Promoting and implementing self care : a mixed methods study of offshore workers and remote healthcare practitioners

The oil and gas industry is a vital contributor to the global economy and a key source of employment within oil-producing countries. Oil production is largely dependent on a skilled population who are adept in coping with the demands of an offshore environment. Due to the high risk nature of work offshore, it is a requisite that personnel engage in health promoting behaviours. The research aimed to identify aspects of offshore workers self care which required behaviour change and the behavioural determinants which were associated with engagement in self care. A mixed methods design was utilised to generate novel data and original findings. Phase 1 used a quantitative cross-sectional online survey to assess offshore workers’ (n=352, 53.6% response rate) health, quality of life, mental wellbeing and self care status. The findings highlighted key areas of concern, as indicated by negative scoring across measures, relating to: overweight/obesity; medication adherence; absenteeism (with regard to travelling offshore); medical evacuation; lack of adherence to 5-a-day fruit and vegetable guidelines; physical activity; smoking; hazardous alcohol use, and insomnia. Phase 2 used qualitative theory-based telephone interviews to explore self care behaviours from the perspective of offshore workers (n=16). Offshore workers who had completed a survey and indicated they would like to receive further information on the interviews were invited to participate. Both the interview schedule and data analysis were informed by the Theoretical Domains Framework (TDF). Healthy eating and physical activity were the behaviours most frequently discussed by offshore workers and identified as areas requiring behaviour change. TDF domains representing both behaviours included: beliefs about capabilities; beliefs about consequences; intentions; goals; memory, attention and decision processes; environmental context and resources; social influences; emotion, and behavioural regulation. Phase 3 used qualitative theory-based telephone interviews to explore offshore workers’ (n=13) self care behaviours from the perspective of remote healthcare practitioners. Both the interview schedule and data analysis were informed by TDF. Healthy eating and harmful/hazardous alcohol use were the behaviours most frequently discussed by remote healthcare practitioners and identified as areas requiring behaviour change. TDF domains representing both behaviours included: knowledge; environmental context and resources; social influences; emotion, and behavioural regulation. The findings, when triangulated suggest that offshore workers may benefit from the implementation of a self care intervention which targets healthy eating, physical activity and alcohol consumption. It is advised that the intervention target multiple self care behaviours and that development is underpinned by behaviour change theory to ensure effectiveness. The intervention may be tailored in accordance with the TDF domains identified in this research as determinants of healthy eating, physical activity and alcohol use behaviours.

The Demographic, Behavioral, and Clinical Factors associated with the Quality of Life of Cancer Survivors

The objective of this thesis is to assess the prevalence of low quality of life in adult cancer survivors and predictors for these low scores, separately for physical and mental quality of life. Possible predictors can include sociodemographic, clinical, and behavioral factors. The data source for this thesis is the National Health and Nutrition Examination Survey (NHANES) 2005-2012. The sample population included 1244 cancer survivors 20 years or older that answered “yes” to the question “have you ever been told by a doctor or other health professional that had cancer or malignancy of any kind?” Mental and physical health scores were determined based on the number of days physical and mental health was not good and dichotomized according to mean value. The four groups identified were low mental health and low physical health (n=148, 11.9%), low mental health and high physical health (n=239, 19.2%), high mental health and low physical health (n=143, 11.5%), and high mental health and high physical health (n=714, 57.4%). The diet quality was calculated using the Healthy Eating Index 2015, which measures adherence to the Dietary Guidelines for Americans 2015-2020. The SAS Survey Analysis Procedures was used to account for the random multi-stage sampling of NHANES. Also, ANOVA and chi-square tests were used to determine the possible predictors of low quality of life while multivariable logistic regression analysis was performed to examine the independent predictors of low physical and mental health. Treatment of Severe Caries and Molar Incisor Hypomineralization and Its Influence on Oral Health-Related Quality of Life in Children: A Comparative Study

Background: Treatment of oral diseases can have a long-lasting impact on a child’s life well beyond its childhood years. The purpose of this study was to compare the impact of treatment on the oral-health-related quality of life (OHRQoL) of children with severe caries and severe molar incisor hypomineralization (MIH). Methods: A total of 210 children (mean age 9 years; 49% female) with severe caries (inner third of dentin) and severe MIH (post-eruptive breakdown, crown destruction) were included in the study. Both groups were matched according to age, gender, and social status. The German version of the Child Perception Questionnaire for 8–10-year-olds (CPQ-G8–10) was used before and after treatment to analyze the impact on OHRQoL. Results: Patients with severe MIH showed a significantly higher total CPQ score (17.8 (±10.6)) before treatment compared to the caries group (13.8 (±14.3)). The mean CPQ score in all subdomains decreased significantly after therapy in the MIH group. Children with severe carious lesions had similar results except in the domain “functional limitations”, as treatment led to only minor changes (2.9 (±3.6) to 2.2 (±2.6)). Conclusions: Despite a narrower treatment spectrum, patients with severe MIH experienced a greater overall improvement in OHRQoL compared to the caries group.

More specifically, it has intended to: identify the social, economical, cultural and demographic profile of adults that participate on activities promoted by the Solidary Economy; evaluate the quality of life of these individuals; describe the understanding that these workers have regarding the concept of quality of life. Sixty nine people were part of this study; workers from three different recyclable garbage collection cooperatives. A questionary especially elaborated for this study was applied containing the workers identification ata; with the purpose of making a databank of social, economical, cultural and demographic aspects. In this questionary the criteria of income classification and social class of the Brazilian Association of Research Institutes (ABIPEME) was incorporated along with the Whoqol Bref Life Quality Scale. It was verified that the workers present a good level of life quality. Regarding the social, economical, cultural and demographic data, there was a dominance of single or married female with children. Regarding the quality of life, there was a significant correlation between the physical and psychological domains and also the correlation between the psychological domains and the social relations. On the other hand, the environmental domain was the one that presented the lowest data regarding quality of life. The hypothesis is that the fact that the workers are unsatisfied with their insufficient incomes, their precarious homes, most of them coming from a very competitive and individualistic economical system and they not having the necessary adaptation that this new system requires, the workers have intervened on the environment domain in a negative way.(AU) / Este estudo buscou estudar qualidade de vida em indivíduos que trabalhavam em cooperativas com a filosofia da Economia Solidária. Mais especificamente procurou: levantar o perfil sócio econômico cultural e demográfico de indivíduos adultos integrantes de programas de Economia Solidária; avaliar a qualidade de vida desses indivíduos integrantes de programas; descrever a compreensão desses trabalhadores sobre o conceito de qualidade de vida. Participaram desse estudo 69 pessoas, trabalhadores de três cooperativas distintas de catadores de lixo reciclável. Foi aplicado um questionário elaborado especialmente para esse estudo contendo dados de identificação dos participantes, com o propósito de fazer um levantamento relativo aos dados sócioeconômicos, culturais e demográficos e, nesse questionário foi agregado o critério de classificação de renda e classe social da Associação Brasileira dos Institutos de Pesquisa - ABIPEME e utilizou-se a Escala de Qualidade de vida Whoqol Bref . Verificou-se, que os trabalhadores apresentaram um bom nível de qualidade de vida. Em relação aos dados sócio-econômicos culturais, observou-se um predomínio da presença de mulheres, solteiras ou com parceiros, mas com filhos. Com relação aos domínios que compõem Qualidade de Vida, houve uma correlação significativa entre os domínios físico e psicológico e também uma correlação entre os domínios: psicológico e de relações sociais. Por outro lado, o domínio meio ambiente foi o que apresentou um índice menor de qualidade de vida em relação aos demais. Levanta-se a hipótese de que o fato dos cooperados estarem insatisfeitos com seus ganhos, por os considerarem insuficientes, acrescido de suas condições de moradia serem precárias, além da maioria ser oriunda de um sistema econômico competitivo e individualista e não terem ainda a devida adaptação que esse novo sistema exige, interferiram no domínio meio ambiente de forma negativa..(AU)
