This work is focused on the evaluation and verification of the criteria of success foster care on the basis of their own experience. Affects the overall trend of the development of the child in the context of the family and the wider society. The key theme of this study is, above all, approach the importance and advantages of foster care, or clarify so much discussed questions of education of children in substitute family environment in terms of celospolečenského scale. To achieve the desired outcome here were used not only to their own knowledge and experience, which are processed in the individual kasuistikách, but also the conclusions of the expert examination of pedagogical-psychological counseling and specially-educational centre. The thesis has been processed on the basis of personal long- standing observation and study the behaviour of children in foster care, which are pursued and carried out directly in the family of the author thesis. The thesis deals with access schools and teachers to handicapped students. Theoretical part follows problems of pupils with special educational needs and its legislation, the concept of handicap and disability forms. I characterize from the forms of disability specific learning disorders, brain dysfunction and behavior disorders, cerebral palsy, autism and Asperger syndrome. The last chapter of this part focuses on the conditions, legislation, documents and statistical data in the field of disabled pupils integration. For the practical part I have set several goals: regarding the run of the lesson, teacher access and work of pupils with special educational needs; realize education with handicapped students to obtain my own experience; conduct interviews with teachers, with teacher's assistant, possibly with a special needs teacher in order to obtain an opinion of teaching staff to integrateion and obtain advice for access to pupils with special educational needs; through questionnaires for schools to determine the conditions for integration of handicapped pupils, and finally summarize the recommendations and create a handbook for teachers beginning to work with pupils with special educational needs.

This thesis deals with a part of the treatment process in patients with acute ischemic stroke. Specifically, it examines the time interval from patient entry to a healthcare facility to initiation of treatment with intravenous thrombolysis - so called Door-to- Needle Time. The theoretical part describes the stroke and its division according to etiology. Furthermore, there are summarized clinical studies that have taken place in the world since the 1960s to the present, the development of care for patients with stroke in Czech Republic, recommended procedures of professional organizations in Czech Republic and the identification and management of treatment of stroke patients. In conclusion, the theoretical part summarizes the studies focused on the reduction of the Door-to-Needle Time, which took place in the world and in the Czech Republic. The aim of the practical part is to evaluate work procedures and conditions of health teams of individual stroke centers and to identify causes of Door-to-Needle Time median differences. Title: Measurement of lower extremities spasticity in patients with cerebral palsy Objectives: The aim of the theoretical part of this thesis is to evaluate possibilities of lower extremities spasticity measurement in adult patients with cerebral palsy. The main focus was given to the concept of French professor J.-M. Gracies: Five- step clinical assessment in spastic paresis. This unique concept presents differentiation of three main factors of motor impairment that emerge as a result of a lesion to central motor pathways: stretch sensitive paresis, soft tissue contracture and muscle overactivity. Ability to distinguish these factors is crucial for specific treatment indication. Finding a correlation between the Five-step clinical assessment in spastic paresis and muscle activity in gait measured with polyEMG was the main objective in the practical part of the thesis. Methodology: This thesis has a theoretical-empirical character. The theoretical part is in a form of a research on the topic of spasticity diagnosis, focused on cerebral palsy patients. The aim of this thesis is to attempt to summarize what communication possibilities children suffering from a serious form of infantile cerebral palsy have, to describe them and possibly to clarify what exactly restricts these possibilities. The thesis also tries to answer the question in what way the communication of these individuals differs from the communication of intact persons. The core of the thesis resides in a theoretical exposition which deals - among others - with the communication process and its verbal and non-verbal parts, the diagnostic characterization of the infantile cerebral palsy and the impaired communication ability in symptomatic speech disorders which are associated with this disability. It describes how especially mobility impairment and mental retardation are reflected in the way and possibilities of communication. Special attention is paid to methods of alternative and augmentative communication which can be used as a therapy in the stimulation of the development of communication and speech skills, and which very often represent one of few possible ways of communicating for persons with this disability. Neuromodulace v léčbě vybraných dystonických syndromů / Neuromodulation in treatment of selected dystonic syndromes

Dystonia is a neurological syndrome characterized by the involuntary contraction of opposing muscles, causing twisting movements or abnormal postures (modified by Fahn, 1987). Writer's cramp is the most common form of task-specific focal dystonia. In the first study, patients with writer's cramp were evaluated for differences in cortical activation during movements likely to induce cramps (complex movements) and movements which rarely lead to dystonia (simple movements). Although complex patient movements during fMRI were never associated with dystonic cramps, they exhibited abnormally decreased cortical activity. This was not observed in simple movements and was unrelated to the character of handwriting or the presence/absence of visual feedback. Our results support the theory of dualistic sensorimotor system behavior in writer's cramp. As the somatosensory system is believed to be affected in focal dystonia, we focused on modulation of the primary somatosensory cortex (SI) induced by repetitive transcranial magnetic stimulation (rTMS) in the second study, in order to improve writer's cramp. In conclusion, 1 Hz rTMS of the SI cortex can improve manifestations of writer's cramp while increasing cortical activity in both hemispheres. Introduction: The surgical and endovascular results of the treatment of pial AVM provided at our Neurosurgical centre are presented. These results are supported by neuropsychological outcomes of subgroup of treated patients. Going by these results and by an overview of literary data on the efficacy and complications of each therapeutic modality, the optimal algorithm of indications is presented Cohort of patients: The main series comprises 222 patients aged 9 to 87 years treated in the years 1998 - 2013. The surgical group consists of 85 patients, 55 patients received solely endovascular treatment. Thirty-four patients were consulted and referred directly to the Radiosurgical unit. The remaining 48 were recommended to abide by the strategy of "watch and wait". A subgroup of 66 patients, who underwent treatment of AVM was neuro-psychologically tested at least two years after treatment using a battery of tests constructed specifically for this study. A control group consisted of 10 subjects without any neurological disease. Results: In the surgical group, serious complications were 3.5% at a 96.5% therapeutic efficacy. As for AVM treated with purely endovascular methods, serious procedural complications were seen in 5.5% of patients, with efficacy totalling 36.4%. One observed patient suffered...

The Focus of this study is to present Pro-Active approach and its use in pediatric occupational therapy. The main goal is to find out the effect of the Pro-active approach to neurorehabilitation on function capability of children with spastic quadruparesis form of cerebral palsy. In the first part is presented an overview of the issue of the Pro-active approach, which is made on the basis of a literature search. Nine patients participate in this study. The pro-active approach was applied to all of them. The first case study acted as a pre- research, she had 4 months of therapy with 25 therapies. The other 8 case studies took place for 5-weeks, with occupational therapy 3 times a week. Participants were assessed with the Pediatric Evaluation of Disability Inventory (PEDI), measurement of active and passive range of motion in joints at upper extremity and spasticity. Evaluation of model activities was also included. Comparison of initial and control assessment showed an improvement in all measured score. 6 participants had better score in PEDI. For most participants was the range of active and passive motion increase. The degree of spasticity remained unchanged, but the spasticity angle improved.
