A Study of Nurse Practitioner on Human Resource Benefits: Using a Certain Southern Teaching Hospital as a Study CaseCheng, Li-Ling 30 June 2009 (has links)
Due to under intense competitions and uneven resource allotments, many medical institutes are not only being pushed ahead with their medical technology but also constantly improving their medical management and efficiency for achieving higher medical quality. In Taiwan, it takes ten years to train a doctor; however, it is possible to have a NP (Nursing Practitioner) to substitute partial capacity of a resident;
as long as the NP has equivalent working experiences, a short-term training and also to pass exams as well. Therefore, using a qualified NP in medical institutes is a more economical and practical way of utilizing human resource benefits; as far as for
the medial caring services it also has a substantive significance.
This study is a research that is based on a certain southern teaching medical hospital¡¦s doctors, nurses, patients, medical services and other medical related people.It is divided into two parts to proceed: first, collect information from quality based
researching interviews of targeting people in order to study their perspectives toward to the system of NP. Then, a quantifiable survey is being designed according to the result of the interviews and some other verified literature content; therefore, the merit
of NP can be proven by the medical hospital¡¦s human resource efficiency trend.Further discussion on whether medical professional personnel attributes can affect theperception of NP; to anticipate an establishment of independent NP system in the
future medical system.
According to the research results: (1) 51.8% participants of the interviews think that the establishment of independent NP system is not only a demand from hospitals, doctors as well as the patients. It may even promote overall hospitals management efficiency. The demand 28.4% accounts come from doctors; to a certain extent that many participants believe to establish an independent NP system can possible to share an appropriate amount of doctors¡¦ workloads. 47.2% participants believed that the act of NPs in medical system is ¡§Physician Assistant¡¨. In addition, about 50% participant thought that the NPs can substitute residents and they can cut down cost,be economized the use of manpower training time, improve efficiency, promote medical service quality and so on. (2) A remarkable difference in ¡§Expanding System Construction Interface¡¨¡]F=3.495¡Ap=.018¡^is found because the participants come from different age groups. The younger participants are, the lower they score.
Thus, it is possibly to say when come to develop nursing care network and community medical caring Expanding System Construction Interface that the
younger medical professionals are, the lower understanding and less recognizing they are. (3) According to different seniority of every single interface variance analysis; the differences of seniority medical professionals in ¡§Expanding System Construction Interface¡¨¡]F=4.153¡Ap=.008¡^, after comparing with Scheffe¡¦s method,has scored above an average: above five years > 3-5. In comparison, one who has
more seniority has obtained a higher score. (4) Monthly income in the SubstitutionConstruction Interface (F=3.174, p=.046) has a remarkable difference. According to the description of personnel attributes; doctors who have the average monthly
income above 120,000NTD have occupied the majority (38.2%). However, in comparing with other categories; to provide resident's immediate supports, as well as others¡¦ understanding and recognizing of the NPs have obtained a lower score.
The purpose of this research is to believe that there should be more explicit direction and further related implementation in all the future hospital policy, in order
to enable the NPs to make full use of their ability. At the moment, there is a rather vague definition of NPs¡¦ job in Taiwan; in fact, the establishment of NPs¡¦ system has not much explication either. Therefore, while evaluating overseas¡¦ system, method and training, it may not be approved by the present medical professionals without
envisaging a thoughtful systematic construction first. However, according to the findings of this research; it is necessary to have a further discussion. It is to suggesting a future specific and comprehensive plan: expanding case acceptance throughout all
level of hospitals in Taiwan to promote the result of inference; designing a further discussion survey of focusing on NPs¡¦ beneficial results as for the future research foundation.

<p>Bakgrund: Dödshjälp eller eutanasi innebär att åtgärder vidtas för att skapa en så lätt ochfridfull död som möjligt. Nederländerna, Luxemburg och vissa stater i USA har en lag somtillåter dödshjälp, dock är dödshjälp fortfarande olagligt i de flesta delarna av världen. Antaletstudier kring sjuksköterskors attityder kring dödshjälp är få. Den tidigare forskning som finnspåvisar att attityderna kring dödshjälp är mycket delade. Olika studier visa mycket olikaresultat beroende på vart studien görs och vilka sjuksköterskor som deltar.Syfte: Syftet med denna litteraturöversikt är att belysa bakomliggande faktorer kringsjuksköterskors attityder till dödshjälp.Metod: En litteraturöversikt har använts i denna studie utifrån Friberg (2008) föreskrifter. 13artiklar har använts som särskilts i en del med artiklar av kvalitativ art och en med kvantitativart. Sökord som har använts är sjuksköterska, dödshjälp och attityder vilka sökts i databasenCINAHL.Resultat: I resultatet framkommer det att den viktigaste och mest betydelsefulla faktorn till attsjuksköterskor har en positiv eller negativ attityd till dödshjälp är religion. De sjuksköterskorsom var troende hade en mer negativ attityd medan agnostiker eller ateister var mer positiva.Ett outhärdligt lidande, stor smärta och det faktum att mediciner ej längre lindrar patientenstillstånd kunde ses som en annan viktig faktor till varför sjuksköterskor hade en positiv attitydtill dödshjälp. En annan faktor till en negativ attityd var maktmissbruk. Studien påvisar att desjuksköterskor som arbetade inom intensiv/akut vård har en mer positiv attityd än desjuksköterskor som arbetade inom palliativ vård.</p> / <p>Background: Euthanasia means different methods that are used to create an easy andpeaceful death. The Netherlands, Luxembourg and certain States in USA have a law thatpermits euthanasia. Euthanasia is still illegal in the most countries in the world and there arefew studies done around nurses' attitudes around this. Previous research demonstrates that theattitudes around death help are divided. Different studies show very various result dependingon where each study is done and which nurses that participate.Aim: To explore the underlying factors to nurses attitudes towards euthanasia.Method: A literature review has been used in this study on the basis of Friberg (2008)regulations. 13 articles has been used and these have been separate in one part which isarticles with a qualitative method and one with quantitative method. The words that has beenused in the seeking process are nurse, euthanasia and attitudes and the seeking is done in thedatabase CINAHL.Result: The result shows that nurses attitudes divides a lot from each other, it is verydepending on underlying factors. It showes that one of the most important factor to i nurseshas a negative or positive attitude to death help is religion. The nurses that are religious had amore negative attitude while agnostics or atheists where more positive. Intolerable suffering,great pain and the fact that medicines could not improve the patient's conditions are importantfactors for the nurses positive attitude to euthanasia. Misfeasance was also as a factor to whysome nurses had a negative attitude to euthanasia. The study reflects that the nurses that wereworking within intensive/the emergency treatment had a more positive attitude than the nursesthat was working within palliative the care.</p>

More people are likely to be diagnosed with cancer and the number of people living with cancer is expected to increase, which means that patients live longer with cancer and different treatments. All patients in Sweden should have access to a nurse navigator to facilitate the cancer trajectory. The purpose of this literature review was to delineate patients' experiences of the care given by the nurse navigator. The result is based upon twelve scientific articles included in this review. The results show that the experiences of the patients can be divided into four different categories: emotional support- being present and offering supportive talks, support for physical symptoms- counseling and relief from symptoms due to illness and treatments, educational support- receiving information and knowledge about the disease and cancer trajectory and coordination support- collaboration with other healthcare professionals involved in patients´care. The result shows that the experiences of the patients were mainly positive and they received the care that was the aim of introducing nurse navigators in Sweden. It is important to continue doing research of the care given by the nurse navigator to ensure that patients with cancer are receiving evidenced based care. / Antalet människor som drabbas av cancer och som lever med cancer förväntas öka, vilket innebär att patienter lever längre med cancersjukdom och olika behandlingar. Alla patienter med cancer skall i Sverige ha tillgång till en kontaktsjuksköterska för att underlätta för patienten genom vårdprocessen. Syftet med litteraturstudien var att beskriva patienters erfarenheter av kontaktsjuksköterskans omvårdnad inom cancervården. Tolv vetenskapliga artiklar inkluderades och ligger till grund för resultatet. Resultatet visade att patienternas erfarenheter av kontaktsjuksköterskans omvårdnad kunde delas in i fyra kategorier: emotionellt stöd - att finnas till hands och ge stödjande samtal, stöd vid fysiska besvär- råd och lindring av symptom på grund av sjukdom och behandling, kunskapsstöd - information och utbildning om sjukdom och vårdprocessen och samordningsstöd - samarbete med andra vårdgivare delaktiga i patientens vård. Resultatet visade att patienternas erfarenheter av kontaktsjuksköterskans omvårdnad i huvudsak var positiva och att de fick den omvårdnad som var syftet med införandet av kontaktsjuksköterskefunktionen i Sverige. Fortsatt forskning om kontaktsjuksköterskans omvårdnad inom cancervården ses som betydelsefullt för att säkerställa att evidensbaserad omvårdnad erbjuds patienter med cancersjukdom.

Background: Self- harm among children is a growing problem in Sweden. The behavior means that persons deliberately hurt themselves. It´s not only the child who suffer from this behavior, it also affect the parents and siblings. It’s important that the nurse can give parents the support and information that they need. Research is limited when it comes to parents’ experience of having a child who deliberately harm her-/himself. Aim: The aim of this study was to illuminate parents’ experiences of living with a child with deliberate self-harm. Method: A literature-based study based on analysis of ten qualitative articles was performed. Results: The result are presented in three categories; "Feeling responsible", "Desire to be a better parent", "Feeling invisible and extradited" Conclusion: Nurses need more knowledge about self-harm and how parents perceive their situation. Parents need more support and information from the nurse.
