Účinky obezity a metabolického syndromu v průběhu infekce Trypanosoma Cruzi / Effects of obesity on the course of Trypanosoma cruzi infection

Obesity is very widespread and detrimental to health. Obesity brings with it many changes including heightened immune function, and a higher prevalence of major cardiovascular disorders, cancer, diabetes, and Alzheimer disease. Obesity is also associated with shortened lifespan. The detrimental effects of obesity are linked to the "metabolic syndrome", a broad range of changes in metabolic processes and immune function. As a first approximation, we agree with this formulation but we will then proceed to document some of its weaknesses. (i) Crude mortality rates increase with increasing body mass index (BMI) but as the BMI approaches the normal range, mortality rates reverse (the now classic "J-shaped curve") so that individuals with reduced BMI have elevated mortality. (ii) A multiplicity of medical and surgical conditions have been reported where short term and medium term outcomes are better for overweight patients. These conditions are placed under the heading of "obesity paradox". (iii) The medical community has introduced a binary system for the metabolic syndrome ---- yes, patient has it or no, the patient does not have it, despite the fact that all of the changes that are considered components of the metabolic syndrome are continuous variables. Obesity negatively affects quality of life of children and its high prevalence is a global problem. Obesity is a multifactorial disease, whose formation influence genetic and exogenous factors. Exogenous factors can act directly on obese individuals and can be defined by the markers of personal history, or may act within the family and then they are define by the markers of family history. The health risks associated with obesity are high and therefore you need to focus on the treatment of obesity in childhood. In childhood it is still possible to change unhealthy habits and lifestyle. The aim of this work is to describe the relationship of severity of obesity of obese children in the selected data from their personal and family history and analyze the success of reducing treatment of children in dependence selected aspects of their personal and family history. Monitored set includes 398 girls and 216 boys aged 6 to 18 years who were examined anthropometric at the beginning and in the end of the stay in the hospital, Dr. Philip in Poděbrady, and whose parents responded to the survey questions for parents of obese children. This dissertation is based on the possibilities of improvement of obesitological care as delivered by general practitioners. The theoretical part is dedicated to marketing, obesitology and to the lesser known problem of gynoid obesity. The practical section describes the quantitative research into GP's delivering adult care in the Prague area, central Bohemian area, and amongst the clients of Centrum Preventivní Medicíny. The needs of GP's in terms of this type of care are examined. The main needs are elementary and further education, information about the problems of gynoid obesity, and the need for GP's to co-operate with obesitological institutions, including centres of gynoid obesity therapy. At the end of the thesis is a suggested list of products for Centrum Preventivní Medicíny. Also suggested is an algorithm of care for gynoid obesity for use in GP's practice. The diploma thesis is focused on evaluating changes in quality of life in overweight and obese persons. The theoretical part discusses quality of life, various aspects of obesity and the most relevant psychological context. The empirical part includes two studies: 1. Study in overweight / obese persons undergoing physical activity intervention - walking, lasting up to 1 year, and covering 24 female participants. The intervention resulted in a significant effect on the quality of life, assessed using the following instruments: IWQOL-Lite, OWLQOL and WRSM. 2. Study in overweight / obese persons undergoing STOB basic intervention lasting 12 weeks, and covering 17 female participants. The intervention resulted in a significant effect on the quality of life, assessed using the following instruments: IWQOL-Lite, OWLQOL and WRSM. Changes in self-esteem and self-efficacy were also assessed in this population using the instruments: RSE and GSE, respectively. Statistical significance was not achieved in either of these cases, although a trend toward change could be seen according to RSE and GSE, respectively. ABSTRACT: This thesis deals with the obesity of older school age children and their dependence on eating and exercising habits. The theoretical part summarizes knowledge regarding overweight and obesity, disease diagnosis, epidemiological trends and complications related to obesity. Emphasis is put on the evaluation of reasons to why this problem occurs, such as disease prevention, dietary and exercise recommendations, its therapy and inclusion of the issue with the secondary school curriculum. The practical part contains research focused on the nutritional status, eating and exercise habits of pupils in secondary school. The survey results are designed for use in pedagogical practice. Lifestyle diabetic 2nd type Aims: The aim of this thesis is to outline alternatives how to improve medical condition of patients who suffer from diabetes mellitus 2nd type by non- pharmacological treatment. Another target is to create and application an intervention exercise program. It is also necessary to evaluate measured physical parameters plus to compare results before the beginning and after the termination of the intervention. Last but not least is important suggestion of recommendations for the future therapy of patients with NIDDM. Methods: In this work, there are stated contemporary information of relationship between physical activity and diabetes of the second type of a patient. Literature research was used to get this knowledge. Based on these facts and available options, the experiment related to training program was designed. This observation takes 20 weeks. Results: The controlled diabetic patient underwent the intervention program without significant complications and in full extent. Non-pharmacological treatment focused primarily on increasing physical activity performed by walking and practicing on a exercise bike, on the contrary, dietary regimen was not changed or a adjusted. Title: The evaluation of physical activity of children of school age Objectives: The aim of the study is to assess physical activity in children of school age in Prague 6 and evaluate the main factors influencing physical activities and the possible correlation between these factors and levels of physical activities. The work includes the spectrum of information about physical activity and its importance in the pediatric population, level overview of physical activities children in the world and its measurement, then the description of programs to support the physical activity. Methods: The research was carried out using non-standardized questionnaire. The questionnaire was distributed to 15 elementary schools located in Prague 6 among students of 2nd and 6th class. We used the Microsoft Excel software to analyse the data. Results: We found that the largest number of children does sports 2-4 hours a day. Boys and younger students were more physically active than girls, and older students. The results of the work suggest that children in the Czech Republic are, in the monitored parameters of physical activity (frequency, time), at a similar level as other children abroad. This thesis examines the surgical treatment of obesity from the perspective of the patient. The theoretical portion deals with etiopathogenesis, diagnosis, complications, related illnesses, conservative and surgical treatments (definitions, overview and types, principles of basic bariatric approaches, as well as effects and indications, side effects, and results of treatment. It also includes the specifics of the pre-operative preparation, immediate and long- term post-operative care, psychological preparation and post-bariatric-surgery nutrition. The empirical portion consists of quantitative research in the form of a survey instrument with closed-end questions with the possibility of individual responses. The respondents were patients who had been diagnosed as morbidly obese or third-degree obesity and who had also been through surgery within the past three years. One hundred and fifty surveys were distributed, 125 were returned, and 10 of those were not used for incompleteness, and another 6 were not used because the respondents did not meet the criteria. The theory focuses on these terms: the health, the determinants of health, the lifestyle, the moves-activity, the nourishment, the psychic hygiene and the lifestyle diseases. The practical part contains the research which is accomplished using questionnaire survey. The main target is getting information about teacher's lifestyle at chosen schools It was fulfilled and we discovered the teachers at schools in Vlašim, Domažlice, Prague and Bělá nad Radbuzou are trying to live healthy, even though not all of them like this lifestyle.
