Bakgrund Patienters rätt till delaktighet understryks i flera lagar och riktlinjer. Trots detta visar studier att personer med demenssjukdom i stor utsträckning helt exkluderas från delaktighet i beslut rörande deras vård. Dessa utmaningar och komplexiteten med att skapa delaktighet trots nedsatta kognitiva funktioner utgör ett ökat behov av kunskap och förståelse för demenssjuksköterskors arbete med att främja delaktighet för personer med demenssjukdom. Syfte Syftet är att beskriva demenssjuksköterskors erfarenhet av att arbeta med delaktighet för personer med demenssjukdom inom kommunal hälso- och sjukvård. Metod Studiens genomfördes som en empirisk kvalitativ intervjustudie med induktiv ansats och analyserades sedan med en kvalitativ innehållsanalys med manifest fokus. Resultatet bygger på semistrukturerade intervjuer med sju demenssjuksköterskor. Resultat Resultatet visar att demenssjuksköterskor arbetar med delaktighet på organisatorisk nivå genom att nå ut med information i ett tidigt skede och genom att samverka med regionen. Ett personcentrerat förhållningssätt präglar arbetet med delaktighet på individnivå genom ett gott bemötande, individanpassad kommunikation, att etablera en relation samt genom dokumentation skapa en bild av personen med demenssjukdom. Vidare främjas delaktighet på individnivå av att arbeta i team och av en underlättande vårdmiljö. Resultatet visar att otydlighet och att få ett uttalat samtycke kan utgöra hinder för delaktighet, liksom att vårdpersonal eller anhöriga fråntar personen med demenssjukdom möjligheten att delta. Konklusion Demenssjuksköterskor har en viktig funktion i arbetet med att på olika nivåer skapa förutsättningar för personer med demenssjukdom att vara delaktiga i sin vård genom hela sjukdomsförloppet. / Background Patients' right to participation is underlined in several laws and guidelines. Despite this, studies show that people with dementia are to a large extent completely excluded from participation in decisions regarding their care. These challenges and the complexity of creating participation despite impaired cognitive functions constitute an increased need for knowledge and understanding of dementia nurses' work to promote participation for people with dementia. Aim The aim of this study is to describe the experiences of dementia nurses´ with the participation of people with dementia within municipal healthcare. Method The study was conducted as an empirical qualitative interview study with an inductive approach and then analyzed with a qualitative content analysis with manifest focus. The results are based on semi-structured interviews with seven dementia nurses. Results The results show that the dementia nurse works with participation at an organizational level by reaching out with information at an early stage and by collaborating with the region. A person-centred approach characterises the work with participation at the individual level through good treatment, individualized communication, establish a relationship and through documentation create an image of the person with dementia. Furthermore, participation at the individual level is promoted by teamwork and a facilitating care environment. The results show that ambiguity and obtaining explicit consent can be obstacles to participation, as well as that healthcare professionals or relatives deprive the person with dementia of the opportunity to participate. Conclusions The dementia nurse has an important function in the work of creating conditions at different levels for people with dementia to be involved in their care throughout the course of the disease.
