There have been various approaches to the treatment of nonorgasmic women, including psychoanalysis, desensitization, relaxation, masturbation, and group therapy. The present study was conducted to examine the efficacy of group therapy combined with home-based training in the treatment of primary nonorgasmic women. A no-treatment control group was also employed. Treatment consisted of two weekly 1- hour group sessions for 5 weeks. Educative processes were employed, such as detailed information on physiology of female sexual response. Structured homework exercises were also utilized, such as mastubatory techniques, role-playing orgasm, strengthening vaginal muscles, and assertiveness training in sexual and nonsexual situations. Results indicated an 88% success rate in the treatment group and no change in the control group.

Bien que l’innocuité de la pratique de l’activité physique (AP) ait été acceptée dans les myopathies y compris dans les dystrophies, il existe très peu de preuve de pertinence dans la littérature. Ce travail de thèse a permis d’apporter pour la première fois les preuves confirmées de l’innocuité et des effets bénéfiques de la thérapie par l’exercice dans le cadre de la dystrophie facio scapulo humérale. Ceci à travers une vision intégrée de ces bénéfices potentiels émanant d’évaluations conjointes fonctionnelles, tissulaires et de qualité de vie. Deux groupes ont été formés par randomisation : groupe contrôle (CG : 44 ± 10ans) et groupe entraîné (TG : 40 ± 13ans). Au terme de 24 semaines d’un entraînement mixte adapté, supervisé et coaché à domicile, les bénéfices fonctionnels importants ont été rapportés notamment en terme de capacité aérobie (VO2pic, PMA), de force (MVC) suivi d’une augmentation de la surface de section (CSA) des fibres musculaires, de fonction musculaire (endurance musculaire, TM6) et de fatigue ressentie par les patients. L’absence de dommages musculaires et la forte tendance à l’amélioration de la qualité de vie confortent l’idée de l’innocuité de notre programme d’AP. La biologie du muscle révèle aussi des améliorations de l’activité de certains enzymes du métabolisme oxydatif (CS et CK). En somme, ce programme d’entraînement mixte, supervisé et coaché à domicile a permis d’aboutir à des améliorations fonctionnelles, tissulaires et de qualité de vie, ce qui ouvre des perspectives d’application à d’autres types de myopathies / Although it is now accepted that physical activity (PA) is not deleterious in myopathies, including muscular dystrophies, there is very little evidence of relevance in the literature. This thesis has allowed to provide for the first time, confirmed evidence of safety and beneficial effects of exercise therapy in FSHD, through an integrative view of the potential benefits of such programs on functional, biological and quality of life. Two groups were randomly formed: control group (CG: 44 ± 10 years) and trained group (TG: 40 ± 13 years). After 6-month of adapted and home-based training, benefits have been reported particularly in terms of aerobic capacity (VO2pic, PMA), strength (MVC) followed by an increase in cross-sectional area (CSA) of muscle fibers, muscle function (muscle endurance, 6MWT) and fatigue experienced by patients. The lack of muscle damage (HES analysis & plasma CK values) and the strong tendency to improving the quality of life, support the idea that our training program is safe. Muscle biology also shows improvements in the activity of some oxidative enzymes (CS and CK). In short, this home-based mixed training program has allowed to achieve functional, tissue and quality of life improvements, which opens perspectives for application to other types of myopathies

Effets de l'entraînement à haute intensité associé au renforcement musculaire sur les capacités physiques et la qualité de vie chez les patients atteints de sclérose en plaques / Effects of high-intensity training combined with resistance training on physical capacities and quality of life in multiple sclerosis patients

Après avoir été contre-indiquée, l’activité physique prend une place de plus en plus importante dans la prise en charge de la sclérose en plaques (SEP). L’entraînement à intensité faible ou modérée est largement documenté, contrairement à la haute intensité. Ce travail s’est intéressé aux effets de l’entraînement en intervalle à haute intensité combiné au renforcement musculaire durant 12 semaines chez 26 patients atteints de SEP. Nos résultats indiquent que la consommation pic d’oxygène, la puissance maximale tolérée, la force isocinétique des quadriceps et ischio-jambiers, ainsi que la qualité de vie sont améliorées. Les femmes présentent des améliorations plus importantes et plus nombreuses que les hommes, cependant le niveau de handicap ne semble pas limiter les améliorations. Notre travail a permis de démontrer que l’entraînement en intervalle à haute intensité combiné au renforcement musculaire est bien toléré et permet des améliorations des capacités physiques et de la qualité de vie. / After being contraindicated, physical activity take an increasingly important place in the management of multiple sclerosis (MS). The training at mild to moderate intensity is widely documented, contrary to the high intensity training. This work has focused on the effects of high intensity interval training combined with resistance training during 12 weeks in 26 MS patients. Our results show that peak oxygen consumption, maximum tolerated power, isokinetic muscle strength in both quadriceps and hamstrings and quality of life are improved. Women show more important and more numerous improvements than men, however, the disability level does not seem to limit improvements. Our work has demonstrated that high intensity interval training combined with resistance training is well tolerated and allows physical capacities and quality of life improvements.

Vliv sensomotorického tréninku na rovnováhu u zdravých dospělých subjektů v krátkodobém terapeutickém plánu / The role of sensomotoric training on balance in healthy adult subjects if included in short therapy plan

Goal The goal of our pilot study was to evaluate the effect of short-term home based sensomotoric Training program (six weeks) in healthy adults on dynamic postural stability. We used the SOT at baseline and post the training program to evaluate the effect. Methodology The number of participants in this experiment was 10 healthy students, the number of males was 8 and 2 females, they were all between the age of 23 and 40 (mean 29.5 and ± S.D = 5.1 years). The sensory organization test was conducted at baseline and post the home based sensomotoric training program by using the dynamic posturography SMART Balance Master System (NeuroCom). The somatosensory (SOM), visual (VIS), vestibular (VEST), preference (PREF) and composite equilibrium outcome measures of the sensory organization test were evaluated. Results The VIS sensory system has significantly improved also the VEST sensory system with p-value < 0.05. The SOM sensory system had no significant improvement also the PREF sensory system with p-value > 0.05, while the overall composite equilibrium had a significant improvement with p-value > 0.05. Conclusion According to our findings the short-term home based sensomotoric training program has a positive effect in terms of improving the overall dynamic balance in healthy adults.
