Correlación de los hallazgos endoscopicos e histológicos en el diagnóstico de metaplasia intestinal gástrica en pacientes en el hospital nacional Hipólito Unánue en el año 2015

INTRODUCCION: La metaplasia intestinal gástrica (MI) es una etapa previa al cáncer gástrico. La apropiada identificación endoscópica con biopsia es vital para confirmación histológica. No conocemos en nuestro medio la relación entre los hallazgos endoscópicos sospechosos de metaplasia intestinal y su confirmación histológica, ni sus factores asociados con claridad. OBJETIVO: determinar la correlación entre los hallazgos endoscópicos de sospecha de metaplasia y su contraparte histológica en nuestro medio , dentro de ellos también determinar algunos factores asociados .a los pacientes con sospecha de metaplasia .Métodos: Estudio observacional analítico, realizado en el Hospital Nacional Hipólito Unanue en pacientes >= 18 años con sospecha endoscópica de MI, excluyendo pacientes con cáncer gástrico previo y pacientes con antecedentes previo de metaplasia . La evaluación estadística se realizó con el software estadístico SPSS. Resultados: 3398 pacientes sometidos a endoscopia alta se identificaron un total de 176 pacientes con sospecha endoscópica de metaplasia l. En 152 de ellos se encontró confirmación histológica MI equivalente a un 86,36% del total de la muestra. Los hallazgos histológicos frecuentes en el grupo fueron 51(29% pacientes ) pacientes presentaron gastropatía atrófica en alguna parte de su estómago , 28( 15,9%) pacientes presentaron gastritis superficial con la toma de biopsia , hiperplasia foveolar en Solo 2 (1,1%) pacientes demostrados por biopsia entre otros. Predominaron pacientes de sexo femenino con MI+ (90) .Los diferentes subgrupos generados por el reporte histológico de metaplasia intestinal incluyeron los siguientes: metaplasia completa (44,3%), metaplasia incompleta (17,6%), metaplasia inespecífica (13,6%), metaplasia mixta (11,4) % y la no presencia de metaplasia 13,1% . solo el 4 % de la población estudiada se reportó metaplasia intestinal con algún tipo de displasia . Con respecto al análisis bivariado se encontró asociación de riesgo con gastritis crónica atrófica OR : 3,733 (IC 95% 0,941-14,819,p = 0.051), gastritis crónica superficial OR 1,378 (ic 95% 0,382-4,973, p = 0.623) edad <=65 años con Helicobacter pylori (HP) y metaplasia intestinal + OR :2,706 (IC 95% 0,342-21,402, p = 0.327) edad <=65 años con hp+ y metaplasia intestinal completa vs mixta , se encontró OR 2,836 (IC 95% 0,341-23,574, p = 0.633), edad <=65 años con hp+ y MI completa vs incompleta OR 1,393 (IC 95% 0,356-5,447, p = 0.633). Entre reflujo y MI , se encontró un OR 1,164 (IC 95% 1,096 -1,238, p = 0.322) ,HP y MI OR 0.537 (IC 95% 0.226 -1,277, p = 0.155) no se puede afirmar que existe una tendencia de asociación entre presencia HP y MI . Conclusiones: Con respecto al diagnóstico histológico de metaplasia intestinal, 86,36 % de los pacientes sometidos a estudio 7 endoscópico con sospecha de metaplasia intestinal correspondientes a un total de 152 fueron confirmados mediante el estudio histológico, aunque la detección fue cercana al 86,36%,se encontró asociación de riesgo en los pacientes con gastritis atrófica , gastritis crónica superficial ,edad<=65años y HP+ ,reflujo biliar . No se encontró asociación en pacientes con Hp+ .Se requieren de estudios prospectivos, multicéntricos y con cromoendoscopia, con el fin de evaluar la concordancia respectiva entre los dos métodos y determinar variables endoscópicas predictoras de severidad, y tipo de metaplasia para determinar protocolos de seguimiento para este tipo de pacientes.

Estudos químico e biológico de taninos e antraquinonas que atuam no sistema gastrointestinal / Chemical and biological studies of tannins and anthraquinones acting on the gastrointestinal tract

Diversas plantas com atuação no trato gastrointestinal - ricas em taninos e antraquinonas - ainda deixam muitas dúvidas quanto a sua caracterização estrutural, o seu mecanismo de ação e toxicidade. O presente trabalho objetivou o estudo químico e biológico de duas plantas utilizadas para doenças relacionadas ao sistema gastrointestinal: barbatimão (Stryphnodendron rotundifolium), rico em procianidinas e taninos (utilizada para combater gastrite); e cáscara sagrada (Rhamnus purshiana), rica em antraquinonas (utilizada para constipação). Inicialmente foi realizado um estudo químico com as plantas envolvendo isolamento e caracterização de componentes do extrato por RMN (1H, 13C, DEPT e NOESY), espectrometria de massas (MS), infravermelho e dicroísmo circular vibracional (VCD). Essas substâncias foram utilizadas para a padronização dos extratos utilizados em estudo biológico. O estudo biológico envolveu testes in vitro para avaliar a toxicidade de diferentes componentes do extrato de cáscara sagrada e teste in vivo com barbatimão, visando, através do tratamento de animais e remoção do estômago para estudo em microscopia confocal, entender o mecanismo de proteção gástrica. Com o desenvolvimento do trabalho foi possível desenvolver novas metodologias para a identificação de antraquinonas utilizando espectrometria de massas e provar a veracidade de regras empíricas utilizadas para a determinação da configuração absoluta de moléculas dessa classe. Os testes in vitro com antraquinonas isoladas e padrões comerciais indicam a possibilidade de aprimoramento dos perfis químicos utilizados para a produção de medicamentos a base de extratos de cáscara sagrada, visando à redução de toxicidade. Quanto ao barbatimão, o estudo químico possibilitou a aplicação da técnica de VCD para a diferenciação de procianidinas diasteroisomericas. Os estudos in vivo confirmaram a eficácia do uso de taninos para combater gastrite através da formação de um revestimento protetor. / Several plants with action in the gastrointestinal tract - rich in tannins and anthraquinones - cause many doubts concerning their chemical characterization, mechanism of action and toxicity. The present work aimed to develop a chemical and biological study of two plants used for gastrointestinal system related diseases: barbatimão (Stryphnodendron rotundifolium), rich in procyanidins and tannins (used to treat gastritis); and cascara sagrada (Rhamnus purshiana), rich in anthraquinones (used for constipation). Initially, a chemical study with plants was performed comprising isolation and characterization of components of the extract through NMR (1H, 13C, DEPT and NOESY), mass spectrometry (MS), and infrared and vibrational circular dichroism (VCD). These substances were used for extract standardization used in biological studies. The in vitro tests were performed in order to evaluate the toxicity of different components of the cascara extract. The in vivo tests with barbatimão aimed to better understand the mechanism of gastric protection, by treating animals with the extract, removing their stomachs, and analyzing them with fluorescence microscopy. Through this work we were able to develop new methods for identifying anthraquinones by mass spectrometry and to prove empirical NMR rules used to determine the absolute configuration of molecules of this class. The tests with isolated anthraquinones indicated the possibility of improving chemical profile to further reduce cascara sagrada toxicity. The barbatimão chemical study allowed the application of VCD technique for the differentiation of diasteroisomeric procyanidins, and confirmed the effectiveness of using tannins for gastritis treatment through the formation of a protective coating.

Enfermedades inducidas por drogas; anemia aplásica adquirida, hepatitis medicamentosa y gastritis hemorrágica en el Hospital Nacional Guillermo Almenara Irigoyen de agosto 1998-diciembre 2001

El presente es un estudio epidemiológico, descriptivo, retrospectivo de tipo deductivo que se realizó en el Hospital Nacional Guillermo Almenara Irigoyen, perteneciente a EsSalud, Hospital de IV nivel de la seguridad social, comprendiendo el periodo entre agosto 1998 - octubre 2001, con el objetivo de establecer relación de causalidad entre medicamento administrado (causa) y la aparición de enfermedades (efecto) tales como: Anemia Aplásica Adquirida, Hepatitis Medicamentosa y Gastritis Hemorrágica. Se trabajó con las historias clínicas de pacientes, en quienes se había diagnosticado las enfermedades comprendidas en el estudio; los pacientes pertenecían a los diferentes servicios del hospital. Los periodos de estudio fueron para: anemia aplásica adquirida de agosto de 1998 a agosto del 2001, hepatitis medicamentosa de enero de 1999 a octubre del 2001, y gastritis hemorrágica del 1 de enero del 2000 al 31 de diciembre del 2000. El análisis deductivo se realizó mediante el Algoritmo de Karch y Lasagna modificado por Naranjo y colaboradores. Los criterios considerados para establecer relación de causalidad entre agente etiológico (medicamento) y la enfermedad inducida fueron: Relación en el tiempo, entre la administración del medicamento sospechoso y la enfermedad inducida por drogas; la resolución favorable de los signos, síntomas y pruebas de laboratorio después del retiro del agente etiológico probable; los datos de laboratorio y exámenes clínicos que relacionen en el tiempo a la droga y la aparición de la enfermedad en estudio. Se estudiaron 18 casos de anemia aplásica adquirida, la edad promedio fue de 26.7 años, el 27.8% fue inducido por drogas mientras otro 27.8% fue inducido por otro agente etiológico y el 50% de los casos presentaba una enfermedad concomitante. Se identificaron 15 casos de hepatitis medicamentosa, la edad promedio fue de 44 años, el 40 % estaban entre la edad de 40 – 50 años y el 93.3% de los casos presentaba otra enfermedad concomitante. Encontramos 41 casos de gastritis hemorrágica, la edad promedio fue de 63 años, el 31.7% estaban entre la edad de 70 – 80 años, el 48.8 % fue inducido por drogas mientras el 29.3% fue inducido por otra causa no medicamentosa, el 53.7 % presentaba otra enfermedad concomitante. Se concluyó que el 27.8% de los casos de anemia aplásica y el 48.8% de los casos de gastritis hemorrágica tenían como agente etiológico probable a drogas, los agentes quimioterapicos con un 11.1% (5-fluorouracilo, doxorubicina, metotrexate) en anemia aplásica, los agentes antituberculosos con un 20.1% ( isoniazida, rifampicina, pirazinamida, etionamida) en hepatitis medicamentosa y los AINES con un 29.3% (ibuprofeno, piroxicam) en Gastritis hemorrágica fueron los principales agentes etiológicos probables de estas enfermedades estudiadas. / This is an epidemiologic, descriptive, retrospective study of deductive tipe done in Hospítal Nacional Guillermo Almenara Irigoyen of EsSalud, Hopital of fourth level of social security, in period from august 1998 to October 2001 and the objective is to stablish relation between administered drug (cause ) and apparition of illness (effect) such as: acquired aplasic anemia, hepatitis induced by drugs and hemorrhagic gastritis. Clinic histories of patients with diagnostic of illness included in study were material of work; patients were of different services of hospital. Period of study was: for acquired aplasic anemia from august 1998 to august 2001, for hepatitis induced by drugs from january 1999 to october 2001, and for hemorrhagic gastritis from january 1 2000 to december 31 2000. Deductive analysis was done with Karch and Lasagna´s algoritme modified by Naranjo and collaborators. Cryteria considered to stablish relation of causality between etiologic agent (drug) and induced illness were: relation in time between administration of suspect drug and illness induced by drug; favorable remission of signs, symptoms and laboratoty tests after quit suspect etiologic agent, laboratory tests and clinic surveys wich were related in time drug with apparition of illness in study. Eighteen cases of acquired aplasic anemia were studied, the average age was 26.7 years; the 27.8% was produced for drugs while other 27.8% was induced for other etiologic agent and 50% of cases showed a concomitant disease. 15 cases of hepatitis produced for drugs were identified, the average age was 44 years, the 40%, the 40% was between 40 50 and 93.3% of cases showed other concomitant disease. 41 cases of hemorrhagic gastritis were discovered, the average age was 63 year, the 31.7% was between 70 – 80 years; the 48.8% was produced for drugs while 29.3% was induced for other cause and 53.7% presented other concomitant disease. 41 cases of hemorrhagic gastritis were discovered, the average age was 63 year, the 31.7% was between 70 – 80 years; the 48.8% was produced for drugs while 29.3% was induced for other cause and 53.7% presented other concomitant disease. Conclusion, the 27.7% of aplasic anemia cases and 48.8% of hemorragic gastritis cases were produced probably for drugs; the quimioterapic agents whit a 11.1% (5-fluorouracile, doxorubicin, metothrexate) in aplasic anemia; the antituberculosis agents with a 20.1% (isoniazida, rifampin, pyrazinamide, ethionamide) in hepatitis produced for drugs and the AINES with a 29.3% (ibuprofen, piroxicam) in hemorrhagic gastritis, which were the principal etiologic agents who produced probably the different studied diseases.

Darbo tikslas: ― išnagrinėti kačių, sirgusių gastritais 2006 – 2012 metais sergamumą. Darbo uždaviniai: 1. susisteminti literatūros duomenis apie kačių gastritu; 2. įvertinti kačių sergamumą virškinimo ligomis; 3. palyginti kačių sergamumą gastritais su kitomis virškinimo sistemos ligomis; 4. įvertinti gastritų paplitimą priklausomai nuo veislės, bei lyties Išvados: Gastritas yra polietiologinis susirgimas, kuriam būdingas klinikinių simptomų įvairavimas nuo mažai pastebimų iki stipriai išreikštų. Ištyrus 726 pacientus, kurie gydėsi Lietuvos veterinarijos gydyklų terapiniuose skyriuose ir jiems buvo diagnozuotas gastritas, nustatėme, kad: 1. Nuo 2006 iki 2012 metų, Lietuvos smulkių gyvūnų gydyklų terapiniuose skyriuose gydytos 2633 katės, sirgusios virškinimo sistemos ligomis.2. Katės gastritais ir kitomis virškinimo trakto ligomis mažiausiai sirgo 2006 metais, daugiausiai 2012 metais. 3. Patelės gastritais sirgo 1,33 karto dažniau, nei patinai. 4. Mišrūnėms katėms gastritas diagnozuotas 14 atvejų daugiau, nei veislinėms katėms. / The information gathering of cats with gastritis took place in three clinics in Lithuania (two of them were held in Kaunas and the other one- in Vilnius) from 2006 to 2012. All the patients who were treated in therapeutical sections were registered, their anamnesis vitae et mobri information was gathered and analyzed according to clinical examination methods. Diagnosis of gastric inflammation was usually based on history and clinical trial data, the abdominal X, endoscopic and laboratory test results. Gastritis is a fairly common digestive tract disorder. Etiotropic gastritis factors are many: it is feeding tolerance, infectious diseases, other organs and systems dysfunction, tumor pathology, the body's sensitization, neurogenic, iatrogenic causes, and so on. Analysis of the collected data showed that the period 2006-2012 cats treated in several Lithuanian veterinary clinics therapeutic sections, often suffered from gastro-intestinal diseases, which accounted for 63 ± 1.26 percent of all therapeutic treated for feline diseases (mean 376.1 ± 95.9 cases per year). In most cases, the assessment of a percentage of the gastrointestinal tract cats had in 2010 - 65 per cent. all cases, the least - in 2008. (61.2 per cent Cases all). Introduction. The operative link for gastritis assessment (OLGA) and the operative link on gastric intestinal meta-plasia assessment (OLGIM) staging systems have been sug-gested to provide risk of assessment for gastric cancer. Objec-tive. To evaluate the distribution of OLGA and OLGIM staging by age and Helicobacter pylori status. Material and methods. We studied 197 subjects undergoing elective upper gastrointestinal endoscopy. The presence of the H. pylori and histological changes were evaluated using the updated Sydney system. Stages III and IV of OLGA/OLGIM were considered high risk stages. Results. The H. pylori rate was 56.85% (112/197). High-risk OLGA/OLGIM cases were rare: 7/112 (6.5%) cases of OLGA in the H. pylori positive group and 6/85 (7%) in the H. pylori negative group; 5 (4.4%) cases of OLGIM in the H. pylori positive and 6 (7%) in the H. pylori negative. The proportion of advanced stages of OLGA and OLGIM increased with age (p < 0.001). High-risk OLGA was not found before age 40 regardless of the presence of H. pylori, but increased to 16.2%, 10.3%, 17.3% and 40.8% in subjects in the fourth, fifth, sixth and seventh decade of life respectively. The OLGIM high risk showed a similar trend: 0% before 40 years and up to 22.6% in people of 70 years. Conclusions. High-risk OLGA/OLGIM cases are infrequent before age 40 and increase significantly with age. No relation was found with the presence of the H. pylori. According to these protocols, only a fifth of the patients would strictly require endoscopic control. / Revisión por pares
