INTRODUCCIÓN: Los altos costos generados por el Linfoma de Hodgkin (LH) justifican buscar marcadores pronósticos fiables y accesibles. Este estudio evalúa la asociación entre el Índice Plaqueta/Linfocito y la Sobrevida Global a los 5 años en pacientes con LH atendidos en el INEN durante el período 2007-2016.
MÉTODOS: Estudio observacional analítico tipo cohorte retrospectivo con análisis de sobrevida basado en revisión de historias clínicas en adultos con LH. El Índice Plaqueta/Linfocito (IPL) se calculó como el cociente entre el recuento de plaquetas y el número absoluto de linfocitos; al igual que los índices Neutrófilo/Linfocito (INL) y Linfocito/Monocito (ILM). Se obtuvieron sobrevidas a los cinco años y se elaboraron las curvas de Kaplan-Meier. La magnitud de asociación se evaluó mediante Hazard Ratios (HR) con intervalos de confianza al 95% (IC95%) usando regresión de Cox cruda y ajustada.
RESULTADOS: Se estudió 224 con una media de edad de 42 años, en su mayoría varones (58.93%). En el análisis crudo tener más de 45 años, albúmina <4, hemoglobina <10.5, un RAL <600, Ann Arbor IV y tener un ILM <1.1 se asociaron a una menor sobrevida. Luego de ajustar por sexo, edad, Ann Arbor y hemoglobina, no se encontró asociación con IPL ≥266.2 (HR: 0.93, IC95%: 0.43-2.02), ni con INL ≥6.4 (HR: 0.90, IC95%: 0.40-2.05), ni con ILM ≥1.1 (HR: 0.53, IC95%: 0.23-1.21).
CONCLUSIONES: En este análisis no se encontró asociación entre ninguno de los tres índices linfocitarios evaluados con sobrevida. Edad ≥45, y estadio Ann Arbor IV se asociaron a menor sobrevida. / INTRODUCTION: The high costs generated by Hodgkin Lymphoma (HL) justify a search for reliable and accessible prognostic markers. This study evaluates association between the Platelet/Lymphocyte Ratio and five-year Overall Survival in patients with HL treated at INEN during the period 2007-2016.
METHODS: A retrospective cohort study with survival analysis based on review of medical charts was performed in patients with diagnosis of HL. The Platelet/Lymphocyte Ratio (PLR) was calculated as the quotient between the platelet count and the absolute number of lymphocytes. Similarly, Neutrophil/Lymphocyte (NLR) and Lymphocyte/Monocyte (LMR). Five-year survival probabilities were and graphed using Kaplan-Meier curves. Magnitude of association was evaluated using Hazard Ratios (HR) with 95% confidence intervals (95%CI) obtained by crude and adjusted Cox regression.
RESULTS: 224 patients older than 18 years were included, with a mean age of 42 years, made up mostly of men (58.93%). In the crude analysis being over 45 years old, albumin <4, hemoglobin <10.5, RAL <600, Ann Arbor IV and LMR <1.1 were associated with a lower survival. In the multivariate analysis, there was no association of survival with PLR ≥266.2 (HR: 0.93, 95% CI: 0.43-2.02), nor NLR ≥6.4 (HR: 0.90, 95% CI: 0.40-2.05) nor LMR ≥1.1 (HR: 0.53, 95% CI: 0.23-1.21).
CONCLUSION: The Lymphocyte ratios were not significantly associated with overall survival in our study. Age≥45, and Ann Arbor stage IV were associated to less survival probability. Studies with a larger population in our region are required in order to offer significant results according to our reality. / Tesis
