Previous issue date: 2016-03-28 / Rheumatoid arthritis (RA) is a chronic autoimmune and inflammatory disease characterized by the production of autoantibodies and the articular and extra articular events, including the Central Nervous System (CNS) of patients. Factors such as the chronic inflammation and the presence of autoantibodies against myelin proteins are potential mechanisms involved with cognitive dysfunction in other autoimmune diseases. Disorders such as mild cognitive impairment and Alzheimer's disease have high plasma levels of autoantibodies against myelin proteins well as important cognitive impairment and increased levels of S100? protein. Based on this, here we explore the relationship between cognitive performance, plasma levels of antibodies and S100? protein of patients with RA. Twenty patients with RA and nineteen healthy controls did not differ significantly by gender, age and schooling, were recruited in this study. Cognitive function, (MMSE, working memory, processing speed, inhibitory control and mental flexibility) declarative memory and depression were assessment through interviews where specific clinical questionnaires were apply. The plasma of patients and controls was obtained by centrifugation to investigate the levels of S100? protein and autoantibodies against Basic Myelin Protein (English MBP - Myelin Basic Protein) and against Glycoprotein Myelin oligodendrocyte (English MOG Myelin Oligodendrocyte Glycoprotein). RA patients had a lower cognitive performance in the MMSE, declarative memory, working memory, processing speed, inhibitory control and mental flexibility compared to healthy controls. Patients present S100? protein levels and anti - MBP and anti - MOG higher than controls which negatively correlated with cognitive performance of these individuals. In regression analyzes peripheral levels of anti - MOG and anti - MBP acted as predictors of executive function scores as declarative and working memory, processing speed and inhibitory capacity. In conclusion, when compared to healthy controls, RA patients have higher serum levels of autoantibodies and S100? protein which were negatively related and predicted the worst cognitive performance of these individuals. / A Artrite Reumatoide (AR) ? uma doen?a autoimune cr?nica e inflamat?ria caracterizada pela produ??o de auto anticorpos e por eventos articulares e extra articulares, atingindo inclusive o Sistema Nervoso Central (SNC) dos pacientes. Fatores como a inflama??o cr?nica a presen?a de auto anticorpos contra prote?nas da mielina s?o potenciais mecanismos envolvidos com a disfun??o cognitiva em outras doen?as autoimunes. Patologias como o dano cognitivo leve e doen?a de Alzheimer apresentam altos n?veis plasm?ticos de auto anticorpos contra prote?nas da mielina bem como altera??es cognitivas importantes e aumento dos n?veis da prote?na S100?. Com base nisto, neste trabalho n?s exploramos as rela??es entre desempenho cognitivo, n?veis de auto anticorpos e da prote?na S100? na circula??o perif?rica de pacientes com AR. Vinte pacientes com AR e dezenove controles saud?veis, que n?o diferiram significativamente em rela??o a sexo, idade e escolaridade, foram recrutados neste estudo. A fun??o cognitiva (MMSE, mem?ria de trabalho, velocidade de processamento, controle inibit?rio e flexibilidade mental) mem?ria declarativa e depress?o foram avaliados atrav?s de entrevistas onde foram aplicados question?rios cl?nicos espec?ficos. O plasma dos pacientes e controles foi obtido por centrifuga??o para investiga??o dos n?veis da prote?na S100? e de auto anticorpos contra Prote?na B?sica da Mielina (em ingl?s MBP ? Myelin Basic Protein), contra Glicoprote?na da Mielina de Oligodendr?citos (em ingl?s MOG Myelin Oligodendrocyte Glycoprotein). Os pacientes com AR tiveram um desempenho cognitivo inferior no MMSE, mem?ria declarativa, mem?ria de trabalho, velocidade de processamento, controle inibit?rio e flexibilidade mental se comparado a controles saud?veis. Embora todos os indiv?duos, de ambos os grupos, tiveram uma pontua??o superior ? estabelecida pelo cutoff do MMSE. Os pacientes apresentaram n?veis de prote?na S100? e de anticorpos anti ? MBP e anti ? MOG mais altos do que os controles o que se correlacionou negativamente com o desempenho cognitivo desses indiv?duos. Nas an?lises de regress?o os n?veis perif?ricos de anti ? MOG e anti ? MBP atuaram como preditores dos escores de fun??es executivas como mem?ria declarativa e de trabalho, velocidade de processamento e capacidade inibit?ria. Concluindo, quando comparados a controles saud?veis pacientes com AR tem n?veis plasm?ticos mais altos de auto anticorpos e prote?na S100?, que se correlacionaram e predisseram negativamente o pior desempenho cognitivo destes indiv?duos.
