Feeding Practices and Nutritional Status of Infants in Northwest Nigeria

Infants and young children in the Northwest province of Nigeria are susceptible to malnutrition. Inappropriate and inadequate breastfeeding and complementary feeding result in stunting, underweight, and wasting. The purpose of this cross-sectional study was to examine current feeding practices of infants not older than 2 years and their nutritional status in Northwest Nigeria. The theory of planned behavior was adopted in this research. With a total sample size of 3,861, multiple linear regression was adopted as a predictive analysis to delineate the correlation between two or more independent variables and one continuous dependent variable. Also, adopted was an independent t test to demonstrate the statistical difference between the mean of the dependent variable and that of the independent variable. The coefficient of determination (R2) indicated that the change in underweight associated with exclusive breastfeeding (EBF) was 8.1%. The overall regression model was significant, F(18, 879) = 4.29, p < .05, adj. R2 = .06 predicted underweight in infants under 6 months of age. The coefficient of determination (R2) indicated that the changes in underweight associated with age appropriate complementary (CP) feeding was 8.0%. The overall regression model was significant, F(18, 2,944) = 14.29, p < .05, adj. R2 = .08. The model predicted underweight in infants 624 months of age. The results from this study can be used in the reinforcement of EBF and age appropriate CP guidelines and policies by the extension of paid leave, implementing flexibility in working hours, and private space to breastfeed.

The Process Young Mothers in New Mexico Undergo to Achieve the Healthy People 2020 Objective of Exclusive Breastfeeding for Three Months

Breastfeeding is an important and relatively affordable health promotion and disease prevention activity with positive health outcomes for both the women who breastfeed and the infants who receive human milk as their first source of nutrition. As these benefits are dose dependent, the duration of breastfeeding and exclusivity of breastfeeding are important in the development of these protective effects. The purpose of this study is to understand the process younger mothers, ages 18 to 24 years, undergo to be able to sustain breastfeeding for longer than most women their age breastfeed. Ten women, ages 21 to 24 years, participated in individual, in-depth interviews in this grounded theory study. Following the iterative process of data collection and analysis, focus of the interviews changed as the data analysis guided the data collection process. Data underwent open, focused, axial, and theoretical/selective coding. The analysis process resulted in the development of a middle-range, descriptive theory grounded in the data. Prolonged Breastfeeding Through Fierce Determination succinctly explains the central phenomenon, fierce determination, and the relationship it shares with the other concepts identified as important contributing factors as the women moved through their breastfeeding journeys. Facing and Deciding to Overcome Obstacles, influenced most heavily by fierce determination, exerted its influence upon the outcome of the theory, "Trucking Along", an in vivo code provided by Participant 10, which captured her ongoing breastfeeding even in the face of obstacles. Normalized Infant Feeding Method contributed to the participants' fierce determination. Availability of Support positively impacted the participants' ability to face and overcome their breastfeeding obstacles. Personal Agency had a reciprocal relationship with fierce determination and, like availability of support, also positively impacted their ability to face and overcome obstacles. The concepts within the theory were found to be congruent with a focused review of the literature around long-term goal achievement, healthful behavior adaptation, and breastfeeding. This study contributed to the breastfeeding literature, providing insight into how young mothers engage in prolonged breastfeeding.

O aleitamento materno é um dos principais instrumentos para a promoção da saúde infantil. O leite materno é o alimento completo para o crescimento e desenvolvimento das crianças até os 6 meses. O objetivo deste estudo foi avaliar as práticas do aleitamento materno entre as crianças de 0 a 6 meses cadastradas em Núcleos de Saúde da Família da cidade de Ribeirão Preto (SP) e caracterizar as mães quanto a variáveis socioeconômicas e demográficas, assistência pré–natal e natal, às características dos recém-nascidos; verificar a associação do aleitamento materno exclusivo com as variáveis e analisar a duração do aleitamento materno exclusivo. Foi realizado inquérito domiciliar com uma amostra de 53 crianças, através de questionários, para caracterizá-las de acordo com as variáveis e verificar a duração do aleitamento materno exclusivo. Na análise da duração do aleitamento materno exclusivo e suas variáveis foi utilizada a curva de Kaplan-Meier. A duração mediana do aleitamento materno exclusivo foi de 70 dias. Encontramos associação positiva entre duração do aleitamento materno exclusivo e as seguintes variáveis: crianças nascidas em Hospital Amigo da Criança, não uso de chupeta, não uso de mamadeira, realização de puericultura em unidade de saúde da família. / Breastfeeding is one of the most important tools to promote children’s health. Breast milk is a complete food for children’s growth and development until the sixth month. The objective of this study is to evaluate the breastfeeding practices in Ribeirão Preto city, state of São Paulo (SP), Brazil and to characterize the mothers regarding social, economical and demographic variables, prenatal and natal assistance and newborn characteristics, verify the relation between variables and exclusive breastfeeding and also, verify the exclusive breastfeeding duration. A domiciliary inquire, through questionnaire, was fulfilled with a sample of 53 children, to characterize them according to the variables and to verify the duration of breastfeeding. In the analysis of the breastfeeding duration and its variables, the Kaplan-Meier curve was used. The median duration of exclusive breastfeeding was 70 days. We found positive association between exclusive breastfeeding duration and the following variables: born in Baby Friendly Hospital, no use of pacifiers, no use of bottle and children accomplishment in Family Health Units.

Entre os indicadores de saúde infantil propostos pela Organização Mundial da Saúde, a prevalência do aleitamento materno exclusivo até 6 meses de idade (AME: quando a criança recebe somente leite materno direto da mama, ordenhado, ou de ama de leite, sem outros líquidos ou sólidos, com exceção de gotas ou xaropes contendo vitaminas, sais de reidratação oral, suplementos minerais ou medicamentos, podendo ser desagregado nos grupos etários 0-1, 2-3, 4-5 e 0-3 meses) é fundamental para o monitoramento das ações para promoção, proteção e apoio à amamentação. Objetivos - Investigar a prevalência e os fatores associados à prática de AME aos 30 dias de vida em Cruzeiro do Sul, Acre. Métodos - Análise de dados do estudo de coorte de nascimentos MINA-Brasil: Materno-INfantil no Acre. Entre julho de 2015 e junho de 2016, mães internadas para parto no Hospital Estadual da Mulher e da Criança do Juruá do município de Cruzeiro do Sul, Acre, foram entrevistadas sobre dados socioeconômicos, demográficos e história de saúde após aceite à participação no estudo. Para a presente análise, o desfecho de interesse -aleitamento materno exclusivo aos 30 dias de vida?, foi obtido 30 a 45 dias após o parto por meio de entrevista telefônica e classificado segundo critério da OMS (2010). Medidas de tendência central e dispersão, frequências absolutas e relativas, intervalos de confiança de 95% (IC95%) e teste de homogeneidade &#967;2 foram calculados segundo prática do AME. A análise de sobrevida por meio do estimador Kaplan-Meier foi utilizada para estimar a probabilidade e o efeito de fatores associados à prática do aleitamento materno exclusivo aos 30 dias de vida. O efeito dos preditores sobre a duração do AME no primeiro mês foi investigado em modelos múltiplos de regressão tempo de vida acelerado. A análise estatística foi realizada com auxílio do pacote estatístico Stata 14.0 ou superior, ao nível de significância de p<0,05. Resultados - No total, 962 mães responderam a entrevista telefônica de acompanhamento no puerpério. A prevalência de AME aos 30 dias de vida foi 36,7% e o tempo mediano para os que interromperam o AME nesse intervalo foi 16 dias. Quando consideradas todas as crianças nascidas no período e elegíveis ao seguimento, a probabilidade de AME aos 30 dias foi 43,7% e a mediana de AME foi 30 dias. Os fatores associados à duração do AME no período estudado foram: número de filhos vivos, uso de chupeta e história de chiado no peito. Observou-se que a duração do AME (time ratio - TR) foi 28% mais prolongada entre os bebês que tinham irmãos (1,28; IC95% 1,11-1,48). O uso de chupeta e história de chiado no peito foram associados à redução no tempo de AME em 33% (0,67; IC95% 0,58-0,78) e 20% (0,80; IC95% 0,69-0,93), respectivamente. Conclusões - A prática de aleitamento materno exclusivo aos 30 dias de vida em Cruzeiro do Sul, Acre, foi aquém das recomendações nacionais e internacionais. Mães primíparas interromperam o AME mais precocemente durante o primeiro mês quando comparadas às mães com mais de um filho; bebês que usaram chupeta ou apresentaram história de chiado no peito apresentaram menor duração do AME no primeiro mês de vida. The outcome of interest was exclusive breastfeeding at 30 days of life, classified according to WHO guidelines (2010). Descriptive statistical data, 95% confidence intervals (95%CI) and chi-square tests were calculated. Kaplan-Meier survival analysis was performed to estimate the median duration and the probability of EBF at the end of the first month of life. In order to explore factors associated with time to EBF cessation, accelerated failure-time (AFT) models were run, following a conceptual framework hierarchical model for determinants of EBF. All analyses were done in Stata 14.0, at p<0.05. Results - During the follow-up period, 962 mothers answered the postpartum telephone interview. The prevalence of EBF at 30 days of life was 36.7% and the median duration was 16 days. Considering all children born in the study period and eligible for follow-up, the probability of EBF in the first month of life was 43.7% and the median duration was 30 days. Factors associated with time to EBF cessation were: having one or more children, pacifier use and history of wheezing. It was observed that duration of EBF (time-ratio - TR) was 28% longer among infants who had siblings (1.28; 95% CI, 1.11-1.48). The knowledge and practices of mothers regarding exclusive breastfeeding in the Mahwelereng Local Area of the Waterberg District, Limpopo Province

Thesis (MPH. ) --University of Limpopo, 2014 / The aim of this study was to determine the knowledge and practices of mothers with regard to exclusive breastfeeding in the Mahwelereng Local Area of the Waterberg District, Limpopo Province. A quantitative study was conducted by using a researcher-administered structured questionnaire at six clinics in the Mahwelereng Local Area. A simple random sampling method was used to select the hundred and seventy five mothers who had babies of less than six months old from whom data was collected. Data was analysed with the assistance of the statistician by using the SPSS software program. Data is presented applying descriptive and inferential statistics. Less than half (74; 42.3%) of the responding mothers were practising exclusive breastfeeding despite the fact that 137 (78.3%) of them were given information about exclusive breastfeeding. There is an association between knowledge of exclusive breastfeeding (Chi-Square= 14.039; Sig=0.000 p<0.05) and exclusive breastfeeding practice. There is no association between exclusive breastfeeding practice and HIV status (Chi-Square=2.444; Sig=0.118; p>0.05). Challenges that mothers faced in relation to practising exclusive breastfeeding were work-related, school-related, health-related, pressure from family to mix-feed, and a lack of knowledge. However, 42.9% of the respondents indicated that they did not have any challenges with regard to the practice of breastfeeding. It was, therefore, recommended that one-on-one counselling about exclusive breastfeeding needs to be intensified and maternity leave should be extended to six months. The majority of respondents showed adequate knowledge of what exclusive breastfeeding was because they were counselled about breastfeeding although they were not practising it. Postnatal Peer Counseling on Exclusive Breastfeeding of Low-birthweight Filipino Infants : Results of a Randomized Controlled Trial

<p>In a Manila hospital, 204 mothers were randomized into three groups: two intervention groups receiving home-based counseling visits, one of them (n=68) by counselors trained to use a locally developed, two-tiered program of breastfeeding counseling, and the other by counselors trained in general childcare (n=67), were compared with a control group of mothers (n=69) who did not receive any counseling. All infants were scheduled for seven visits to the hospital for follow-up. During hospital visits, maternal and infant body measurements were made and an independent interviewer asked the mothers individually to recall how the infant had been fed. One study physician, blind to participant groups, was consulted at all scheduled and unscheduled infant visits.</p><p>At six months, 44% of the breastfeeding-counseled mothers, 7% of the childcare-counseled mothers and none of the mothers in the control group were exclusively breastfeeding. Twenty- four mothers breastfed exclusively during the first six months, of whom 22 received breastfeeding counseling and 2 had no breastfeeding counseling. Among 24 infants who were exclusively breastfed from birth to six months there were no episodes of diarrhea. All infants had gained in weight, length and head circumference. Mean maternal weight loss at six months was similar whether her breastfeeding was exclusive or partial.</p><p>The reasons why mothers without breastfeeding counseling introduced non-breast milk feeding before six months reflected lack of knowledge and support. Breastfeeding support during the first six months focusing on how to prevent and solve breastfeeding problems, particularly during the first two weeks, will enable mothers to choose to breastfeed exclusively up to six months. </p><p>This study has provided fundamental evidence of successful intervention by breastfeeding counseling to achieve six months of exclusive breastfeeding among term, low-birthweight infants. The locally developed training program in breastfeeding counseling, which successfully prepared volunteers to counsel mothers at home, could be incorporated into primary health care in the Philippines. Mothers who received breastfeeding counseling appreciated how this helped them to achieve their breastfeeding goals for the first six months. Improved breastfeeding practices as a result of breastfeeding counseling provided infants with protection from diarrhea and respiratory infections, contributing to their health and development.</p>
