Dermatopathology in Endocrine Disease

Through multiple complex mechanisms, the endocrine system plays a vital regulatory role in virtually every organ system including the skin. Cutaneous lesions may be seen incidentally in patients with endocrine disorders. Alternatively, certain skin conditions are seen exclusively in the setting of endocrine disorders and may rarely be the first clinical symptom of the underlying endocrinopathy. Although clinical examination is often sufficient, a tissue biopsy may still be needed to arrive at or confirm the diagnosis and to direct therapy. In many cases, the cutaneous condition might improve or resolve completely after treatment of the underlying endocrine disorder. In this review, we summarize the most common skin conditions associated with endocrine disorders, uncovering their relationship to the underlying endocrinopathy, demonstrating their clinicopathologic presentation, and highlighting their typical modalities of treatment.

La compatibilité des arrestations de masse avec le droit de manifester

Quoique le droit de manifester semble acquis au Canada depuis un certain temps, l’exercice de ce droit demeure précaire. Considérant la fréquence des arrestations de masse au Canada, une réflexion s’impose sur la conformité de telles interventions avec les droits et libertés de la personne prévus dans la Charte canadienne des droits et libertés. En précisant l’étendue de cette protection, il serait possible de tracer une limite pour mieux encadrer les arrestations de masse. Le développement du droit de manifester au Canada depuis l'adoption de la Charte canadienne sera abordé pour démontrer comment la manifestation a historiquement bénéficié d’une protection en vertu de l’al. 2 b) de la Charte canadienne, qui garantit la liberté d'expression. Partant de la prémisse que cette protection lui est mal adaptée, l’hypothèse sera avancée que le rattachement du droit de manifester à la liberté de réunion pacifique prévue à l’al. 2 c) de la Charte permettrait aux tribunaux de développer une protection qui y serait mieux adaptée. Après avoir établi la protection dont devrait bénéficier le droit de manifester au Canada, nous porterons notre attention sur les dispositions du Code criminel qui permettent les arrestations de masse. Il sera conclu que ces dispositions du Code criminel violent la liberté d’expression et la liberté de réunion pacifique et que l’utilisation d’arrestations de masse devrait être circonscrite aux cas les plus manifestes, soit ceux où il y a de la violence ou un danger réel de violence. Finalement, une réflexion sera effectuée quant à la possibilité d’appliquer l’article premier de la Charte pour préserver les dispositions du Code criminel qui permettent ces arrestations de masse. Considérant l’importance politique que devrait revêtir le droit de manifester, il sera conclu que ces dispositions ne devraient pas être sauvegardées par l’article premier. / While the right to protest may appear as a right acquired in Canadian law for some time, its exercise is precarious. Considering the frequency of mass arrests in Canada, how can such actions by the state be reconciled with the rights and freedoms guaranteed by the CanadianCharter of Human Rights? Determining the contours of the right to protest will allow us to better understand how mass arrests are to be regulated under the law. In this thesis, I analyze the evolution of the right to protest in Canada since the adoption of the Canadian Charter and examine how protesting historically fell under article 2b) of the Charter of Rights and Freedoms,which protects freedom of expression. Starting from the premise that the right to protest is not sufficiently protected under article 2 b) as a form of expression, I argue that including the right to protest under section 2 c) of the Charter’s right to peaceful assembly would allow courts to more effectively protect the right to protest. Following a discussion of how the right to protest should be safeguarded in Canadian law, this thesis then examines the sections of the Criminal Code that allow for mass arrests. I conclude that these provisions of theCriminal Code infringe on the right to peaceful assembly as well as freedom of expression and that the use of mass arrests should be limited to the most manifest cases, namely those where there is violence, or a real risk of violence. Finally, this thesis discusses whether article 1 of the Charter could be applied to protect the provisions of the Criminal Code that allow for mass arrests. Considering the political importance of the right to protest, this thesis concludes that these cannot be considered a reasonable limit to the Charter under article 1.

Psychiatric diagnosis vs medical diagnosis: Are mental health professionals aware?

For years research has demonstrated a varying incidence of medical disorders manifesting with psychiatric symptoms. A relatively conservative estimate of such so called "medical masquerades" is around 10%. It is important to ascertain whether health care professionals are aware of possible medical masquerades perhaps most especially in a mental health center outpatient setting where non-medically trained clinicians are the first line therapists for treatment in the majority of cases. This study set about to find out how aware three types of health care clinicians (psychiatrists, nonpsychiatrically trained medical doctors, and non-medically trained mental health psychotherapists) are of the prevalence of medical masquerades, and whether these three types of clinicians perform differently on three types of clinical vignettes (psychiatric, somatoform, and medical masquerades). Results revealed that all health care professionals surveyed are aware that there are a percentage of medical masquerades in the clinical population. Results also revealed that the three types of clinicians performed differently on the case vignettes.
