The effects of a nursing intervention on maternal perception of the infant and postpartum adjustment

The purpose of this study was to investigate the effect of a nursing intervention on the mother's postpartum adjustment and perception of her infant. The intervention was designed to give information about infant behaviour patterns; and to assist the mother in identifying the behavioural responses and reflex behaviours characteristic of her infant. A quasi-experimental design was utilized with random assignment of the twenty-five married primiparous participants to either an experimental or a control group. The subjects in the experimental group took part in the nursing intervention approximately twelve days following their infants' births; the fathers of the infants were also present during the intervention. All participants completed the Neonatal Perception Inventory and the Postnatal Research Inventory one month postpartum. Data were also obtained from the mothers' hospital records. Statistical analysis comparing the scores for the two groups indicated that experimental group mothers had a more positive perception of their infants. In addition, they reported less depression and irritability and fewer negative feelings towards caring for their babies. There were no differences between experimental and control group mothers on seven other scales of maternal adjustment. Correlations between the perception scale and maternal adjustment inventory across all subjects showed that positive perception of the infant was associated with low scores on scales of depression, irritability, fear or concern for the baby, and the mother's need for reassurance. These findings were discussed in relation to their implications for clinical practice. Mothering by the book : constructions of mature student mothers' identities in the context of mothering and study practices and mother/child relationships

This project investigates the development of mature student mothers’ identities in the dual contexts of constructions of the women’s at-home study practices and of their ‘responsibilities’ for their children’s development. Interviews were conducted with 23 women – all ‘new university’ students – and with their schoolaged children. I used discourse analysis focussing on language as performative and constitutive in order to understand positioning of, and by the women. I also drew upon critical developmental psychological theory and the concepts of discourse, intersubjectivity and dialogicality as epistemological resources in order to understand the women’s and children’s accounts. The methodology yielded a diversity of constructions of the women, these drawing upon a variety of discourses. The first empirical chapter addresses constructions of mature student mothers, the second, constructions of child development and the third, constructions of ‘influence’. The organisation of the empirical chapters reflects not only the importance I accorded to particular themes, but also the idea that separating mothers’ concerns and those of their children can be less fruitful in examinations of identity construction than addressing these together. The key issues that are a thread connecting the empirical chapters are time (requiring ‘balancing’ of social positions); change (in mothering practices and confidence); perceived ‘influence’ on children’s development, and relationships (including the ways in which identities are constructed in the ‘space in the middle’). Participants addressed these issues in different ways with some women positioning not spending ‘quality’ time with their children as meeting children’s developmental needs (addressed in Chapter Seven). Other important themes were mothering constructed as mundane and undervalued (in Chapter Five), children’s constructions of ‘roles’ as helpers (in Chapter Six) and perceived intersubjectivity in mother/child interactions (examined in Chapter Seven). In the concluding chapter I discuss the implications of the findings in terms of the contribution of the research to theoretical debates about motherhood, mothering and child development. I also reflect on my position as a mature student mother, examining my involvement in the research process and finally, suggest applications for the findings reported in the thesis.

Bonding experiences in mothers of infants with severe congenital heart disease

Mothers who have an infant with severe congenital heart disease (CHD) face an uncertain and emotionally challenging postpartum period as their baby is hospitalised and undergoes life-saving cardiac surgical treatment. There are many potential risk factors to mother-infant bonding, that is, the emotional tie a mother develops with her baby, in the context of infant illness. Having an infant with a diagnosis of severe CHD could be seen as a threat to the mother's experience of bonding. However, there is limited understanding about the maternal perception of bonding with an infant with severe CHD. This study aimed to explore mothers' bonding with their infant with severe CHD throughout antenatal, perinatal and postnatal periods and how they coped with any challenges to this bond. Interviews were conducted with eight mothers of infants aged between eight and fifteen months with severe CHD, who were recruited from a children's hospital and who were diagnosed either antenatally or postnatally. Interpretative Phenomenological Analysis was used to identify themes across the mothers' accounts. Four superordinant themes were identified: ‘An Emotional Start to Motherhood and the Mother-Infant Bond', ‘Losing Control in the Context of CHD', ‘Keeping Connected to the Baby' and ‘Moving on Together'. The findings identify mother-infant bonding as a process that can withstand challenges such as maternal-infant separation, potential loss of the infant and maternal feelings of disconnection from the baby. Practical strategies were used by mothers to maintain their bond with their infant following diagnosis and during hospitalisation. These included being close to their infant and taking over caregiving duties from the nurses. Mothers described strength and resilience from the experience and a process of increasingly feeling closer to their infant. Several potential research implications and clinical recommendations for healthcare professionals are suggested.

Trata-se da análise de 2222 consultas de enfermagem para o binômio mãe-filho executadas por 14 enfermeiras obstetras, no ano de 1983 em um Hospital Universitário, com o propósito de verificar se os procedimentos normatizados na consulta de enfermagem estão possibilitando o levantamento de informações relativas à clientela atendida e direcionando as intervenções da enfermeira, permitindo efetiva individualização das condutas de enfermagem. Foram levantados 5024 problemas, dos quais 3548 nos recém-nascidos e 1476 nas puérperas. Quanto às condutas de enfermagem executadas, estas foram em número de 4768 e conforme o registro, assim distribuídas: 714 - cuidados executados; 3270 - orientações ministradas; 252 - encaminhamentos para nova consulta com enfermeira; 458 - encaminhamentos para consulta médica; 48 - encaminhamentos para nutricionista, e 26 - encaminhamentos para a assistente social. Não tiveram suas condutas registradas, 1485 problemas. Os tipos de problemas levantados pelas enfermeiras não variaram: a atuação das enfermeiras não se restringe às condutas estabelecidas no guia de orientação e as orientações independentes do registro dos problemas foram em número bastante significativas. A maneira como a consulta de enfermagem se desenvolveu, possibilitou a caracterização da enfermeira na sua função independente, mas utilizando a interdependência de forma coerente, científica e individualizando a clientela. / This paper reviews 2.222 nursing consultations for the mother-child binomial, carried out by 14 obsteinic-nurses in 1983, at a teaching Hospital with the objective to find out if the standardized procedures for the nursing consultations are enabling the survey of information related to the clientele assisted and if the said procedures are directing the nurse\'s interference, to the actual individualization of the nursing approach. 5024 problems have been surveyed, of which 3548 in newborns and 1476 in mothers. As to the nursing approach utilized, there were 4768 and, according to the data, they were distributed as follows: 714 - care services performed; 3270 - instructions given; 252 - references to a new nursing consultation; 458 - references to medical consultation; 48 - references to nutritionists; and 26 - references to social workers. 1485 problems did not have their related nursing approach registered. The types of problems surveyed by the nurses did not show any variation; the nurse\'s activities are not limited to the ones established in the instructions manual and the number of orientations, regardless of the registration of problems, was significant. The way in which the nursing consultation was developed characterizes the nursing approach, as an independent function using, however, the interdependent function in a consistent and scientific manner as well as the individualization of the clientele.
