Cobertura de inmunización contra el virus de hepatitis B (HBV) en hijos nacidos de madres portadoras de la infección, Huanta (Ayacucho) 2014-2018

Determina el nivel de cobertura de inmunización contra el HBV en hijos nacidos de madres portadoras de la infección en el Hospital de apoyo de Huanta, Ayacucho, entre los años 2014 y 2018. Se realizó un estudio observacional de tipo descriptivo y retrospectivo. En el estudio se incluyó a los hijos nacidos de madres portadoras de la infección por HBV, cuyos nacimientos fueron registrados en el Hospital de Apoyo de Huanta, Ayacucho, entre enero de 2014 y diciembre de 2018. No se contó con criterios de exclusión. Se utilizó fuentes de información secundarias (Libro de Nacimientos del Hospital de Apoyo de Huanta y bases de datos otorgadas por la Unidad Ejecutora Red de Salud Ayacucho Norte Sede Huanta). Este estudio fue aprobado por el Instituto de Ética de la Facultad de Medicina de la Universidad Nacional Mayor de San Marcos. En el análisis estadístico se utilizó medidas de frecuencia y porcentaje. Se encontró que la cobertura total de inmunización contra el HBV de 64.47%, siendo la cobertura de la dosis de nacimiento de la vacuna contra el HBV de 100%, la cobertura de aplicación de la Inmunoglobulina de 77.78%, y la cobertura de la aplicación de la tres dosis de la vacuna pentavalente fue 97.61%, 93.90% y 80.26% respectivamente. Se concluye que se encontró una cobertura total de inmunización contra el HBV por debajo de lo esperado según las recomendaciones de la OMS. Individualmente se encontró una cobertura adecuada de la dosis de nacimiento y de la primera dosis de la vacuna contra el HBV incluida en la vacuna pentavalente, mas no en la segunda y tercera dosis de la misma ni en la aplicación de Inmunoglobulina. Se recomienda realizar futuros estudios en donde se lleve a cabo el seguimiento de la población y se utilice fuentes de información preferentemente primarias, además de complementar y reforzar el plan de eliminación de transmisión madre - hijo del HBV en el Perú. / Tesis

Novel approaches to an improved understanding of the epidemiology and control of hepatitis B virus infection in Australia

Background: The most recent estimate for the number of Australians living with chronic hepatitis B virus (HBV) infection is 150,000, with over one million ever having been infected. One in four people with chronic infection will die as a result. Worldwide, the burden of chronic HBV infection is great. As many as 400 million people are chronically infected, and the World Health Organisation estimates that as a result HBV infection is the tenth leading cause of death. / Aim: The aim of the research presented in this thesis is to improve the accuracy and relevance of our understanding of the epidemiology and control of HBV infection in Australia, through the development of new methodological approaches to the collection and analysis of relevant epidemiological data. / Methods: Three novel approaches were adopted. First, a serosurvey of a randomised, age-structured convenience sample of over 3200 specimens was performed spanning the period from 1995 to 2005 to estimate the prevalence of markers of infection with, and immunity to HBV. Secondly, a comparative analysis of the serosurvey results with national surveillance notifications since 1971 and migration records since 1945 was undertaken. Finally, a complex deterministic mathematical model of HBV infection in Australia was constructed simulating the entire population between 1951 and 2050. / Results: The serosurvey indicates that chronic infection with HBV is more common in the Victorian population than existing national serosurvey estimates suggest, and the coverage of immunisation programs (particularly of adolescents) is far from universal. Significant geographic, age, and gender disparities in the prevalence of chronic HBV infection were identified in the serosurvey, which appear in part to relate to historical migration patterns and which could be used to develop a targeted and effective public health response. The comparative analysis of the serosurvey results with notifications and migration data demonstrates coherence of these disparate sources of information, and suggest that knowledge of migration patterns can lead to robust predictions of future notifications. The novel regression model developed implies that at least 50,000 people with chronic HBV infection are undiagnosed. The mathematical model of HBV infection in the Australian population is unique in many respects, and has been validated against external data to provide reassurance regarding the accuracy of the simulated outcomes. Some of these outcomes include an estimated 160,000 Australians living with chronic HBV infection in 2009, increasing by several thousand people every year, and that less than 5 per cent of chronic infections entering the population are able to be addressed by domestic vaccination or other prevention programs. / Conclusion: The new insights into the epidemiology of HBV infection in Australia provided by the approaches described all suggest a large and increasing burden of chronic HBV infection. New approaches are needed to provide essential policy outcomes to assist and empower Australians living with chronic HBV infection. If this does not occur, the economic and human costs to our community are likely to become great.

Mutation Pattern of Lamivudine Resistance in Relation to Hepatitis B Genotypes

Es gibt wenige Erkenntnisse über den Zusammenhang zwischen Lamivudin induzierten Resistenzmutationen und Hepatitis B Genotypen. Die vorliegende Studie untersucht das Verhältnis zwischen diesen Mutationen und den Hepatitis B Genotypen A-D. Die Datenbank der US-amerikanischen Kongressbibliothek (Pubmed) wurde nach den Begriffen „HBV OR hepatitis B”, „YMDD”, „genotype”, und „lamivudine” durchsucht. Alle in dieser Suche gefundenen Arbeiten, die bis Juni 2009 veröffentlicht worden waren, wurden in die Studie eingeschlossen. Die Ergebnisse der Literaturanalyse wurden mit den Hepatitis B-Genomdaten zweier Referenzlabore in Tübingen und Melbourne verglichen. Insgesamt konnten 29 Arbeiten aus der Datenbankrecherche in die Literaturanalyse eingeschlossen werden. Diese Studien enthielten Daten zu insgesamt 827 Patienten, deren Hepatitis B Genotyp bekannt war und die eine Lamivudinresistenzmutation aufwiesen. In statistischen Untersuchungen konnte nachgewiesen werden, dass die rtM204V-Mutation die dominierende Mutation bei Infektionen mit Genotyp A ist. Dieses Ergebnis konnte durch die Analyse der Genomdaten der Referenzlabore bestätigt werden. Ferner konnte gezeigt werden, dass bei den Genotypen A, B, und D die rtL180M-Mutation hochsignifikant mit der rtM204V-Mutation verknüpft ist. Die Dissertationsschrift enthält neben dem Artikel „Mutation pattern of lamivudine resistance in relation to hepatitis B genotypes: hepatitis B genotypes differ in their lamivudine resistance associated mutation pattern“ (Damerow, H, Yuen L et al.; J Med Virol. 2010 Nov; 82(11):1850-8) eine Einführung in die Rationale der Studie, eine Zusammenfassung der Ergebnisse sowie ein Fazit.
