Bakgrund Svårläkta sår kan innebära lidande och sänkt livskvalitet för individen som drabbas samt ökade kostnader för samhället. Sår som inte läker förekommer särskilt bland äldre människor och kan bero på bakomliggande faktorer såsom venös och arteriell insufficiens samt diabetes. Vårdens primära mål är vanligtvis att uppnå sårläkning men för patienter med svårläkta sår kan välmående och symtomlindring vara minst lika viktigt. Det finns dock begränsad kunskap om hur vården ska tillgodose behoven hos denna patientgrupp på bästa sätt. Att sammanställa hur patienter med svårläkta sår upplever omvårdnaden kan ge insikt om och belysa faktorer som är betydelsefulla för dessa patienter och på så vis bidra till att öka kunskapen hos vårdpersonal när det gäller omvårdnad vid svårläkta sår. Syfte Syftet var att belysa patienters upplevelser av omvårdnad vid svårläkta sår. Metod En icke-systematisk litteraturöversikt där 15 vetenskapliga originalartiklar med kvalitativ och kvantitativ ansats inkluderades. Artiklarna inhämtades genom sökordskombinationer och manuella sökningar i databaserna PubMed och CINAHL. Artiklarna kvalitetsgranskades och en integrerad analys genomfördes av inkluderade artiklar. Resultat Den integrerade analysen resulterade i sex kategorier: upplevelse av vårdkontinuitet, upplevelse av vårdpersonalens kunskap om sårvård, upplevelse av vårdrelation, mötet och delaktighet vid omvårdnad, upplevelse av kommunikation och information, upplevelse av förband, förbandsbyten och tekniska omvårdnadsåtgärder samt upplevelse av tidsåtgång kopplat till omvårdnad. Slutsats Studien belyser ett behov av personcentrerad och holistisk omvårdnad som beaktar individens behov och tillvaratar dennes resurser samt främjar delaktighet. Det upplevs viktigt att vårdpersonalen har kunskap om sårvård samt beaktar patienternas kunskap om sina egna tillstånd. Patienters upplevelser av mötet med vårdpersonal beskrivs som såväl positiva som negativa. Studien tyder även på att lidande i form av smärta är en framträdande upplevelse som förbises av vårdpersonal och är otillräckligt lindrad. / Background Chronic ulcers may cause suffering and reduced quality of life as well as increased economic costs to society. Ulcers that do not heal are most common among older people and might be caused by underlying conditions such as venous and arterial insufficiency or diabetes. Wound healing might usually be the primary goal of health care but well-being and symptom relief might be just as important for the patient if the ulcer is chronic. The current knowledge of how to meet the health care needs of patients with chronic wounds is however limited. Compiling the experiences and perceptions of nursing care among patients with chronic ulcers might give insight into factors that are considered important for these patients, which could possibly contribute to raising the healthcare personnel’s knowledge of nursing care for chronic ulcers. Aim The aim of this study was to illuminate how patients with chronic ulcers experience nursing care. Method A non-systematic literature review which included 15 scientific original articles with qualitative and quantitative approaches. The articles were retrieved from the databases PubMed and CINAHL by using combinations of keywords and by manual searches. The quality of each article was reviewed, and the content of the included articles was analyzed with an integrated analysis. Results The integrated data analysis resulted in six themes: experience of continuity of care, experience of health care personnel’s knowledge of wound care, experience of care relationship, the meeting and participation in nursing care, experience of communication and information, experience of dressings, dressing changes and technical nursing care interventions and experience of time consumption related to nursing care. Conclusions This study revealed a need for person centered care and holistic nursing that encourage patient participation in care and considers the individual’s needs as well as resources. It is important that healthcare personnel have knowledge of wound care and to take the patients´ knowledge of their own condition in consideration. Patients describe having both positive and negative experiences of encountering with health care personnel. Findings of this study also indicate that patients are suffering due to physical pain. The study also shows that the patients´ pain is overlooked by healthcare personnel and the patients are receiving insufficient pain relief.
