A survey of the occlusal traits in an adolescent population in Uganda

Magister Chirurgiae Dentium (MChD) / Epidemiological studies on malocclusion have been primarily concerned with its aetiology and distribution. However, due to the varied and often subjective methods of assessment, many of these studies provide conflicting data related to malocclusions (Solow, 1970). To address this, the Fédération Dentaire Internationale (FDI) in close collaboration with the World Health Organisation (WHO) developed an objective method for measuring occlusal traits (Baume et al., 1973). This method was used to obtain the epidemiological data on occlusal traits for 14-year-old children Kampala in order to provide baseline data related to malocclusion. A total of 402 subjects were examined. Of these 65% were female and 35% were male. Thirty percent of the sample had at least one dental anomaly. The most commonly extracted teeth were mandibular first molars (43.6%) and maxillary canines (17.3%). The high frequency of extracted permanent canines in this sample is unique. By and large, many of the occlusal traits related to the canine are attributable to the practice of ebinyo, a form of dental mutilation, which still seem to be rife in many communities in Uganda. Similar to other studies (Massler and Frankel, 1951; de Muňiz, 1986; Ferguson, 1988; Kaka, 1993), mandibular first molars were five times more likely to be missing than maxillary first molars while the ratio of missing maxillary to mandibular canines was found to be 2.5:1. Crowding was most frequently observed in the mandibular incisal segment while spacing was mostly in the maxillary incisal region, thus supporting the view of Brunelle et al., (1996) that although prevalence may vary from study to study, more people have malaligned mandibular incisors than maxillary incisors concurs with these studies. Of the subjects studied, 6.8% had a diastema of 3mm and more. Regarding space measurements, 17.9% of the sample population had at least one segment with crowding, 18.2% had at least one segment with spacing while 33.6% had some degree of incisor malalignment and 54.2% of the sample population had some form of space anomaly. About 70% of the subjects had symmetric molar relationships of which 54.2% were Class I, 9.2% Class II and 3.2% Class III similar to the universal distributional pattern. Crossbites were not a major finding, and were present in only 7% of the sample. The distribution of overjet was relatively symmetrical on the left and right. Negative overjet (anterior open bite) was a rare occurrence in an average of 2.4% of the population. In 77.6% of the population, the overjet ranged from 1 to 4mm, and in 22.4%, the overjet was either edge-to-edge, reverse or 5mm and over. About 7.3% had an overjet of 5-6mm while only 2.1% had an overjet greater than 6mm. Approximately 53.7% of the population had an overbite of 1-3mm. Edge-to-edge and open bite incisor relationships were found in less than 10% of the sample. Although ideal occlusion as described by Angle (1907) in the Ugandan sample is very rare, using the data derived from this study, normal occlusion as defined by statistical distribution can be described. / South Africa

O ajuste oclusal (AO) como parte da terapia periodontal é um tema controverso, principalmente porque a literatura não fornece evidências suficientes sobre a influência do trauma de oclusão (TO) na doença periodontal (DP). A necessidade de ajuste oclusal na terapia periodontal é considerada incerta e requer investigação. O objetivo desta revisão sistemática foi identificar e analisar estudos que investigaram os efeitos do AO associado à terapia periodontal, sobre os parâmetros periodontais. Um protocolo foi desenvolvido incluindo todos os aspectos de uma revisão sistemática: estratégia de busca, critérios de seleção, seleção de métodos, coleta de dados e extração de dados. A pesquisa bibliográfica foi realizada utilizando MEDLINE via PubMed, Cochrane Central Register de estudos controlados e EMBASE. Os títulos e resumos de artigos foram selecionados de acordo com critérios estabelecidos na metodologia. Cada artigo que indicava uma possível correspondência com estes critérios, ou não poderia ser excluído baseado nas informações presentes no título ou no resumo, foi considerado e avaliado. Na seleção final, quatro artigos foram incluídos. Embora os estudos selecionados sugerem uma associação entre AO e uma melhora nos parâmetros periodontais, suas questões metodológicas (exploradas nesta revisão) sugerem a necessidade de novos estudos com maior qualidade. Atualmente as evidências presentes são insuficientes para presumir que AO é necessário para reduzir a progressão da doença periodontal. / Occlusal adjustment (OA) as part of periodontal therapy is a controversial theme, mostly because the literature does not provide enough evidence regarding the influence of trauma from occlusion (TO) on periodontitis. The need for occlusal adjustment in periodontal therapy is considered uncertain and requires investigation. The aim of this systematic review was to identify and analyze those studies that investigated the effects of OA, associated with periodontal therapy, on periodontal parameters. A simetria da parte superficial do músculo masseter e da parte anterior do músculo temporal foi mensurada durante o período da mínima estabilidade mandibular, que ocorre no início da intercuspidação. Participaram da pesquisa 26 voluntárias com disfunção temporomandibular (DTM), dentre as quais 13 foram avaliadas exclusivamente após a realização do Ajuste Oclusal Funcional; 12 voluntárias foram avaliadas após a realização de uma simulação de ajuste oclusal (placebo) e em outro momento, após a realização do ajuste oclusal efetivo. Uma voluntária participou exclusivamente na condição placebo, em vista de sua desistência de participação na segunda etapa. Foram observados 13 casos tratados de forma não efetiva (placebo) e 25 casos tratados com Ajuste Oclusal Funcional efetivo. Em cada caso foram efetuadas avaliações em 3 fases: pré tratamento (Pré), imediatamente após (Pós) e 1 semana após o procedimento (Pós1S). Os índices de simetria se alteraram de forma significativa em todos os músculos do grupo tratado entre as fases Pré e Pós (p<0,05) e não foram observados indícios de diferenças entre as fases Pós e Pós1S (p>0,05). No grupo placebo não foram detectadas quaisquer indicações de efeito significativo (p>0,05). O aumento da simetria da atividade muscular, da simetria do tempo de ativação e a redução significativa dos níveis de dor (p<0,05), observados exclusivamente no grupo tratado, sugerem a eficácia do AOF como fator de estabilidade mandibular, e como tratamento de DTM / Abstract: The effectiveness of Functional Occlusal Adjustment (FOA) on the mandibular stability, which is based on the evaluation of palpation and articular paper brands generated over the teeth, was assessed in this study by the symmetry of the masticatory muscles and reporting pain. The symmetry of the superficial part of the masseter muscle and the anterior part of the temporal muscle was measured during the period of minimal mandibular stability, which occurs in the beginning of the intercuspation. Participants were 26 volunteers with temporomandibular disorders (TMD), among which 13 were evaluated only after the completion of the FOA, 12 patients were evaluated after performing a simulation of occlusal adjustment (placebo) and at another time, after effective realization of occlusal adjustment. One voluntary participated only in placebo condition, in view of its withdrawal of participation in the second stage. A total of 13 cases treated non-effective (placebo) and 25 cases treated with effective FOA. Each case was evaluated based on three phases: pre-treatment (Pre), immediately after (Post) and 1 week after the procedure (1WP). The symmetry indices changed significantly in all muscles of the treated group between the pre and post phases (p<0.05) and revealed no evidence of differences between phases Post and 1WP (p>0.05). In the placebo group were not found any indication of a significant effect (p>0.05). The increase of the muscular activity of symmetry, the symmetry of the activation time and a significant reduction in pain levels (p<0.05) only observed in the treated group, suggesting the effectiveness of the FOA as a factor of mandibular stability and for the treatment of TMD / Mestrado / Anatomia / Mestra em Biologia Buco-Dental

Orientador: Renata Garcia Fonseca / Resumo: Esta tese consiste em dois estudos, ambos investigando materiais monolíticos fresados por tecnologia CAD-CAM. Primeiro estudo. Objetivo. A resistência ao desgaste e abrasividade do Lava Ultimate, Vita Enamic, Vita Suprinity, IPS e.max CAD e Lava Plus, bem como algumas propriedades que podem estar relacionadas, foram analisadas. Métodos. Espécimes desses materiais tiveram sua rugosidade, dureza e coeficiente de atrito avaliados, respectivamente, em microscópio confocal, microdurômetro e tribômetro. O teste de desgaste conhecido como "2-body", no qual os materiais atuaram como abrasivos e, juntamente com o esmalte bovino, também como antagonistas, também foi realizado. A taxa de desgaste foi determinada com perfilômetro de superfície e as superfícies desgastadas foram observadas por microscopia eletrônica de varredura (MEV). Resultados. Vita Enamic e Lava Ultimate mostraram a maior rugosidade, enquanto IPS e.max CAD e Vita Suprinity, a menor. O resultado da dureza foi Lava Plus> (Vita Suprinity = IPS e.max CAD)> Vita Enamic> Lava Ultimate. O Lava Ultimate exibiu maior coeficiente de atrito do que o IPS e.max CAD e o Lava Plus. O Lava Plus e o IPS e.max CAD mostraram um potencial significativamente maior para desgastar o Lava Ultimate. Estes dois materiais, juntamente com o Vita Suprinity, foram os que mais desgastaram o esmalte e o Vita Enamic. Vita Suprinity e IPS e.max CAD promoveram o maior desgaste do Lava Plus, e o inverso também ocorreu. Vita Enamic e Lava Ultimate causar... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: This thesis consists of two studies, both investigating the computer-aided design and computer-aided manufacturing (CAD-CAM) monolithic materials. First study. Aim. The wear resistance and abrasiveness of Lava Ultimate, Vita Enamic, Vita Suprinity, IPS e.max CAD, and Lava Plus, as well as some properties that might be related to, were analyzed. Methods. Specimens from these materials had their roughness, hardness, and coefficient of friction evaluated, respectively in confocal microscope, microdurometer, and tribometer. The 2-body wear test, wherein the materials acted as abraders and, together with bovine enamel, also as antagonists, was also carried out. The wear rate was determined with surface profilometer and the worn surfaces were observed by scanning electron microscopy (SEM). Results. Vita Enamic and Lava Ultimate showed the highest roughness, whereas IPS e.max CAD and Vita Suprinity, the lowest. The hardness result was Lava Plus > (Vita Suprinity=IPS e.max CAD) > Vita Enamic >Lava Ultimate. Lava Ultimate exhibited a higher coefficient of friction than IPS e.max CAD and Lava Plus. Lava Plus and IPS e.max CAD showed significantly higher potential to wear Lava Ultimate. These two materials, together with Vita Suprinity, provided the highest wear of enamel and Vita Enamic. Vita Suprinity and IPS e.max CAD exhibited the highest wear against Lava Plus, and the inverse also occurred.
