Role sestry ve farmakoterapeutickém týmu / The role of nurse in the pharmacotherapy team

This diploma thesis deals with current pharmacotherapeutic care with a focus on activities that nurses take part in. The aim was to collect and evaluate the views and experiences of selected nurses regarding their role in pharmacotherapy team, particularly what activities they perform in such team and how their pharmacotherapy teams work in general. SWOT analysis was then used firstly to evaluate the current role of nurses and secondly to suggest a way of implementation of possible extension of nurses' competencies in pharmacotherapy. In order to collect the required data, semi-structured interviews were conducted. These were recorded, so they could be easily processed afterwards. During transcription of the interviews into written text, all the collected data was briefly analysed. The data was further analysed using the open coding method, when three main categories with specific subcategories and subcodes were identified. All the selected men and women, which were asked for cooperation, are people working as a nurse without supervision and mentoring. Based on their career history all of them were expected to have a more specific experience with the issue. The coaching is currently and dynamically developing access and newly emerging qualifications, and it is especially in the field of management and professional leadership of staff in the field of management. It is usable as the method of the support of proffesional growth and develop of staff in the various type of the contexts. The coaching is widely used in today's time, no only in personal life, but in proffesional life too. The coaching style is very often aplicated to the nursing practise in abroad, however in czech nursing it's completely new problem. In theoretic part of my thesis it's aimed for the basic characteristic of the coaching and its priciples. I pursue the problematic of the coaching and its possibilities of application, especially in field of conversional process at the graduates at the department of the intensive care. Organizational ethics is concept which focues on support and aplication ethical principles and values in organizational action. According to theory of social responsibility have organization duties to stakeholders; this theory is background of concept organizational ethics. Stakeholders are clients, but also employees too. I intrested in duties of organization or more precisely hospital to empolyees in this thesis. For research about ethical conduct of employees in hospital in context of organizational ethics I chosed qualitative methodology and interview as the major method of research. Accordinc to analysis I identified main phenomenon as ethical problems of employees in hospital. Employees was selected by proffessional orientation and working position for research. The present dissertation, which focuses on the needs of particularly vulnerable older people in home health care, was written within the framework of the PhD study in Longevity at the Faculty of Humanities, Charles University and was carried out as part of the project supported by the Charles University Grant Agency "Met and Unmet Needs of Particularly Vulnerable Older Patients in Home and Inpatient Care" (GA UK No. 760219). The overall aim of the three researcher project, where I was the research coordinator, was to clarify the needs of particularly vulnerable older patients in both home health care and inpatient care, including the needs of patients living with dementia. The aim of this dissertation was to identify the needs of vulnerable older patients in home health care. This dissertation consists of four papers. At the time of writing, two articles had been published in peer-reviewed journals and two articles were under review. The thesis has chapters in the introduction and conclusion that set the professional articles in context, creating a comprehensive view of the needs of particularly vulnerable older people in home health care. Psychicky náročné momenty v práci sestry v intenzivní péči / Mentally challenging moments of intensive care's nurse

The profession of general intensive care nurse is specific in its high demands on physical, technical, ethical and mental abilities of each individual. The thesis focusses on the mental stress of the nurse, analyses potentially high-risk professional situations the nurse encounters and proposes methods for their management. The topic of mental stress and its handling has, and will, have important impact on the field. The goal of the thesis is to map the psychologically challenging situations that all of the 129 nurses approached in a smaller local medical facility considered as difficult to handle and thus have impact on their ability to cope with a demanding job. The situations are defined and evaluated using predefined hypotheses. The results of an anonymous quantitative questionnaire clearly show that the largest levels of stress and discomfort are caused by situations which are "directly connected to medical care provided by the general intensive care and accident and emergency nurse". The least discomfort is perceived in situations described as "indirectly connected to intensive care / A&E nurse duties", such as irregular working patterns and work during public holidays. Subjects: This diploma thesis is dedicated to the topic of time management in nursing - it deals with four generations of time management and with its different techniques. Furthermore it describes functions and roles of a manager. Objectives: The main objective of this thesis is to analyse time management issues that concern nurse managers working in intensive care. The main goal of the research is to answer following question: How do nurse managers in intensive care use and organise their working time? Secondary goals are: to name priorities of nurse managers on an ordinary working day, to find out which timekeeping devices do nurse managers use for direct measuring of time consumption, to analyze managerial strategies used by nurse managers for their time management, to sum up the most frequent tasks postponed by nurse managers for a later time and find the reasons of procrastination. Methodology: Data were gained through a combined quantitative and qualitative research design. The topic of this diploma thesis is "Influence of footwear on foot deformities in women - the role of nurses in prevention". The theoretical part is devoted to the foot anatomy, it describes the issue of heels on the foot and represents the Giraffe certification. The practical part of the qualitative research shows the role of nurses in preventing foot deformities for patients. This thesis has two goals. To find out what footwear affects the deformities of the legs in women and to find out what role the nurse plays in preventing this issue. The research questions are 1. What is the importance of wearing high-heeled shoes for women and how will this be reflected later? 2. What role do nurses attribute to the prevention in this issue? Data collection was collected by semi-structured interviews with nurses in the orthopaedic ward and orthopaedic clinics. The research sample consists of 20 respondents, the choice was deliberate and formed by snowball sampling. In February 2019, data was collected, which was subsequently terminated at the moment of theoretical saturation. The interview covered themes: wearing high-heeled shoes, leg deformities caused by high-heeled shoes, nurse-led prevention and education. The results show that only a fraction of nurses adhere to the role of a nurse who helps patients prevent leg deformities. The rest of the nurses leave prevention and education to doctors. This diploma thesis presents as a work output a poster, which can be an information material for nurses from the orthopaedic clinics and beds. The output of the thesis is an educational poster that will reflect the entire nursing issue in prevention (the role of a nurse, the deformity of the feet caused by high-heeled shoes, etc.).

Role perioperační sestry v bezpečnostní proceduře na operačním sále / The role of perioperative nurses in the safety procedure in the operating theatre.

Preoperative safety procedure is a departmental safety goal laid down by the Ministry of Health of the Czech Republic. The providers of health care directly controlled by the Ministry of Health of the Czech Republic are obliged to putpreoperative safety procedure and its documentation into practice, other health care organizations are recommended this implementation. However, the Ministry does not specify the formof this procedure nor its process in the operating room, there is no uniform documentation prescribed and last but not least, it is not clearly laid down which person of the operational team is responsible for the correct implementation of this procedure. The goal of the research is to map the preoperative safety procedure in the operating theatres in selected hospitals, to compare the process of the preoperative safety procedures in the operating theatres among the addressed hospitals, to describe the competencies of perioperative nurses during the preoperative safety procedures in the operating theatres in the selected hospitals and to find out the attitude of the perioperative nurses to the preoperative safety procedure. In the selected hospitals the safety procedure is introduced and implemented in different ways. The preoperative safety procedure in the operating theatres is introduced as an administrative act, but not as a practical tool to improve patient safety. The competences of perioperative nurses in preoperative safety procedures are not clearly specified in all of the respondent hospitals. The perioperative nurses adopt entirely positive approach to that, in most cases they believe it is important for ensuring the safety of the patient.

Současná praxe a možnosti využití supervize v ošetřovatelství / Current practice and ways of the supervision use in nursing

The thesis named "Current practice and possible ways of implementing clinical supervision in nursing" is the first doctoral dissertation establishing the concept of supervision in the Czech nursing environment. Itscentralresearch questionis as follows: How do nurses and university educators in nursing construct, interpret and practically apply theconcept ofclinical supervision and the supervisor's role in the context of nursing education at specific institutions of tertiary education and clinical nursing practice in the Czech Republic? The author draws on international theoretical literature and local empiricalevidence.Her qualitative research took the form of 26 semi-structured interviews with academic nursingeducators from the entire Czech Republic. In justified cases those are supplemented with qualitative data obtained through the study of legislative, policy and strategic documents in nursing and health care, including publicly available information and sources on clinical supervision, in order torefine the resulting picture of her interpretation of current clinical supervision practice in Czech nursing. A shift to the constructivist paradigm enabled one to conceive the term "clinical supervision" mainly in relation to learning, training and professional development of supervisees, also in the context of transformation of education and supervision not only of nurses but also of supervisors themselves in the Czech Republic. The author also focuses on the philosophical and theoretical foundations of clinical supervision and application of the postmodern approach in egalitarian clinical supervision. The empirical part of this dissertation is based on the qualitative research conducted. The author gradually presents her results as individual categoriesand relations between them that ensued from the analysed data in the process of open coding. The author draws a link between the category of implementation of clinical supervision in the context of nurses' professional education andexternal conditions, the environment and a broader professional and social-cultural context. Clinical supervision in nursing is constructed mainly as a unidisciplinaryconcept and a part of the nursing profession. The university nursing educatoris typically constructed as a role professional, but there is also an ongoing process ofdiversification, stratification, profiling and professionalization of non-academic nursing educators. Based on a content analysis of documents and post-modern philosophical-theoretical foundations and strategies of legitimizingclinical supervision, the author views the concept of clinical supervision in nursing as socially constructedand discursively legitimated. The author also defines the limitations of her research that arise from the choice of a qualitative research design. In the conclusion she outlines some future trends of clinical supervision in particular educational institutions that can be interesting and inspiring not only for supervisors but also for teachers of supervision themselves. Based on the results of her empirical analysis the author emphasises the need to integrate the theory and practice of education, clinical supervision and nursing. A comprehensive structure and methodology of system-wide implementation of clinical supervision, based on the situation of clinical nursing practice and supervision needs in the Czech Republic,should in future formthe basis of innovation of existing university programmes in nursing and approaches to the education and training of supervisors, as well as the subject of new research in clinical supervision.

Integrace rodičů do péče o kriticky nemocné dítě / Integration of parents into the care of critically ill child from the perspective of nurse

Nowadays the parental presence in the pediatric intensive care units (PICU) is quite common. Building a fellowship between parents and health care staff and high quality parents' integration into the care is a very demanding process. First of all, the success of this process largely depends upon the nurse. It is the very nurse who leads, educates, supports and also professionally integrates parents into their child's care. In addition, critically ill child's care in cooperation with parents is complicated by a high parental stress level, child's serious condition and high professional and technical requirements posed to the nurse. The graduation thesis concentrates on the parents' integration into the care of a critically ill child from the nurse's point of view. The goal of this thesis was to describe the general conditions of parents' integration into the critically ill child's care and to map the actual state of the cooperation within the nurse ? parent ? critically ill child relation from the nurse's point of view. Other goals included the analysis of nurse's feeling of readiness for work with parents of critically ill children, and elaboration of a booklet concerning the initial introduction of an intensive care and resuscitation unit for infants and older children for better parents' awareness. In the research part of the thesis a qualitative research was used. The data collection technique was a semi-standardized interview. A research sample was represented by seven nurses working in the sphere of the critically ill children care in four selected hospitals in the Czech Republic. The study took place in the period starting May 2011 till July 2011. The research results revealed that the parental integration conditions are not quite optimal. First of all, in this sphere the nurses pointed to a limited accommodation capacity for parents, lack of supporting services and unsatisfactory site layout of the intensive care units. The nurses expressed their readiness to the closer cooperation with critically ill children's parents in the basic nursing sphere, mostly, thereafter, in the sphere of hygiene care. The parental cooperation in the special-care sphere was accepted rather negatively by the respondents. As the research results show, most of the nurses consider the work with parents psychologically very demanding and during their school education they had never been prepared for it by anybody. Findings flowing from the research results gave birth to the information booklet that is a basic informational and educational material for parents of children admitted to the intensive and resuscitation care unit for older children and infants in Hradec Králové University Teaching Hospital. It also represents a detailed instruction for a similar material for other facilities of this type and, last but not least, it facilitates the whole process of initial parents' education for nurses. This graduation thesis can also assist in the education of children's nurses, help students and nurses working with critically ill children's parents understand the "Family-centered care" principles and their practical introduction.
