Efeitos da modifica??o do estilo de vida nos par?metros metab?licos e na qualidade de vida de pacientes com s?ndrome metab?lica

Previous issue date: 2014-08-29 / Purpose : Lifestyle intervention programs can reduce the prevalence of Metabolic Syndrome (MS) and, therefore, reduce the risk for cardiac disease: one of the main public health problems nowadays. The aim was to compare the effects of three types of approach for lifestyle change programs in the reduction of metabolic parameters and identify its impact on quality of life (QOL) of individuals with MS. Methods : A randomized controlled trial included 125 individuals with MS between 30-59 years old, from the MERC Study at Hospital S?o Lucas of PUC-RS. Individuals were randomized into three groups of interdisciplinary intervention: Standard Intervention (SI), control group, Group Intervention (GI) and Individual Intervention (II), during 12 weeks. The primary outcome was change in the metabolic parameters and, secondarily, the improvement in QOL measures at three moments: baseline, 3 and 9 months. Results : Only 72 individuals concluded the study: SI: 19, GI: 25 and II: 28. GI and II resulted in significant reduction of body mass index, waist circumference, SBP at 3 months and the improvement of QOL, although it was significantly associated with the physical functioning domain. However, these changes did not remain 6 months after the end of intervention. Depression and anxiety were significantly associated with worse QOL, although they have not shown effect on the response to intervention. Conclusion : Interdiscliplinary intervention, especially in a group, might be an effective and economically feasible strategy in the control of metabolic parameters of MS and improvement of QOL compared to SI, even in a dose-effect relationship. / Objetivos : Programas de interven??o de estilo de vida podem diminuir a preval?ncia da S?ndrome Metab?lica (SM) e, assim reduzir o estabelecimento da doen?a card?aca: um dos principais problemas de sa?de p?blica da atualidade. O objetivo desta pesquisa foi comparar os efeitos de 3 tipos de abordagens de programas de modifica??o de estilo de vida na redu??o dos par?metros metab?licos e identificar o seu impacto na qualidade de vida (QV) de indiv?duos com SM. M?todos : Foi realizado um estudo controlado randomizado, o qual incluiu 125 indiv?duos de 30-59 anos com SM, conduzido pelo grupo de pesquisa MERC, no Hospital S?o Lucas da PUC-RS. Os indiv?duos foram randomizados em tr?s grupos de interven??o interdisciplinar: Interven??o Padr?o (IP), grupo controle, Interven??o em Grupo (IG) e Interven??o Individual (II), com dura??o de 12 semanas. O desfecho principal foi modifica??o nos par?metros metab?licos e, secundariamente, a melhora na QV, medidos em tr?s momentos: no in?cio do estudo, aos 3 meses e aos 9 meses. Resultados : Conclu?ram a interven??o somente 72 indiv?duos: IP: 19, IG: 25 e II: 28. A IG e II promoveram significativa redu??o do ?ndice de massa corporal, circunfer?ncia abdominal, e na press?o arterial sist?lica aos 3 meses, e na melhora da QV, apesar de ter demonstrado significativa associa??o apenas com o dom?nio capacidade funcional. Entretanto estas modifica??es n?o se mantiveram 6 meses ap?s o t?rmino da interven??o. A depress?o e ansiedade foram significativamente associadas ? piora da QV, apesar de n?o ter sido demonstrado efeito na resposta ? interven??o. Conclus?o : A interven??o interdisciplinar, especialmente em grupo, pode ser uma estrat?gia eficaz e, economicamente vi?vel no controle dos par?metros metab?licos da SM e melhora na QV, comparado ? interven??o padr?o, ainda que numa rela??o dose-efeito.
