Previous issue date: 2007-02-01 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Objective: The purpose of this study was to investigate two methods for obtaining the real-ear to coupler difference (RECD) in infants: with an insert earphone and the measure in free field with a linear hearing aid. The test -retest reliability and reproducibility of both methods were compared as well as the reliability of between-ear RECD measures. The values of RECD obtained were compared with the database of DSL [i/o] 4.1 and 5.0. Design: The RECD was measured in 25 hearing impaired infants aged 3 months to 24 months with no malformations or visible syndrome and with normal middle ear function. The RECD values were obtained via both methods twice each with removal and reinsertion of the probe microphone between each measure and using the infant s earmold. Results: The insert earphone method resulted in significantly better test-retest reliability than the free field because the values were most similar for all frequencies and ages. The RECD values obtained in the current study were similar to those proposed by DSL [i/o] 4.1 and DSLm 5.0 database. There were no significant differences between the mean RECDs obtained using an insert earphone compared with those obtained via free field method but the standard deviation is significantly higher in free field measurement. Conclusions: The results of this study support the use of the insert earphone method of measurement of the RECD as a reliable tool in assisting with the hearing aids fitting in infants. The variability is smaller in this method and the values were more similar to DSL[ i/o] 4.1 and DSLm 5.0 database / Objetivo: Analisar comparativamente a RECD medida em bebês por meio de dois métodos de mensuração: a medida realizada com o fone de inserção e a medida em campo livre com utilização de um AASI linear, verificando a confiabilidade e reprodutibilidade do teste e reteste de cada medida. Método: Foi mensurada a RECD em 25 bebês deficientes auditivos, entre 3 a 24 meses, sem mal formação ou síndrome aparente que apresentavam função de orelha média dentro da normalidade. A medida da RECD foi realizada com fone de inserção e em campo livre com um AASI linear duas vezes cada uma delas, com molde do bebê. A ordem de aplicação dos dois métodos e da orelha a ser testada foi estabelecida de forma aleatória. A coleta foi realizada no Centra da Audição DERDIC, com aprovação do comitê de ética e consentimento do responsável pelo bebê. Resultados: As medidas com fone de inserção mostraram-se mais confiáveis do que as realizadas em campo livre com AASI, pois apresentou menor variação intra e intersujeitos. Os valores da RECD encontrados nesse estudo estão próximos aos propostos pelos bancos de dados do DSL [i/o] 4.1 e DSLm 5.0. Não houve diferença significante entre as orelhas na medida da RECD. Conclusão: Na comparação entre médias, controlando a faixa etária, houve diferença estatisticamente significante entre os valores da RECD medidos com fone de inserção e em campo livre com AASI na maioria das freqüências estudadas. Portanto as medidas não são equivalentes. A medida da RECD realizada com fone de inserção mostrou-se mais confiável e precisa que a realizada em campo livre com o AASI, pois a diferença entre teste e o reteste foi menor, as variações nas medidas intrasujeito e intersujeitos foram menores do que em campo livre com o AASI, assim como o desvio padrão dos valores obtidos
