Estudo genético-clínico de 144 pacientes portadores de deficiência auditiva não sindrômica\". / Clinical-genetic study of 144 patients with nonsyndromic hearing loss

A deficiência auditiva constitui uma importante categoria de defeitos congênitos que podem se manifestar isoladamente ou fazer parte do espectro fenotípico de várias síndromes. O presente trabalho refere-se ao estudo genéticoclínico de pacientes portadores de deficiência auditiva não sindrômica, com os objetivos de estabelecer a razão sexual, tipo, grau, simetria, lateralidade e progressão da deficiência auditiva; determinar diagnósticos etiológicos prováveis da deficiência auditiva, estabelecendo, quando possível, o padrão de herança genética e fornecer meios para o aconselhamento genético. Foram avaliados 306 pacientes no serviço de Genética Clínica do Centro de Atendimento aos Distúrbios da Audição, Linguagem e Visão do Hospital de Reabilitação de Anomalias Craniofaciais-USP, no período de dezoito meses. Dos 306 pacientes avaliados, 278 eram portadores de deficiência auditiva, 27 portadores de distúrbio de linguagem e 1 portador de deficiência visual. De acordo com o critério estabelecido (presença de deficiência auditiva isolada), 144 pacientes foram selecionados para este estudo, através da história clínica, história familial, dados pré, peri e pós-natais e audiometria (dados de prontuário). A análise estatística não mostrou associação significativa entre a ocorrência de deficiência auditiva não sindrômica e o sexo dos propósitos. Dos 144 pacientes estudados, 95.8% dos casos tinham etiologia genética e 4.2% etiologia ambiental. Nos portadores de deficiência auditiva de etiologia genética, o padrão de herança indefinido (53.6%) foi o mais freqüente, seguido o de herança autossômica recessiva (23.9%) e autossômica dominante (22.5%); as anomalias maiores e menores associadas à deficiência auditiva não estavam relacionadas com esta deficiência. O tipo de perda auditiva predominante foi o neurossensorial (99.3%) e deficiência auditiva severa a profunda foi a mais freqüente (34.8%). Da amostra, 68.1% dos casos apresentaram perda auditiva pré-lingüística, 54.9% apresentaram perda auditiva assimétrica, 95.8% perda auditiva bilateral e 95.1% perda auditiva não progressiva. O estudo permitiu concluir que história clínica e familial são informações importantes no diagnóstico etiológico da deficiência auditiva; que as configurações da curva audiométrica são avaliações de apoio ao esclarecimento diagnóstico da deficiência auditiva; que o aconselhamento genético é dificultado nos casos com padrão de herança indefinido e que análises moleculares são dados subsidiários à definição etiológica da deficiência auditiva. / Hearing loss constitutes an important category of congenital defects that can be expressed isolately or making part of the phenotypical spectrum of several syndromes. The aim of this study is to establish the sex-ratio, type, degree, symmetry, laterality, progression , and to find out the etiology of non syndromic hearing loss, establishing, when possible, the inheritance pattern providing ways to genetic couseling. During 18 months, 306 patients were evaluated at Centro de Atendimento aos Distúrbios da Audição, Linguagem e Visão of the Hospital de Reabilitação de Anomalias Craniofaciais-USP. Nonsyndromic hearing loss was observed in 144 of them; syndromic hearing loss in 134; only speech impairment in 27, and only visual impairment in 1. According criteria only the 144 patients with nonsyndromic were selected for this study. There were 83 females (57.6%) and 61males (42.4%); genetic etiology was found in 95.8% of the sample and environmental etiology in 4.2%; major and minor associated anomalies were not related with hearing loss. Sensorineural hearing loss was found in 99.3% and severeprofound hearing loss was the most frequent (34.8%). Others results were: asymmetrical hearing loss in 54.9%, bilateral hearing loss in 95.8%, and non progressive in 95.1%. This study allow to conclude that the familial and clinical data were important to define etiological diagnosis; the shape of audiograms were evaluations that help to clarify, but not define, the diagnosis. Genetic counseling is difficult when the pattern of inheritance is unknown; and molecular studies are important subsidiary test to define the etiology of the hearing loss.

The Effect of Rapid Weight Loss on the Efficiency of Wrestlers Under Simulated Competitive Conditions

The purpose of this study was to determine the effects of rapid weight loss on wrestlers' efficiency incorporating competitive aspects of the sport into the research procedures. Six subjects were selected from the North Texas State University Wrestling Club. Eight cardiovascular and neuromuscular measurements were taken and recorded before treatment and following treatment. Astrand's bicycle ergometer test was utilized to obtain exhaustive, heart rates. Data were analyzed by a one-tailed t test at the .05 level. Conclusion of the investigation was that following 6.25 per cent rapid loss of body weight, over a period of one week, no loss of efficiency occurred among wrestlers under simulated competitive conditions.

The focus of this study was on traumatic suicide attempt due to lack of remuneration to the clergy by the church: a pastoral care perspective. The assumption was that no one in the church took responsibility of caring for clergy families. Trauma referred to a spiritual wound in the soul of humankind and it is a spiritual event when humankind was experiencing loss of faith, loss of hope, loss of trust, loss of meaning, loss of innocence and when the spiritual wound was being healed spiritually. Suicide was defined as purely personal act that appeared to be entirely the outcome of the extreme personal unhappiness of the minister who as a result of non-payment of their stipend by the church. The operational framework of research methodology included: the research design, literature review and interviews; data collection method and data analysis and interpretation. The Uniting Presbyterian Church in Southern Africa belongs to the Reformed family of the Protestant churches. In this project, semi-structured scheduled interview open-ended Questions were designed to allow interviewers free movement. Examination of Perceptions of Weight Loss and Weight Loss Methods

The purpose of this study was to examine individuals’ perceptions of weight loss and weight loss methods. Respondents were employees and customers attending health/wellness events at local supermarkets in June, July, and August 2019. Participants completed a survey to assess their understanding and perceptions of weight loss and methods used to achieve weight loss. The majority of participants were white females, 60 years or older with a college degree and a Body Mass Index (BMI) classified as overweight or obese. Data revealed most participants had a variety of misconceptions relating to weight loss and weight loss methods that were obscure and fad-diet oriented, including low-carbohydrate diets. Participants did understand benefits of weight loss but expressed maintaining weight loss was a challenge. These findings suggest that fad diets are alluring to individuals and individuals did not incorporate evidence-based behavior changes to promote or sustain weight loss.

Energy Losses in Cross Junctions

Solving for energy losses in pipe junctions has been a focus of study for many years. Although pipe junctions and fittings are at times considered "minor losses" in relation to other energy losses in a pipe network, there are cases where disregarding such losses in flow calculations will lead to errors. To facilitate these calculations, energy loss coefficients (K-factors) are commonly used to obtain energy losses for elbows, tees, crosses, valves, and other pipe fittings. When accurate K-factors are used, the flow rate and corresponding energy at any location in a pipe network can be calculated. K-factors are well defined for most pipe junctions and fittings; however, the literature documents no complete listings of K-factors for crosses. This study was commissioned to determine the K-factors for a wide range of flow combinations in a single pipe cross and the results provide information previously unavailable to compute energy losses associated with crosses. To obtain the loss coefficients, experimental data were collected in which the flow distribution in each of the four cross legs was varied to quantify the influence of velocity and flow distribution on head loss. For each data point the appropriate K-factors were calculated, resulting in over one thousand experimental K-factors that can be used in the design and analysis of piping systems containing crosses.

Runoff and soil loss under different tillage and cropping system practices at Ginchi Vertsol in Ethiopia

Published Article / To assess and predict runoff and soil loss on different tillage methods coupled with alternative cropping systems in the central highland vertisols of Ethiopia, a study was carried out at Ginchi, Agricultural Research Sub-Center during 1996. The experiment was conducted on runoff plots of 4 meter wide by 22 meter long, on surface slopes that range between 0.1% and 2.3%. The data collected was analyzed using regression models and an empirical formula developed by the Soil Conservation Service of America (SCS, 1964; 1972), known as curve number (CN). Both the regression model and the SCS simulated the mean daily runoff reasonably well with R2 93% and 83%, respectively. The overall results obtained explain that the improved tillage practice, BBF could drain the excess surface water safely.

Operational risk events in banks and practices for collecting internal loss data

This research study had two distinct objectives. The first objective was to determine in which areas in South African banks the most severe operational risk losses are likely to occur (based on the Basel II seven loss event types and eight business lines). Severity was assessed based on single operational risk events that might have significant monetary values attached to them. The likely frequency of single operational risk events was also assessed. The investigation of the aforementioned research problem was explorative and quantitative of nature, as the researcher made extensive use of survey research in the form of a questionnaire to all registered banks. The second part of the research study’s objective was to assess the range of practices in collecting internal loss data for operational risk purposes as required by Basel II. This part was approached from a qualitative perspective, by benchmarking the research findings against the Basel II text, the researcher’s experience in risk management in banks, the Basel Committee on Banking Supervision’s Sound Practices for the Management and Supervision of Operational Risk, and related literature. The literature review, including reference to certain surveys and studies, focuses on the main concepts of operational risk within banks that are pertinent to the research problem. The literature review also includes several references to the Basel II text and other relevant publications and papers issued by the Basel Committee on Banking Supervision. The research results revealed that respondents in South African banks believed that ‘business disruption and system failures’ is the loss event type that is likely to result in the most severe single operational risk loss. ‘Trading and sales’ scored the same high average rating as ‘business disruption and system failures’ as the business line where the most severe single operational risk loss is likely to occur in South African banks. ‘External fraud’ and ‘execution, delivery and process management’ scored the highest average ratings as the loss event types where the most frequent operational risk losses are likely to occur. Respondents indicated that ‘retail banking’ is the business line where the most frequent single operational risk losses are likely to occur in South African banks. Based on the above-mentioned findings the researcher recommends that these high-risk areas be highlighted to the Bank Supervision Department of the South African Reserve Bank, the boards of directors and senior management of banks in order for them to strengthen banks’ internal controls. The researcher recommends the inclusion of near misses and opportunity cost in operational risk loss databases. Banks should at least capture the date of the discovery of an operational risk event as this represents acceptable practice among the majority of banks. Operational risk losses should be assigned to the multiple business activities in which it occurred on a pro-rata basis. All recoveries of operational risk losses should be processed separately, but associated with the original loss event. Replacement cost is seen as the most appropriate way to capture gross loss amounts for the damage to fixed assets. The researcher encourages the recording of overtime cost for fixing systems failures. Market risk losses due to operational risk events should be treated as market risk losses, while loan-related losses due to operational risk failures should be treated as credit risk losses by banks. The researcher’s view is that banks should set different thresholds for the collection of operational risk losses for its various business units based on each business unit’s operations and nature of business. Banks should, as a starting point, map operational risk events to the Basel II 8x7 matrix. Operational risk losses should be assessed by both legal entity and on a consolidated basis.

Therapists and sense of self : themes of loss

This study constitutes an exploration of the role of the experience of loss in the social processes of meaningmaking - regarding self, other and the world - especially as it applies to therapists. Traditionally a focus on loss has entailed a description of the mourning processes brought about by this inevitable but dreadful event. Here the lens is widened to evolve a description of how the loss experience can contribute to a transformation of a person's sense of her self and of her patterns of relating. This description. rests on a social constructionist understanding of the experience of self. A person's sense(s) of self is seen to evolve within the usual and seemingly predictable patterns of connecting and disconnecting that constitutes the social webs of the discursive communities that people move in. Thus her sense of I is indelibly linked to 'Nho and 'Nhere her Yous are. Indeed, all meaning is proposed to evolve from a template of connection-disconnection patterns. The experience of loss is seen to be able to disrupt these seemingly stable patterns within such a community sufficiently, as to be able to bring about in depth transformation of the meanings evolving from these habits of relating. One nuance of these meanings in transformation, entails a person's sense of her self. Experiences of, and struggles around connection and disconnection are centrally important in the world of therapists. It does not only constitute a basic focus of their 'M)rk, but is also the template in their personal lives that contributes to their O\Ml evolution as therapists.
