Factors of Supervision and the Impact on Intentional Nondisclosure by Counselors-in-Training

Supervision of counselors-in-training (CITs) is complex and multifaceted. While supervision is critical to the professional development of CITs, they are also being evaluated for a grade that will advance them academically. Therefore, CITs may feel pressured to intentionally withhold information in supervision that could reflect badly on them or their supervisor and/or hinder their progress. Indeed, one study indicated that 97.2% of CITs intentionally withheld relevant information in supervision (Ladany, Hill, Colbert, and Nutt, 1996). Fortunately, there are ways to reduce supervisee non-disclosure. For example, when CITs perceive a strong supervisory relationship, nondisclosures occur less frequently (Mehr, Ladany, and Caskie, 2010). There is some evidence that the supervisory relationship is impacted by the supervisory working alliance and supervisee attachment styles. More general social perceptions may also impact the supervisory relationship, but that has not been assessed in the context of CIT supervision. Therefore, the purpose of this quantitative study was to examine these factors, supervisory working alliance, supervisee attachment styles, and social perceptions, and their impact on intentional nondisclosure by CITs. This quantitative study included a sample of 112 master's-level counselor-in-training students enrolled in internship at a CACREP-accredited (Council for Accreditation of Counseling and Related Education Programs) counselor education program. Results indicate that CITs report more discomfort in disclosing supervision-related issues compared to client-related issues. An exploratory factor analysis of the three constructs of interest resulted in a four-factor model. These factors were: (1) Perception of a Supervisor, (2) Anxious Attachment in Supervision (3) the Supervisory Working Alliance, and (4) Avoidant Attachment in Supervision. A multiple regression analysis indicated that a model including social perceptions of supervisors, the supervisory working alliance, and supervisee attachment styles explained 64% of the variance in intentional nondisclosure. Avoidant attachment styles and rapport of the supervisory working alliance were significant predictors of intentional nondisclosure. Implications for counselors and counselor educators are included. Limitations and recommendations for future studies will be discussed based on the unique findings from this study. / Ph. D.

“CFL has its patient zero”: A Critical Examination of HIV Nondisclosure in the Trevis Smith Case

Canada is currently the world leader in the number of per capita prosecutions of HIV nondisclosure (Hastings, 2017). Many of these cases garner the attention of media and receive sensational and dramatic coverage. This thesis provides a feminist critical discourse analysis of the juridical and mediated content on Trevis Smith who was a former Canadian Football League linebacker convicted of aggravated sexual assault for not disclosing to his sexual partners that he was living with HIV. Mobilizing an intersectional (Crenshaw, 1989, 1991) and masculinities (Connell, 2005/1995; Messerschmidt, 2012; 2016) theoretical framework, I specifically explored the ways in which the media and juridical content constituted Smith by considering how the different discourses were shaped by race, gender, class, criminality, sexual orientation, and seropositivity. The analysis revealed three main discourses: Dr. Jekyll and Mr. Hyde, The “Sodom and Ghomorrah of Shared Sexual Partners” and Playing a Dangerous Game. These discourses demonstrate that Smith was initially shown sympathy by way of his construction as a philanthropist, and sports hero. However, as the case progressed his masculinity was discursively linked to racialized tropes of hypersexuality, dangerousness, and criminality due to his failure to disclose that he was HIV positive. I conclude that these discursive connections are part of a broader historical narrative that subjugates and controls Black men while also working to symbolically and literally segregate the Canadian HIV-negative social body from the hypersexual Black ‘AIDS fiend’.

Are All Nondisclosures Created Equally? An Exploratory Factor Analysis of the Content of Supervisee Nondisclosure

abstract: This study examined the factor structure of supervisee disclosure in clinical supervision. An original survey measure was created for this study, the Supervisee Disclosure Scale (SDS). Through exploratory factor analysis eight specific content areas of supervisee disclosure were identified. The eight specific content areas of supervisee disclosure include: Perceived Clinical Inadequacy, Transference Issues, Strengths of the Supervisory Relationship, Clinical Successes, Self, Weaknesses of the Supervisory Bond, Dissatisfaction with the Clinical Setting, and Own Clinical Voice. Furthermore, this study examined the potential relationship of clinical experience with the content areas of supervisee disclosure. The results of this study support a relationship between greater clinical experience and disclosure of items related to Self but not with the other content areas. Additionally, the bi-level factor structure of the Working Alliance Inventory/Supervision-Short (WAI-SS) was validated via confirmatory factor analysis. The bi-level factor structure of the WAI-SS identifies a hierarchical structure of general alliance in addition to the specific factors of task, bond, and goal. Lastly, this study preliminarily evaluated the relationship between WAI-SS factors of general alliance, task, bond, and goal and the preliminary specific content areas of supervisee disclosure. The hierarchical factor of general alliance was a statistically significant predictor for all specific content areas of supervisee disclosure. The preliminary findings of this study, highlight the important differences in the relationships among the specific factors of the supervisory working alliance and content areas of supervisee nondisclosure. The factor of task was not significantly correlated with content areas of supervisee disclosure and the factor of goal was only a significant predictor for two content areas of disclosure: Strengths of the Supervisory Relationship and Dissatisfaction with Clinical Setting. The factor of bond was significantly correlated with six content areas of supervisee disclosure and significantly predicted five content areas: Strengths of the Supervisory Relationship, Clinical Successes, Self, Weaknesses of the Supervisory Bond, and Dissatisfaction with the Clinical Setting. This study contributes specificity to the supervision literature on supervisee disclosure and nondisclosure. The results of this study provide a psychometrically sound foundation for future research to identify aspects of the supervisory working alliance that may reduce supervisee nondisclosure. / Dissertation/Thesis / Doctoral Dissertation Counseling Psychology 2018
