Previous issue date: 2009 / A Portaria nº 648/GM de 28 de março de 2006, Anexo I, Capítulo II, Item 4,diz respeito às especificidades da Estratégia Saúde da Família - ESF, com ênfase para o processo de trabalho, estabelecendo diretrizes e normas para seu funcionamento. Este estudo buscou descrever o processo de trabalho da Estratégia Saúde da Família do município de Fortaleza CE no biênio 2007 - 2008, tendo como parâmetro a Portaria mencionada e a percepção dos atores sociais envolvidos. Como objetivos específicos (1)foram relacionadas as ações estruturantes da ESF do município com as prescrições da Portaria nº 648/2006, Anexo I, Capítulo II, Item 4; (2)identificadas as percepções dos profissionais de saúde das EqSF sobre as estratégias, os momentos marcantes, os principais conflitos, e fatores que exerceram influência determinante no processo de trabalho, e (3)propostas 64 estratégias, adequadas à Portaria em estudo, para facilitação do processo de trabalho e otimização da implementação da Saúde da Família no município. Foi realizado um estudo de caso, utilizando-se de entrevistas semi estruturadas,observação participante e pesquisa documental. Os sujeitos da pesquisaforam os médicos, enfermeiros, agente comunitário de saúde e coordenadores de dois centros de saúde da família. Os resultados revelaram que o município passa por variadas dificuldades na implementação da Estratégia, dentre elas: a (a)coexistência de modelosde Atenção Básica; (b)cenário urbano sobrecarregando sua implementação; (c)violência urbana e (d)baixa integração entre os profissionais das EqSF e os profissionais veteranos da Atenção Básica. / The Decreed Nº648/GM from March 28th 2006, Attachment I, Chapter II, Item 4, concerns on specific matters of Family’s Health Program Strategies, emphasizing work process, establishing operation directives and rules. The present study tried to describe the work process of the Family’s Health Strategies of the city of Fortaleza from 2007 and 2008, based on the decreed mentioned above and the professionals involved in the process. As specific objectives (1) list the structuring actions carried out by the city of Fortaleza to the Family’s Health Program Strategies according to what is
recommended by the Decreed 648/2006, Attachment I, Chapter II, Item 4; (2) identify the perception of the health professionals of Family’s Health Program Teams about the
strategies, important moments, the conflicts and the factors that have a determining influence on the work process, and (3) 64 strategies ad equated to the decreed were proposed, in order to facilitate work process and optimize the implementation of the Family’s Health Program in the city. A case study was carried out through semistructured interviews and the observation of the people involved as well as a documental research. The subjects of the research were doctors, nurses, health communitarian agents and coordinators of two health centers of the Family’s Health Program. The results showed that the city of Fortaleza is coming through a series of difficulties in the implementation of the Strategies, among them there is: (a) the
coexistence of other models of Basic Health Attention; (b) the urban scenario that
overloads its implementation; (c) urban violence and (d) the low level of integration between the professionals of the Family’s Health Program Teams and the veterans professionals of the Basic Health Attention Teams. The final consideration revealed is that the city of Fortaleza is facing many difficulties to adequate to the 648/2006,
although it has been investing a lot quantitatively and qualitatively to the consolidation and to the quality of the Strategy. The outstanding landmarks identified were: the
democratization of the management with the management debate circles; the creation of
the Municipal System Health-School and the implementation of the disposal to welcome the patients according to the risk evaluation and classification.
