Aim: The aim was to evaluate the effect of laser therapy combined with collagen sponge scaffold in the healing of a cranial critical defect in rats. Methods: Experimental study, prospective, in vivo. Forty five Wistar rats were divided into 5 groups. The G0 group was the positive control group ( with defective and no treatment); The group G1  with defective was treated with collagen sponge; the group G2  with defective was treated only with laser ; The G3 group with defective was the group treated with collagen and laser sponge; Group C  was the negative control group ( no bone defect and without  treatment). The laser was used at a dosage of 120J / cm2 (50 mW power, exposure time 10 sec), wavelength 780nm, applications performed on 4 points after the procedure, and every 48 h twice. The defect of 5mm was held in the skull, and after 21 and 30 days the evaluations were made by x-ray, histological slides and immunohistochemistry (RANK, OPG, TGF-β and FGF-2).  Results:  Through the radiographic findings the group had more radiopaque area was the G3 30 days. For histological analyzes the G3 30 days had more new bone formation, with less inflammatory infiltrate and large amount of osteoid. For immunohistochemical analysis were used only G3 blades 30 days and G0 30 days because there was a significant difference between these groups, and there was a higher markup OPG, indicating the reconstruction of bone tissue.  Conclusion: It was observed that the combined low-level laser therapy to the collagen scaffold showed positive results for bone regeneration within 30 days.
