Bakgrund: Fotkomplikationer relaterat till diabetes är den vanligaste komplikationen och kan orsaka mycket lidande. Det är viktigt med stöd, råd och utbildning till personen med diabetes för att undvika fotkomplikationer. Sjuksköterskan i kommunal hemsjukvård har en viktig roll i att stödja patienter i sin sjukdom och få dem delaktiga i sin vård för att kunna hantera sin egenvård på ett adekvat sätt. Syfte: Att beskriva sjuksköterskors erfarenheter av att stödja patienter med diabetesrelaterade fotkomplikationer i kommunal hemsjukvård.Metod: En kvalitativ intervjustudiestudie där sex sjuksköterskor från tre olika kommuner deltog. Intervjuerna analyserades utifrån kvalitativ innehållsanalys.Resultat: I resultatet framkom två huvudkategorier. I den ena huvudkategorin: Patientens medverkan och dess betydelse för sjuksköterskan att stödja framkom det att i kommunal hemsjukvård är patienterna ofta i behov av stöd från sjuksköterskan gällande egenvård vid diabetesrelaterade fotkomplikationer. Det är viktigt med en delaktig patient och stödet till patienten baseras på vilket behov av stöd som finns. Sjuksköterskan har ett ansvar i att vägleda och råda patienterna kring egenvård. I den andra huvudkategorin: Organisationens betydelse för sjuksköterskan att stödja patienten framkom det brister gällande fortbildning men även varierande kompetens inom den egna organisationen. Informationsöverföring från andra vårdgivare fungerade inte helt optimalt, ofta fick sjuksköterskorna ingen information om patientens besök hos exempelvis diabetessjuksköterskan.Konklusion: Sjuksköterskorna har en viktig roll i att stödja patienterna genom att förklara och anpassa information till varje enskild patient kring egenvård för att förhindra fotkomplikationer, både muntligt och skriftligt. Det finns ett behov av utbildning och kompetensutveckling för både sjuksköterskor samt omvårdnadspersonal i kommunerna. Utebliven information mellan vårdgivare är en riskfaktor som äventyrar patientsäkerheten. / Background: Foot complications related to diabetes is the most common complication and can cause a lot of suffering. It is important with support, counseling and education for the person with diabetes to avoid foot complications. The nurse has an important role in supporting patients in their illness and involving them in their care in order to adequately manage their self-care. Purpose: To describe the nurse's experience of supporting patients with diabetes-related foot complications in municipal home care. Method: A qualitative interview study in which six nurses from three different municipalities participated. The interviews were analyzed on the basis of qualitative content analysis. Results: In the result two main categories emerged. The first main category: The participation of the patient and its significance for the nurse to support: showed that in municipal home care, the patients are often in need of support from the nurse regarding self-care in diabetes-related foot complications. It is important with a participant patient and the support for the patient is based on the needs of the patient. The nurse has a responsibility to guide and advise the patients regarding self-care. The second main category: The significance of the organisation for the nurse to support the patient showed deficiencies regarding continuous education but also varying competence within the own organization. Information transfer from other healthcare providers did not work optimally, often the nurses received no information about the patient's visit to, for example, the diabetes nurse. Conclusion: The nurses have an important role in supporting the patients by explaining and adapting information to each patient about self-care to prevent foot complications, both verbally and in writing. There is a need for education and competence development for both nurses and nursing staff in the municipalities. Failure to provide information between healthcare providers is a risk factor that can jeopardize patient safety.
