Bakgrund: Palliativ vård i livets slutskede innebär lindrande vård och grundar sig på de fyra hörnstenarna: symtomlindring, stöd till närstående, kommunikation och teamarbete. Målen inom palliativ vård är att främja livskvaliteten, lindra lidande, samt att beakta individens fysiska, psykiska, sociala och existentiella behov vid obotligt sjukdomstillstånd. Genom ett personcentrerat förhållningssätt finns förutsättningar att uppnå målen. I den palliativa vården i livets slutskede i hemmet har sjuksköterskan en ledande roll, vården kan vara komplex, utmanande och förutsättningarna för omvårdnaden är beroende på sjuksköterskans kunskap kommunikationen och samarbetet inom teamet. Syfte: Syftet var att beskriva sjuksköterskors erfarenheter av att vårda patienter i livets slutskede i hemmet. Metod: Kvalitativ metod med induktiv ansats tillämpades för att genomföra litteraturöversikten. Tio kvalitativa artiklar utgjorde resultatet vilket analyserades enligt Fribergs fem steg (Friberg, 2022). Resultat: Resultatet belyser sjuksköterskors erfarenheter i att vårda patienter i livets slutskede i hemmet. Analysen resulterade i tre teman: vårda i relation, teamarbete och förutsättningar för att vårda och sju subteman: egna känslor, att vägleda, närvara i hemmet, symptomlindring, samverka med patienten, närstående, sjuksköterskans kompetens och stöd från omgivande team. Slutsats: Sjuksköterskors erfarenheter av att vårda patienter i livets slutskede i hemmet var utmanande på många sätt och deras egna känslor kunde påverka vården. För en erfaren sjuksköterska var det lättare att ge adekvat symtomlindring än för en oerfaren. Samarbete och stöd från kollegor samt tillgängligheten till primärläkare ansågs vara viktiga aspekter för en fungerande palliativ vård i hemmet. / Background: Palliative care at the end of life is based on symptom relief, support for relatives, communication and teamwork. The goals of palliative care are to promote quality of life, alleviate suffering and to take into account the individual’s physical, psychological, social end existential needs in case of incurable illness. Through a person-centered approach, it is possible to achieve these goals. In palliative care at the end of life at home, the nurse has a leading role. Care can be complex and challenging and the conditions for the care depend on the nurse’s knowledge and cooperation within the team. Aim: The aim of this study was to describe the nurse’s experience of palliative care in end of life care at home Method: The method is a literature review based on ten health science articles with a qualitative design. The analysis was done according to Friberg’s analytical model. Results: The results highlight nurses experiences in caring for palliative patients at end of life at home and is presented in three themes: caring in relationship, teamwork and conditions for care and seven subthemes: own feelings, to guide, to be present in the home, symptom relief, to cooperate with the patient and relatives, the nurse’s competence and support from the surrounding team. Conclusion: Nurses’ experience of caring for palliative patients at the end of life at home was challenging in many ways and their own feelings could affect care. For an experienced nurse, it was easier to provide adequate symptom relief than for an inexperienced one. Cooperation and support from colleagues as well as the availability of primary care physicians were important aspects for a functioning palliative care at home.
