Spolupráce psychiatrických sester s rodinami duševně nemocných / Cooperation of psychiatric nurses with families of the mentally ill

I was inspired to choose this diploma thesis by reading an article written by an anxious family member of a person suffering from a mental illness. The stigma of a mentally ill patient does not affect only the patient themselves, it also extends to psychiatrists and nursing staff who work with these people, but it also affects their families and close relatives. For them mental illness is a burden from the psychological as well as social perspective; there are changes in family roles, the emotional climate is influenced too. The theoretical part focuses on the cooperation of nurses, families and a multidisciplinary team. It also deals with the personality of the nurse in psychiatric care, psychiatric rehabilitation limitations, communication and education in psychiatry. It also deals with the family of a mentally ill person and forms of support provided to families. The objective of this thesis was to determine whether psychiatric nurses involve families of the mentally ill in nursing care. The secondary objective was to determine whether the families of the mentally ill are aware of all the possibilities of psychiatric treatment in all areas of care. These objectives have been met. A quantitative research, the techniques of a questionnaire and a standardized interview were chosen for this work. The first research set consisted of 212 nurses working in psychiatric care; the second research set consisted of 33 families. Based on the results, it was found that nurses do not have time to educate families during their shifts. The nurses encourage families to participate actively in caring for their relatives and they have no problems in communicating with the families. Furthermore, the results indicate that families do have access to information and they are satisfied with the information from healthcare professionals; families use the information provided in the care of their relatives. Six hypotheses were set to obtain the research objectives, of which 5 hypotheses have been confirmed and a hypothesis has been refuted. This work also included an educational lesson that can help non-medical professionals to orientate in education. An educational lesson on ``Appropriate behaviour of family members of the mentally ill{\crqq}, which can be used in educating families, was prepared for family members.

Autism spectrum disorders, as some of psychiatric disorders, which also include childhood autism, atypical autism, Asperegs´syndrome, childhood disintegrative disorder, and Rett syndrome, are considered to be significant anomalies in children´s development, which are characterized by problems in communication, social interaction and behavior. These are the typical symptoms creating barriers between children and their families, medical personnel, or environment. Despite all efforts to improve the care for children with autism spectrum disorders, the situation is generally unsatisfying. Counseling services are not available in all regions and specialized preschool and school facilities are missing in a lot of places. Also, specialists´ awareness is often insufficient and even in health care facilities unfamiliarity with the issue of children with autism spectrum disorders may be seen. On the part of the public a negative reaction, misunderstanding and prejudice against these children and their families are still being expressed, making autism spectrum disorders stigmatizing. A nurse as a professional provides the care through the nursing process, in which her role consists not only in provision of the direct nursing care, but she also takes on the role of the child´s rights advocate and a mediator for information exchange between the child, the parent, the doctor and herself. In the care of children with autistic spectrum disorders, a nurse provides not only nursing care, but also educational care with the aim to support the child's development. In order to succeed in her efforts, the nurse needs a large portion of empathy, patience and communication skills and all the support and cooperation on the part of the child's parents. The above conclusions are related to objectives of the research carried out using in-depth interviews, during which we wanted to determine if this disease is stigmatizing for families caring for a child with autism and also the way a nurse participates in cooperation with the family. The third objective was to define the specifics of nursing care for children with autism spectrum disorders, from which standards of nursing care for these children were established. Based on the objectives and the study of available resources, four research questions were stated. First: Is the disease stigmatizing for families caring for a child with autism? Second: What is the impact of autism on families caring for a child with this disease? Third: What is the way a nurse participates in cooperation with families caring for a child with autism? And fourth: What are the specifics of nursing care for a child with autism? All the objectives were met. We hope that this thesis will provide nurses with so much information on the issue of nursing care for children with autism spectrum disorders, so that nurses could provide quality care based on the child's needs, and the cooperation with the family could become beneficial and joyful.

Historie a současnost vzdělávání dětských sester a jejich postojů k vlastní profesi / Past and present of education of paediatric nurses and their attitudes towards their own profession

This thesis deals with the history and present state of the education of paediatric nurses and their attitudes towards their own profession. The first objective was to find out information regarding the education in the nursing profession in Bohemia, with a focus on child care, and to create an overview of the education in the profession Paediatric Nurse. The second objective was to find out about the attitudes of paediatric nurses towards their profession. Interviewed were sisters who chose the profession after finishing elementary school and underwent qualifying studies as well as nurses who chose the profession after graduating from a secondary school or university education and achieved their education through specialization studies. The last objective was to compare these two groups and to assess whether there are any differences between them. The theoretical background of the thesis defined basic concepts such as the definition of education, education in nursing, a profession, and attitude towards a profession. Documents related to education since the foundation of nursing education to the present were also used and studied. The chapters on the status of nursing staff and its employment chances focus on the development of the relationship to the profession and shaping attitudes towards it. In the research part of the thesis, the collected information related to education and derived from archival documents, literature, laws and regulations is presented. The results can be used as a basis for creating educational material focused on the history of the field and intended for the education of paediatric nurses to strengthen their identification with the profession. The thesis also presents interviews with in-service paediatric nurses where they express their views on the work in the field and their attitudes towards their profession. The results of the interviews are documented and organized so that it is possible to assess whether there are any differences between these groups. The anticipated differences can be used for further investigation focused on attitudes to the profession. Verified knowledge could then be used for training paediatric nurses aimed at creating desired attitudes towards the profession.

Specifika ošetřovatelské péče u dětí s cystickou fibrózou / Specifics of nursing care in children with cystic fibrosis

The topic of the thesis is ?Specifics of nursing care about children with cystic fibrosis?. Cystic fibrosis, in some countries known as mucoviscidosis, occurs as a part of a complex disorder. It is a hereditary, incurable disease, which may occur at any time during the life. Timely diagnostic of this disease is very important for maintenance of good condition of an affected child. As a late diagnosis involves frequent visits to doctors or hospitals, it leads to complicated examinations with anxiety in the family and postponement of efficient therapy. This is why infant screening is performed in all maternity wards nowadays. The research was performed in the University Hospital Praha Motol. Qualitative research in the form of interviews with 4 nurses from the ward for children with cystic fibrosis, with 5 parents of children affected by cystic fibrosis and with 6 children with cystic fibrosis was applied. Nurses from an anthropologic and spirometry departments and from a department for adults suffering from cystic fibrosis were also interviewed. The responses from the interviews were processed in the form of case studies.

This thesis treats the issues regarding the role of healthcare assistants in the nursing process and their mutual cooperation with general nurses. Four goals were set. The first goal was to ascertain in which way the general nurses allow the healthcare assistants to engage in the nursing process. The second goal was to ascertain what are the healthcare assistants? opportunities to participate in the nursing process. The third goal was to determine what problems are seen by managers regarding the integration of the healthcare assistants in the nursing care system. The last, fourth goal, tried to ascertain whether the system of integration of general nurses and healthcare assistants in the nursing team contributes to improvement of the patient?s care. The thesis is divided into two parts: theoretical and practical one. The theoretical part deals with the nursing process as a method of work of a nurse, qualification training of general nurses and healthcare assistants and possibilities of finding vacancies in the nursing practice. The practical part consists of quantitative-qualitative research survey. For the qualitative research a method of individually conducted, pre-prepared interviews with representatives of the above mentioned professions. Four research questions were determined, i.e.: 1. To what extent are the healthcare assistants, general nurses and managers in positions of ward sisters aware of the nursing process? 2. Have the healthcare assistants a possibility to use their knowledge of the nursing process pursuant to Act No. 96/2004 Coll.? 3. Is the cooperation of healthcare assistants and general nurses perceived as a positive element for improvement of nursing care? 4. What experience have managers in the position of ward sisters in integration of healthcare assistants into the nursing team? The surveyed group consisted of six healthcare assistants, six general nurses and eight ward sisters-managers from minor inpatient facilities in the Pilsen region and Cental Bohemian region. Qualitative research was realized through a survey by questionnaire. Students of the last grade of the study field ?Healthcare assistant? and the study programme ?General nurse? were addressed. For the qualitative part of the research survey the following hypotheses were set: 1. Students of the study field ?Healthcare assistant? are oriented in the competences of healthcare assistants and general nurses in accordance with At No. 96/2004 Coll. 2. Students of the study programme ?General nurse? are oriented in the competences of healthcare assistants and general nurses in accordance with At No. 96/2004 Coll. 3. Students of the study field ?Healthcare assistant? perceive the determined competences for individual fields as a tool for quality professional care. 4. Students of the study programme ?General nurse? perceive the determined competences for individual fields as a tool for quality professional care. In total, 200 questionnaires were distributed, and subsequently, 142 of correctly completed questionnaires were evaluated. The output of this thesis is the verification of the present status of the issue in question. The results will be provided to nursing care managers in the researched healthcare facilities where they could help to improve the management of healthcare activities. The results may be also used to prepare informational and educational sessions clarifying the rules of cooperation of healthcare assistants and general nurses. Furthermore, the results could be offered to the Ministry of Health of the Czech Republic, namely to the Working Committee for transformation of non-medical healthcare professions in the Czech Republic the main work objective of which is the amendment of Act No. 96/2004 Coll.

Achievement of harmony in interaction between a nurse and a patient/client creates better conditions for attainment of the care goals and thus saturation of his/her needs. In combination with Gordon?s model better satisfaction of needs and thus improvement of the care at the orthopaedic department would be achieved.

Over the last few years there has been a significant differentiation of medical professions. At the same time their role and position has changed in the nursing process. Genesis of new professions and distribution of employees in the work field, based on their expertise, has led to changes in the work organization in individual departments. Therefore we can encounter highly specialized care in the intensive care departments, which places on its staff a number of specialized requirements. Aim of this thesis was to map the positions of individual workers and especially on the application of medical assistants. The thesis continues on with the issue of setting the optimal amount of nursing staff in the intensive care departments. Outcome of the thesis is that workers under supervision are currently not finding the proper use. However, in the future, we can count on more of these workers, in the intensive care units, to be more needed. Introduction: Burnout syndrome is a topical issue in the world and in the Czech Republic, not only in the healt sector. The diploma thesis is focused on finding the conection between burnout syndrome and sexuality of general nurses in emergency care. I chose the topic for my diploma thesis because it is close to me and I work in emergency care myself. In addition, the topic is not very well explored, and the research data will contribute to an international study examining the imapct od burnout syndrome on human health. This study in the Czech Republic is under the guidance of PhDr. Šárky Dynákové, Ph.D. and doc. PhDr. Jana Kožnara, CSc. Methodology: Data collection for research was conducted using two anonymous questionnaires. The first questionnaire focused on burnout syndrome using the Czech version of the Shirom-Melamed scale (Shirom, Melamed, 2006 In Ptáček, 2013). The second questionnaire examined the sexual function of a woman according to Mellan in Kratochvíl, S. (1999). The research sample consisted of general nurses working in emergency care at a teaching hospital. Results: The impact of burnout syndrome has been demonstrated by research in sexual desire (H1), self-assessment as a sexual partner (H2), and mood before sexual intercourse (H4). The communication between patient and healthcare professional is one of the keys to patients' adherence to treatment and to successful therapy; hence communication is a relevant and widely discussed issue of our healthcare. The relationship between a patient and a health worker is rather asymmetric, which affects the course of their communication. This asymmetry is based on the patient made weak (helpless) by illness whereas the expert/physician is in the position to prescribe treatment with the health staff administering such treatment. The aim of the study is to explore the extent of asymmetry in Czech hospitals, the situations where it is most pronounced and the changing structure of asymmetry as seen from the patients' point of view. We conducted a qualitative analysis of 16 interviews with respondents who have experienced hospitalization. The analysis focuses on the psychological level of communication and on the dynamic process in which the asymmetry is being negotiated. We have proved that there is an a priori asymmetry in the relationship, which, under certain circumstances, is effective for the treatment.
