Previous issue date: 2016-12-02 / Coordena??o de Aperfei?oamento de Pessoal de N?vel Superior (CAPES) / Altera??es no status de zinco no sistema nervoso t?m sido implicadas em uma gama de doen?as neurodegenerativas, incluindo a esclerose lateral amiotr?fica (ELA), cuja defici?ncia ou o excesso de zinco pode influenciar sua patog?nese e progress?o. Diante disso, objetivou-se avaliar o consumo diet?tico e o status de zinco em pacientes com ELA, assistidos em um ambulat?rio especializado no Hospital Universit?rio Onofre Lopes, Natal-RN, Brasil. Foi realizado um estudo do tipo caso-controle n?o pareado composto por 57 indiv?duos, sendo 20 pacientes com ELA (grupo caso) e 37 indiv?duos saud?veis (grupo controle). Foi realizada uma avalia??o cl?nica no grupo caso, em rela??o ao in?cio e tempo dos sintomas, via de alimenta??o e escore da escala de avalia??o funcional. Para ambos os grupos determinou-se o ?ndice de massa corporal (IMC) e o consumo diet?tico de energia, macronutrientes, fibra e zinco, por meio da an?lise de 2 recordat?rios de 24h. Ademais, para ambos os grupos foram verificadas as concentra??es de zinco no plasma e na urina por meio de espectrofotometria de absor??o at?mica. Observou-se que a maioria dos participantes estavam eutr?ficos segundo IMC, correspondendo a 73,7% dos casos e 62,2% dos controles. O consumo m?dio de energia, prote?na, carboidrato e gordura foi significativamente menor para o grupo caso comparado ao grupo controle. Houve maior preval?ncia de inadequa??o da ingest?o de zinco no grupo caso (35%) em compara??o ao grupo controle (27%). A m?dia de zinco no plasma foi significativamente menor no grupo caso em rela??o ao grupo controle (77,13 ? 22,21 vs. 87,84 ? 17,44 ?g/dL) (p=0,05). N?o houve diferen?a significativa para o zinco na urina entre casos e controles (p=0,155). No grupo caso, as concentra??es de zinco no plasma e na urina estavam abaixo dos valores de refer?ncia em 50,0% e 52,6% dos pacientes, respectivamente. Grande parte dos pacientes com ELA apresentou altera??o no status de zinco corporal, demonstrada pela baixa concentra??o de zinco no plasma e baixa ingest?o alimentar. A defici?ncia de zinco encontrada na metade dos pacientes com ELA pode contribuir para um pior progn?stico e deve ser alvo de interven??es nutricionais espec?ficas que visem a corre??o dessa defici?ncia. / Disturbances of zinc homeostasis in nervous system, represented by its excess or deficiency, are associated with numerous neurodegenerative diseases, including amyotrophic lateral sclerosis (ALS), contributing to its pathogenesis and progression. Thus, the purpose of this study was to evaluate the dietary intake and zinc status in ALS patients attending a specialized ambulatory care at Onofre Lopes University Hospital, Natal-RN, Brazil. This is a case-control study of 57 individuals, 20 patients with ALS (case group) and 37 healthy subjects (control group). Case group patients were clinically characterized in terms of symptom onset, symptom duration, feeding pathway and score on the ALS functional rating scale. In both groups, body mass index (BMI) was calculated, and dietary energy, macronutrients, fiber and zinc intake were obtained from two 24-hour recalls. Moreover, in both groups, plasma and urinary zinc concentrations were determined by atomic absorption spectrophotometry. Most of the participants were eutrophic, according to BMI, accounting for 73.7% of the cases and 62.2% of controls. Mean energy, protein, carbohydrate and fat intake was significantly lower for the case group. There was greater prevalence of inadequate zinc intake in the case group (35%) compared to control group (27%). Mean plasma zinc was significantly lower in the case group than in control group (77.13 ? 22.21 vs. 87.84 ? 17.44 ?gZn/dL) (p=0.05). Urinary zinc did not differ significantly between cases and controls (p=0.155). In the case group, urinary plasma and zinc concentrations were below reference values in 50.0% and 52.6% of patients, respectively. A large portion of patients with ALS exhibited changes in body zinc status, demonstrated by the low plasma zinc concentration and low food intake. The zinc deficiency found in half of the ALS patients may contribute to a worsened prognosis in patients with ALS and should be the target of nutritional interventions that aim to correct this deficiency.
