Criação e aplicação de instrumento para verificação de aderência à técnica psicanalítica em sessões de psicoterapia

Introdução: A pesquisa em psicoterapia carece de métodos que visem garantir a aderência à técnica psicanalítica em sessões de psicoterapia. Tal fato é um potencial gerador de vieses em pesquisas de resultados. A padronização e manualização dos tratamentos restringem a técnica e afastam o tratamento daquele aplicado na realidade. Objetivos: Desenvolver um instrumento confiável e de fácil aplicação a fim de verificar se determinada sessão pode ser considerada como psicanalítica e realizar análises iniciais em relação à confiabilidade do instrumento (Artigo 1). Além disso, pretende-se testar se o instrumento é capaz de diferenciar sessões de psicoterapia psicanalítica de outros tipos de psicoterapia (Artigo 2). Métodos: O estudo foi dividido em 5 fases: fase 1- seleção do conjunto de itens, através de grupos focais e construção das sentenças; fase 2 – testes preliminares com aplicação em sessões transcritas de psicoterapia psicanalítica e terapia cognitivo-comportamental (TCC) por 5 avaliadores, problemas discutidos em grupos focais; fase 3- cálculo da consistência interna e confiabilidade inter-avaliadores, utilizando-se 16 sessões de psicoterapia psicanalítica (avaliadas por experts), 2 típicas de TCC e 2 típicas de terapia interpessoal (TIP) provindas da literatura, com 8 avaliadores, tendo sido calculado alfa de cronbach, kappa, sensibilidade e especificidade do ponto de corte e realizada análise fatorial (SPSS 16.0); fase 4 – realizada validade de face, através de opiniões de experts (4 primeiras fases – Artigo 1); fase 5 – analisada capacidade de discriminação entre psicoterapia psicanalítica e TCC, através da aplicação em 25 sessões de psicoterapia psicanalítica e 15 de TCC, por 5 avaliadores, com treinamento prévio de 1h, sendo realizado cálculo do kappa (concordância entre o tipo de psicoterapia e o ponto de corte sugerido) e curva ROC para o ponto de corte (Artigo 2). Resultados: Fase 1- foram selecionados 6 domínios: natureza da intervenção, neutralidade, uso de interpretações, qualidade das interpretações, uso da teoria psicanalítica, clima da sessão, sendo que cada item com escore de 0 a 5, em uma escala Likert; Fase 2- 5 domínios, excluído o item qualidade das interpretações, uma vez que o objetivo é avaliar se uma sessão é ou não psicanalítica e não a sua qualidade; Fase 3- Análise dos Componentes Principais: 1 único fator significativo (eingenvalues = 3,930), explicando 78,61% da variância total, sendo que todos os itens tiveram participação relevante (item loadings > 0,84) na solução fatorial, alfa de Cronbach= 0,81, com ponto de corte de 13: kappa de 0,81, com sensibilidade de 98% e especificidade de 94%; Fase 4- avaliação por experts de outros países, concluindo que os termos selecionados têm o mesmo significado em outras culturas, descrevendo aspectos teóricos e técnicos em diferentes contextos (4 primeiras fases – Artigo 1); Fase 5 – na diferenciação com TCC, o kappa variou entre 0,79 e 1, sendo o kappa conjunto calculado de 0,83. Curva ROC demonstrou que sensibilidade e especificidade de 100% são obtidas com o ponto de corte de 13,2 (Artigo 2). Conclusões: O presente estudo demonstrou que o instrumento criado é útil em verificar aspectos fundamentais da técnica psicanalítica em sessões de psicoterapia, com boa capacidade de diferenciação entre sessões de psicoterapia psicanalítica e de TCC. Mostrou boa confiabilidade, ainda, nos testes iniciais. / Introduction: The psychotherapy research lacks methods to ensure adherence to psychoanalytic technique in psychotherapy sessions. This fact is a potential generator of biases in research results. The standardization of treatment and manualization restrict the treatment technique and deviate from that applied in reality. Objectives: To develop a reliable and easily applied instrument to check whether a session can be considered as psychodynamic psychotherapy and perform initial analysis regarding the reliability of the instrument (Article 1). In addition, we intend to test whether the instrument is capable of differentiating psychoanalytic psychotherapy sessions of other types of psychotherapy (Article 2). Methods: The study was divided into five phases: Phase 1 - Selection of the item pool, from focal groups and construction sentences; Phase 2 - Preliminary tests using transcribed sessions of psychoanalytic psychotherapy and cognitive-behavioral therapy (CBT) by 5 raters, problems were discussed in focal groups; Phase 3 – Tests to evaluate internal consistency and interrater reliability were performed, using 16 sessions of psychoanalytic psychotherapy (assessed by experts), two typical sessions of CBT and two of interpersonal therapy (IPT) from literature, 8 raters. Cronbach's alpha, kappa, sensitivity and specificity of the cut point were obtained and factorial analysis was performed (SPSS 16.0); Phase 4 - face validity through expert views (4 early stages - Article 1), Phase 5 - analyzed the capacity of discrimination between psychotherapy and CBT, using 25 sessions of psychotherapy and 15 of CBT, by 5 raters with prior training of 1 hour, kappa (agreement between the type psychotherapy and the cutoff suggested) was obtained and ROC curve for the cutoff (Article 2). Results: Phase 1 – Six main domain were selected: nature of interventions, neutrality, use of interpretations, quality of interpretations, use of psychoanalytic theory, ambiance of the session, each item should be score of in a Likert scale (0-5); Phase 2 - 5 domains, the evaluation of the quality of interpretations was ruled out, because the objective of the instrument was to evaluate whether or not a session was psychoanalytic, rather than but not its quality; Phase 3 - Principal Component Analysis revealed a unique meaningful factor (eingenvalues = 3.930), explaining 78.61% of total variance, all items had a relevant participation (item loadings> 0.84) in the factor solution. Cronbach's alpha was 0.81 and, using a cutoff of 13, kappa was 0.81, sensitivity was 98% and specificity was 94%; Phase 4 – the evaluation by experts from other localities ensured that the selected terms have the same meaning in other cultures, describing theoretical and technical aspects in different contexts (4 early stages - Article 1); Phase 5 – In the analysis of the ability to differentiate psychoanalytic psychotherapy CBT, kappa ranged between 0.79 and 1 and the aggregated kappa was 0.83. ROC curve showed that sensitivity and specificity of 100% are obtained with a cutoff of 13.2 (Article 2). Conclusions: This study demonstrated that the instrument is useful in verifying the fundamental aspects of psychoanalytic technique in psychotherapy sessions, with a good ability to differentiate sessions of psychoanalytic psychotherapy CBT. It obtained a good reliability, even in initial tests.
