A System-wide Planning Tool to Evaluate Access from Crash Sites to Medical Facilities in Virginia

Crash response planning is a vital component of emergency management and highway emergency response planning. Evaluation of coverage of medical facilities is required to determine adequate access from crash sites to medical facilities. This study proposes a proof of concept for a planning tool that evaluates fatal and serious injury crash response coverage from crash sites to medical facilities in the Commonwealth of Virginia. Calculated travel times from fatal crash sites to medical facilities are compared with reported travel times to better estimate travel time modification factors. The modified travel times are used to determine coverage areas and evaluate serious injury crash response coverage of medical facilities in Virginia. A geo grid approach is used to demonstrate the proof of concept for a crash response planning tool. A risk grid is developed based on the aggregate number of fatal and serious injury crashes. This study includes serious injury crash response coverage because the number of serious injuries and serious injury rate are now included as reportable safety performance measures for state highway safety agencies. A mitigation grid is developed based on the travel time to the closest facility. Finally, a planning grid that combines risk and mitigation factors based on a decision matrix is presented. The resulting tool serves as a proof of concept for developing a crash response planning tool which enables planners to identify areas that do not have timely access from crash sites to medical facilities. / Master of Science / An objective of emergency responders is to safely transport crash victims from crash sites to medical facilities. Ensuring adequate access is an important goal of highway safety professionals. This study proposes a proof of concept for a planning tool that evaluates this access in the Commonwealth of Virginia. This study focuses on serious injury crash sites because the number of serious injuries and serious injury rate are now included as reportable safety performance measures for state highway safety agencies. Travel times from serious injury crash sites to medical facilities are used to identify areas that do not have timely access. Risk and mitigation assessments are performed by dividing the study area into equal sized cells. Risk and mitigation assessments are based on number of crashes and response travel times to the closest medical facility, respectively. These assessments are used to generate a proof of concept for a crash response planning tool which enables planners to identify areas that do not have timely access from crash sites to medical facilities.

The diploma thesis focused on the issue of communication between the medical staff and the patient deals with attitudes, knowledge and ethical isssues of communication between the doctor and the patient. The target group is the medical staff of the hospital ward and also the specialized outpatient service. The theoretical part explains the importance of communication between medical staff and the patient in a particular health care system. It also explains why ethics is important in relation to communication. All theoretical knowledge is based on available literary sources. The practical part describes the preparation, implementation and results of the research. Individual steps are taken to identify the current situaton in the issue. It has been found that attitudes and knowledge in patient communication vary. Furthermore, it was found that current educational programs and teaching plans of selected schools meet the level of school curriculum, but the practical teaching of communication skills is neglected. The subject of the present thesis is the architectural aspect of healthcare facilities with a focus on the work of the architects František Čermák and Gustav Paul. The initial part outlines the general classification and development of the building system of these facilities - ranging from the "pavilion" type to the extensive hospital blocks - illustrated by examples. The next part describes types of hospitals and their designs, especially after 1948. It includes a brief overview of both Czech and foreign projects that are of significance. The subsequent part introduces the life and oeuvre of the architects František Čermák and Gustav Paul, detailing several particular projects and realizations. Among those chosen for further elaboration are the projects of the Municipal Infectious Diseases Hospital in Bratislava, Clinical and Foundation Hospital in Zagreb, Tuberculosis Sanatorium in Vyšné Hágy and the Hospital in Prague-Motol; and further their realizations including the Sanatorium in Vráž u Písku, Provisional Hospital in Prague-Motol, completion of the main pavilion of the Hospital in Litomyšl and finally the Hospital in Chrudim.

This master thesis is focused on marketing of Santé, the private clinic. The first part is theoretical and has two main chapters. Marketing in the field of health care is the first chapter, including the differences between health care marketing and marketing in other fields and other characteristics of the medical system. Next a product is defined, in health care it is the provided service. Marketing mix is described not as 4P but as 4C. Then there is the practical point of view on the health care system. Very important is the patients' satisfaction, which is measured by many different surveys, such as the best hospital, or quality perceived by the patients. The other chapter discusses used methods, defines marketing research and the S-T-P strategy. The practical part of thesis introduces the company Santé followed by the description of their clientele, which is differentiated as individual and companies' clientele. Marketing mix of the company consists of the 4 components already mentioned higher. Next is the SWOT analysis. Analysis of the competition focused on the main competitors of Santé. Very significant moment is the research project created to suit the needs of this thesis only. A complex questionnaire researching the privet clinic market as a whole was prepared. Analysis of the data was complex as well. Open questions were analyzed separately, closed questions were processed with the help of SPPS computer program. By the end of the thesis there are some recommendations for the company that originate in the research project and also from the confrontation of the theory and practice. Last is the conclusion, bibliography and the annex, where you can find already mentioned questionnaire.

The goal of this graduation work is to create a strategic analysis of Hospital Znojmo (state institution). Znojmo hospital is one of the medical institution which is managed by South Moravian region. Znojmo hospital offers komplex health care that includes sick-bed detachement, ambulance and diagnostics. From the geographical aspects the biggest competitor is Hospital Třebíč which has the shortest run distance. The other main competitors are Faculty hospital in Brno and Faculty hospital saint Anna in Brno. The strenghts of the hospital are in her extensive history and the comprehensive services. The weaknesses are poor condition of the hospital buildings and insufficient financial investments form founder of the hospital. The conclusion of this work are outputs and recommendations in the form of various strategis analysis.

The topic of this thesis is the evaluation of education in a healthcare facility. The aim of the thesis is to evaluate selected educational events in a healthcare facility and to propose changes in educational activities based on the acquired knowledge. In the theoretical part, which is divided into three chapters, the introductory chapter identifiesd the concept of human resources and its development from the identification of educational needs, fromr planning to implementation. The importance of managers in the management and education of human resources and the subsequent evaluation process is also mentioned. The second chapter defines the concept of evaluation, including individual models of subsequent evaluation, which constitute a key source of information on the effectiveness of selected educational programs. The concept of efficiency is also clarified here. The third chapter deals with education in medical facilities with a focus on teaching hospitals. Its specifics are defined here, which must be taken into account in particular in accordance with the applicable legislation. Society today is increasingly exposed to the effects of emergencies. These situations not only affect the normal functioning of the society but also the functioning of medical facilities. The preparedness of medical facilities is a very important factor influencing the success in dealing with emergencies and crisis situations. The preparedness of the health care system as a whole consists of all medical facilities in the Czech Republic. Therefore, it is desirable that they are prepared to address them. The objective of the thesis, in its theoretical part, was to provide a comprehensive overview of the current state of crisis preparedness in medical facilities. The practical part was intended to introduce in more detail the territory of the South Moravian Region and its issues and to map out emergency preparedness in the specified research set. The research set included medical facilities in the region that are not university hospitals but that are part of the region traumatology plan. The data on this issue was collected using a questionnaire. The questionnaire was in a format of selected work indicators of emergency preparedness and it was uniform for all the respondents. The work indicators were as follows: theoretical preparedness, organizational preparedness and practical preparedness. Based on the questionnaire survey, analysis of the results and determining the average level of preparedness of all the participating medical facilities, it was concluded that the preparedness of the medical facilities of a non-university type in the South Moravian Region is very diverse: from hospitals whose preparedness is excellent to those who do not reach the required basis. In the overall assessment, 40.0 % of the respondents did not reach the determined average. Overall, the level of preparedness of the research set can be seen as slightly below average. To increase the level of preparedness of non-university medical facilities, it is suggested, for example, to create unified procedures for processing various types of plans, to assign each medical facility the obligation to employ a professional emergency manager, and to establish cooperation among individual medical facilities.

Zajišťování požární ochrany ve vybraných nemocnicích v hlavním městě Praha / The provision of fire protection in selected hospitals of the Capital City of Prague.

The aim of this work is to map fire safety in selected hospitals in the capital city of Prague. The main aim is to answer whether is the fire protection by the corporate fire brigade unit considering the vastness of the area and the number of beds measurable benefit for the fire safety of medical facility. Custom work is also examined whether the unit is reduced by the presence of the corporate fire brigades in the hospital requirements for fire protection using dedicated kinds of firefighting equipment. Another part of the research is dedicated to the fact whether is the load of the fire rescue service (FRS) of the capital city of Prague reduced by the presence of the corporate fire rescue service in the hospital and thus has to adjust the area coverage? In the first part of the thesis is summary of the regulations in the area of fire prevention. Above the Law on Fire protection and the Ministry of Interior Decree on fire prevention. There are described in detail obligations of legal entities and individuals, competence and training of fire protection. From the perspective of repression is described by the distribution of fire protection units, and also their rights and obligations. For purposes of this thesis is defined by the fire and method of propagation of fire. This will take the necessary measures to prevent its spread. Solution is also the fire safety of buildings according to the CSN 73 0802 Fire protection of buildings - nonproductive objects. Specifically resistance of structures and the use of fire safety equipment. It is important to define the hospital as a medical facility and its basic characteristics and distribution. They are described in detail in terms of medical facilities and fire safety through technical standard CSN 73 0835 Fire safety of buildings - Buildings for health care facilities. Rated objects are the Motol University Hospital, the University Hospital of Royal Vinohrady and Thomayer Hospital, all with headquarters in Prague. The main criteria for the selection of the hospital's number of beds, number of patients treated and the site area. Another criterion is the number of employees and the difficult conditions for intervention. At first, the comparison parameters hospitals for fire prevention, such as the number of professionally qualified persons, the number of trainees, preventive fire patrols. In the Motol University Hospital are installed sprinkler system and a special spraying equipment and also there is an entity of its own corporate fire rescue brigade. The next section analyzes the number of incidents in each hospital complexes or the number of fires. We found a relatively large number of fires in the Motol University Hospital. And with an average of 5.7 fires per year in the past decade, compared to 0.3 fires in the Thomayer Hospital and 1.0 of fire in the University Hospital of Royal Vinohrady. Space is analyzed with the most complex variant of fire, especially the requirements for forces and resources. The results are compared to their inclusion in areal coverage depending on the degree of danger cadastral municipality. In all three cases, these conditions suit. The result of the research is that the selected areas of hospitals in the capital city of Prague in terms of fire protection are at a very high level. Whereas there is the unit with its own fire protection or not. Its presence is a matter of fire protection concepts during the design of the hospital complex. Establishment of fire protection units in hospitals, where it currently is, would only increase the comfort of visitors and hospital patients on the one hand and financial wastage on the other. Functioning FRS in the Motol University Hospital is an objective benefit for the fire safety of the hospital complex. Analyses of the current state of the recommendation by the FRS of the capital of Prague conduct more tactical exercises, mainly in the hospital where they have their own fire protection unit.
