Interakce lidského patogenu Bordetella pertussis s krevním sérem / Interaction of the human pathogen Bordetella pertussis with blood serum

Bordetella pertussis is a Gram-negative strictly human pathogen and the major causative agent of whooping cough or pertussis. The incidence of this highly contagious respiratory disease in developed countries has increased in the last decades. One of the less characterized virulence factors of B. pertussis is the type three secretion system (TTSS) which is responsible for the secretion of the effector proteins into host eukaryotic cells. This diploma thesis sheds light onto factors influencing TTSS in vitro activity. Although TTSS of laboratory strain Tohama I was induced by biologically active compounds present in blood (e. g. complement proteins), TTSS of recent clinical isolate B1917 seems to be induced permanently. Furthermore, BB0302 encoding a GntR family transcription regulator in B. bronchiseptica RB50 (homologous to BP0209 of Tohama I) was studied, however, the deletion of this gene did not affect the TTSS functionality. Serum resistance is a factor that plays a key role in the pathogenesis of B pertussis. We show that Czech recent isolates (2008-2015) are significantly more resistant to serum killing in vitro than the original vaccine strains (1954-1965). This work is part of a project focused on the study of the variability of human cytomegalovirus (HCMV) among clinical isolates with the aim to map the geographical distribution of HCMV genotypes, reveal the relationships between genotypes and the severity of HCMV-associated diseases, and identify regions in the HCMV genome with a potential for use as diagnostic and therapeutic targets. Attention was paid to the development of the methodology for the preparation of the material for next-generation sequencing (NGS) from HCMV clinical isolates and evaluation of the obtained sequencing data. Blood and urine samples collected from hematopoietic stem cell transplantat recipients and congenitally infected children were analyzed. Samples suitable for NGS were sequenced by the Illumina platform and sequences were created by de novo assembly followed by mapping assembly. Urine samples in comparison to blood samples had higher yield of material for NGS. Of the samples positive for HCMV DNA (7 of 50) after amplification in the cell cultures, only one sample had high purity of the viral DNA (98%) while six samples had purity of less than 7%. This master's thesis deals with the problems of anomalies detection in data from clinical trials and medical registries. The purpose of this work is to perform literary research about quality of data in clinical trials and to design a personal algorithm for detection of anomalous records based on machine learning methods in real clinical data from current or completed clinical trials or medical registries. In the practical part is described the implemented algorithm of detection, consists of several parts: import of data from information system, preprocessing and transformation of imported data records with variables of different data types into numerical vectors, using well known statistical methods for detection outliers and evaluation of the quality and accuracy of the algorithm. The result of creating the algorithm is vector of parameters containing anomalies, which has to make the work of data manager easier. This algorithm is designed for extension the palette of information system functions (CLADE-IS) on automatic monitoring the quality of data by detecting anomalous records.

The thesis focuses on the use of Evidence Based Medicine (EBM) methods and principles at the Psychiatric Clinic of the First Faculty of Medicine of Charles University and the General University Hospital in Prague. There is a theoretical introduction to EBM - its origin and development, EBM information resources as well as general basic principles of this approach to work with clinical evidence. Part of this work introduces the history and current activities of the Psychiatric Clinic and the related Psychiatric Association of the Czech Medical Association of J. E. Purkyně. The practical part of the thesis describes the preparation of research and its results. Based on semi-structured interviews with physicians of the Psychiatric Clinic, the results of this research represents their current attitude towards EBM and use of EBM methods in their daily work. The aim of the thesis is also a proposal to raise awareness of EBM information sources in the form of information materials and educational seminars.

Computational Fluid Dynamic Simulation of Intracranial Aneurysms Analysis of time-dependent changes of hemodynamic parameters - the road the clinical use Hemodynamics are involved in the genesis of intracranial aneurysms and time- dependent changes of their parameters lead to aneurysm growth, stabilization or rupture. Definition of these changes using computational fluid hemodynamics could significantly contribute to the understanding of aneurysmal development and rupture and could enable the routine use of mathematical simulations. In this study, computational fluid dynamics were performed for nine incidental aneurysms. Five aneurysms were monitored throughout time and factors leading to aneurysm rupture were analyzed. In four aneurysms the influence of the hemodynamics on the growth was defined. Major growth occurred in areas of low wall shear stress and oscillatory index. These areas increased in size during growth time. Contrary to this, neck shape remodeling occurred in areas with large wall shear stress and pressure. Throughout the follow-up of ruptured aneurysms, the minimal wall shear stress decreased, and the area of low wall shear stress increased significantly. Title: The effect of physiotherapy on balance in multiple sclerosis Objectives: The aim of this study is to compare efficacy between ambulatory individual physiotherapy and complex inpatient rehabilitation in people with multiple sclerosis on balance and subjective evaluation of general health. In ambulant program to compare effect of different physiotherapeutic methods (Motoric program activating therapy, Manual physiotherapy correction, Reflex Locomotion according to Vojta). Methods: A combined design of study. Ambulant program has randomised assessor blind design, in which subjects were divided into one of the three groups of ambulatory physiotherapy. A clinical study without control arm design is used in case of comparison between complex and ambulatory program. Results were analysed in 45 patients with multiple sclerosis (25 subjects of ambulatory program, 20 subjects of complex program). Subjects of the ambulatory program were divided into one of the three groups of ambulatory physiotherapy (Motoric program activating therapy, Manual physiotherapy correction, Reflex Locomotion according to Vojta), in which they absolved during two months 20 therapies (2-3 times per week, length 60 minutes).
