Potřeby klientů vybraných doléčovacích center v oblasti bydlení / The needs of clients in selected aftercare centers in the area of their housing

Background: Aftercare services with an aftercare program in the Czech Republic significantly help to integrate persons with addictological problems into the majority society. However, according to the National Monitoring Center for Drugs and Drug Addiction, more than 50 % of users of these services are unsuccessful in their aftercare. Objective: The aim of the research is to find out the needs of clients in the area of housing at selected after-treatment centers. Identify whether these needs are met and whether clients' success in aftercare programs is not affected by unmet housing needs. Methodology: The theoretical and practical part of the thesis delimits the research findings. The theoretical part deals with aftercare services with aftercare program and the effect of relapse and relapse in aftercare for addiction.The practical part defines the area of research and with the use of quantitative methods of research comes up with statistical data, which are subsequently interpreted. Results: 7 residential facilities with a capacity of 85 beds participated in the research. The research sample consisted of 76 respondents. This thesis is focused on the revitalization of public green areas in the municipality Adamov, near České Budějovice in area "Babský vrch". This work is divided into two parts, the first part is literature review, which is generally discusses history of the landscape, the landscape structure, the notion of a revitalized green, the planting and about aftercare of course. The first part of this thesis is finished with a chapter about use of the subsidy policy in this area. The second part is focused on the project itself, therefore revitalization of public green area. In this part are new planting of green area, which is followed aftercare, as well as the creation of quiet zones for citizens of Adamov.

The objectives of the work The objective of this work has been set as the mapping of degree of patient's contentment/ rate of patient's satisfaction in view of the nurse's approach within the scope of the post-thoracic operation care and the evaluation/ assessment whether the needs of the aforementioned patients have been satisfied. Another objective of the research enquiry has been set as finding out the level of the nurse's awareness regarding the ways and possibilities of the subsequent care. Methodology For the purpose of the research enquiry the form of qualitative research has been chosen and adopted, which has been applied and performed by means of using a method of interviewing i.e. putting questions which have been prepared beforehand, well in advance. Research array of patients consisted of 10 patients from one hospital (which is not going to be specified in this work) as well as 8 nurses whose job was to take care of the post-operative patients/ the post-thoracic operation patients. The results of the research have been processed by applying the method of open coding, pencil-paper method. Assets and Benefits of this Work The research enquiry has showed certain level of absence of desirable knowledge as well as ignorance of the staff taking care of the patients regarding the ways of subsequent care. Nurses should be educated about the possibilities of home care, hospice care as an integral part of their profession. The results gained by research enquiry process could be presented at seminars, conferences aimed at the subsequent care.

Role sestry v následné péči o bariatrického pacienta / The Role of a Nurse in after care about bariatric patient

Current status Obesity is a disease characterized by the increased proportion of fat in the body over a certain limit (Müller 2009). According to the World Health Organization in 2014, more than 1.9 billion people suffered from overweight, of which more than 600 million people were obese (WHO 2015). Subject The aim of the work is the definition of competence nurses have in the aftercare of bariatric patients. Determination of preparation, care, patient awareness and procedures for patients undergoing bariatric treatment. And findings of specific needs of bariatric patients undergoing treatment for obesity. Three research questions have served towards these targets: What competences do nurses have in the aftercare of bariatric patients? How are patients informed throughout the bariatric treatment? What are the needs of patients undergoing bariatric treatment of obesity? Methods Qualitative research techniques were used to obtain the necessary data for the practical part. The practical part was realized through individual semi-structured interviews with nurses working on surgical wards and patients who had undergone bariatric treatment. At the beginning of each interview, the nurse and the patient were apprised of the reason of the research conducted and were assured of anonymity in the processing of the dialogue. The central theme was the assertion of nurses in the subsequent care of bariatric patients and to determine the specifics of these patients. Participants The sample consisted of nine nurses working in a specialized center, where bariatric procedures are performed and seven patients who had undergone bariatric surgery. The survey was conducted from February to March 2015 Results: Before the operation patients undergo surgical, internal, nutritional and psychological examinations. Blood is collected for hematological and biochemical laboratory tests. Among other examinations clients go through gastroenterology, pulmonary testing, also examination in a sleep laboratory and swabs are taken from the throat and nose. Before the particular operation clients should reduce weight on a doctor's recommendation. It was also found that patients are informed about the preoperative and postoperative care, as well as about the need for dispensarisation, the frequency of follow-ups and possibilities of participation in bariatric clubs and associations. Nurses cover the following competences. Before the surgery, the nurse educates the clients about the preoperative and postoperative care, takes their blood, fulfills what the doctor prescribes and motivates the patient. After the surgery, the nurse provides general nursing care, monitors physiological functions, and educates patients about their diet and carries out the doctor´s prescriptions. The nurse must know the dietary restrictions, when it is determined by the general procedure, which varies in some detail according to the type of surgery and the patient's condition. The research showed that nurse cares more about the biological needs of the patients, rather than their psychosocial needs. Patients lack psychological support and motivation in aftercare. Patients undergoing bariatric treatment need to prepare for surgery (weight loss) and acquire post operation eating habits. Their other needs are psychological support and motivation conveyed by nurses. Conclusion: As the number of people suffering from overweight and obesity at present is constantly growing, obesity treatment becomes a necessity. Currently, bariatric surgery is widely used for patients suffering from obesity of the II. and higher degree, so it is advisable to know the needs, knowledge, practices in patient care before and after such an operation and, ultimately, which competences do nurses have during them.

This thesis deals with the urgent and follow-up measures in case of chlorine leak and a retrospective analysis of past cases. Chlorine is a widely used toxic gas. It is known from history as the first practically used warfare agent. Today, it is an important chemical raw material that is used widely in industry. It is also commonly transported by road and rail. It is one of the substances whose leakages occur very often. The objective of this paper is to assess the impacts of chlorine leakage on the lives and health of people and to provide an overview of urgent and follow-up measures in case of chlorine leakage in the perspective of the integrated rescue system and the general public. In addition, to locate, describe and analyze past cases. The theoretical part contains information about chlorine, the principles of behaviour of the population in case of an accident with chlorine leakage and an overview of measures undertaken at the site of chlorine leakage. Through studying available literature and resources on the Internet, overview of major chlorine leakages in the Czech Republic as well as abroad has been created. Selected events are described in detail. The practical part simulates, using computer programme called TerEx, a chlorine leakage from a water treatment plant in Písek, South Bohemia. To compare the results, several model situations were created with varying amounts of chlorine leaked at different times of accident and atmospheric conditions. A specific example is used to assess the risks to people living near the plant and to outline the urgent and follow-up measures.

The goal of this diploma thesis is to introduce urgent and follow-up precautions in case of leakage of chlorine - simulated accident of vehicle transporting chlorine. The major objective of this study is to analyse the risk of chlorine-transport for people with the aid of available simulation programme and to specify urgent and follow-up precautions. The next part of this study is to investigate the knowledgeability of people. The theoretical part of this study is systematical structured into a number of chapters and worked out on the basis of available legislative regulations, literature and internet sources. At first basic legislation of transport of dangerous substances is described, from the european, as well as national perspective. The next chapter deals with transport of dangerous substances, danger classes of these substances, their labeling in transport, as well as labeling of transport units. The following chapter is devoted to aspects of an accident in case of transporting of dangerous substances. This chapter demonstrates an accident with leakage of dangerous substance, its impact, escape of this substance and its spread. Further the sources and risks of transportation of dangerous substances and accident rate in this transport are described. The next chapter deals with the specific dangerous substance - chlorine. It is aimed at physico-chemical characters of chlorine, its history and its current utilisation, its effect on human health, on environment and first aid in case of contact. At the end urgent and follow-up precautions in case of an accident inkl. leakage of chlorine. The empirical part of this study is devoted to two basic research questions. Qualitative, as well as quantitative methods of research were used to answers these questions. The empirical part is devided into two major chapters. The primary purpose of this study is to simulate an accident of vehicle transporting chlorine with the aid of the programme TerEx and to determine its impact on people and surrounding. The first question was to analyse the potential dangerous for people and surroundings in case of an accident with chlorine. For this purpose the town České Budějovice was used. First the transport of liquid chlorine in small containers in the Czech Republic was analysed focusing on the southbohemian region and especially the regional city. Thereafter input data were entered. On the basis of obtained outputs were determined threats, that arise from the transport of dangerous chlorine and on the basis of simulation were characterized activities carried out by Integrated Rescue System using appropriate urgent and follow-up precautions. Part of this work was also the second research question - the level of knowledge of people located close to the simulated accident about this problem. This survey was aimed at the risk of chlorine, its transport and characteristics and impact on human health as well as protection against its adverse effects. For this purpose a group of second-year students of the local gymnasium located close to the place of simulated accident was chosen. The data were collected through a questionnaire, which contained a group of questions relating to this problem. The obtained data were evaluated using basic methods of elemental statistical analysis and graphic processing. Findings resulting from the analysis of transporting of chlorine and from the simulation using the programme TerEx show, that the danger to the population of České Budějovice in case of an accident with chlorine is significant. The issue of chlorine-transport and its risks is additionally complicated by insufficient regulation. Another negative factor of transporting of these substances is the complicated traffic situation in the country town. Based on the results of the questionary can be stated, that these students have average knowledge in this issue.

This thesis is focused on the problematics of aftercare in patients with brain damage. The thesis has an informative character and its aim is to introduce the problematics to the readers and give them valuable advices. This topic has come to the fore of the interest of the professional and general public especially in recent years. Partly because numbers of brain damage especially those of the traumas are increasing, but also because of the development of acute medicine, which is nowadays at a very high level, there is an increase in the number of patients who survive brain damage. However, the problem is a large quantity of these patients have permanent consequences, and they and their closest neighbors have to deal with. These permanent consequences are manifested as cognitive impairment, impairment of sensory functions, physical disability and changes in behavior and emotions. Following care in patients with brain damage is a major problem in our country. We see the problem especially in the continuity and availability of care for locally available services and rehabilitation care for these individuals. The continuity of care after the patient is released from the acute bed is very low and sometimes chaotic. To obtain the necessary informations for thesis and to achieve the goals, it was necessary to study a large amount of professional literature. Used sources are specialized Czech and foreign literature, professional journals and articles searched in professional databases. This thesis should show the possible problems of patient aftercare with brain damage. The results of the thesis can be used as a subsidy to professional conferences, seminars or courses to deal with the issue. The thesis could increase public interest about this topic.

Heart rhythm disorders alias arrhythmias rank among the most frequent heart diseases. They result from a disorder of generation or conduction of electric impulses at the heart. There are often arrhythmias of which one is not conscious and which may be detected in long-term ECG monitoring. Patients{\crq} problems mostly arise from rhythm disorders that are paroxysmal or permanent. These may endanger the patient{\crq}s life. Subjective troubles reported by patients include uncomfortable heart palpitation, fatigue, breathlessness, distress, shorter breath, dizziness or even unconsciousness; sometimes patients may feel unpleasant chest pain. These subjective feelings are very uncomfortable for a person suffering from heart arrhythmia and can make it impossible for him/her to manage routine daily activities. The task of a nurse is to help a patient overcome this difficult period and instruct him/her of a healthy lifestyle as a prevention of heart diseases. In this respect, cooperation between the nurses in hospitals and the nurses in cardiologic out-patient departments is important. Four hypotheses were raised. The first hypothesis was to confirm that the care of patients with heart arrhythmia after their discharge to home-care is sufficient in respect of their needs. This hypothesis was confirmed. The second hypothesis was to confirm that the patients are sufficiently instructed on prevention of recurrence of heart arrhythmias. This hypothesis was not confirmed. The third hypothesis was to confirm that patients know the impacts of heart arrhythmias on their overall health state, and was not confirmed. The fourth hypothesis was to confirm that patients are aware of other options of the care after discharge from hospital. This hypothesis was not confirmed. In the practical part of the thesis I used a quantitative research. The research was conducted in hospitals in České Budějovice, Jihlava, Plzeň and Prague, in the form of anonymous questionnaires distributed to patients in cardiologic out-patient departments. The results have been processed in charts and tables in the SPSS programme. The objectives were fulfilled. The appendix to the thesis contains a brief proposal suggesting on what the nursing care should be focused after a patient{\crq}s discharge from hospital. The results will be offered to deputy managers for the nursing care and to nurses in cardiologic out-patient departments.

This thesis deals with consequential care after release from institutional care and relations between absence of binding person and incorporation of a young adult terminating his or her institutional care into society. The thesis focuses on institutional care, stressing its termination, it suggests changes for improvement and offers an example of good practices from Germany. Also, the thesis identifies the most serious issues of children after institutional care and describes, in detail, consequences of lack of emotional support. Utilizing case studies, the experimental part looks into whether children living in children's home can find a binding person and how presence or absence of such a person can affect their future life.
