Male menopause and decision-making: a qualitative study.

The purpose of this study was to explore how a small group of white South African men going through menopause attached meaning to this major event in their lives, and also how it affected the decisions they took as leaders in the financial sector. In view of the fact that menopause is a natural process that, according to the existing literature, has a strong influence on men’s behaviour, the following research question was formulated: “Do behavioural changes due to menopause ultimately affect men’s social lives, and particularly their behaviour in the work environment where as leaders they take important decisions?” In collecting data I followed a field research approach where focused interviews were used involving asking questions, listening, expressing interest and recording what was said. Five participants were selected, including myself (I provided an autoethnographic narrative). Having opted for a particular application of the grounded theory approach (Strauss & Corbin, 1990) and in keeping within the requirements of modernist qualitative research, i.e., providing sufficient information to establish the study’s credibility, and at the same time giving the participants a voice, I conveyed the data, the findings of the study, and the inferences I made according to grounded theory’s three analytical steps namely, open coding, axial coding and selective coding. The findings indicated that menopause symptoms in particular (their physical, psychological and sexual dimensions) had a profound influence on the systemic male. The themes that I inferred from the data through continuous and systematic comparisons were related to decision-making in the work environment. This continuous comparative approach crystallised in the study’s emerging core theme, namely: “the decreased need for work power in the work environment due to psychological integration occurring during their menopause” as well as its substantive theory: “work power trade-offs result in decreased decision-making power during the male menopause phase”.

Métodos: Estudo populacional de corte transversal, com questionário anônimo auto-respondido por 378 mulheres brasileiras residentes em Belo Horizonte, Minas Gerais, entre 40 e 65 anos, com 11 anos ou mais de escolaridade, no período de maio a setembro de 2005. o instrumento de avaliação baseou-se no Short Personal Experiences Questionnaire (SPEQ). O escore de sexualidade foi calculado através da análise multivariada de sete componentes: presença de fantasias sexuais, grau de desejo, freqüência de atividade sexual, satisfação nas atividades sexuais, freqüência de excitação, orgasmo e a autoclassificação da vida sexual. Considerou-se escore de sexualidade abaixo da mediana como sexualidade ruim e maior ou igual à mediana como sexualidade boa. Para se investigar os fatores associados, os dados foram analisados usando-se a análise bivariada, com aplicação dos testes qui-quadrado e exato de Fisher. Por último, a análise por regressão múltipla de Poisson foi realizada. A razão de prevalência (RP) e o respectivo intervalo de confiança de 95% (IC 95%) foram calculados. Resultados: 276 questionários foram avaliados e, destes, 219 mulheres relataram ter parceiro sexual. A mediana do escore de sexualidade foi 9,0 (variação: 2,45-13,77). A análise de regressão múltipla mostrou que a prevalência de escore de sexualidade abaixo da mediana foi maior nas mulheres com mais idade e naquelas com insônia. Ter parceiro sexual e sentir-se bem indicaram efeito protetor contra baixo escore de sexualidade. Em mulheres com parceiro sexual, a prevalência de escore abaixo da mediana foi maior nas que moravam com o parceiro, que estavam na transição menopausal ou na pós menopausa e nas hipertensas. Ter relações sexuais com penetração e satisfação com o parceiro como amante indicaram efeito protetor contra escore de sexualidade abaixo da mediana. Conclusões: Mulheres com mais idade e que relataram insônia tiveram maior chance de ter sexualidade ruim, enquanto as com parceiro sexual e que se sentiram bem tiveram maior probabilidade de apresentar sexualidade boa. No grupo de mulheres que tinham parceiro sexual, a sexualidade foi negativamente associada ao fato de residir com o parceiro, estar na transição menopausal ou pós-menopausa e ser hipertensa. A satisfação com o parceiro como amante e ter relações sexuais com penetração diminuíram a probabilidade de apresentar sexualidade baixa / Abstract: Objectives: To evaluate aspects of sexuality and associated factors in middle aged women with 11 or more years of formal education. Methods: with living in Belo Horizonte (Minas Gerais), between 4, in the period between May and September 2005. The evaluation instrument was based on the Short Personal Experíences Questíonnaíre (SPEQ). Sexuality score was calculated from the multivariate analysis of the seven components: presence of sexual fantasies, intensity of desire, frequency of sexual activities, satisfaction in sexual activities, frequency of arousal, orgasm and self-classification of sexual life. The sexuality score below the median was considered bad sexuality and equal or higher than median as good sexuality. To investigate associated factors, data were analyzed using bivariate analysis and chi-squared and Fisher's exact tests were applied. Finally, Poisson multiple regression analysis was performed. The software used was Stata 7.0. Prevalence ratios (PR) and their 95% confidence intervals (95% CI) were calculated. Results: 276 questionnaires were evaluated and from these 219 women reported having sexual partners. The median sexuality score was 9.0 (range: 2.45-13.77). Multiple regression analysis showed that the prevalence of below median scores was higher in older women and in those with insomnia. Having a sexual partner and feeling well was associated with a protective effect against a below median sexuality score. In women with sexual partner, the prevalence of below median scores was higher in women who lived with their sexual partner, were in the menopausal transition or postmenopausal and in hypertensive women. Sexual activities involving penetration and a score of 6 for satisfaction with partner as a lover were indicative of a protective effect against below median sexuality score. Conclusions: Older women and those with insomnia were more likely to have a bad sexuality, whereas the ones with a sexual partner and who felt well were more likely to have a good sexuality. In the group which had sexual partner, the sexuality of women in midlife was negatively associated with the fact of living with a sexual partner, being in the menopausal transition or postmenopausal and being hypertensive. The efficacy of Sepia® in the management of climateric symptoms

M.Tech. / The purpose of this study was to determine the effectiveness of the homoeopathic complex remedy, Sepia® from Natura (Pty) Ltd. in South Africa, in alleviating climacteric symptoms in climacteric women as compared with a placebo remedy. Volunteers were recruited using flyers and posters placed at the Technikon Witwatersrand Clinic, and at pharmacies and health shops within the central Johannesburg area. Newspaper advertisements were placed in local newspapers advertising free homoeopathic treatment for climacteric symptoms by taking part in a research project. A sample size of forty subjects was chosen according to the delimitations of the study. A signed consent form was compulsory for participation in the study. The study was a two group experimental design consisting of an experimental group, who were administered Sepia®, and a control group, who were administered a placebo preparation. The study was conducted in a double-blind manner. The subjects were given sufficient medication to last for the duration of the study and were given instructions on how and when to take the medication, and how to store it. The duration of the study was eight consecutive weeks (two consecutive months). At the end of the treatment period there were fifteen subjects in the treatment group, and fourteen subjects in the placebo group During the treatment period the participants were required to fill out a Climacteric Symptom Questionnaire on a weekly basis and a Hot Flush Score Sheet on a daily basis. All the daily Hot Flush Score Sheets and weekly Climacteric Symptom Questionnaires were completed by each participant. They were collected after the eight week treatment period and subjects ceased taking the medication

Low sexual function is associated with menopausal status in mid-aged women with human immunodeficiency virus infection

OBJECTIVE: To assess the association between menopausal status and female sexual function among mid-aged women with human immunodeficiency virus (HIV) infection. METHODS: We carried out a cross-sectional study of 221 sexually active HIV-infected women ages 40 to 59 years, based on a secondary analysis of a three-hospital survey in Lima, Perú. We classified menopausal status according to Stages of Reproductive Aging Workshop criteria (STRAW+10); this exposure variable was categorized as binary (non-postmenopausal and postmenopausal) and-for exploratory analysis-as multinomial (pre-, peri-, and postmenopausal). We defined low sexual function (LSF) using the 6-item Female Sexual Function Index (total score ≤19). Socio-demographic and clinical variables were assessed, including age, used highly active antiretroviral therapy scheme, disease duration, depressive symptoms, and co-morbidities. We performed Poisson generalized linear models with a robust variance to estimate 95% confidence interval (CI), crude prevalence ratios (cPRs), and adjusted prevalence ratios (aPRs) by epidemiological and statistical approaches using nonparametric method of bias-corrected and accelerated bootstrap resampling with 1,000 repetitions. RESULTS: Studied women had a median age of 47.0 years (interquartile range: 7.5); 25.3% were premenopausal, 25.8% were perimenopausal, and 48.9% were postmenopausal. Also, 64.3% had LSF. The frequency of LSF was 53.6% in non-postmenopausal and 75.0% in postmenopausal women. Postmenopausal status was associated with LSF in both the crude (cPR = 1.39; 95% CI: 1.13-1.71) and the adjusted regression models (aPR = 1.38; 95% CI: 1.12-1.71). CONCLUSIONS: HIV-infected postmenopausal women have a higher prevalence of LSF than those non-postmenopausal ones, even when adjusting for multiple potential confounders. / Revisión por pares

Medicina tradicional chinesa: acupuntura e tratamento da síndrome climatérica / Traditional chinese medicine: acupuncture and climateric syndrome treatment

O objetivo do estudo foi avaliar em mulheres com sintomas menopáusicos, os efeitos da acupuntura ou da eletroacupuntura-placebo no Índice Menopausal de Kupperman (IMK) e na intensidade de fogachos e secundariamente avaliar se a ordem de execução desses tratamentos interfere nos resultados dessas medidas e se há alteração nos parâmetros laboratoriais. Foi realizado estudo prospectivo randomizado com 122 pacientes divididas em: Grupo 1 Acupuntura com 88 pacientes (período de um ano 10 sessões semanais e quinzenalmente até completar um ano, seguida de 6 meses de eletroacupuntura-placebo) e Grupo 2 Eletroacupuntura-placebo com 34 pacientes (6 meses 10 sessões semanais e quinzenalmente até completar 6 meses, seguida de um ano de acupuntura). Resultado: As pacientes tratadas com acupuntura e eletroacupuntura-placebo tiveram alívio das ondas de calor em 86,8% e 90,4% respectivamente, entretanto quando realizado quinzenalmente houve aumento das ondas de calor no Grupo 2. As pacientes tratadas com acupuntura apresentaram redução acentuada do IMK de 93,7% em relação ao Grupo 2 de 15,1%. O grupo que inicia com eletroacupunturaplacebo possui redução média na glicemia de 8,8%. Os valores médios de hemoglobina, triglicérides e HDL das pacientes aumentaram durante o estudo independente do grupo em 2,4%, 13,2% e 4,2%, respectivamente, enquanto os níveis de colesterol não modificaram. Nas pacientes tratadas com acupuntura observou-se redução do LDL em 9,1%, aumento do VLDL em 10,5% e redução do número de plaquetas em 5,0%. Houve redução significante do conteúdo mineral ósseo da coluna e do colo do fêmur em ambos os grupos em 1,4%. Conclusão: O uso da acupuntura para o alívio dos sintomas das pacientes climatéricas foi eficaz, sem apresentar efeitos colaterais. / This study aimed at evaluating the effects of acupuncture and placebo electroacupuncture on the Kupperman Menopausal Index (KMI) and on the intensity of hot flashes in women with menopausal symptoms. A secondary objective was to check whether the order in which these two treatments were carried out interfered in the measurement results and laboratory tests. A prospective randomized study was conducted with 122 patients assigned to two groups: Group 1: Acupuncture on 88 patients (one session a week for ten weeks and then one session every other week throughout one year followed by six months of placebo electroacupuncture) and Group 2: Placebo electroacupuncture on 34 patients (one session a week for ten weeks and then one session every other week throughout six months followed by one year of acupuncture). Results: : Hot flashes decreased by 86.8% and 90.4% in Group 1 and Group 2, respectively, during the one-year acupuncture treatments, but they increased in Group 2 during the biweekly placebo electroacupuncture treatment. In the acupuncture phase, the KMI had a 93.7% drop in Group 1 in sharp contrast with the 15.1% decline in Group 2. The glucose decreased 8,8% in group 2. The hemoglobin, triglycerides and HDL increased during this study in 2,4%, 13,2% and 4,2%, respectively, in group 1. The leucocytes and total cholesterol dont change during this study. The LDL decreased 9,1%, VLDL increased 10,5% and platelet decreased 5% in group 1. The mineral bone in lumbar spine L1-L4 and neck of femur decreased 1,4% in both groups. Conclusions: The use of alternative therapy such as acupuncture for menopause symptoms is beneficial and relief almost all symptoms without any side effects.
