Μεταβολή της ενδοφθάλμιας πίεσης μετά από μακρυχρόνια χρήση ρινικών ψεκασμών

Η επίδραση της συστηματικής λήψης κορτικοστεροειδών στην ενδοφθάλμια πίεση είναι επαρκώς τεκμηριωμένη και εξαρτάται από σημαντικό αριθμό παραγόντων. Αντίθετα, ελάχιστες και αντικρουόμενες πληροφορίες υπάρχουν για την επίδραση των ρινικών κορτικοστεροειδών στην ενδοφθάλμια πίεση. Σκοπός της μελέτης ήταν η διερεύνηση της επίδρασης του ρινικού κορτικοστεροειδούς fluticasone υπό μορφή ρινικών ψεκασμών στην ενδοφθάλμια πίεση σε ασθενείς που έπασχαν από αλλεργική ρινίτιδα. Η ενδοφθάλμια πίεση μετρήθηκε πριν και κάθε πέντε ημέρες από τη χορήγηση του φαρμάκου. Η στατιστική ανάλυση δεν αποκάλυψε σημαντικές διαφορές, γεγονός που φανερώνει ότι το συγκεκριμένο κορτικοστεροειδές δεν επηρέασε την ενδοφθάλμια. / The effect of systemic administation on intraocular pressure is well established. However less attention has been paid to the effect of steroids when administered in a nasal spray. We conducted a study to investigate a possible association between nasal steroids and elevated IOP in 54 patients who were being treated for allergic rinitis. IOP was measured before the patients started therapy and thereafter every five days during that therapy. Follow up ranged from 27 to 35 days. Statistical analysis revealed no significant elavation in IOP after nasal steroid administration.

Pressão intra-ocular, condições endoteliais e prostaglandina E2 no humor aquoso de cães com catarata senil madura ou hipermatura /

Orientador: José Luiz Laus / Banca: Adriana Morales / Banca: Alexandre Lima de Andrade / Resumo: Estudaram-se a pressão intraocular, a densidade e a hexagonalidade de células endoteliais, níveis de Prostaglandina E2 e sua correlação com catarata senil madura (n=8) ou hipermatura (n=8). Adicionalmente, foram utilizados valores de Prostaglandina E2 do humor aquoso de cães oftalmologicamente saudáveis (n=7). Para avaliação da pressão intraocular empregou-se tonometria digital de aplanação. A avaliação da densidade e da hexagonalidade das células endoteliais foi realizada por meio de microscopia especular de não contato. Sob anestesia geral, 0,2mL de humor aquoso foram colhidos por meio de paracentese da câmara anterior. As amostras de aquoso foram congeladas e armazenadas à -80 oC para mensuração da concentração de PGE2, por imunoensaio enzimático competitivo. Os valores foram comparados estatisticamente, empregando-se análise de variância de via única, e o teste de múltiplas comparações de Bonferroni. Teste t não pareado foi utilizado para se avaliar os dados obtidos por microscopia especular. Possíveis correlações entre os resultados foram comparadas aplicando-se o teste de Person. Adotou-se nível de significância de p<0,05. Em ambos os grupos observou-se diminuição significativa da pressão intraocular (PIO), entretanto não houve diferença entre eles (p = 0,90). As médias dos valores de densidade celular (CD) e de hexagonalidade (HEX) foram 2253,363 cell/mm² e 59,03% para GSM e 2058,325 cell/mm² e 63,75% para GSH, respectivamente. Entretanto não houve diferença significativa entre ambos, tanto para CD (p=0,201), quanto para HEX (p=0,61). Observou-se incremento nos valores de PGE2 nos grupos com catarata em relação aos cães oftalmologicamente saudáveis (p<0,001), entretanto, não houve diferença significativa entre os grupos com catarata... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Intraocular pressure, endothelial cell density and hexagonality, and aqueous humor prostaglandin E2 (PGE2) were studied and correlated in dogs with mature (n=8) and hypermature (n=8) senil cataratcs. In addition, aqueous humor PGE2 values of ophthalmic healthy dogs (n=7) were used. Non contact specular microscopy was used to evaluate density and hexagonality of endothelial cells. 0.2mL of aqueous humor was collected by means of anterior chamber paracentesis. Aqueous humor samples were frozen at -80 oC and concentration of PGE2 were quantified by means of competitive enzyme immunoassay. PGE2 values were statistically compared with one way analysis of variance and Bonferroni's multi comparison test. Specular microscopy data were assessed by means of unparied T test. Person's test was used in order to assess possible correlations among data. Significance was set at value of P < 0.05. Values of endothelial cell density and hexagonality did not change significantly between groups (P = 0.61). Investigating the Effect of a Micelle-Based Drug Delivery System in Reducing IOP and Glaucomatous Effects in a Partially Open Angle Mouse Model of Glaucoma

This project explores the use of a novel sustained release mucoadhesive micelle-based drug delivery system in combination with 0.005% latanoprost (LTP) on our partially open angle mouse model of glaucoma (AP-2β TMR-KO). We previously tested for LTP treatment in our model and found a reduction in intraocular pressure (IOP) 20 minutes following treatment. This information led us to investigate the long-term effect of LTP treatment and micelle loaded with LTP (MLTP) treatment in our model. We hypothesized that the MLTP treatment would be more effective in reducing IOP and preventing glaucomatous effects than LTP treatment alone in the AP-2β TMR-KO mice. The MLTP groups of animals (wildtype and mutant) were treated every 3 days, and this was compared with animals treated with LTP daily as well as animals treated every 3 days with LTP alone for comparison’s sake for 60 days. IOP measurements were taken every 3 days. Following long term LTP treatment alone, mutant mice showed a consistent decrease in their baseline IOPs with a significant reduction in baseline IOP at 35 days of treatment across all cohorts (P<0.0001). In comparison, mutants treated with MLTP exhibited an even greater reduction in baseline IOP following long term treatment. After the treatment period, mice were euthanized, and their eyes were enucleated, fixed, sectioned, and stained for retinal ganglion cells (RGCs) using Brn3a. Mutant mice exhibited a significant decrease in RGC cell number when compared to wildtype, and this loss was not rescued by treatment with LTP. However, mutants treated with MLTP demonstrated significant RGC cell protection compared to eyes of untreated mutants, as well as everyday LTP treated mutants. / Thesis / Master of Science (MSc) / An effective treatment strategy is required to prevent irreversible blindness caused from glaucoma. Unfortunately, compliance with current medications is extremely poor, as they require frequent administration due to their low ocular bioavailability and short-term effect. As such, this thesis aims to explore an alternative drug delivery approach in a partially open angle mouse model of glaucoma to prevent the worsening of glaucoma and ultimately improve patient compliance.

Latanoprost reduced the IOP 1.6 mm Hg more than timolol. The IOP-reduction was maintained with timolol and slightly enforced with latanoprost up to 6 months of treatment. The mean IOP-reduction was maintained during 2 years of latanoprost treatment. The overall risk of withdrawal due to insufficient IOP-reduction with latanoprost was 8%. </p><p>The statistical methodological issues are of a general and reoccurring character in trial design of the IOP-reduction: should the statistical hypothesis testing be based on the mean intraocular pressure (IOP) or the proportion of patients who reach a specific IOP level, should the estimate of the IOP or IOP-reduction be based on single eyes, mean of bilaterally eligible and identically treated eyes or the difference between an eye with active treatment and a placebo treated contralateral eye, and is mean of replicated recordings useful? Statistical methodological conclusions: the most effective response variable varies with the selected patient population. Therefore, the trial design process should include a comparison of the variability, test power and required sample size for the possible response variables in a sample of the target population. At minimum a statistical consideration should be done.</p>
