Reamer-Irrigator-Aspirator Multiuse Application in the Treatment of Chronic Osteomyelitis

INTRODUCTION: The treatment of chronic intramedullary infection of the long bones relies on microbe-specific antibiotics in conjunction with surgical removal of infected necrotic material. We discuss the use of reamer-irrigator-aspirator(RIA) for debridement of the intramedullary canal instead of conventional reaming techniques. This is the first case report to explore the use of RIA for osteomyelitis. CASE REPORT: We discuss the use of the RIA in treatment of a 26-year-old female presenting with chronic osteomyelitis of the left distal femoral shaft. She had normalization of infection laboratories at 6 weeks and complete resolution of symptoms at 3 months and was released at 6 months. CONCLUSION: Recent exploration of the RIA system's (Synthes®, Inc. West Chester, Philadelphia) multipurpose applications has indicated use in long bone debridement. While further exploration and high-quality studies are needed to make robust claims of efficacy, we believe that the use of RIA in the context of chronic osteomyelitis is a superior alternative to conventional reaming techniques.

The main goal of this diploma thesis is to increase cost savings on cutting tools, which are used to ream piston hole inside the housing of CP3 diesel pump made by Bosch Diesel s. r. o. Diploma thesis is divided into a theoretical description of the issue, which is initial knowledge for machining tests, and experimental part, whose task it is to find a suitable reamer for this technological operation. Cost savings will be achieved by selecting suitable tool instead of the serial tool that has been used so far.

The Master’s thesis is focused on reaming technology using PVD coated reamers with cermet cutting edge. This work point out a historical evolution of reaming technology as well as recent requirements to the quality of produced surfaces. Reamers characteristic, splitting, mechanical designs, cutting tool geometry as well as the assumption of a reamer potential evolution are presented. Application of cermet tool material and PVD coatings to reaming tools is considered. The work contains a review of present statement at reamer market including cermet reamers and provided single cells (elements) of the Tool Management. The experimental part of the work is focused on product manufacture qualities verification of the changeable PVD coated reamer head made by HAM-FINAL ltd. Economic analysis in case of investment to the reamer purchase is demonstrated on the example of the practical application of the reamer.

The diploma thesis deals with the design of the hole-making technology for hydraulic components that are specific for its high requirements on size and shape accuracy. The aim of diploma thesis is to compare the productivity, efficiency and accuracy of reaming with two different reamers – the solid carbide reamer and the newly designed reamer that combines two cermets on cutting part of the tool. In the practical part, the data analyses and calculations were made. It was discovered that the newly designed tool brings economic benefits while the qualities of final hole are improving or remaining at the same level.

The gauge is unique and is designed for a company manufacturing concerete. It was designed in accordance with requirements and a prototype was tested for functionality. There is made analysis, development, design, theoretic estimation and practical verification of accurancy.

The aim of the thesis is to design, conduct and assess an experiment seeking to look into the utility properties of recent tools manufactured by HAM-FINAL. The tools include polycrystalline diamond (PCD) and CVD diamond cutting edges. In the theoretical part, the attention is devoted to cutting materials with an emphasis on diamond materials, as well as to issues associated with the wearing of the cutting tools, requirements specified for precision of bores and tools used in the manufacture of precision bores. The paper further provides an overview of a selection of world’s leading manufacturers of PCD blanks, CVD diamond coatings, CVD diamond layers and reamers with PCD cutting edges.
