Epidemiological studies of childhood diabetes and important health complications to the disease

Background and aims: The overall aim of this thesis was to increase knowledge regarding the occurrence of childhood onset T1D and T2D in Sweden and in relation to that describe and elucidate important aspects on two grave complications to diabetes; end-stage renal disease (ESRD) and mortality. The two first studies included in this thesis aimed to describe and analyze the cumulative incidence of childhood onset T1D in Sweden and to assess the occurrence of undetected T2D in Swedish children. The aim with the third study was to describe the cumulative incidence of ESRD, and to analyze how ESRD risk differs with age at-onset and sex. The aim of the fourth study was to show how parental socioeconomic status (SES) affects all cause mortality in Swedish patients with childhood onset T1D. Study populations: The foundation for the studies on T1D was data from the Swedish Childhood Diabetes Registry (SCDR). When studying ESRD we also included adult onset T1D cases from the Diabetes Incidence Study in Sweden (DISS). The study on T2D was a population-based screening study where BMI was measured in 5528 school-children and hemoglobin A1c was measured in children with overweight according to international age and sex specific BMI cut-offs. To study ESRD and mortality, we linked the SCDR to various nationwide registers containing individual information on SES, mortality and ESRD. Results: The incidence rates of childhood onset T1D has continued to increase in Sweden 1977–2007. Age- and sex-specific incidence rates varied from 21.6 (95% CI 19.4–23.9) during 1978–1980 to 43.9 (95% CI 40.7– 47.3) during 2005–2007. Cumulative incidence by birth-cohorts has shifted to a younger age at-onset over the first 22 years of incidence registration. From the year 2000 there was a significant reverse in this trend (p<0.01). In contrast to the increase of T1D, we found no evidence of undetected T2D among Swedish school children. Despite a relatively high incidence in T1D in Sweden there is low cumulative incidence of ESRD, 3.3% at maximum 30 years of duration. We found difference between the sexes regarding long-term risk of developing ESRD that was dependent on the age at onset of T1D. When analyzing how socioeconomic status affects mortality in different age at death groups, we found that having parents that received income support increased mortality up to three times in those who died after 18 years of age. Conclusion: The incidence of childhood onset T1D continued to increase in Sweden 1978-2007. Between the years 1978-1999 there was a shift to a younger age at-onset, but from the year 2000 there is a change in this shift indicating a possible trend break. The prevalence of T2D among Swedish children up to 12 years of age is probably very low. There is still a low cumulative incidence of T1D associated ESRD in Sweden. The risk of developing ESRD depends on age at-onset of T1D, and there is a clear difference in risk between men and woman. Excess mortality among subjects with childhood onset T1D still exists, and low parental socioeconomic status additionally increased mortality in this group.

Renal Disease Risk and Risk Perceptions Among African-American Women with Type 2 Diabetes

<p>Abstract</p><p>Problem: African Americans face a disparate risk for renal disease development secondary to type 2 diabetes (T2D), and African-American women have shown to be at the highest risk. Despite this, there is minimal research on African American's awareness of renal disease and existing renal disease risk perceptions, and none focused specifically on African-American women with T2D. Although the literature has shown that a portion of this disparate risk is due to modifiable social and cultural factors, there is still a significant amount of unexplained risk. Since past research has shown that risk perceptions can influence preventative behaviors, it is important to gain an in-depth understanding of renal disease beliefs and existing risk perceptions among high-risk African-American women with T2D. Once risk perceptions are better understood in this population, interventions can be developed to correct inaccurate beliefs and risk perceptions and aim to decrease renal disease risk.</p><p>Methods: Three different methods of analyses were employed in this dissertation, including: 1) a systematic review of the literature, 2) an exploratory, descriptive, qualitative study, and 3) a quantitative secondary analysis, including descriptive statistics, a cluster analysis and mixed modeling. The Common Sense Model guided all three studies and these three methods of evaluation helped us to gain a more complete understanding of renal disease risk perceptions in African Americans, particularly African-American women with T2D, and provided guidance for future intervention research in this population. </p><p>Conclusions: The findings of this dissertation illustrated there is a significant gap in the literature on African American's renal disease awareness and risk perceptions, yet the available research was used to guide the in-depth interviews with African-American women with T2D. Overall, African Americans underestimate their renal disease risk and lack an understanding of the disease, even in the presence of risk factors. African-American women, in particular, related renal disease directly to the end-stages of the disease, perceived a greater risk for other complications of diabetes, and exhibited significant fear related to their perceived consequences of the disease. This fear frequently initiated maladaptive coping mechanisms, which influenced risk perceptions negatively and hindered preventative behaviors. This study also found that health care providers rarely discussed the disease and often exhibited provider control. Therefore, these findings suggest an urgent need for clinical practice suggestions and intervention research aimed at correcting inaccurate risk perceptions. The secondary analysis findings showed that a culturally relevant intervention with coping skills training resulted in significant improvements in renal disease risk factors among high-risk African-American women with T2D; however, we cannot be sure which facets of the intervention or control care for equal attention may have influenced these outcomes, and renal disease beliefs and risk perceptions were not assessed in the parent study. Therefore, the knowledge gained from this dissertation can be used to guide intervention research that evaluates change over time in renal disease risk representations, risk perceptions, coping procedures and outcomes among participants at high-risk for renal disease.</p> / Dissertation

The Effects of Acid-Base Parameters, Oxygen and Heparin on the Ability to Detect Changes in the Blood Status of End-Stage Renal Disease Patients Undergoing Hemodialysis Using Whole Blood-Based Optical Spectroscopy

Relative changes are detectable in the blood of end-stage renal disease (ESRD) patients during hemodialysis (HD) treatment using optical spectroscopy. However, the potential impacts of several confounding factors that could affect the detection of these changes have not been evaluated. The objectives of this thesis were to: 1) investigate how the variations and/or changes in acid-base and oxygen parameters during HD treatment can affect the optical signature of whole blood of ESRD patients, 2) to investigate the effect of heparin on the optical properties of whole blood and its impact on our method. Blood samples were drawn from 23 ESRD patients at 5 time points during a 4 hour HD treatment and sent for blood gas and blood spectroscopy analyses. No significant correlations were found between the changes in the blood transmittance spectra and acid-base and oxygen parameters. This indicates that the perturbations in these parameters due to HD procedures do not confound the detection of changes in the blood transmittance spectra of ESRD patients during HD treatment. Additionally, the effect of heparin in modifying the optical properties of whole blood does not confound the detection of changes in the blood of ESRD patients due to HD treatment using whole blood-based optical spectroscopy. ANOVA revealed significant (P<0.05) measurable changes in the blood transmittance spectra of ESRD patients during HD treatment. Orientador: Camila Renata Corrêa / Resumo: A obesidade está associada a diversas comorbidades, como dislipidemia, hipertrigliceridemia, resistência à insulina (RI) e diabetes mellitus tipo 2, além de estresse oxidativo e inflamação. Além disso, na obesidade ocorre um desequilíbrio na produção de adipocinas, com aumento de citocinas proinflamatórias e diminuição de adiponectina, o que pode ocasionar disfunções renais e metabólicas. O γ-orizanol, composto presente no arroz, têm demonstrado potencial ação antiadipogênica, anti-inflamatória e antioxidante. Sendo assim o objetivo deste estudo foi avaliar os efeitos do γ-orizanol sobre a ativação dos receptores Adipo- R1 e Adipo- R2 e seus papéis na melhora da função renal. Foram utilizados ratos Wistar machos (n=60 animais), 21 dias de idade, provenientes Biotério Central da Faculdade de Medicina de Botucatu. Os animais foram divididos em seis grupos (n=10 animais/grupo) para receberem dieta padrão (G1), dieta padrão + γ-orizanol (G2), dieta rica em carboidratos (G3), dieta rica em carboidratos no início e depois com γ-orizanol (G4), dieta rica em carboidratos no início e depois dieta padrão + γ-orizanol (G5) e dieta rica em carboidrato + γ-orizanol (G6) por 30 semanas. Detectada a proteinuria, o grupo G4 começou a receber γ-orizanol e o grupo G5 teve a troca da ração. Dialysis Modality and Health-Related Quality of Life of Persons with End Stage Renal Disease

abstract: Health-related quality of life (HR-QOL) is a significant treatment outcome for persons with end-stage renal disease (ESRD); however, little is known about the HR-QOL of Mexican patients with ESRD. This pilot study describes relationships between demographics, sleep disorders, spirituality, mood, folk practices and dialysis modality on the HR-QOL of patients with ESRD residing in Guanajuato, Mexico. Mexican patients receiving continuous ambulatory peritoneal dialysis (CAPD), automated peritoneal dialysis (APD) and hemodialysis (HD) provided information on demographics, clinical health data including body mass index (BMI), and folk health practices. Measures included the Short Form (SF)-36 HR-QOL survey, Sleep Habits Questionnaire, Latin Spirituality Perspective Scale and Hospital Anxiety and Depression Scale. Data were analyzed using SAS software (V9.1). Signifi¬cance level for this pilot study was set at p<0.10. The Quality-Adjusted Life Year method was utilized to examine cost effectiveness for each dialysis modality. Demographics and clinical data showed participants (N=121) to be 59 (SD=13) years, predominantly men (55.4%), married (66.9%), Catholic (92.6%), and not currently working (78.3%). The majority were diabetic (72%) and slightly overweight (BMI M=26.1; SD=5.1). The CAPD group (n=39) demonstrated significantly lower HR-QOL scores compared to the APD (n=42) and HD (n=40) groups. Patients on HD reported higher rates and greater numbers of sleep disorders, including insomnia symptoms, non-restorative and insufficient sleep, and daytime somnolence compared to patients on CAPD and APD. Patients on CAPD reported more anxiety and depression compared to patients on HD and APD. Overall linear regression for HR-QOL found dialysis type, sleep disorders and income to be significant predictors and the model accounted for 31% of the variance. Cost analysis indicated APD as the preferred treatment because it is less costly and results in the best HR-QOL compared to the other treatment modalities. Findings provide the first SF-36 norms for Mexicans with ESRD. Sleep disorders and dialysis type greatly impinge on the HR-QOL of these patients, particularly their mental health. APD was identified as the preferred treatment based on low cost and improved HR-QOL. End-stage renal disease patients have a greatly increased risk of cardiovascular disease partly attributed to the elevated levels of systemic inflammation observed in uraemia. One of the key mechanisms underlying inflammation appears to be the immune dysfunction that afflicts almost every aspect of the uraemic immune system. As a consequence patients experience immunosuppression and reduced responsiveness to antigen as well as a simultaneous over-activation leading to a pro-inflammatory environment. In addition, the haemodialysis (HD) treatment itself induces a proinflammatory response but may provide an otherwise opportune time to complete supervised exercise.

Coping with chronic illness: A study with end-stage renal disease patients / Afrontamiento a la enfermedad crónica: estudio en pacientes con insuficiencia renal crónica terminal

This study identifies coping styles and strategies used by 40 end-stage renal disease patients over 20 years old who receive treatment in a general hospital in Peru. The instruments applied were a personal sociodemographic questionnaire and the Coping Inventory (Carver, Scheier & Weintraub, 1989). Results showed that emotion focused coping were most frequently used followed by problem focused coping. Planning, acceptance and positive reinterpretation-growth coping strategies were more frequently used by these patients, whereas mental disengagement, suppression of competing activities and behavioral disengagement were the less frequently used coping strategies. Other differences between coping styles and strategies and sociodemographic and medical variables were analyzed. / Esta investigación identifica y describe los principales estilos y estrategias de afrontamiento utilizados por un grupo de pacientes diagnosticados con insuficiencia renal crónica terminal. Participaron 40 pacientes mayores de 20 años, aceptados al programa de transplante de riñón del hospital de seguro social. Se utilizaron una encuesta personal y el Inventario sobre Estilos y Estrategias de Afrontamiento (Carver, Scheier & Weintraub, 1989). Los hallazgos indicaron que los estilos enfocados en la emoción fueron utilizados en mayor frecuencia por los participantes, seguidos por los estilos enfocados en el problema. Entre las estrategias de mayor uso se encontraron la planificación, la aceptación y la reinterpretación positiva-crecimiento, mientras que las estrategias de menor uso fueron desentendimiento conductual, supresión de actividades competentes y desentendimiento cognitivo. Se analizaron las posibles diferencias entre los estilos y estrategias de afrontamiento de acuerdo a las condiciones sociodemográficas y médicas de los participantes.

Identificação e descrição dos achados histológicos não-neoplásicos do parênquima renal em espécimes de nefrectomia por neoplasias renais primárias

Introdução: Nefrectomia radical tem sido o tratamento mais utilizado para o carcinoma de células renais. Atualmente, a preservação da função renal do paciente após a cirurgia tem sido um fator de preocupação, já que a sobrevida desses pacientes vem aumentando, assim como a detecção de lesões pequenas, incidentais e assintomáticas. Vários estudos tentam identificar fatores preditores da função renal pós-operatória, no entanto poucos avaliam os achados não-neoplásicos do parênquima renal. Objetivos: O estudo tem como objetivos: 1) descrever as alterações nãoneoplásicas do parênquima renal encontradas nos espécimes de nefrectomia; 2) correlacionar os achados com a função renal pós-operatória dos pacientes e 3) correlacionar essas alterações com o laudo anatomopatológico original. Materiais e métodos: Foram revisados 262 exames anatomopatológicos de espécimes de nefrectomias realizadas em pacientes adultos por neoplasias primárias renais no Hospital de Clínicas de Porto Alegre durante o período de 2005 a 2009. O laudo anatomopatológico e o prontuário eletrônico desses pacientes foram revisados de forma cegada. Um patologista renal realizou a revisão de todas lâminas histopatológicas do estudo. Posteriormente, nos pacientes com dados clínicos e laboratoriais disponíveis, foram correlacionadas as alterações encontradas com a variação da função renal dos mesmos (creatinina sérica e o cálculo da fórmula de Cockroft-Gault, antes e cerca de seis meses após a cirurgia). Resultados: Na amostra inicial, houve uma predominância de pacientes masculinos (57,6%), de raça branca (88%), com uma idade média de 60 anos. Cerca de 76% foram submetidos a nefrectomias radicais e 70% dos pacientes apresentavam estágio tumoral T1 e T2, sendo o tamanho tumoral médio de 6,0 cm e o tipo histológico mais frequente o de células claras (67%). Hipertensão, tabagismo, diabetes melitus e obesidade foram encontrados em, respectivamente, 53%, 32%, 19% e 9% dos pacientes. Apenas 10% dos casos cujas lâminas foram revisadas não apresentavam alterações parenquimatosas. As alterações vasculares foram as mais frequentes, e destas a aterosclerose a mais comum. Esclerose diabética estava presente em quatro pacientes. As variações das estimativas da função renal entre o período pré e pós-operatório não apresentaram diferença significativa entre os grupos de pacientes com e sem alterações parenquimatosas. Foi encontrada diferença significativa entre os valores da creatinina pré e pós-operatória em ambos os grupos e dos valores da fórmula de Cockroft-Gault no grupo com alterações (p<0,001). Em apenas 50 laudos anatomopatológicos originais, haviam sido descritas alterações parenquimatosas. Conclusões: O estudo mostrou que os patologistas gerais se mostram despreparados ou não conscientes acerca da recente inclusão da descrição dos achados não-neoplásicos nos checklists dos laudos anatomopatológicos de neoplasia renais primárias, portanto acreditamos que a mesma deva ser encorajada. Além disso, investigações futuras prospectivas que acompanhem os pacientes por um longo período e utilizem formas mais acuradas de estimativa da função renal devem ser conduzidas para que o real impacto desses achados na função renal dos pacientes após a cirurgia seja bem estudada. / Background: Radical nephrectomy has been the most common treatment for renal cell carcinoma. Besides, the preservation of the renal function after surgery has been a worrisome factor. Patient’s survival rates are increasing, along with the detection of small, incidental and asymptomatic lesions. Several studies have attempted to identify predictors of postoperative renal function, yet few assessed the findings of non-neoplastic renal parenchyma. Objectives: The study aims at: 1) describing the findings in non-neoplastic renal parenchyma found in nephrectomy specimens, 2) correlating these findings with postoperative renal function and 3) correlating these changes with the original pathology report. Materials and Methods: The specimens of two-hundred and sixty-two nephrectomies performed in adult patients for primary renal neoplasms at Hospital de Clinicas de Porto Alegre during the period of 2005 to 2009 were reviewed. The pathology report and electronic medical records of these patients were reviewed in a blinded fashion. A renal pathologist performed the histopathological review of the renal parenchyma slides. After that, in patients with clinical and laboratory data available, the alterations found were correlated with the renal function. Results: There was a predominance of male patients (57.6%) and caucasians (88%), with a mean age of 60 years. About 76% underwent radical nephrectomy and 70% of patients had tumor stage T1 or T2. The average tumor size was 6.0 cm and the most common histological type was clear renal cell carcinoma (67%). Hypertension, smoking, diabetes mellitus and obesity were found in respectively 53%, 32%, 19% and 9% of the patients. Only 10% of the cases whose slides were reviewed had no abnormalities. The vascular changes were the most frequent, and of these the most common was atherosclerosis. Diabetic sclerosis was present in four patients. Renal function variations between the pre and postoperative estimates showed no significant difference between the groups of patients with and without parenchymal abnormalities. Significant difference was found between the pre and postoperative creatinine in both groups and the Cockroft-Gault formula in the group with changes (p<0.001). Only 50 original pathology reports described non-neoplastic abnormalities. Conclusions: The study showed that general pathologists are unaware of the recent inclusion of the non-neoplastic findings in the primary renal neoplasm’s protocols. Therefore, we believe that the evaluation of these findings should be encouraged. In addition, future prospective investigations monitoring patients over a long period of time and using more accurate ways of estimating renal function on postoperative period should be performed.

Na doença renal crônica (DRC) a manutenção da homeostase de água e sódio é o primeiro problema a ser contornado pelo organismo e com o agravamento das lesões renais surgem outros problemas graves relacionados à homeostase de cálcio e fósforo. O presente estudo tem por escopo avaliar a excreção renal de cálcio, fósforo, sódio e potássio, e o perfil sérico destes eletrólitos em cães normais e em cães com DRC naturalmente adquirida. Foram avaliados três grupos de cães adultos, machos ou fêmeas, de raças variadas. Animais normais compuseram o grupo controle (G1) e os cães com DRC foram distribuídos em dois grupos de acordo com os estágios de comprometimento da função renal (G2 e G3, respectivamente, estágios 1-2 e estágios 3-4, descritos pela IRIS 2006 staging CKD). Os cães do G3 apresentaram aumento das concentrações séricas de cálcio ionizado e fósforo, além de diminuição da concentração sérica de sódio. Quanto à excreção renal dos eletrólitos analisados, os animais dos grupos G1 e G2 apresentaram diminuição de carga filtrada e aumento de excreção fracionada, mas as excreções urinárias não variaram significativamente. Os resultados são indicativos de que os rins de cães com DRC podem manter a excreção urinária dos eletrólitos em valores semelhantes aos dos normais. O mecanismo envolve aumento da excreção fracionada à medida em que haja diminuição da filtração glomerular. Esse processo de compensação, entretanto, pode perder a eficiência nos estágios mais avançados da enfermidade no que se refere à manutenção das concentrações séricas de fósforo e sódio. / The kidneys play a fundamental role in the balance of water and electrolytes. In chronic kidney disease (CKD), the first problem to be solved by the organism is to maintain water and sodium homeostasis and, with the worsening of the renal injuries, other severe problems related to the calcium and phosphorus homeostasis emerge. The present study has as a purpose to evaluate the renal excretion and serum profile of calcium, phosphorus, sodium and potassium in healthy dogs and in dogs with naturally acquired CKD. Three groups of adult male and female dogs of varied breeds were evaluated. Normal animals were in the control group (G1) and the CKD dogs were distributed in two groups in accordance with the stage of renal function impairment (G2 e G3, respectively, stages 1-2 and stages 3-4, proposed by IRIS 2006 staging CKD). The G3 dogs showed increased serum levels of ionized calcium and phosphorus, in addition to the reduction of sodium level. Regarding the renal excretion of the analyzed electrolytes, G1 and G2 groups of animals showed a decrease of filtered load and increase of fractional excretion, yet there were no significant variations on the urinary excretions. The results suggest that the kidneys of the CKD dogs can maintain similar values of electrolytes urinary excretion as the normal dogs do. The mechanism involves an increase of fractional excretion while glomerular filtration decreases. This compensation process, however, can lose its efficiency in the later stages of the disease, in relation to the maintenance of phosphorus and sodium serum levels.

Além disso, na obesidade ocorre um desequilíbrio na produção de adipocinas, com aumento de citocinas proinflamatórias e diminuição de adiponectina, o que pode ocasionar disfunções renais e metabólicas. O γ-orizanol, composto presente no arroz, têm demonstrado potencial ação antiadipogênica, anti-inflamatória e antioxidante. Sendo assim o objetivo deste estudo foi avaliar os efeitos do γ-orizanol sobre a ativação dos receptores Adipo- R1 e Adipo- R2 e seus papéis na melhora da função renal. Foram utilizados ratos Wistar machos (n=60 animais), 21 dias de idade, provenientes Biotério Central da Faculdade de Medicina de Botucatu. Os animais foram divididos em seis grupos (n=10 animais/grupo) para receberem dieta padrão (G1), dieta padrão + γ-orizanol (G2), dieta rica em carboidratos (G3), dieta rica em carboidratos no início e depois com γ-orizanol (G4), dieta rica em carboidratos no início e depois dieta padrão + γ-orizanol (G5) e dieta rica em carboidrato + γ-orizanol (G6) por 30 semanas. Detectada a proteinuria, o grupo G4 começou a receber γ-orizanol e o grupo G5 teve a troca da ração. A obesidade foi caracterizada pelo índice de adiposidade e comorbidades: pressão arterial sistêmica, perfis glicêmico, lipídico e hormonal. No tecido adiposo foi analisado o perfil inflamatório por ELISA. A função renal foi avaliada pela proteinuria e foi também analisado o perfil inflamatório e estresse oxidativo. Após o período experimental, os animais que receberam o tratamento com o composto tiveram uma recuperação da função renal, melhora da inflamação e estresse oxidativo renais. Como conclusão, o composto se mostrou efetivo na recuperação da função renal associada à obesidade. / Obesity is associated with several comorbidities such as dyslipidemia, hypertriglyceridemia, insulin resistance (IR) and diabetes mellitus type 2, and also oxidative stress and inflammation, situations that trigger an imbalance in production adipokine, or increase of proinflammatory cytokines and decreased adiponectin. This condition can lead renal and metabolic disorders. γ-oryzanol, main phytosterol present in rice has shown potential antiadipogênica, anti-inflammatory and antioxidant action. Thus the aim of this study was to evaluate, the effects of γ-oryzanol on the activation of Adipo-R1 and Adipo-R2 receptors and their role in improvement of renal function. It was used male Wistar rats (n = 60 animals), 21 days old, from Central Animal Laboratory of the Botucatu Medical School. The animals were be divided into six groups (n = 10 animals / group) to receive standard diet (G1), standard diet + γ-oryzanol (G2), high-carbohydrate diet (G3), high-carbohydrate diet at first and then with γ-oryzanol (G4), high-carbohydrate diet at first and then standard diet + γ-oryzanol (G5) and high carbohydrate diet + γ-oryzanol (G6) for 30 weeks. When detected proteinuria, the G4 group started receiving γ-oryzanol and the G5 group was the exchange of the feed. Obesity was characterized by adiposity index and co-morbidities: blood pressure, blood glucose, lipid and hormonal. In adipose tissue was analyzed the inflammatory profile for ELISA. Renal function was evaluated by proteinuria. It was analyzed the inflammatory status and oxidative stress in renal tissue. After the treatment, the animals presented a recovery of renal function, improvement of inflammation and oxidative stress. So, it is possible to conclude that γ-oryzanol was effective to recovery renal function associated to obesity.
