Feasibility study of dam deformation monitoring in northern Sweden using Sentinel1 SAR interferometry

Dams are man-made structures that in order to keep functioning and to be considered structurally healthy need constant monitoring. Assessing the deformation of dams can be time consuming and economically costly. Recently, the technique of Interferometric Synthetic Aperture Radar (InSAR) has proved its potential to measure ground and structural deformation. This geodetic method represents a cost-effective way to monitor millimetre-level displacements and can be used as supplemental analysis to detect movements in the structure and its surroundings. The objective of this work is to assess the practicality of the method through the analysis of the surface deformation of the Ajaure dam located in northern Sweden, in the period 2014-2017, using the freely available Sentinel-1A images. The scenes, 51 in ascending and 47 in descending mode, were processed exploiting the Persistent Scatterer (PS) technique and deformation trends, and time series were produced. Built in the 60’s, the Ajaure embankment dam is considered as high consequence, meaning that a failure would cause socio-economic damages to the communities involved and, for this reason, the dam needs constant attention. So far, a program of automatic measurements in situ has been collecting data, which have been used partly to compare with InSAR results. Results of the multi temporal analysis of the limited PS points on/around the dam show that the dam has been subsiding more intensely toward the centre, where maximum values are of approximately 5 ± 1.25 mm/year (descending) and 2 ± 1.27 mm/year (ascending) at different locations (separated of approximately 70 m). Outermost points instead show values within -0.7 and 0.9 mm/year, describing a stable behaviour. The decomposition of the rate has furthermore revealed that the crest in the observation period has laterally moved toward the reservoir. It has been observed that the operation of loading and unloading the reservoir influence the dam behaviour. The movements recorded by the PS points on the dam also correlate with the air temperature (i.e. seasonal cycle). The research revealed that the snow cover and the vegetation could have interfered with the signal, that resulted in a relative low correlation. Therefore, the number of PS points on and around the dam is limited, and comparison with the geodetic data is only based on a few points. The comparison shows general agreement, showing the capacities of the InSAR method. The study constitutes a starting point for further improvements, for example observation in longer period when more Sentinel1 images of the study area are collected. Installation of corner reflectors at the dam site and/or by use of high resolution SAR data is also suggested.

Remote sensing for water quality monitoring in oligotrophic rivers : Using satellite-based data and machine learning

Water quality monitoring is crucial globally due to the vital role of freshwater in providing drinking water, irrigation, and ecosystem services. Highly polluted water poses risks to both ecosystems and human health. Current water quality monitoring methods deployed in the field are often expensive, labor-intensive, and invasive. To overcome these issues, this degree project investigated the use of remote sensing to assess critical water quality parameters in the Swedish river Indalsälven. The research questions focus on determining the accuracy of predicting chemical oxygen demand (COD), river color, turbidity, and total phosphorus (TP) using satellite data and machine learning algorithms. The findings revealed that COD can be predicted with a cross-validated coefficient of determination (R²CV) of 0.7, indicating a robust predictive capability. The study suggests that while approximate quantitative prediction of COD in oligotrophic rivers is feasible using Sentinel-2 imagery, predictions for the other parameters remain challenging in the context of Indalsälven. Improvements in prediction accuracy were achieved through optimized band combinations, reduced datasets encompassing satellite data collected within two days of field measurements, and suitable pre-processing methods. / Airborne Monitoring of Water Quality in Remote Regions

Retrieval of High-Resolution Vegetation Optical Depth from Sentinel-1 Data over a Grassland Region in the Heihe River Basin

Vegetation optical depth (VOD), as a microwave-based estimate of vegetation water and biomass content, is increasingly used to study the impact of global climate and environmental changes on vegetation. However, current global operational VOD products have a coarse spatial resolution (~25 km), which limits their use for agriculture management and vegetation dynamics monitoring at regional scales (1–5 km). This study aims to retrieve high-resolution VOD from the C-band Sentinel-1 backscatter data over a grassland of the Heihe River Basin in northwestern China. The proposed approach used an analytical solution of a simplified Water Cloud Model (WCM), constrained by given soil moisture estimates, to invert VOD over grassland with 1 km spatial resolution during the 2018–2020 period. Our results showed that the VOD estimates exhibited large spatial variability and strong seasonal variations. Furthermore, the dynamics of VOD estimates agreed well with optical vegetation indices, i.e., the mean temporal correlations with normalized difference vegetation index (NDVI), enhanced vegetation index (EVI), and leaf area index (LAI) were 0.76, 0.75, and 0.75, respectively, suggesting that the VOD retrievals could precisely capture the dynamics of grassland.

Impact des polluants agricoles sur la génétique des populations d’une espèce sentinelle : le ouaouaron (Rana catesbeiana)

L’expansion agricole ne cesse d’agir sur la perte d’habitats essentiels et nécessaires au développement des espèces. Bien que plusieurs espèces réussissent à survivre dans ces habitats peu adéquats, la persistance et la santé de plusieurs populations semblent compromises par l’utilisation souvent intensive de polluants chimiques agricoles et de fertilisants. Cette étude a pour but de déterminer l’impact des contaminants et de l’écologie du paysage sur la diversité génétique des populations de ouaouarons retrouvées en milieu agricole. Notre hypothèse de départ stipule qu’une exposition chronique aux polluants agricoles induira des différences génétiques au niveau des populations exposées. Le bassin versant de la rivière Yamaska a été désigné comme site d’étude puisqu’il fait partie de la région agricole la plus importante du Québec et parce qu’on y retrouve un gradient d’utilisation des terres pour l’agriculture (faible, moyen, élevé). Le ouaouaron a été choisi à titre de modèle biologique puisque ses caractéristiques physiologiques et écologiques en font une espèce sentinelle capable de rendre compte de l’état de santé global des écosystèmes. La caractérisation génétique des populations a été effectuée à partir de marqueurs d’AFLP (Amplified Fragment Length Polymorphism). Les résultats montrent que la diversité génétique est liée à la colonisation à partir de l’embouchure de la rivière Yamaska et que quelques populations sont génétiquement différenciées. De plus, nous avons démontré une relation positive entre le nombre de locus polymorphes et l’atrazine, l’indice de contamination et le métolachlore et la concentration en azote ainsi qu’entre l’hétérozygotie attendue et la concentration en phosphate. / Agricultural expansion continues to act on the loss of critical habitats for the development of species and affects the integrity of remaining habitats. Although several species manage to survive in such inadequate ecosystems, persistence and health of many populations seem compromised by the intensive use of pesticides and fertilizers. This study aims at determining the impact of pollutants and landscape ecology on the genetic diversity of bullfrog populations inhabiting an agricultural area. Our hypothesis states that chronic exposure to agricultural pollutants induces genetic differences in exposed populations. The watershed of the Yamaska River was designated as the study site because it is part of the largest agricultural region in Quebec and because the sampling sites represent a gradient of land use for agriculture (low, moderate, high). The bullfrog was chosen as the biological model, since its physiological and ecological characteristics make it a sentinel species, which can reflect the overall health of an ecosystem. Population genetic characterization has been conducted from AFLP (Amplified Fragment Length Polymorphism) markers. The results show that genetic diversity is correlated to colonization from the mouth of the Yamaska River and that some populations are genetically differentiated. Furthermore, we demonstrated a positive relationship between the number of polymorphic loci and the atrazine, the contamination index and the nitrogen concentration as well as between expected heterozygosity and phosphate concentration.

Modelos estatísticos para previsão de metástases axilares em câncer de mama em pacientes submetidos à biópsia de linfonodo sentinela / Statistical models for predicting axillary metastases in breast cancer patients submitted to sentinel lymph node biopsy

INTRODUÇÃO: O status axilar é o fator prognóstico mais importante em câncer de mama. A biópsia de linfonodo sentinela (BLS) tornou-se o procedimento padrão no estadiamento axilar em pacientes com axila clinicamente negativa. O procedimento recomendado para pacientes com metástase em linfonodo sentinela (LS) inclui a linfadenectomia axilar completa (LAC). Porém questiona-se a necessidade da LAC em todos os pacientes com metástase em LS (MLS), particularmente naqueles com baixo risco de metástase em linfonodos axilares adicionais (Não-LS). Estimar de forma precisa a probabilidade de MLS e metástase em Não-LS (MNão-LS) pode auxiliar em muito o processo de decisão terapêutica. Os dados publicados referentes aos fatores preditivos de MLS e MNão-LS são de certa forma escassos. A forma de como esses dados são apresentados – geralmente expressos como razão de chance (odds-ratio) – dificulta o cálculo de probabilidade de MLS ou MNão-LS para um paciente em específico. Nesta tese, dois modelos de previsão computadorizados de fácil utilização foram desenvolvidos, utilizando-se um grande número de casos, para facilitar o cálculo de probabilidade de MLS e MNão-LS. MÉTODOS: Todos os dados clínico-patológicos foram coletados do banco de dados prospectivo de LS do Memorial Sloan-Kettering Cancer Center (MSKCC), Nova York, EUA. Os projetos de desenvolvimento de ambos os modelos foram aprovados pelo Institutional Review Board do MSKCC. Dois modelos foram desenvolvidos: Modelo de MLS (Modelo LS) e Modelo de metástase em Não-LS (Modelo Não-LS). No Modelo LS, os achados clínico-patológicos de 4.115 procedimentos subseqüentes de BLS (amostra de modelagem) foram submetidos à análise de regressão logística multivariada para se criar um modelo de previsão de MLS. Um software baseado nesse modelo foi desenvolvido utilizando-se as variáveis: idade, tamanho do tumor, tipo histológico, invasão vásculo-linfática (IVL), localização do tumor e multifocalidade. Esse modelo foi validado em uma amostra distinta (amostra de validação) com 1.792 BLSs subseqüentes. No Modelo Não-LS, os achados patológicos do tumor primário e das MLS, obtidas de 702 procedimentos de BLS (amostra de modelagem) em pacientes submetidos à LAC, foram submetidos à análise de regressão logística multivariada para se criar um modelo de previsão de MNão-LS. Um nomograma e um software baseados nesse modelo foram desenvolvidos utilizando-se as variáveis: tamanho do tumor, tipo histológico, grau nuclear, IVL, multifocalidade, receptor de estrógeno, método de detecção da MLS, número de LS positivos, número de LS negativos. Esse modelo foi validado em uma amostra distinta (amostra de validação) com 373 casos subseqüentes. RESULTADOS: O software do Modelo LS na amostra de modelagem mostrou-se adequado, com a área sob a curva de características operacionais (ROC) de 0,76. Quando aplicado na amostra de validação, o Modelo LS também previu de forma acurada as probabilidades de MLS (ROC = 0,76). O software e o nomograma do Modelo Não-LS na amostra de modelagem apresentaram uma área ob a curva ROC de 0,76 e, na amostra de validação, 0,77. CONCLUSÕES: Dois softwares de fácil utilização foram desenvolvidos, utilizando-se informações comumente disponíveis pelo cirurgião para calcular para o paciente a probabilidade de MLS e MNão-LS de forma precisa, fácil e individualizada. O software do Modelo LS não deve ser utilizado, porém, para se evitar a BLS. Sentinel lymph node biopsy (SLNB) has become the standard of care as the axillary staging procedure in clinically node negative patients. The standard procedure for patients with sentinel lymph node (SLN) metastasis includes complete axillary lymph node dissection (ALND). However, many experts question the need for complete ALND in every patient with detectable SLN metastases, particularly those perceived to have a low risk of additional lymph node (Non-SLN) metastasis. Accurate estimates of the likelihood of SLN metastases and additional disease in the axilla could greatly assist in decision-making treatment. The published data on predictive factors for SLN and Non-SLN metastases is somewhat scarce. It is also difficult to apply these data – usually expressed as odds ratio – to calculate the probability of SLN or Non-SLN metastases for a specific patient. In this thesis, two user-friendly computerized prediction models based on large datasets were developed, to assist the prediction of the presence of SLN and Non-SLN metastases. METHODS: All clinical and pathological data were collected from the prospective SLN database of Memorial Sloan-Kettering Cancer Center (MSKCC), New York, USA. The development projects of both models were approved by the Institutional Review Board of MSKCC. Two models were developed: Model for predicting SLN metastases (SLN Model) and Model for predicting Non-SLN metastases (Non-SLN Model). In the SLN Model, clinical and pathological features of 4,115 sequential SLNB procedures (modeling sample) were assessed with multivariable logistic regression to predict the presence SLN metastases. A software based on the logistic regression model was created using age, tumor size, tumor type, lymphovascular invasion, tumor location and multifocality. This model was subsequently applied to another set of 1,792 sequential SLNBs (validation sample). In the Non-SLN Model, pathological features of the primary tumor and SLN metastases, identified in 702 SLNBs (modeling sample) on patients who underwent complete ALND, were assessed with multivariable logistic regression to predict the presence of additional disease in the Non-SLNs of these patients. A nomogram and a software were created using tumor size, tumor type and nuclear grade, lymphovascular invasion, multifocality, and estrogen-receptor status of the primary tumor; method of detection of SLN metastases; number of positive SLNs; and number of negative SLNs. This model was subsequently applied to another set of sequential 373 procedures (validation sample). RESULTS: The software of the SLN Model for the modeling sample was accurate and discriminating, with an area under the receiver operating characteristic (ROC) curve of 0.76. When applied to the validation sample, the SLN-Model accurately predicted likelihood of SLN metastases (ROC = 0.76). The software and nomogram of the Non-SLN Model was also accurately predicted likelihood of Non-SLN metastases, with an area under ROC curve of 0.76 for the modeling sample, and 0.77 for the validation sample. CONCLUSION: Two user-friendly softwares were developed, using information commonly available to the surgeon to easily and accurately calculate the likelihood of having SLN metastases or additional, Non-SLN metastases for individual patients. Validade do mapeamento do linfonodo sentinela na detecção de metástase linfática cervical do carcinoma papilífero da glândula tireoide / Impact of sentinel lymph node mapping to detect papillary thyroid carcinoma lymph node metastasis

INTRODUÇÃO: O mapeamento do linfonodo sentinela (MLNS) é largamente utilizado em pacientes com melanoma cutâneo, câncer de mama e outras neoplasias malignas sólidas com a finalidade de estadiá-las e indicar esvaziamento linfático apenas na presença de metástase. Nos últimos anos aumentou o interesse pelo uso do MLNS em pacientes com carcinoma papilífero de tireoide (CPT) sem metástases linfáticas detectáveis clinicamente (estádio cN0), devido à alta frequência de metástases ocultas nesses pacientes. O MLNS pode evitar o esvaziamento linfático do compartimento central (ECC) em portadores de CPT sem metástase. Mesmo que não se planeje o ECC eletivo, o MLNS pode também ser usado para estadiar adequadamente o pescoço e indicar tratamento posterior com radioiodoterapia em casos com metástase. Esse estudo tem como objetivo verificar: 1) a efetividade da técnica do MLNS nos pacientes com CPT; 2) a acurácia do MLNS em diagnosticar as metástases linfáticas; 3) se o MLNS pode modificar o estadiamento dos pacientes com CPT cN0. MÉTODOS: Trata-se de estudo de acurácia, prospectivo longitudinal de 38 casos consecutivos de CPT clinicamente N0, atendidos em um único centro, no período de 2010 a 2015. Todos foram submetidos à tireoidectomia total, MLNS com radiofármaco e ECC eletivo. Os resultados relativos ao MLNS foram comparados com o exame anatomopatológico do conteúdo do ECC. RESULTADOS: O tempo médio de seguimento dos doentes foi de 36 ± 13 meses. Os LNS foram localizados com maior frequência nos níveis VI e III. O LNS foi detectado em 95% da casuística e os valores de avaliação de testes diagnósticos foram: 1 falso negativo, 95% de sensibilidade, 100% de especificidade, 94% de valor preditivo negativo e 97% de acurácia. O MLNS foi capaz de reestadiar 49% dos pacientes inicialmente N0 para pN positivo. Quanto ao grupo de estadiamento AJCC (que leva em conta a idade), 3% dos pacientes foram reestadiados para estádio III e 18% para estádio IV. CONCLUSÕES: 1) a técnica de MLNS utilizada nos pacientes com CPT foi efetiva em 95% dos procedimentos realizados; 2) a acurácia do MLNS em diagnosticar as metástases linfáticas cervicais foi de 97%; 3) o MLNS re-estadiou os portadores de CPT, classificados clinicamente como N0, para pN positivo em 49% da casuística, e para estádio III e IVa em 21% / INTRODUCTON: Sentinel lymph node mapping (SLNM) is widely perfomed in melanoma, breast cancer and other solid tumors, to adequately stage these diseases. More recently, the interest in SLNM for clinically N0 Papillary Thyroid Cancer (PTC) has increased due to the high rate of occult metastases in these patients, to avoid unnecessary central neck dissection (CND), and its complications. Even if routine elective CND is not planned, SLNM can be used to adequately stage the neck, and to indicate further treatment with radioiodine in cases with neck metastases, for instance. This study aims to evaluate 1) SLNM effectiveness in PTC patients, 2) SLNM accuracy to diagnosis lymph node metastases and 3) if SLNM can upstage cN0 PTC patients. METHODS: This is a prospective longitudinal, diagnostic test accuracy study with 38 consecutive cN0 PTC patients, treated in a single center between 2010 and 2015. Surgical treatment in all cases included total thyroidectomy and elective CND after SLNM. Results of SLNM were compared to CND pathological findings, in order to verify if sentinel lymph node (SLN) predicted the occurrence of PTC occult lymph node metastasis. RESULTS: The mean patients\' follow-up was 36 ± 13 months. 133 SLN were found in the neck, on levels VI and III. The SLN was identified in 95% of the patients with 1 false negative, 95% sensitivity, 94% negative predictive value and 97% accuracy. The SLNM upstaging from cN0 to pN+ was 49%, with 3% stage III and 18% stage IVa. CONCLUSION: 1) SLNM was effective in 95% of procedures, 2) SLNM accuracy was 97%, 3) SLNM upstaging from cN0 to pN+ was 49%, and to stage III and IVa was 21%

Le Varan du Nil (Varanus niloticus), indicateur de la pollution des zones humides d’Afrique Sub-Saharienne / The Nile monitor (Varanus niloticus), an indicator species for pollution in sub-Saharan wetlands

En Afrique, la contamination des zones humides par les métaux et les pesticides va se poursuivre durablement. Pour autant, le statut toxicologique de ces milieux reste trop peu documente. Le but du présent travail est d’estimer la valeur du varan du Nil (Varanus niloticus) en tant qu’espèce sentinelle pour la contamination des zones humides continentales d’Afrique sub-Saharienne. Plomb, cadmium, et pesticides organochlorés et organophosphorés ont été quantifies, par spectrométrie d’absorption atomique et chromatographie gazeuse (respectivement), dans plusieurs tissus provenant de 71 spécimens issus de quatre sites (au Mali et au Niger) juges a priori inégalement contamines. Bien que des différences claires apparaissent, la contamination environnementale s’avère modérée sur les quatre sites, et ne semble pas constituer un risque notable pour les varans ni pour les humains qui s’en nourrissent occasionnellement. Toutefois la variabilité interindividuelle est importante. Les organotropismes des polluants détectés sont cohérents avec ceux préalablement décrits. Si l’on n’a pu mettre en évidence de différence liée au sexe en ce qui concerne les pesticides, les femelles présentaient des charges en métaux supérieures. La relation entre d’autres facteurs (taille, proportion de graisse) et les concentrations tissulaires a également été considérée. Les varans sont susceptibles de révéler des différences subtiles de contamination environnementale entre sites, et la résolution spatiale de l’outil semble très fine. La possibilité pratique d’utiliser cet indicateur se trouve donc validée. Un travail expérimental sur des varans captifs a par ailleurs été mené pour approfondir l’étude / In Africa, metal and pesticide contamination of wetlands is supposed to continue on a permanent basis. However, the ecotoxicological status of these ecosystems remains poorly documented. The aim of the present work is to assess the value of the Nile monitor (Varanus niloticus) as a sentinel species for the environmental contamination of continental wetlands in sub-Saharan Africa. Lead and cadmium on the one hand, and organochlorine and organophosphate pesticides on the other, have been quantified in several tissues by atomic absorption spectrophotometry and gas chromatography, respectively. Samples come from 71 specimens originating from four sites considered unequally contaminated (in Mali and Niger). Although clear differences appear between sites, the environmental contamination turns out to be moderate at the four sites, and does not seem to represent a significant risk neither for the monitors themselves, nor for occasional human consumers. However, the interindividual variability is important. The organotropisms relative to the detected pollutants are consistent with those described in previous studies. Concerning the pesticides, no gender effect has been found, whereas females were more contaminated by metals. The relation between other factors (size, proportion of fat) and tissue concentrations has been considered too. Nile monitors can reveal subtle differences in local pollution and the spatial resolution of this tool seems to be very sharp. Its practical relevance is thus validated. Linfonodo sentinela e dissecção axilar: análise da disfunção de membros superiores / Axillary and sentinel lymph node dissection: analysis of the dysfunction of the upper limbs

O câncer de mama é a neoplasia maligna que mais acomete as mulheres na população brasileira. Atualmente o tratamento para o câncer de mama está fundamentado em três pilares: radioterapia, cirurgia em suas diversas técnicas, quimioterapia/hormonioterapia. A qualidade de vida pode ser afetada de uma forma geral, envolvendo os aspectos físicos, psicológicos, sociológicos, emocionais, entre outros. O objetivo deste estudo foi comparar a função do membro superior em pacientes com câncer de mama que foram submetidas à biópsia de linfonodo sentinela com as submetidas à dissecção axilar, e que não foram acompanhadas por um protocolo de reabilitação pré-estabelecido. Pacientes e métodos: foram avaliadas 36 pacientes que passaram por dissecção axilar (grupo DA), 29 que foram submetidas à biópsia de linfonodo sentinela (grupo BLS) e 11 sujeitos normais (grupo SN). Resultados: Verificamos que o grupo DA apresenta um risco de ter perda funcional para os movimentos de flexão e abdução de ombro na ordem de 5 e 3,7 vezes maior que o grupo BLS, respectivamente. Encontramos também que pacientes submetidas à BLS evoluem com disfunção significativa para a flexão e abdução de ombro quando não submetidas à intervenção fisioterápica. Conclusão: A DA apresenta uma maior disfunção em membro superior quando comparada com BLS. A reabilitação de pacientes submetidas à BLS não pode ser ignorada, pois esta intervenção cirúrgica pode levar a disfunção significativa do membro superior ipsilateral / Breast cancer is a malignancy that affects more women in the Brazilian population. Currently the treatment for breast cancer is based on three pillars: radiotherapy, surgery in its various techniques, chemotherapy / hormone therapy. Quality of life may be affected in general, involving the physical, psychological, sociological, emotional, and others. The aim of this study was to compare the upper limb function in patients with breast cancer who underwent sentinel lymph node biopsy with axillary dissection underwent, and were not accompanied by a rehabilitation protocol pre-established. Patients and methods: we evaluated 36 patients who underwent axillary dissection (group DA), 29 who underwent sentinel lymph node biopsy (SNB group) and 11 normal subjects (group SN). Results: We found that the DA group presents a risk of loss of function for flexion and shoulder abduction in the order of 5 and 3.7 times higher than the BLS group, respectively. We also found that patients who underwent SNB evolve with significant dysfunction for flexion and shoulder abduction when not undergoing physiotherapy intervention. Conclusion: The DA has a higher dysfunction in upper limb when compared with BLS. The rehabilitation of patients who underwent SNB can not be ignored because this surgery can lead to significant dysfunction of the ipsilateral upper limb

A presença e a extensão do envolvimento linfonodal axilar é um importante fator prognóstico, com grande impacto na decisão terapêutica. Por outro lado, a linfadenectomia axilar (LA) associa-se a considerável morbidade. Neste sentido, a biópsia do linfonodo sentinela (BLS) constitui modalidade atualmente aceita para uso em tumores T1 e T2, associados a axila negativa. Fato é que pacientes com tumores T3 e T4 e axila negativa são, muitas vezes, submetidas a LA de maneira desnecessária, visto o limitado número de pacientes em séries publicadas. Assim, o objetivo deste estudo é avaliar a negatividade e segurança da BLS em tumores T3 e T4, associados à axila clinicamente negativa, baseada em série de casos institucionais e revisão sistemática da literatura. The presence and extent of axillary lymph node involvement represents an important prognostic factor, with an impact on therapy. On the other hand, axillary lymph node dissection (ALND) is associated with considerable morbidity. In this sense, sentinel lymph node biopsy (SLNB) is currently accepted modality for tumors T1 and T2 with negative axilla (N0). The fact is that many patients with T3-T4 tumors with negative axilla, are often submitted to unnecessarily ALND. There are limited number of patients in published series and selected cases may have axillary preservation. Thus, the objective of this study is to evaluate the negativity and safety of SLNB in T3 and T4 tumors, associated to the clinically N0, based on a series of institutional cases and a systematic review of the literature. Materials and Methods: A retrospective, observational study of patients treated for breast cancer at a tertiary cancer center between 2008 and 2015, with T3 / T4 tumors and N0 who underwent SLNB. In order to perform this treatment it’s needed a vascular access (VA) that offers an adequate flow rate, a long use-life and a low rate of complications. The Arteriovenous Fistula (AVF) is the closest access to meet these requirements. It can, however, present complications and, during the cannulation that usually happens three times per week, adverse events (AE) can occur to the patient. The arteriovenous fistula cannulation must happen with safety in order to prevent future patency problems. There are three methods of cannulation: area, rope ladder and buttonhole. In the area method, the insertion points of the needles are in the same area; in the rope ladder method there’s the varying of the place of the puncture, at a distance defined by the previous puncture, all along the VA; and in the buttonhole method, the needle’s insertion happen in the same place, angle and deepness, forming a subcutaneous tunnel that will be cannulated with the blunt needle. Each one of these methods has its own particularity and can influence in the need to repair the fistula. This is a prospective cohort study, during the course of six months, from April to September of 2017, conducted with the participation of 347 patients using the vascular access by autologous arteriovenous fistula, within three hemodialysis clinics in the city of Goiânia - GO. The data collection happened by weekly interview to the patients, using a structured instrument online. The research was approved by the Ethics Committee and the participation conditioned to signing of the consent form by the patient. The general objective was to analyze the factors that can influence in the necessity to repair the arteriovenous fistula of patients in hemodialytic treatment cannulated by different cannulation methods. The specific objective was to relate the adverse events and complications in the different arteriovenous fistula cannulation methods. We’ve found that in the buttonhole method, the most frequent AE was dermatitis and misscannulation, and in the area/rope-ladder methods, the most frequent AE were haematoma and peri-punction bleeding. The patients in the buttonhole method group received the hemodialytic treatment with a higher blood flow compared the other group. We’ve observed that the dual lumen catheter (DLC) is a predictor to the need of AVF repairments, due to enhancing in 28% the risk of need for AFV repair. The “arterial” retrograde cannulation has presented itself as a protection factor, diminishing the need to AVF repairments in 1%. In conclusion, the buttonhole method is recommended, since there is an intermittent surveillance of the arteriovenous fistula by the nurse in the touching exam. The area method is not recommended, and the rope ladder method should be individually evaluated in future studies. The nurse must act by monitoring the AFV, surveillance of the patency parameters and health education to the patients for the AVF self-care, as well as continued education to the nursing team in order to promote safe and scientifically based practices. / A hemodiálise é a modalidade de terapia renal substitutiva mais comumente instituída para a doença renal crônica. Para esse tratamento é necessário um acesso vascular que ofereça fluxo sanguíneo adequado à necessidade dialítica, meia vida longa e baixo índice de complicações. A fístula arteriovenosa é o acesso que mais se aproxima desses requisitos. Porém, não obstante, pode apresentar complicações, e durante as punções, que comumente se repetem três vezes por semana, pode haver eventos adversos (EA) ao paciente. A punção da fístula arteriovenosa deve ser realizada com segurança a fim de prevenir futuros problemas de perviedade. Há três métodos de punção: regional, escada de corda, e buttonhole. No método regional, os pontos de inserção das agulhas são na mesma região; no método escada de corda, há rotação do sítio de punção, a uma distância definida a partir da anterior ao longo de todo o AV; e no buttonhole, a inserção da agulha é no mesmo local, ângulo e profundidade, formando de um túnel subcutâneo que será puncionado com agulha romba. Cada um desses métodos tem sua particularidade e podem influenciar na necessidade para reparos na fístula. Este é um estudo longitudinal de coorte prospectiva, no período de seis meses, abril a setembro de 2017, realizado com 347 pacientes em hemodiálise usando acesso vascular por fístula arteriovenosa autóloga, em três clínicas satélites do município de Goiânia - GO. A coleta de dados foi por entrevista semanal aos pacientes, por meio de instrumento estruturado online. A pesquisa foi aprovada por comitê de ética, e a participação condicionada à assinatura do Termo de Consentimento Livre e Esclarecido do paciente. O objetivo geral foi analisar fatores que influenciam na necessidade de reparo à fístula arteriovenosa de pacientes em hemodiálise puncionados por distintos métodos de punção. E os objetivos específicos foram identificar e relacionar os eventos adversos e complicações em distintos métodos de punção da fístula arteriovenosa, e caracterizar os preditores de complicações da fístula arteriovenosa. Encontramos como resultados que no método de punção de fístula arteriovenosa buttonhole o EA mais frequente foi dermatite e reinserção de agulhas de punção, e nos métodos escada/regional foram hematoma e sangramento peripunção. Os pacientes no grupo puncionado pelo método buttonhole receberam hemodiálise sob fluxos de sangue mais altos comparado ao outro grupo. Observamos que o uso do cateter venoso central de duplo lúmen (CDL) caracteriza-se um preditor de necessidade de reparo da fístula arteriovenosa, pois aumenta em 28% o risco dessa necessidade. A punção “arterial” retrógrada apresentou-se como fator de proteção, diminuindo em 1% a necessidade de reparos. Concluímos que o método de punção buttonhole é recomendado desde que haja a monitoração intermitente da fístula arteriovenosa pelo enfermeiro durante exame físico. O método regional é desestimulado. E o método escada de corda deve ser avaliado individualmente em estudos futuros. O enfermeiro deve estabelecer a vigilância dos parâmetros de perviedade, educação em saúde para autocuidado da fístula arteriovenosa, bem como educação continuada para a equipe de enfermagem a fim de promover práticas seguras e cientificamente embasadas.
