Thesis deals with ?the issue of awareness of nosocomial infections from undergraduate students of Bachelor and Master degree from Health education field at the University of South Bohemia in the Czech Budejovice.? This is very timely topic as nosocomial infections are a global economic and clinical problem not only for the present time but also for the future. Thesis is divided into two parts. Details of nosocomial infections, epidemic processes, agents of these infections, hand hygiene, prevention, health education, healthy lifestyle, immunity and support options for immune system are described in the theoretical part. The practical part describes the research problem, formulates hypothesis and shows statistical methods for research. The aim of this thesis was to find out the awareness of nosocomial infections from students of Bachelor and Master degree of VKZ at JU in the Czech Budejovice. Further I tried to compare this knowledge with respect to the age, education and field in which they act. I am trying to find out the possible way of prevention, personal protection against nosocomial infections moreover subjective perception of information related to this problem from respondents. I used the basic way of collecting data for achievement the given aim such as questionnaire in the aim group. There were used basic statistical methods suitable for analysis of categorical data. These results are illustrated in pie chart and bar chart for better interpretation. Test of the main hypothesis and working hypotheses are statistically made by 2 ? test. There was used Fisher factorial test in case there would be break prerequisite for correct usage of the previous test. The results of these tests are interpreted on the base of , with 95 % of reliability. I set this hypothesis in my thesis: ? Better knowledge will have students whose high school education was connected with the problems of health care. ? The result shows better knowledge for these students than for students from other high school however this difference was not statistically important. From all tests was found out that all students know the right definition of nosocomial infection however they have significantly less information related to the prevention against nosocomial infection. This thesis should give us food for thought about this problem and should change the attention to the health and the health of others and the possibility of further education.

Problematika infekcí spojených se zdravotní péčí u vybraných ošetřovatelských činností / Problems of healthcare-associated infection in selected nursing activities

The diploma thesis deals with problems of healthcare-associated infection in selected nursing activities. The thesis is a theoretical one; it has been processed by means of review and synthesis. In this thesis, four objectives were set, namely to create a comprehensive overview of the problems of healthcare-associated infection in selected nursing activities. Other objectives were to map and analyze clinically relevant sites of transmission and possibilities of prevention of healthcare-associated infection in selected nursing activities and to propose recommendations for the prevention of these infections. The thesis is divided into several chapters that deal with the characteristics of healthcare-associated infection, focusing on the causative agents, formation, spreading and prevention. It also deals with particular nursing activities. The first chosen activity was the care of medical devices intended for repeated use, with a particular focus on the implementation of their disinfection. Secondly, hand sanitation as a basic factor affecting the transmission of healthcare-associated infection activity was selected. This chapter deals primarily with the hygienic hand disinfection, its indications, methods and implementation of disinfection products. It also deals with strategies for improving hand hygiene and glove use in providing nursing care. As the third nursing activity, aseptic techniques, which are an essential part in providing nursing care, were chosen. The thesis describes three specific clinical nursing procedures: aseptic collection of venous blood from a peripheral vein, preparation and administration of a drug into an existing peripheral venous catheter, and uncomplicated (aseptic) wounds dressing. Processing of the thesis was based on a range of relevant sources from the Czech Republic but also from abroad. It also contains various recommendations of national and international institutions. The thesis can serve as a basis for more effective prevention of healthcare-associated infection in certain areas.

Kvalita ošetřovatelské péče na vybraném oddělení následné péče / The quality of nursing care at selected department subsequent care

Current state: The goal of nursing is a quality of nursing healthcare. Each department has its own specific features and therefore has its own quality indicator. This work focuses on two quality indicators related to healthcare, specifically on patients with MRSA infection and their falls. Methodology: In the research were applied three methods of data collection: 1) statistical data from the hospital system; 2) intentional structured observation; 3) structured interviews. Research file: In order to evaluate the incidence of falls and cause of falls, were examined a total of 85 patients and a total of 98 patients for evaluation of decolonized patients with MRSA infection. The investigated group of people for observation and in-depth interviews were ONP nurses from the 6th floor of České Budějovice Hospital, plc. Results: The hypothesis that the most common cause of the fall is leaving the bed without any accompaniment has not been confirmed and the incidence of falls at the patients throughtout the observation period of the year 2015 dropped down significantly in comparison with the year 2014. The most common source of MRSA infection was a wound. Also, has not been confirmed the hypothesis that the number of decolonized patients with MRSA infection who were observing in the year 2015 will be statistically higher than in the year 2014. Most nurses know the cause of MRSA infection, but the problems they have are the precise definitions. By observing the practical activities and skills of nurses during the treatment of patients with MRSA infection were revealed certain shortages, especially the improper use of protective equipment and the improper disinfection of aids after their use. Conclusion: The results of the research show the need for more nursing education in this subject. Adverse events are a serious complication of the nursing process. The number of falls as well as the number of decolonized patients with MRSA infection is not successful significantly decrease in comparison year-on-year. The research also found inadequate theoretical preparedness of the nurses in the area of infectious diseases and a certain discrepancy between verbally presented experiences and the results of observation real activities.

The main aim of this thesis is to investigate the role of Impl2 gene in the regulation of immune response to bacterial infection in Drosophila melanogaster. The theoretical part provides a conceptual background concerning immunity in drosophila and also it summarizes what is known about the Impl2 gene. In the practical part, the function of Impl2 gene during infection is examined, starting with the location of its production during immune response, the changes of Impl2 gene amount during infection and its essentiality for a proper immune response. Furthermore, it examines the metabolic changes underlying an immune activation in connection to Impl2 gene. Last but not least, it tests how Impl2 gene is connected with the mobilization of energy during Chill Coma Recovery assay.

Okrasné parkové dřeviny zásobárnou virů čeledi Rhabdoviridae / The ornamental park tree species as a resource of the Rhabdoviridae family viruses

Rhabdoviridae family viruses attacking the plant hosts were only described at the angiosperms. In this work, a gymnosperm rhabdoviridae infection was described for the first time ever ? specifically at Ginkgo biloba. Even though there were not observed any kinds of obvious infection symptoms on any of randomly chosen plant samples, through the molecular methods and detection primers the rhabdoviridae infection was proved at six of the plant samples. The acquired nucleotide and amino acid sequences, which were compared with the GenBank sequences, confirm the Rhabdoviridae family viruses occurrence. These given sequences demonstrated a certain analogy with a Strawberry crinkle virus assigned to the genus of Cytorhabdoviruses. The analyses proved a different reciprocal homology among the nucleotide sequences of the individual isolates, and in all likelihood an occurrence of two up to now unknown viruses in the Ginkgo biloba samples was proved for the first time. A definite categorization will be dependent on an acquisition and comparison of other sequences from the isolates genome and also on certain biological characteristics observation.

The diploma work concerns the actual problem of papilomavirus infection and the vaccination against its originators. The gravity lies in the human-organism attack and the further proceeding changes in the human cells and mucous membranes. In many cases this leads to the pre-cancer states or even worse, to the cervix carcinoma. The work is divided into two parts, first theory, and second practice. The aim was to evaluate the vaccination against papilomviruses at the female-students of the University firstly, secondly to map the topic knowledge, and thirdly to monitor if the female-students visit their gynaecologist on regular basis. The theory concerns all the epidemiology, virology, clinical and diagnosis aspects of the papilomavirus infections, including the vaccination as a prevention possibility. In practice, there were run two anonymous questionnaire searches, first aimed on the female-students, second on the male-students only. The questions were focused on the level of the topic knowledge and the level of the both sex students vaccination, including the reasons of their non-interest in the possible vaccination, next the frequency of the girls? screenings in the gynaecologist practices. The research proved certain level of the respondents? topic knowledge and compared the existing differences among faculties. Another research conclusion is that the vaccination of the female-respondents is not very high, only 23,75% has already been vaccinated against the virus. Many young ladies underestimate the vaccination, and have more or less serious reasons why not to have them vaccinated. More than half of the respondents don?t do so because of the vaccine high price. This work should serve the women and young ladies, but also the young men as a source of the needed information. This means to improve the virus knowledge itself, its means of transmission, the diagnosis process or the treatment possibilities; overall the ways of primary prevention, with the ways of infection expanding prevention.

Mládeží uplatňovaná prevence vybraných alimentárních nákaz v regionu Milevsko / The Youth of selected infectious alimentary disease preventiv in the region Milevsko

Foodborne diseases form a large group of infectious diseases. These substantial diseases are occurring more in developing countries but they do not avoid developed countries. The originators of the disease are bacteria and their toxins, viruses, parasites and prions, whose source is an infected person or animal. The disease is passed on by contaminated soil, contaminated water and food, sometimes contaminated items. Greatest importance in the prevention of foodborne disease are nonspecific measures, protection of drinking water, safe food production, waste disposal, functional sewer system, rodenticides and insecticides measures and of course the principles of personal hygiene, certain standards of hygiene and health education of the population. Intestinal infections are preventable. The consumer can effectively protect by following the fundamental principles established by the World Health Organization. The aim of this study was to explore knowledge of foodborne disease prevention measures and compliance with these principles by adolescents in the Milevsko region. The theoretical part of the study summarizes the basic information about intestinal diseases and describes their effect on health from a variety of perspectives. The preventive measures which everyone should be aware of and apply them in everyday life to protect health are described in great detail. The paper summarizes the information about the process of spreading foodborne diseases and epidemiological measures focused on supply, routes of transmission and susceptible individuals. The theoretical part includes basic information about specific intestinal infections which are most significant for the population in the Czech Republic. Briefly described are treatment of these diseases and the most important legislative measures. In the research part quantitative research was applied by questionnaire. The research groups were students of higher secondary vocational schools and vocational schools in the region Milevsko. Three hypotheses were formulated. Statistical hypothesis testing was performed using the t test. The values of the achieved significance level are compared with a predetermined level of significance. The first hypothesis assumed that knowledge about prevention of intestinal diseases is higher among girls than boys. This statement has not been confirmed. Knowledge of the principles of prevention does not differ according to the criteria of gender. Two further hypotheses assumed that the observance of hygienic measures in the prevention of foodborne disease is higher among girls than among boys and higher for high-school graduates than apprentices. These hypotheses are valid. Precautionary principles are indeed respected more by girls than boys and high-school graduates than apprentices. The carried out research shows that adolescents in the region Milevsko have a good knowledge of foodborne infections and do known also essential preventive measures. However, knowledge of these rules does not mean they are applied in practice. The results showed that some rules, such as washing fruits and vegetables before eating, adequate boiling or roasting of food, especially hand hygiene before eating food or after contact with an animal are observed by a smaller percentage of respondents than that reported knowledge of these principles. A positive finding is that respondents comply with certain rules, without being aware of them, such as the principle of proper preservation of precooked food. Foodborne diseases are diseases which can be prevented in compliance with precautionary principles. Especially in the Czech Republic, where nonspecific principles are at a high level. It is up to each user/consumer how consistent these principles will be. Respecting preventive measures is a prerequisite for reducing the incidence of foodborne disease in the population.

The present thesis focuses on nosocomial infections and their causes. The theoretical part summarizes information on especially the sources of these infections, the way of their transfer and the most frequent infection agents. Clostridium difficile is studied in more detail. The illness caused by this pathogen leads to the complications especially with in-patients with a previous antibiotic therapy and immuno-compromised individuals. And this is the problem the present thesis is also concerned with. The thesis also divides nosocomial infections by their clinical manifestations and contains basic anti-epidemic measures and surveillance methods. Its empirical part focuses on the description of an occurrence of select nosocomial infections in Strakonice Hospital, a.s., especially an occurrence of the Clostridium difficile pathogen and methicillin-resistant Staphylococcus aureus (MRSA).

4 Abstract The prevalence of obesity and its related cardiometabolic complications in children remains high across the world. Obesity is a multifactorial disease caused by interaction between genes and environmental factors. Genome-wide association studies have discovered several single nucleotide polymorphisms associated with obesity. A causal role of infection in the pathogenesis of obesity has also been considered, particularly the role of adenovirus 36 (Adv36). The aim of the Ph.D. thesis was to investigate the associations of obesity susceptibility loci (TMEM18, SH2B1, KCTD15, PCSK1, BDNF, SEC16B, MC4R, FTO) and Adv36 infection with obesity-related characteristics and complications in the Czech adolescent population. The results are described in eight publications, of which six are original papers and two are reviews. Studies were performed on a cohort of Czech adolescents recruited either from the general population (1,533 individuals from the epidemiological study) and from in-patient or outpatient weight management clinics (562 overweight/obese individuals underwent an intervention). The results demonstrated an association of TMEM18, SEC16B and FTO gene variants with obesity. Total hip and knee arthroplasty surgeries form an integral part of orthopaedic practice. With an increase of the primary arthroplasty surgeries performed, comes also an increase in the number of complications. The most common complications of these otherwise very successful procedures is periprosthetic joint infections which are also one of the most difficult to treat. In the first part of the study, we evaluated the primary bacterial resistance of the 14 most commonly used materials in the construction of joint prostheses. More specifically, we concentrated on how their surface treatment resists colonisation by specific bacterial species (Staphylococcus aureus, Staphylococcus epidermidis, Pseudomonas aeruginosa, Enterococcus faecalis and Escherichia coli). The studied materials included metal alloys that are commonly used in the weight bearing parts of implants - CoCrNo, FeNiCr and Ti6A14V - but also polymeric and ceramic materials used in the bearing materials, represented by ultra-high- molecular-weight polyethylene (UHMWPE) and aluminium oxide (Al2O3).
