Kärlek till droger eller begär efter kärlek : En litteraturstudie om erfarenheter av omvårdnad hos patienter med en beroendesjukdom / Love to drugs or craving for love : A literature review of experiences of nursing care in patients with addictive disorders

Bakgrund: Idag lever 425 000 människor med ett substansrelaterat beroende i Sverige. En stor andel av personerna lever med samtidiga somatiska sjukdomar vilket innebär att det är en patientgrupp som ofta förekommer inom somatisk sjukvård. Syfte: Studiens syfte var att belysa erfarenheter av omvårdnad inom somatisk vård hos patienter med en substansrelaterad beroendesjukdom. Design: En litteraturstudie genomfördes med systematisk ansats. Metod: Litteratursökning utfördes i databaserna CINAHL, PsycINFO och PubMed vilket resulterade i elva artiklar med kvalitativ ansats. Artiklarna kvalitetsgranskades och materialet analyserades med inspiration från Graneheim och Lundmans (2004) innehållsanalys. Resultat: Analys en mynnade ut i tre olika teman: respektlös behandling, vård på olika villkor och god omvårdnadsrelation. Huvudsakliga fynd visade att patienter upplevde personal som misstänksamma, skuldbeläggande samt Upplevt bristande engagemang. Resultatet visar på att patienter upplevde sig ha fått mottagaren annorlunda vård än andra patienter. Goda upplevelser manifesterades genom att personalen visade omtanke, bekräftade samt upprätthöll en personcentrerad omvårdnad. Konklusion: Patienter med en substansrelaterad beroendesjukdom upplever sig i huvudsak bli respektlöst behandlade och särbehandla de gentemot övriga patienter inom vården. Förstärkt medvetande hos vårdpersonal krävs för att eliminera de stereotypa uppfattningarna och därmed skapa förutsättningar för en god vårdande relation. / Background: 425 000 people are today living with a substance-related addictive disorder in Sweden. A large percentage of these people also live with concurrent somatic diseases which means that this group of people often encounter the somatic health-care system. Aim: The study aimed to highlight the experience of nursing in somatic care in patients with a substance-related addictive disorder. Design: The study was conducted as a literature review with a systematic approach. Method: The literature search was conducted through three databases: CINAHL, PsycINFO and PubMed, which resulted in eleven articles, all with a qualitative approach. The articles were checked for accuracy and the material was analyzed with inspiration from Graneheim and Lundmans (2004) concept analysis. Findings: Three themes were identified: disrespectful treatment, care on different terms and good nursing relationship. Main findings revealed that the patients experienced nursing staff as suspicious, blaming and showing a lack of dedication. The result demonstrates that the patients with a substance-related addictive disorder received inferior care than other inpatients in the nursing wards. Good experiences were manifested by the staff showing concern, confirmation and maintained a person-centered care. Conclusion: Inpatients with a substance-related addictive disorder tend to experience disrespected treatment and discrimination compared to other inpatients. Enhanced awareness is needed among health professionals to erase the stereotypical perceptions and by that create conditions for a good caring relationship.

"En riktig våldtäkt" : - en kunskapsöversikt om föreställningar om våldtäkt

This essay is a selective review of Nordic literature with the aim to explore contemporary conceptions of male-on-female rape, to increase the understanding of how these can construct “a real rape”. To achieve this two questions were formulated: What circumstances can cause an incident not to be defined as rape? What circumstances can cause an incident to be defined as rape? Searches in international and Swedish databases led to initial selection of peer-review journal articles and dissertations that were reviewed against set criteria. These criteria were met by 8 studies, that were assigned codes, from which comprehensive categories were developed. The categories were analysed using Yvonne Hirdman’s concepts gender, separation and male superiority. The analysis shows that circumstances connected to the overall categories Situation, Victim and Offender construct “a real rape”. The circumstances are Relation, Alcohol, Saying no and how it is done, The woman’s sexuality, Resistance, How the woman feels afterwards, Physical violence, and Ordinary, normal and good men. It seems easier to define an incident as rape when these circumstances occur the “right” way. In conclusion, rape is constructed as a rather narrow concept which could make it difficult for women to name experiences as rape.
