Úloha sestry v zajišťování potřeby spánku u hospitalizovaných dětí / The nurses role in ensuring the need of sleep in hospitalized children

Abstract Sleep disorders occur frequently during child development. Cause of these disorders are physical but more frequently psychogenic. In diploma work we addressed the nurse role dealing with sleep necessity for child in-patients.. In the theoretical part we dealt with sleep characteristic,sleep disorders and child needs which can be detected not only with child in-patients in hospital ward. Furthermore we engaged in sleep fysiology and other factors which can influence the sleep quality. In the quest for that I did use the semi-structured conversation conducted in hospital child ward in Tábor hospital a.s .and České Budějovice hospital a.s. The conversation contained 15 questions which were answered individually by nurses. Data collection took place in April, 2012. The total number of questioned nurses was 16. In the connection with the theme 2/two objectives and 2 research questions were set. To achieve the objective it was necessary to find what is important to secure child in-patient sleep and find if nurses provide conditions for child in-patient quality sleep. The research questions were raised: ? What principles are considered by nurses to be important to secure the child in-patient sleep mode? ? and secondly ? How the nurses secure the conditions for quality sleep of the child in-patient? ?. As for the theoretical field concerns the nurses do not have the basic knowledge of the sleep fysiology (REM, non REM). As it comes out from response of the nurses, each nurse has an individual opinion of the factors which ensure the child in-patient quality sleep. The conditions for quality sleep are secured by nurses in creating quiet environment, cool room and where necessary also their presence with falling asleep children. The diploma work results are individual On the basis of these findigs, I consider to be necessary, the educative material to be worked out and acquaint the child nurses with its content.

Naplňování kritérií pro saturaci potřeb pacienta / Fulfilling the criteria for saturation of the patient's needs

The thesis deals with the realization of the criteria for saturation of the patient's needs. In the current system of nursing care, the principles of modern nursing are failing to realize and therefore we want to find out the main obstacles of this situation and the possibilities of their removal. For the purpose of the thesis there were set three objectives. The first of them has set the objective of finding out the most common problems in meeting the criteria for saturation of the patient's needs. The research showed that the nurses see the problem in a lack of staff at the wards, a lack of time, excess of documentation, excessive demands for nurses. The purpose of the second objective was to find out what supports the realization of the criteria for saturation of the patient's needs. Results have shown that what helps nurses is a positive work environment, motivation, nurse´s personality, own scale of values, salary, good leadership, organization, nursing care. The third objective was to find out the opinion of nurses on meeting the criteria for saturation of the patient's needs. It turned out that the majority of respondents think that nurses do not effectively satisfy all the needs of patients for a number of reasons: there are few nurses and they are overloaded, the nurses do their best but still cannot satisfy all the patients´ needs. We also found out that the higher patients´ needs are omitted. Less than a third of the nurses indicated that nurses effectively meet all the patients´ needs. For the qualitative part of the research, we set two research questions in advance. Which criteria nurses use in patients´ needs saturation? The research showed that time, which is very valuable for nurses and very often is in short supply, is an important criteria for the saturation of the patient's needs. Furthermore, it is knowledge and manual dexterity when nurses additionally reported a lack of education of some nurses especially within the nursing process. As the next criteria, it was mentioned the cooperation with the patient as it is necessary to engage patient himself in care planning. The next criteria is monitoring of the patient´s response, which is included in every phase of the nursing process. Furthermore, the nurses agreed on criteria, which include communication as a key skill of every nurse, then individual care, a suitable environment and utilities, education, critical thinking, effective nursing process and empathy. The second research question tried to find out which obstacles prevent nurses in effective saturation of the patients´ needs. We found out that nurses are concerned about a lack of staff at wards and the excess of administrative load, which is related to a lack of time for patients. These three factors are also obstacles for nurses and prevent them from effective satisfying of the patients´ needs. The next obstacles in satisfying the needs are burnout, reluctance of nurses to work, excessive demands for nurses, poor workplace relationships, a lack of evaluation both in a form of praise and appreciation, and finance. We also found a lack of information, which is related to a lack of time, as nurses do not have a chance to collect all the information concerning the patient. As another possible obstacle, nurses also mentioned personal problems. The results of the thesis can be used as a small contribution to currently ongoing effort of the Czech Association of Nurses.

Handicap jako stigma, aneb přístup sestry ke znevýhodněnému jedinci / Disability as a stigma, or on approach of nurses to persons with disability

The aim of the research is to find out the existence of prejudices about patients with handicap, to map caregivers' awareness of stigmatization, to identify the specific needs of handicap patients/clients and how they reflect on the nursing care. The notions of handicap, stigma, stigmatization, development of nursing care for handicapped patients, specification of nursing care, and importance of family members for nursing care are defined in the theoretical part. The practical part consists of the research, which was done by semi-structured interview. A pre-research was conducted before the research itself. In the pre-research, it was ascertain if the questions are understandable and the interviewees' answers are relevant. Ten nurses participated in the pre-research. Based on their remarks, the list of questions was updated and reworked. Thirteen representatives of nurses working in hospitals participated in the research. The selection of interviewees was deliberate and the only criterion for participation in the research was providing care for handicapped patients. Interviews were recorded and then transcribed in Microsoft Word. Individual answers were repeatedly read and then the parts related to the aims were highlighted in the text. All utterances were divided into groups according to the interviewees. After the realization of interviews and their transcription, the data was reduced, analysed, and divided into categories and subcategories. In some cases, there were similar subcategories, so they merged into superior category. The interviewees' awareness of stigmatization is not accurate and the caregivers' prejudices originate from mentally and socially handicapped. The undesirable factors, that affect nurses, are wrong communication, incorrect monitoring of specific needs, restricted or negative previous experience, and fear of the contagion of infectious disease that patients/clients can have. The interviewees state that after providing nursing care for the patients they suffer from higher amount of stress. We think, that understanding this problem can lead to the improvement of monitoring specific needs, avoiding complications, and more effective nursing care for handicapped patients/clients.

Annotation Subdeprivation is one of the most aspect of today´s time. In my dissertation I accent the complicated situation in which our children´s future is. At the theoretic part of elaboration I issued from researches Prof. Matějček. He was engaged in these questions. For this research were chosen two groups. Experimental group of children´s homes and control group, what was chosen according to the same age and sex the child. Both groups visit the same primary school and the same form. Children were monitoring in socialization, school results, health. Quest is processed, evaluationed, the answers you can see in the chart in the practical part of dissertation.

Posouzení stravovacích návyků a nutriční zásobenosti u seniorů / Evaluation of nutritional tendencies and nutritive supply at seniors

Thesis was solving in senior home of Mistra Křišťana in Prachatice. The goal of thesis was to evaluation nutritional tendencies and nutritive supply at seniors institutional care it also structure on menu and analyses filling of norm of need from the view of intake energy, macronutrients and selected micronutrients. The concept of thesis is divided into two parts. The first part comparances intake energy and the norm of need at seven men and women during ten months. The second part presents results of questionary which was selected three hypothesis. The first hypothesis: Monitoring health nutrition of respondents concidering their education. The second hypothesis: Respondent{\crq}s physical activites concidering their sex. The third hypothesis: Monitoring of observance dribling regime concidering their sex. The result of thesis say that all the women who were monitored were overweight. It was analyses on rate body mass index and percent of fat at also perimeter of waist. Body Mass Index rate at man was usually in optimal range 20 - 25, despite this high percent of body fat, which is created by decreasing of muscle tissue and increasing of body fat. Discovered rates correspond with results comparising intake energy and nutrients with norm of need. All respondents had high intake of lipid and high intake proteins especially women. Intake of sacharide and dietary fiber was optimal. Be results show that respondents have low intake of vitamin D. When compared average intake vitamins and minerals with the norm of need. At minerals the increased intake selenium was found-approximately about fourteen percent from norm of need. At evaluation calcium and phosphorus was analysed opposite rate 1:1,2. Calcium intake is long time under norm of need in divide 20 - 40 %. The same conclusion is valid for magnesium. Magnesium intake is 15 - 30 % under norm of need. The results were consulted with a diet nurse who had information of higher creation of osteoporosis at seniors. The results of three testing hypothesis in the second part of thesis are: monitoring health of food of respondents depends on their education. The second testing hypothes showes that physical activites at seniors doesn{\crq}t depend on their sex. The last testing hypothes found out that krepiny drinking regime doesn{\crq}t depend on their sex. Thirty questionary were analyses twenty women and ten men took a part. Women were more interested in the questionary than man because they were more interested in questions of health food. A questionary included fourty-six questions which were divided into four parts. The first part is focused on information about respondents. The second part is about correct dribling regime and the fourth part of the questionary is focused on satisfaction on seniors in senior home. Part of thesis is recommendation on what concentrate attention in nutrition client senior home.

The diploma emphasizes the rediscovered holistic view of humans as bio-psycho-socio-spiritual beings, characterizing the spiritual dimension in a more detailed way and clarifying what the role of Christian denominations clergy may be in fulfilling spiritual needs of the clients. The theoretical part of the work deals with the spiritual dimension of a human in general, then it explains the term of religion regarding Christian roots of Europe and the position of spirituality among the needs of a human. It also deals with characteristics of possible spiritual needs of the clients, the role of clergy in fulfilling spiritual needs of the clients and the concept of sacramental care in selected Christian congregations. The work itself aims to characterize the ministry of clergy and its place in fulfilling spiritual needs of the clients. In the conclusion the author{\crq}s views of the field of spiritual needs of the diseased are presented as well as recommendations for practice.

The thesis is structured into the theoretical part, which deals with the needs of children in relation to the pre - school facility garden. Large part is aimed to movement and play. Thesis also deals with natural gardens and a list of natural game elements. The practical part describes in detail the current state and equipment of the school garden belonging to the faculty pre - school facility called Sluníčko pod střechou which is connected to Faculty of Education of Charles University. The analysis of the conditions of this garden is based on research methods of direct observation, questionnaire survey with teachers, and then method of interview with the school head. Furthermore, this part of the thesis is also devoted to research aimed at finding out what children's ideas about the equipment of this garden are. The research was based on the work of children with a pre - prepared research tool. Research has shown that the teachers are happy with the current look of the garden. They would only add to it some small gaming elements. Children like elements that are part of the current garden design. They prefer them to the new modern ones. From the point of view of small toys and aids, the conditions of the kindergarten are less suitable. Children mainly mentioned things that are not a regular part...

The master's thesis "Educational needs of headmasters at various school levels" deals with education of headmasters of various school levels. In particular, it deals with educational needs of headmasters at kindergartens, primary schools and secondary schools. The aim of the thesis is to find differences among educational needs of headmasters of various school levels and its mutual comparison. Theoretical part works with resources for identifying and comparison of educational needs. Those educational needs are in this thesis identified through "Competency model of the headmaster of the school". Headmasters of mentioned school levels specify required level of partial competencies and evaluate its real situation. Headmasters also have the possibility to bring their own proposal for type of educational need and provide also their recommendation for supplementation of current offer in the field of education. Research was realized through the form of questionnaire. Findings of the research brought, that educational needs of headmasters of secondary schools differ a lot from needs of headmasters of kindergartens and primary schools.
