the research and was added.The empirical part was carried out using qualitative research method using semi-structured interviews both with health professionals working at pediatric wards, as well as children with Crohn's disease and their parents.Survey results show that children with Crohn's disease come in contact with health workers at various departments of health facilities. The most common early symptoms of Crohn's disease in children included abdominal pain, diarrhea with blood or mucus and failure to thrive. The most common areas affected were the gastrointestinal tract, the small intestine, the colon in one case and the rectum in another.Some workers did not know which tests or treatments their hospital prescribes to the children.They report pain as the main nursing challenge, but this did not reflect in nursing practice.In all departments, except one, there was no material on Crohn's disease, which would have been helpful to the workers.Interviews with children and parents, as well as with health workers confirmed that the disease has a socio-cultural impact on the child and those around it. The results of this diploma thesis were presented at three conferences and will be published.
