On the Utility of Surface Electromyography-Based Biofeedback on Rehabilitation from Total Knee Arthroplasty: A Clinical Trial

Knee osteoarthritis affects approximately 25 million adults. In severe cases, total knee arthroplasty (TKA) is the most common solution. TKA is effective at addressing pain and reducing continued degeneration of articular cartilage. However, effective physical therapy (PT) following TKA is vital for a full functional recovery. Despite the importance of PT, half of patients never achieve a full functional recovery. Decreases in proprioceptive feedback, severe atrophy, and pain inhibition all likely contribute to the variability in effectiveness. Surface electromyography-based biofeedback (sEMGBF) may allow clinicians to address some of these barriers by supplementing proprioceptive feedback and targeting small muscle contractions before eventually increasing the contraction requirement. Using a between group design, we compare the effectiveness of sEMGBF (7) to neuromuscular stimulation (NMES) (6), and a control group (6) in recovery following TKA. Effectiveness was evaluated across 4 metrics (quadriceps strength, range of motion, functional improvement, and quality of life) in a pre-test/post-test fashion. At the statistical level this study suggest that sEMGBF leads to greater improvements in quadriceps strength relative to the NMES and control group. Additionally, visual analysis suggests that sEMGBF may also lead to greater improvements in range of motion, and functional improvement relative to the NMES and control group. The results for quality of life are mixed. Overall, the study provides initial clinical support for the utility of sEMGBF following TKA.

Summary: Title: Kinesiologic analysis of function of selected upper limb muscles after female breast augmentation Objectives: The aim of this research is to analyze the functionality and scope of involvement of the injured muscles due to plastic surgical augmentation procedure at pre-selected physical activities before and after surgery a few months later (specifically, three months - the time the patient left the recovery) using a noninvasive surface electromyography (EMG). Methods: A Case Study measuring method of noninvasive surface electromyography in combination with the method of temporal phase shifts involved in the activation of muscle movement, accompanied by a support method of integrated EMG. Results: There differences have been observed in the timing of individual muscles measured during performing selected testing exercises before and after completion of the augmentation plastic surgery operatios. Furthermore, the possibility of the loss of muscle strength has been detected by some measured muscles and this has the potential to take over functions of damaged muscles. Objectives: The main aim of this thesis was to measure and compare the level of muscle activity and the level of cocontraction of Muscle quadratus lumborum sinister, obliquus internus abdominis sinister, rectus abdominis sinister and dexter obliquus externus abdominis, which were exposed to different stabilization techniques - rhythmic stabilization and technique used in practice - technique "pushing". Methods: This is an experimental study which is analyzed and then assessing the level of cocontraction of antagonists and the degree of muscle strength in two different ways- the technique of PNF and technique pushing. We use a surface electromyography as an objectification method. The sample consisted of 7 probands. Results: It was discovered that left side lying position not enlarged to the level of activity of all muscles required, therefore the muscles involved in PNF diagonals. And then consider whether there is a possibility to use the method of PNF in a combination with Thera- Band during volleyball players' compensatory exercise. Methodology: The thesis has a character of a case study. Using surface electromyography on five probands, we recorded the electrical activity of musculus biceps brachii, musculus deltoideus pars posterior, musculus trapezius pars caudalis, and their antagonists. This diploma thesis is focused on the observation of influence of Vojta's reflex locomotion on the activity of shoulder girdle's muscles at patients with the subacromial impingement syndrome. We examined ten patients with the subacromial impingement syndrome as well as twenty asymptomatic individuals. We evaluated the impact of just one Vojta's reflex locomotion therapy on the activity of muscles using the Method of the Surface Electromyography. At the same time we evaluated if the reaction on the therapy had some impact on measured values. The therapy always took at least time necessary to obtain any reaction of each of the individuals, although the maximum limit of the therapy was 32 minutes. The impact of Vojta's reflex locomotion therapy had not statistical importance with regard on changes of muscle activity after and before the therapy, with regard on the symmetry of muscle activity, as well as the difference of beginning of activation of every muscle before and after the therapy. The statistical importance was influenced just by the corporeal disposition of every measured individual, which was observed on the timing. Title: Consequences of forced hypokinesia on human movement aparatus. Objectives: The aim of this study was to find out the influence of forced hypokinesia after total hip replacement on the excitability of α motoneurons. I deal with the changes of H-reflex and M-wave latencies and the changes of the recruitment curves. The results of the operated and non-operated (kontrol) lower limb were compared. Methods: Five volunteers 3 - 4 weeks after total hip replacement participated in this study. The age was 62,4 (± 3,01). The H-reflex was elicited by stimulation of tibial nerve in a poplitel fossa. The H-reflex and M-wave latencies and its recruitment curves were obtained. After that the values of the operated lower limb were compared with the non-operated one (control). The results were evaluated as case studies. The EMG device Grass Telefactor was used to detect the electrical potential of soleus muscle. The rectangular pulses of 0,5 ms duration were used for stimulation tibial nerve by Grass 88 Stimulator. Then the data were transduced by CED Power 1401 device. Results: We found out that the H-reflexes of both lower limbs were not elicited in the case study no. 2 and the H-reflex of a control lower limb was not elicited in case study no. 1. The Master's thesis is focused on monitoring of the core muscles and m. biceps femoris activity in postural stability examination by Dynamic Neuromuscular Stabilization tests using surface electromyography in patients with LBP. Trunk extension test, trunk flexion test and squat test were chosen. The aim of this study was comparing muscle activity timing, symmetry of muscle activation and average activation between healthy subjects and patients with LBP. The changes of muscle activation pattern were observed and average activation was determined. In theoretical part the principles of DNS, surface electromyography and the relationship between postural stabilization and LBP are discussed. Two groups were measured - patients with LBP and a control group, 20 subjects together. The onset times were measured and rank of activation for each test and for each subject was determined. Then the average order for whole group was specified. Measured data were evaluated according to Wilcoxon signed-rank test. In average activation there was analysed change of activation in time. Results were compared between two measured groups. There is significant difference between the two groups in the rank of activation of m. ES l. dx. in trunk extension test and m. EO in squat test.
