Psykodynamisk eller kognitiv beteendeterapeutisk behandling över internet vid social ångest: en preferensstudie / Psychodynamic or cognitive-behavioral therapy for social anxiety disorder: a pilot preference study.

Social ångest drabbar omkring en av nio svenskar. Effektiva behandlingar finns, med begränsad tillgång och höga kostnader gör att många inte får behandling. Internetbehandling är ett potentiellt kostnadseffektivt behandlingsalternativ som är lätt att sprida, men mer forskning behövs. Syftet med studien var att jämföra två internetförmedlade behandlingar. Trettiosex deltagare fick välja affektfokuserad psykodynamisk behandling (iPDT, N=23) eller kognitiv beteendeterapi (iKBT, N=13). Behandlingarna bestod av tio veckor vägledd självhjälp. Liebowitz Social Anxiety Scale-Self Report var det primära utfallsmåttet. Behandlingarna hade medelstora förbättringar på social ångest och depression (d = 0,51–0,83), samt stora försämringar på interpersonella problem (d = 1,13–1,43). Fler av de som valt psykodynamisk behandling slutförde hela behandlingen. Allians efter tre veckor och preferensstyrka visade sig vara lovande utfallsprediktorer. Resultaten tyder sammanfattningsvis på att behandlingarna har positiva effekter på symtom av social ångest samt negativa effekter på interpersonella problem. / Social anxiety disorder affects approximately one out of nine swedes. Effective treatments exist, but limited access and high costs causes many sufferers to remain untreated. Internet-delivered treatment is a potentially cost-effective treatment alternative, which is easy to disseminate, but more research is warranted. The aim of the study was to compare two internet-delivered psychological treatments. Thirty-six participants choose affect-focused psychodynamic treatment (iPDT, n=23) or cognitive behavioral treatment (iCBT, n=13). The treatments consisted of ten weeks of guided self-help. Liebowitz Social Anxiety Scale-Self Report was the primary outcome measure. The treatments caused moderate improvements in social anxiety and depression (d = 0,51–0,83), as well as large worsening of symptoms of interpersonal problems (d = 1,13–1,43). Adherence to treatment was higher in the iPDT-group. Alliance at three weeks and preference strength were promising predictors of change. In summary, the results suggest that both treatments have positive effects on symptoms of social anxiety and negative effects on interpersonal problems. / SOFIA
