Nurse practitioners are trained to use the electronic medical record (EMR) to document. Documentation in the EMR is often found to be incomplete, inaccurate, and unreliable, which affects the quality of care and patient safety outcomes. The purpose of the project was to improve the efficiency and effectiveness of nurse practitioners' documentation in the EMR. Malcolm Knowles' adult learning theory was used in this project to develop the education program. Kirkpatrick's training evaluation model was also used to analyze and evaluate the project. The study population included 5 primary care nurse practitioners in an ambulatory care setting using Cerner EMR. The practice-focused question was centered on whether a structured scenario-based training in Cerner would improve the completeness, accuracy, and reliability of EMR documentation. The 5 nurse practitioners were educated using structured, scenario-based training in EMR. The Cerner Advance database showed that there was an average decrease of two seconds in the documentation post-education when compared to the documentation time pre-education. Results for patient quality outcomes indicated that 2 out of 3 quality measures were performed above the national mean. The implication of this study for positive social change includes providing structured education using scenario-based training to help nurse practitioners provide quality care and promote better patient outcomes.

The purpose of this study has been to analyze how managers within the elderly care who operate according to the Social Services Act work to support the assistant nurses with the social documentation. In order to achieve my purpose and to be able to answer the research questions, a qualitative approach was selected. My empirical material consists of seven interviews, four with assistant nurses and three with managers.   Anthony Giddens’ structuration theory in Månsson, (2003 s. 422) has been my theoretical starting-point and the concepts of practical and discursive consciousness have been used in the analysis. Furthermore, I have used the concept zone of indifference in the theory regarding authority relations in organizations by Chester Barnard (Barnard, 1968). My research questions are: How do managers and assistant nurses perceive the purpose and content of the social documentation? Which approaches do managers use to inform and educate assistant nurses in the execution of the social documentation? How and when is the social documentation executed? I have used the analytical method meaning condensation, as I am interested in managers’ and assistant nurses’ perceptions and experiences of social documentation. The main headings which are developed from the results are purpose, method and organization. The essay shows that elderly care staff believe that the social documentation’s primary use is to ensure good care for patients/clients. It is the contact person’s responsibility to prepare an implementation plan together with the patient and to continuously write daily notes. There is an understanding amongst assistant nurses and managers that the implementation plan is a tool which is going to simplify the elderly care. However, it is difficult to specify exactly what should be documented. Managers’ strategies to help staff in maintaining a good social documentation is mostly done by informing that documentation should be kept. Sometimes case studies are used during planning days and workplace meetings. Training in social documentation is virtually non-existent; instead managers choose to educate the assistant nurses in computer programs that are used for documentation. / Syftet med denna uppsats har varit att studera hur chefer i verksamheter inom äldreomsorgen som bedriver vård enligt Socialtjänstlagen arbetar för att stödja undersköterskorna i arbetet med den sociala dokumentationen. För att uppnå mitt syfte och kunna besvara frågeställningarna valdes ett kvalitativt tillvägagångssätt. Mitt empiriska material utgörs av sju stycken intervjuer med fyra undersköterskor och tre chefer.    Anthony Giddens struktureringsteori i Månsson, (2003 s. 422) har varit min teoretiska utgångspunkt där begreppen praktiskt medvetande samt diskursivt medvetande har använts i analysen. Mina frågeställningar är: Hur uppfattar chefer och undersköterskor syftet med den sociala dokumentationens ändamål och dess innehåll? Vilka tillvägagångssätt använder chefer sig av för att ge undersköterskor information och kunskap om genomförandet av den sociala dokumentationen? Hur och när utförs den sociala dokumentationen? Jag har använt mig av analysmetoden meningskoncentrering, då jag är intresserad av chefer och undersköterskors uppfattningar/upplevelser av social dokumentation. Huvudrubrikerna som är framtagna ur resultatet är ändamål, metod och organisation.   Uppsatsen visar att omsorgspersonal anser att den sociala dokumentationen främst är till för att säkerställa en god omsorg för patienter/klienter. Det är kontaktpersonens ansvar att upprätta en genomförandeplan tillsammans med patienten och att skriva löpande daganteckningar. Det finns en förståelse hos undersköteskor och chefer att genomförandeplanen är ett arbetsredskap som ska underlätta omsorgsarbetet. Däremot är det svårt att precisera vad som ska dokumenteras. Chefernas strategier för att hjälpa personalen i att upprätthålla en god social dokumentation görs främst genom att informera om att dokumentation ska utföras. Ibland används även fallbeskrivningar under planeringsdagar och arbetsplatsträffar. Utbildning i social dokumentation är nästintill obefintlig, det som cheferna väljer att utbilda undersköterskorna i är hanterandet av datorprogrammet som används för att dokumentera i.
