Previous issue date: 2015-03-03 / Introduction: The oldest old (80 years and older), are the segment of the population which has increased the most in Brazil. Among the leading causes of death in the oldest old are respiratory diseases, which average annual rate is 10 times higher than in the age group 60-69 years-old. Loss of muscle mass is a common factor in the aging process and can be associated with a decrease in the respiratory function. Objective: to investigate the association between handgrip strength (HGS) and respiratory muscle strength (MIP and MEP) in the oldest old, as well as understand the factors potentially associated with HGS.Methods: descriptive and analytical study with convenience sampling. Results: The oldest old with higher HGS showed higher values of MIP and MEP. Functional disability was the variable most associated with HGS, the greater the degree of dependence, the worse the test result. Conclusion: The HGS is an important predictor of MIP and MEP. Therefore, motor physical therapy in the oldest old should also be prioritized in the rehabilitation of respiratory diseases. The maintenance of muscle strength can be a determining factor in the preservation of functional ability in the oldest old.
