Tomáš Kavka Abstract: Nonspecific back pain (nonspecific LBP) is the most common reason for visiting a doctor and it is the leading cause of activity limitation and inability to work by patients. The theoretical part of this thesis describes the issue of LBP and related clinical guidelines. In addition, it summarizes current research results of the influence of health professionals' fear-avoidance beliefs (FA beliefs) on their clinical decision making for LBP patients. The practical part relates to the research of FA beliefs and non-adherence to clinical guidelines regarding care of individuals experiencing LBP among health professionals in the Czech Republic. The degree of FA beliefs was evaluated using a translated and cross-culturally adapted questionnaire "Fear- Avoidance Beliefs Tool", while the degree of non-adherence was evaluated using a self- reported questionnaire in connection to fictitious clinical vignette. Title: Therapeutic intervention of low back pain - comparing of two therapeutic approaches Objectives: The main objective of this experimental study was to compare two therapeutic approaches: Sensorimotor stimulation by Janda Vávrová and Proprioceptive neuromuscular facilitation in patients with low back pain. Methods: Experimental comparative study aimed at comparing the effect of Proprioceptive neuromuscular facilitation method of Kabat and Sensorimotor stimulation by Jandy and Vávrová in patients with chronic low back pain. The probands were selected from among patients of private healthcare facilities Medi-centre Skalka s.r.o. The assessment of pain, we used a standardized questionnaire Oswestry. The data will be processed using statistical method Mann-Whitney U test. Results: The study showed no significant difference in treatment effect low back pain between Sensorimotor stimulation method by Janda and Vávrová and Proprioceptive neuromuscular facilitation. Posilování u pacientů s nespecifickými bolestmi dolní části zad / Strengthening in patients with nonspecific low back pain

In the present work we compare the effectiveness of therapy in patients with chronic non-specific low back pain (CNLBP) using classical analytical strengthening and strengthening program based on principles of physiotherapy techniques that focus on the stabilization of the spine and are used in the treatment CNLBP. The experiment included a total of 14 patients diagnosed with CNLBP who were divided into two groups (n = 7), each of who underwent a 12-week training either classical or physiotherapy conceived strengthening. As for the evaluated parameters, we chose the size and quality of pain (visual analogue scale and Short Form McGill Pain Questionnaire), restrictions in daily life in relation to LBP (Oswestry Disability Index); we tried to capture a change in the stabilizing muscle function by using a collection of tests according to Kolar (2006). The evaluation took place at the beginning and after the program and then at intervals of six months. The results showed that there was a statistically significant improvement in both groups in the monitored parameters. The difference in results in pain intensity and functional limitations in life when comparing the two groups showed statistically insignificant. Prevence posturálních vad v populaci / Prevention of postural defects in the population

This dissertation aims at verifying methodology used for somatographic evaluation of the shape and statics of the spine. The potential of the methodology verified lies in increased objectivity of upright body posture evaluation and thereby contributes to the specification of preventative measures for postural defects, and their consequences. Under a cross-sectional pilot study, a non-invasive somatographic method-a diagnostic DPT-3 system designated for spine shape diagnostics-was used to examine selected biomechanical parameters (spinal curves and statics in the sagittal plane in the upright position) in a total of 508 probands. Of this, 326 were school-age children and 182 adults. Among the key parameters monitored were: Cl (depth of cervical lordosis); LI (depth of lumbar lordosis); the sagittal balance of Cl/Ll, and VThk (distance of the peak of the thoracic kyphosis from the ideal verticals-IV). A questionnaire was used to evaluate the level of movement activity in probands and the relation between the occurrence of painful back and headaches and the measurement of biomechanical parameters and movement activity. The abstract of the diploma thesis is focused on the acupressure and its influence of backache of medical staff. We chose the theme of acupressure because it is a non-invasive treatment method which can be practiced by everyone in the comfort of the home and no aid is necessary to have, just the basic knowledge and training how to perform acupressure. Acupressure is not physically or time consuming and that is why it is considered as a suitable means as an additional treatment of pain and other health disorders. It can improve nursing care. The theoretical part deals with pain and pain of medical staff who are the highest risk group of backache in the lumbar region. The backache is the most frequent cause of incapacity for work. Furthermore, in the theoretical part we deal with acupressure, one of the methods of alternative medicine. For the processing of the theoretical part, we used the available foreign and Czech literary sources. The empirical part of the diploma thesis is a quantitative method. The results obtained from the research were processed in the overview tables and case reports. For the thesis we set two research objectives. The first objective was to find out the experience of medical staff with acupressure. The second objective was to verify the influence of the techniques of acupressure for backache of medical staff. On the basis of the aim two hypotheses were created. The first hypothesis should show whether the backache will be softened by application of the acupressure techniques. The second one shows whether there is the difference in approach of nurses and doctors to perform acupressure.

Title: Effect comparison of educational-movement therapy "stabilization function"of a trunk and traditional rehabilitation therapeutic approach for patients with nonspecific low back pain. Nonspecific low back pain is a main cause of disability and the principal cause for injury-related lost work-days. The various therapies for nonspecific low back pain have been elaborated in many studies. The recommended therapeutic guideline for primary care of patients with nonspecific low back pain is early mobilization combined with a specific rehabilitation therapeutic approach. One such specific rehabilitation therapeutic approach is "trunk stabilization" therapy. The objective of this dissertation is to compare a therapeutic approach involving "stabilization exercises" with the traditional clinical approach that has been clinically proven to be most efficacious for patients with acute nonspecific low back pain. This study compares the effects of the prescribed therapies on 24 patients with non- specific acute low back pain. Title: Analysis of plantar pressures distribution at patients after microdiscectomy L5/S1 Author: Petra Végsöová Objective: The topic of the thesis is to analyze distribution of plantar pressure at "broken" and "healthy" foot. The work is based on comparison of plantar pressure distribution on "broken and healthy" footprint in static position before and after intensive daily rehabilitation. Method of solution: Twelve 40-65 years-old individuals were chosen for the study. They were treated for L5/S1 microdiscectomy and rehabilitated daily at Rehabilitation clinic Malvazinky in Prague. This group of individuals was examined in two ways; by a physiotherapist, who analysed their state of health using kinesiology analysis, and by Plantograph V07, which is used for examination of state of pressure between sole of foot and miniature pressure sensors shaped in a grid. Three weeks of rehabilitation was ended by the same ways of examination used at the beginning of therapy. The results were processed and compared by a computer. Title: Physiotherapy of patients with low back pain of viscerovertebral etiology. Objectives: The objective of this study is to verify the significance of inclusion of procedures of a comprehensive therapeutic approach according to Eastern techniques to classical physiotherapy and to find out whether these Eastern techniques can contribute to the enrichment of standard physiotherapy. Another goal is to develop appropriate regimen measures according to TCM and Ayurveda, to create a yoga regimen suitable for patients who have renal and bladder dysfunction according to TCM and whose main common symptom is chronic non-specific low back pain. The study is based upon a comparison of a four-week physiotherapy program according to the physician's indication and a physiotherapy program enriched with compiled regimen measures and yoga exercises in selected probands. The aim of this study is to compare anthropometric parameters of the trunk between a group of patients with low back pain and a control group without pain. These findings would support the theory that low back pain may be caused by non-ideal postural-locomotion development in childhood, resulting in measurable morphological variations in adulthood. 60 individuals with low back pain (patients) and 60 healthy volunteers (controls) participated in the study. It was confirmed that the distance between the jugular fossa and xiphoid process is shorter in relation to the body height in the group of patients versus the control group. Conversely, there was a significantly longer distance between the xiphoid and symphysis relative to the body height among the patients. Furthermore, diastasis of rectus abdominis muscle was observed more frequently in the low back pain group. These findings suggest that patients with low back pain present with trunk morphological differences when compared with a matching healthy group.
