Previous issue date: 2003 / Resumo: A respeito dos cuidados perioperatórios do paciente cirúrgico ambulatorial, chamam a atenção os pacientes submetidos à cirurgia para extração da catarata infantil, provenientes do ambulatório de oftalmologia, pelo desconhecimento manifesto no pré-operatório imediato, dos seus pais/responsável sobre a afecção ocular, indicação cirúrgica e aos cuidados perioperatórios com a criança. Acredita-se que essa falta de informações suscita o aparecimento de temores e dificuldades no cuidado com a criança no período que antecede à cirurgia e no pós-operatório no domicílio. Os estudos relatam a seriedade do problema das cataratas em crianças quanto às implicações visuais e sociais, apontando a participação dos pais como de extrema importância, no difícil tratamento e enfatizam a necessidade de orientação dos mesmos. O presente estudo teve como objetivos: verificar o conhecimento prévio dos pais/responsável sobre a afecção ocular e cuidados perioperatórios na criança portadora de catarata; identificar as dificuldades e expectativas dos pais/responsável perante a cirurgia; elaborar instrumentos de informação e orientação quanto à catarata e aos cuidados perioperatórios; realizar uma reunião em grupo com os pais/responsável e verificar o conhecimento apreendido e o auxílio da reunião em grupo e dos instrumentos de informação e orientação, nos cuidados com a criança antes e após a cirurgia. Fizeram parte do estudo dez mães cujas crianças preencheram os critérios de inclusão na amostra. Para a coleta de dados foi realizada uma entrevista com as mães no período pré-operatório, no dia da indicação da cirurgia, uma reunião em grupo 15 dias antes da cirurgia e uma outra entrevista com elas no 5° dia do pós-operatório, em ambas, seguindo um instrumento elaborado previamente. Para a reunião em grupo utilizou-se uma estratégia que procurou contemplar alguns pressupostos das ¿Aulas vivas¿ de MASETTO (1992), cuja abordagem privilegia a interação e a participação do adulto no processo de ensino-aprendizagem. Com base nos resultados obtidos, no pré-operatório, as mães apresentaram um conhecimento razoável quanto à afecção ocular e aos cuidados perioperatórios com a criança; as expectativas delas refletiam sentimentos de angústia e de indefinição, a preocupação estava relacionada a todas as fases do período perioperatório e havia uma previsão de dificuldades no cuidado com a criança tanto no pré como no pós-operatório. Após a reunião em grupo e a entrega dos folder verificou-se a melhora das respostas em relação às alternativas relacionadas à afecção ocular, tratamento, sentimentos e cuidados perioperatórios com a criança. Conforme relato das próprias mães, elas não encontraram dificuldades no cuidado com a criança nem antes nem após a cirurgia / Abstract: About patients¿ perioperative care at an Ambulatory Surgery Center, usually there is a complete fault of knowledge, specially in relation to the cataract surgery in infants, when their parents are questioned about the immediate preoperative period, regarding the primary care prior and after the surgery, surgical indication and the ocular affection. It is believed that this lack of knowledge is responsible for the fears and care dificulties in dealing with the children before the surgery and after surgery at home. There are studies stating the severity of the problems regarding the visual and social implications of the cataract in children and indicating crucial participation of the parents in the problems and the importance of parental orientation. The objectives of the present study were to verify the previous knowledge of children¿s parents abouth their ocular affection and perioperative cares in cataract disease; to identify the parents¿ dificulties and expectations towards the surgery; to elaborate information and orientation brochures directed to clarify the cataract disease and perioperative care; to create a group meeting with the parents; to verify the knowledge and the usefulness of the group meeting and the information and orientation brochures in the perioperative care of the children. The study population was composed by ten mothers whose children fulfilled the inclusion criteria. For data collection an interview with the mothers was made after the cataract surgery indication in the ophthalmology departament. There was also a group meeting fifteen days prior to the surgery and another interview five days following the surgery, and all questions were based on a previous elaborated questionnaire. The group meeting was based on Masseto¿s propositions, where he points out the interaction and participation of the adults in the learning and teaching procedures. Data analysis has shown that the mothers had a reasonable degree of knowledge about ocular affection and perioperative cares, and it has been also noted that they had anxiety and uncertanties towards all the perioperative period. After the group meeting and the offer of the brochures there was an increase in the knowledge about the ocular affection, its treatment and the postoperative cares. The data suggest that the group meeting and the information and orientation brochures about cataract and perioperative cares were useful to the mothers, because they contribute to the better diminution of their anxiet and uncertanties; the mothers¿s preocupation direct to only a few items of perioperative period.
